1.Clinical and Pathological Features of Canceration of Epidermoid Cyst

Objective To study and research the clinical pathologic features, immunohistochemistry and prognosis of epidermoid cyst with cancerous changes. Methods Six cases of epidermoid cyst with cancerous changes were studied of their clinical pathologic features and immunohistochemistic expression and their prognosis was evaluated by follow-up vis-its. Results Clinical features: All six cases showed local enclosed mass and underwent extensive resection. No recur-rence nor metastasis were shown in one year follow up. Pathological examination show irregular cystic mass with bean curd like fillings in it. Microscopic examination showed those cysts were lined with squamous epithelium which indicate invasive growth. Pathological diagnosis: Epidermoid cyst canceration to squamous cell carcinoma. Immuno-phenotype: CK 5/6 and P63 were positive in all six squamous cell cancer cases. Conclusion Epidermoid cyst with cancerous changes are quite rare. Diagnosis of the tumor largely relies on histopathology and immunohistochemical markers also assist diagnosis. Local appropriate expanded resection is preferred. Its malignant degree is low and prognosis is good.

3.Effect of Tetramethylpyrazine on IL-8,IL-12 and IL-33 Levels in Serum and MMP-10 and NF-κB Protein Expression in Joint Synovial Tissues in Rats with Collagen-induced Arthritis

Objective To observe the effect of tetramethylpyrazine (TMP) on serum interleukin 8 (IL-8),interleukin 12 ( IL-12) and interleukin 33 ( IL-33) levels and MMP-10 and NF-κB protein expression in joint synovial tissues in rats with collagen-induced arthritis ( CIA) ,and further to investigate the mechanism for treating rheumatoid arthritis. Methods Rats were randomly divided into normal control group( n=10) ,model control group,dexamethasone group,low-dose of TMP and high-dose of TMP groups ( n=9 each) . The CIA model was established in all the groups except for the normal control group. Rats of dexamethasone group were give 2 mg.kg-1 of dexamethasone;Rats of low-dose and high-dose of TMP group were given 50,100 mg.kg-1 of TMP, respectively.The toe volume and arthritis index (AI) were used to evaluate joint inflammation. The levels of IL-8,IL-12 and IL-33 in the serum of rats were detected by ELISA. The expression levels of MMP-10 and NF-κB in joint synovial were detected by Western blotting. Results Compared with CIA group,the toe volume and AI were significantly reduced in high-dose of TMP groups and dexamethasone groups,and levels of IL-8,IL-12 and IL-33 and the expression levels of MMP-10 and NF-κB were decreased significantly (P<0.01). TMP (50 mg.kg-1) could obviously decrease the toe volume and the levels of IL-8 and IL-33. There was no statistical difference in other indices (P>0.05). Conclusion TMP at 100 mg.kg-1 showed obvious inhibition on CIA rats. The mechanism may be correlated to balancing the effect of proinflammatory factor and anti-inflammatory cytokines,improving immune function in rats,reducing joint synovial inflammation,and alleviating the destruction of the articular cartilage.
4.Diagnosis and treatment of bilateral bronchial foreign body in children.

OBJECTIVE:
To discuss the clinical characteristics of bilateral bronchial foreign body in children, increase the curative rate and reduce the complications.
METHOD:
The clinical data of 35 cases with bilateral bronchial foreign body in children were retrospectively analysed, including surgery, key surgery points, and postoperative combined therapy.
RESULT:
The foreign bodies of all cases were removed under intravenous general and tropical anesthesia without complications.
CONCLUSION
The bilateral bronchial foreign body is a serious case, the timely and effective treatment can lower the mortality rate and postoperative complications. The children lack of oxygen for a long time before and in operation should be give comprehensive therapy, for example: sedation and hyperbaric oxygen.

5.Mophological changes after percutaneous transluminal angioplasty in experimental cerebral vasospasm

Objective Experimentally to investigate the changes in vasospastic canine basilar arteries. Methods Vasospasm of basilar arteries was induced by double-injecting autologous blood into the brain cisterns. 28 adult mongrel dogs of both genders served as the subjects, of them 20 involved in experimental group and accepted the PTA treatment and the other 8 doges were in negative control group without any treatment. PTA was performed by a single inflation of balloon for 10 seconds at a peak pressure of 2.0 atmospheres. Angiography was re-performed at 30 day, 90 day and 300 day following PTA, respectively. The histologic changes were then evaluated using light and electron microscopy. Results Caliber of basilar arteries of all canines decompressed obviously, from 1.3 mm to 0.8 mm. 7.Long-term outcomes of carotid artery stenting versus endarterectomy for carotid stenosis: A meta-analysis of randomized controlled trials

ObjectiveTo evaluate the long-term outcomes of carotid endarterectomy versus carotid artery stenting for carotid stenosis.MethodsPubMed, EMBASE, and the Cochrane databases were retrieved.The randomized controlled trials of comparing CEA with CAS in patients with carotid artery stenosis were enrolled.The data such as the research basic characteristics and the long-term outcomes including stroke or death combined endpoints, any stroke or any death were extracted.The Stata software was used to conduct statistical analysis.ResultsA total of 7 randomized controlled trials and 8 210 patients were included.The median follow-up time was 2-7.4 years.The overall quality of the included studies was high and the risk of bias was low.The meta-analysis showed that the risks of the combined endpoint of stroke or death (hazard risk [HR] 1.21, 95% confidence interval [CI] 1.04-1.39), any stroke (HR 1.32, 95% CI 1.15-1.51) and ipsilateral stroke (HR 1.26, 95% CI 1.02-1.55) in the CAS group were significantly higher than those in the CEA group;the risks of death (HR 1.06, 95% CI 0.95-1.18), disabling stroke (HR 1.23, 95% CI 0.95-1.60), non-ipsilateral stroke (HR 1.12,95% CI 0.81-1.55) and restenosis (HR 1.18,95% CI 0.91-1.52) were not significantly different between between the CAS group and the CEA group.Conclusions CAS and CEA are associated with similar risks of long-term death, disabling stroke, non-ipsilateral stroke and restenosis.The risks of long-term combined endpoint of stroke or death, any stroke and ipsilateral stroke significantly higher with CAS.These results suggest that CEA remains the treatment of choice for carotid stenosis.
8.Prediction of subacute infarct lesion volumes by processing apparent diffusion coefficient maps based on apparent diffusion coefficient cut-off values in patients with acute stroke

Objective To explore the feasibility of prediction of subacute infarct lesion volumes by processing ADC maps based on ADC cut-off values in patients with acute stroke.Methods MRI was performed in 20 patients with clinically diagnosed acute infarct less than 6 h after stroke onset.The MRI included a DWI and conventional MRI.The follow-up MR examinations of all the patients were carried out within 4-7 days after admission.The brain tissue, which showed abnormal high intensity both in DWI at the baseline and FLAIR at the reexamination , was regarded as the area of the initial ischemia core.Graphic-penumbra was regarded as the difference between initial DWI and follow-up FLAIR.The ADC values of the ischemia core, graphic-penumbra and the contralateral normal brain tissue , the relative ADC (rADC) were measured.The rADC value of the graphic-penumbra was defined as the ADC cut-off values.GE medical system based on ADC cut-off values was also tested in these patients to obtain ADC maps.The lesion volumes, the abnormal area seen on the DWI , ADC maps and follow-up FLAIR, were also measured.rADC values in different areas were analyzed by paired Student t test.Relationship between baseline DWI , ADC map and follow-up FLAIR was analyzed using Spearman rank-order correlation test , and Kruskal-Wallis H test was used to compare the volumes among three groups.Results Absolute ADC and rADC values gradually increased from the core to the periphery of the ischemic lesion.The absolute ADC values statistically differed from those on the contralateral side for both ischemia core and graphic-penumbra.The rADC values were significantly decreased in the ischemia core ( 0.620 ±0.116 ) compared with the graphic-penumbra values (0.809 ±0.097;t =8.083,P <0.01).The lesion volume of follow-up FLAIR [16.3 (4.7, 29.0) cm3 ] correlated highly with both volume predicted with the ADC map [13.5 (4.8, 25.4) cm3] (r=0.967, P<0.01) and baseline DWI volume [11.0 (4.4,30.4) cm3] (r=0.950, P<0.01).There was no difference between the volume of DWI , ADC map and follow-up FLAIR(H=0.168, P>0.05).Conclusions Data shows the high feasibility of prediction of subacute infarct lesion volumes by processing ADC maps based on ADC cut-off values in patients with acute stroke , without intravenous contrast material, and it provides a new method for outcome prediction.
9.The comparing of the semen quality of different operation method of varicocele with 561 cases

Objective To research the effect to the semen quality of the three operation methods of superiority of highly selective varicocele high ligation,transinguinal canal and laparoseopic of renovated polomo management of varicocele.Methods 561 patients in our hospital who charged by infertility had the operation were analyzed retrospectively and were recruited with primary varicocele above grade II for this study.These patients were divided into three group according to three kinds of operation methods of varicocele :group A was treated with highly selective varicocele high ligation and had 300 patients;group B was treated with transinguinal canal operation and had 181 patients; and group C was treated by laparescopic of renovated polomo operation with 80 patients.Through the comparing of the fertility ability of the three operation methods,to evaluate the therapeutic effect of the three methods.Results The increasing rates of the quality of semen in group B was higher than other groups if patients' ages were lower than 30.The increasing rates of the quality of semen in every group was also higher if patients' ages were lower than 30.No difference was found between three groups in natural conception rate (P>0.05).Conclusion The methods of highly selective varieecele high ligation would be a better choice in the management of patients with primary varicecele because of higher quality of semen.
