1.Preliminary research of Er:YAG laser used for pulpotomy of Beagle dogs

Objective:To investigate the effects of erbium:yttrium aluminium garnet (Er:YAG)laser irradiation on pulp and formation of dentin bridge.The beagle dogs'coronal pulp is amputated by Er:YAG laser.Methods:In the study,24 premolar teeth of two one-year-old Beagle dogs were used.They were divided into 3 groups:bur group,200 mJ laser group,100 mJ laser group.The coronal pulp were removed by traditional bur or 200 mJ/20 Hz or 100 mJ/20 Hz Er:YAG laser.Then,they were dressed with mineral trioxide aggregate (MTA)and sealed with resin.Clinical,radiological and histological ana-lyses were performed 1 day,2 weeks,4 weeks and 8 weeks after treatment.The mobility and gingiva si-tuation were evaluated for clinical evaluation.Periapical films were used for radiological evaluation.The extracted teeth were preserved in 10% formalin.After decalcification,tissue processing,paraffin embed-ding,serial sectioning at 5 μm thickness,staining (Hematoxylin and Eosin,HE),the samples were as-sessed by an independent observer for calcified bridge formation and radicular pulp inflammation.The mean thickness of dental bridge was measured if there was complete dental bridge.Results:Clinical evaluation:there were no signs of fistula or mobility or any abnormal symptoms in Er:YAG laser groups and bur group during the observation period.Radiological evaluation:there were no signs of widened pe-riodontal ligament or root absorption or periapical radiolucency in Er:YAG laser groups and bur group. Histological evaluation:there was no severe inflammation reaction in 200 mJ/20 Hz,100 mJ/20 Hz Er:YAG laser groups and bur group 1 day,2 weeks,4 weeks,8 weeks post-operation in Hematoxylin and Eosin staining.A complete dentin bridge could be observed in 200 mJ/20 Hz,100 mJ/20 Hz Er:YAG laser groups,while no complete dentin bridge in bur group 2 weeks post-operation.Complete dental bridge could be observed in each group 4 and 8 weeks post-operation.The mean thickness of dental bridge 4 weeks post-operation in 200 mJ laser group was 77 μm,100 mJ laser group 87 μm,and bur group 101 μm,and 8 weeks post-operation in 200 mJ laser group was 222 μm,100 mJ laser group 160μm,and bur group 152 μm.Conclusion:The 200 mJ/20 Hz and 100 mJ/20 Hz Er:YAG laser pulpoto-mies show no harm to radicular pulp and can promote the formation of dentin bridge.
2.Evaluation of right ventricular function in patients with pulmonary arterial hypertension by real-time three-dimensional echocardiography

Objective To evaluate right ventricular systolic function in patients with pulmonary hypertension (PH) by real-time three-dimensional echocardiography (RT-3DE),and compared with cardiac magnetic resonance.Methods A total of 23 patients with PH who underwent MRI and echocardiography in the study.Right ventricular index of myocardial performance (RIMP),fractional area change (RVFAC),tricuspid annular plane systolic excursion (TAPSE) and tissue Doppler-derived tricuspid lateral annular systolic velocity (S') were measured by echocardiography.RV end-diastolic volume (RVEDV) and endsystolic volume (RVESV) were measured by RT-3DE and cardiac magnetic resonance and RV ejection fraction (RVEF) was calculated.Results A positive correlation was found between RVFAC and RVEF (r =0.595,P =0.003),there was a negative correlation between RIMP and RVEF (r =-0.745,P =0.000),has no correlation between TAPSE and RVEF (r =-0.029,P =0.896),a positive correlation was found between S' and RVEF (r =0.489,P =0.018).There were close correlation between RVEDV,RVESV and RVEF measured by RT-3DE and MRI (P <0.001,respectively);Bland-Altman analyses showed good agreement between them.Conclusions RT-3DE can noninvasive,accurate assessment right ventricular systolic function in patients with PH,and provide prognosis and treatment choice for clinical demands.
3.Effects of sleep quality on cognitive function in elderly patients undergoing hemodialysis

Objective:To investigate the effects of sleep quality on cognitive function in elderly patients undergoing hemodialysis.Methods:In this cross-sectional study, the cognitive function in hemodialysis patients was evaluated by Montreal Cognitive Assessment(MoCA)and the sleep quality was accessed by Pittsburgh sleep quality index(PSQI). The related indexes of cognitive function were compared and the relationship between cognitive function and sleep quality was analyzed by using multiple binary Logistic regression method.Results:A total of 121 elderly hemodialysis patients aged 67.4±7.2 years with cognitive impairment of 76 cases(62.8%)were included in the study.Numbers(%)of cases with very good, good, general and poor sleep quality were 33(27.3%), 39(32.2%), 14(11.6%)and 35(28.9%), respectively.The proportion of patients with pretty good sleep quality was lower in cognitive impairment group than in normal cognitive group(13/76 or 17.1% vs.20/45 or 44.4%, P<0.001). The proportion of patients with poor sleep quality was higher in cognitive impairment group than in normal cognitive group(29/76 or 38.2% vs.6/45 or 13.3%, P<0.05). Multiple binary Logistic regression analysis showed that age( OR: 1.128, 95% CI: 1.083-1.175, P<0.001), a education level( OR: 0.353, 95% CI: 0.151-0.556, P<0.05), hypertension( OR: 2.508, 95% CI: 1.189-5.291, P<0.05), diabetes( OR: 1.913, 95% CI: 1.045-3.502, P<0.05), stroke( OR: 4.044, 95% CI: 1.439-11.365, P<0.05), dialysis age( OR: 1.016, 95% CI: 1.010-1.023, P<0.001), KT/V( OR: 0.025, 95% CI: 0.005-0.122, P<0.001)and parathormon(iPTH)( OR: 1.002, 95% CI: 1.000-1.003, P<0.05)were associated with cognitive impairment.After adjusting for above factors, sleep quality was also correlated with cognitive impairment( OR: 1.180, 95% CI: 1.108-1.258, P<0.001), showing that the risk of cognitive impairment increased by 1.18 times with every one point increase in PSQI scores. Conclusions:Cognitive impairment is associated with age, education level, hypertension, diabetes, stroke, dialysis age, KT/V and iPTH in elderly hemodialysis patient, and sleep quality is also an independent risk factor for cognitive impairment.
4.Reconstruction with 3D-printed modular pelvic endoprostheses after pelvic tumor resection

Objective To investigate the ideal reconstruction methods after pelvic tumor resection and to assess the feasibility of three-dimensional (3D) printing technology for pelvic reconstruction,we performed a retrospective study of patients with pelvic tumors who received tumor resection and reconstruction using 3D-printed pelvic endoprostheses.Methods Thirty-five patients received tumor resection and functional reconstruction with 3D-printed pelvic endoprostheses in our center from Sep.2013 to Dec.2015.According to Enneking' s classification,there were 3 cases of Type Ⅰ,12 cases of Type Ⅱ+Ⅲ,5 cases of Type Ⅰ+Ⅱ,2 case of Type Ⅰ+Ⅱ+Ⅲ,10 cases of type Ⅰ+Ⅱ+Ⅳ and 3 cases of type Ⅰ+Ⅱ+Ⅲ+Ⅳ resection.Methods of reconstruction included 3 patients with 3D-printed iliac endoprosthesis,12 patients with 3D-printed standard hemipelvic endoprosthesis and 20 patients with 3D-printed screw-rod connected hemipelvic endoprosthesis.Results All 35 patients underwent en bloc resection.Margins were wide in 15 cases,marginal in 14 cases and intralesional in 6 cases.After a mean followed-up of 20.5 months (6 to 30),25 patients survived without evidence of disease,5 patients were alive with diseases and 5 patients died of distant metastasis.Complications included 7 cases of delayed wound healing,2 cases of hip dislocation and no cases of deep infection.The average MSTS 93 score was 19.1 (9-26) for all of the 30 alive patients,22.7 (20 to 25) for 3 patients with iliac endoprosthesis reconstruction,19.8 (15 to 26) for 12 patients with standard hemipelvic endoprosthesis reconstruction,and 17.7 (9 to 25) for 15 patients with screw-rod connected hemipelvic endoprosthesis at the last follow up.Conclusion The application of 3D-printing technology could facilitate precise matching and osseointegration between the implants and host bone.Our clinical results proved that application of 3D-printed pelvic endoprostheses for reconstruction of bony defect was safe without additional complications,and that good functional results could be expected during short-term follow-up.
5.Relationship between echocardiographic and magnetic resonance derived measures of right ventricular function in patients with chronic thromboembolic pulmonary hypertension

Objective To evaluate right ventricular function of chronic thromboembolic pulmonary hypertension (CTEPH) patients by echocardiography and cardiac magnetic resonance (CMR),and to evaluate the value of clinical application of those right heart function indexes.Methods 32 patients with CTEPH who underwent CMR and echocardiography were involved in the study.Right ventricular index of myocardial performance (RIMP),fractional area change (FAC),tricuspid annular plane systolic excursion (TAPSE) and tissue Doppler-derived tricuspid lateral annular systolic velocity (S') were measured by echocardiography.Right ventricular end-diastolic volume (RVEDV) and end-systolic volume (RVESV) were measured by cardiac magnetic resonance and right ventricular ejection fraction (RVEF) was calculated.Results A positive correlation was found between FAC and RVEF (r =0.423,P =0.022),there was a negative correlation between RIMP and RVEF (r =-0.387,P =0.048),there was no correlation between TAPSE and RVEF (r =0.451,P =0.22),a positive correlation was found between S' and RVEF (r =0.689,P =0.000).Conclusions The echocardiographic parameters FAC,RIMP and S' can reflect right ventricular function in patients with CTEPH and can be used as a routine clinical parameters.
6.Concentrations of fine particulate matters and ultrafine particles and influenced factors during winter in an area of Beijing

Objective:To study the concentrations of fine particulate matters and ultrafine particles and influenced factors during winter in an area of Beijing .Methods:Real-time monitoring of particles ’ mass and number concentrations were conducted in an area of Beijing from February 7th to 27th , 2013.At the same time, the meteorological data were also collected from the Beijing meteorological website .Differences of the particles ’ mass and number concentrations during different periods were analyzed using Mann -Whitney U test.Meanwhile, the influenced factors were also analyzed .Results: The mean concentra-tions of fine particulate matters and ultrafine particles were ( 157.2 ±142.8 ) μg/m3 and (25 018 ± 9 309) particles/cm3, respectively.The particles’ number and mass concentrations in haze days were 1.27 times and 2.91 times higher than those in non-haze days, respectively.The mass concentrations of fine particulate matters in the self-monitoring site were higher than those in the nearest central monitoring sites, and the hourly-average concentrations of particles were significantly consistent with those at the commuter times.Meanwhile, the setting off of fireworks/firecrackers during the Spring Festival could lead to short-term increases of the particles ’ number and mass concentrations .When the wind speed was low and the related humidity was high , the concentrations of particulate matters were relatively high , and the mass concentrations of fine particulate matters were lagged about 1-2 d.Conclusion: The level of the particulate matters in this area was high .Heavy traffic , setting off of fireworks/firecrackers and meteoro-logical factors may be some of the main factors affecting the concentrations of the particulate matters in this area.Among those factors, the effect of setting off of fireworks/firecrackers didn’t last long and the effect of the meteorological factors had a hysteresis effect .
7.Association between hypersensitive C reactive protein and the incidence of acute kidney injury insubarachnoid hemorrhagic patients-a research on the clinical perspective

Objective To evaluate the association between hypersensitive C reactive protein (hs-CRP) and the incidence of acute kidney injury in subarachnoid hemorrhage(SAH) patients.Methods It retrospectively recruited 213 cases of computerized tomography validated SAH patients from the neurology ICU from Beijing Tiantan Hospital between January 2012 and January 2015.The average age was (56.29±11.95) years old,and the patients were divided into AKI and non-AKI groups according to Kidney Disease: Improving Global Outcomes (KDIGO) diagnosis standards, Clinical features of AKI and Non-AKI patients including serum levels of hs-CRP were compared and multi-logistic regression was applied to find the risk factors concerning with the incidence of AKI.Receiver operating characteristics (ROC) curve was also plotted to evaluate the diagnostic value of hs-CRP towards the incidence of AKI.Results A total of 25 (11.74%) patients developed AKI.Average age of the SAH patients in both AKI and non-AKI groups were (63.60±12.21) years old vs.(55.31±11.60) years old(t=-3.33, P<0.05).The ratios of diabetics were were 28.00% vs.11.17% (χ2=5.47,P<0.05) and the ratio of proteinuria were 80.00% vs.34.57%, respectively (χ2=3.83, P<0.05).The median of serum creatinie were 63.72(51.45, 79.72)μmol/L vs.53.21(45.27, 65.62)μmol/L (P<0.05), and serum hs-CRP were (14.12±5.03)mg/L vs.(10.23±6.76)mg/L (P<0.05), and the ratios of antibiotics application were 84.00% vs.43.08% (P<0.05 for all).Multi-logistic regression analysis showed that serum hs-CRP was an independent risk factors for AKI after age, serum creatinine at admission were adjusted.[OR (95% CI) was 3.33(1.13, 9.85),P<0.05 for all].The area under curve of ROC was 0.69 (P<0.05), and the cut-off point of serum hs-CRP under the maximum Youden index was 13.85 mg/L.Conclusion Serum hs-CRP is an independent risk factor of theincidence of AKI in SAH patients, the significantly increase of serum hs-CRP might be an important predictor of the incidence of AKI in SAH patients.
8.Effect of sevoflurane postconditioning on microRNA-133a expression during myocardial ischemia-reperfusion in mice

Objective To evaluate the effect of sevoflurane postconditioning on microRNA-133a (miR-133a) expression during myocardial ischemia-reperfusion (I/R) in mice.Methods Thirty adult male C57 mice,weighing 20-30 g,were randomized to 3 groups (n =10 each) using a random number table:control group (group C),I/R group,and sevoflurane postconditioning group (group SP).In I/R and SP groups,hearts from adult male C57 mice were exposed and subjected to 30 min of ischemia and 180 min of reperfusion in anesthetized mice according to the method described by Das et al.In group C,only thoracotomy was performed without ligation of the coronary artery.In group SP,2.4% sevoflurane was inhaled for 5 min starting from the onset of reperfusion to perform sevoflurane postconditioning.At 180 min of reperfusion,blood samples from the femoral vein were collected for determination of serum lactic dehydrogenase (LDH) and creatine kinase (CK) activities using the colorimetric method.The mice were then sacrificed,and myocardial specimens were obtained for determination of myocardial infarct size,miR133a and caspase-9 mRNA expression (by real-time reverse transcriptase polymerase chain reaction),and caspase-9 expression (by Western blot).Results Compared with group C,the serum LDH and CK activities and myocardial infarct size were significantly increased in I/R and SP groups,the expression of miR-133a was significantly down-regulated,and the expression of caspase-9 protein and mRNA was significantly up-regulated in group I/R,and the expression of miR-133a and caspase-9 protein and mRNA was significantly up-regulated in group SP (P<0.05).Compared with group I/R,the serum LDH and.CK activities and myocardial infarct size were significantly decreased,the expression of miR-133a was significantly up-regulated,and the expression of caspase-9 protein and mRNA was significantly downregulated in group SP (P<0.05).Conclusion The mechanism by which sevoflurane postconditioning inhibits cell apoptosis during myocardial I/R is related to up-regulation of miR-133a expression in mice.
9.Comparison of respiratory diseases and symptoms among school-age children in areas with different levels of air pollution

Objective:To compare the differences of children’ s health in different area, and to confirm if the prevalence of respiratory diseases and symptoms among children are closely associated with the air pollution. Methods:A cross-sectional study was conducted in an urban area A and a suburban area B with different levels of air pollution in Beijing. Using a cluster sampling method, we recruited 4 564 chil-dren from 3 primary schools in urban A and 4 primary schools in suburban B. Respiratory symptoms were investigated using an international standardized questionnaire including characteristics of children, living conditions, respiratory diseases and symptoms and situation of parents. The concentrations of air pollu-tants for recent five years were obtained from Reports on the Quality of the Beijing Environment. SPSS 16. 0 was used to analyze data. Results: The prevalence of cough, persistent cough, phlegm, persistent phlegm, wheeze and asthma in A area were higher than those in B area[(62. 2% vs. 59. 9%), (6. 3%vs. 3. 1%), (42. 4% vs. 37. 4%),(3. 6% vs. 2. 4%),(13. 3% vs. 9. 9%) and(9. 5% vs. 5. 4%)]. Except for cough, cough with cold, cough without cold, the prevalence of respiratory diseases and symp-toms in A area were significantly higher than those in B area ( P <0 . 05 ) . Logistic regression analysis showed the prevalence of persistent cough, phlegm without cold, asthma in A area were significantly higher than those in B area ( P<0 . 05 ) . Conclusion:Respiratory diseases and symptoms among school-age children were closely associated with the level of air pollution.
10.Predictive value of circulating miR-143 and miR-182 for the outcome in patients with acute ischemic stroke

Objective:To investigate the predictive value of circulating miR-143 and miR-182 for the short-term clinical outcomes in patients with acute ischemic stroke (AIS).Methods:Patients with AIS admitted to Danzhou People's Hospital from January 2018 to June 2020 were included prospectively. The modified Rankin Scale was used to evaluate the short-term clinical outcome at 14 d after onset or at discharge. 0-2 was defined as good outcome, and >2 was defined as poor outcome. Multivariate logistic regression analysis was used to identify the independent risk factors for poor short-term clinical outcomes in patients with AIS. The receiver operating characteristic (ROC) curve was used to evaluate the predictive value of miR-143 and miR-182 for the short-term clinical outcomes in patients with AIS. Results:A total of 158 patients with AIS, aged 65.80±12.36 years, were enrolled, including 105 males (66.46%), 95 patients with good outcome (60.1%) and 63 with poor outcome (39.9%). The age, total cholesterol, triglyceride, low-density lipoprotein cholesterol, baseline National Institutes of Health Stroke Scale (NIHSS) score, serum miR-143 and miR-182 level in the poor outcome group were significantly higher than those in the good outcome group (all P<0.05). Multivariate logistic regression analysis showed that age (odds ratio [ OR] 1.984, 95% confidence interval [ CI] 1.315-3.617; P=0.036), low-density lipoprotein cholesterol ( OR 2.108, 95% CI 1.406-4.103; P=0.013), baseline NIHSS score ( OR 2.584, 95% CI 1.675-4.505; P=0.005), miR-143 ( OR 3.205, 95% CI 2.370-6.180; P<0.001) and miR-182 ( OR 2.802, 95% CI 1.905-5.516; P<0.001) were the independent risk factors for poor outcomes in patients with AIS. ROC curve analysis showed that the combined area under the curve of miR-143 and miR-182 to predict the poor outcome in patients with AIS was 0.935 (95% CI 0.873-0.992), the sensitivity and specificity were 96.5% and 87.0% respectively. Conclusions:The increase of serum miR-143 and miR-182 was closely associated with the poor short-term outcomes in patients with AIS. The combination of the two has a good predictive value for the poor short-term outcomes in patients with AIS.
