Objective To compare the complications of end colostomy and loop colostomy for preventive colostomy, then to evaluate which one is superior to the other. Methods Studies and relevant literatures comparing end colostomy with loop colostomy for defunctiong colostomy were searched by PubMed,Springer and Embase Library. The rates of complications were pooled and compared using a Meta analysis. The risk ratios were calculated with 95% confidence intervals to evaluate the safety and efficacy of each teachnique. Results Six nonrandomized studies were included, with 1999 patients in total. The Meta-analysis of the non-randomied studies showed a lower risk of stoma retraction (RR:0.21,95% CI:0.04 ~ 0.99,P =0.05) and stoma prolapse (RR:0.23,95 % CI:0.05 ~0.99,P =0.05) in the end colostomy group, but the higher risk of ischemia and necrosis (RR:5.08,95% CI:1.94 ~ 13.22,P =0.05).No other statistically significant difference was observed for other complications. Conclusions Each type of defuncyioning stoma has its advantages and disadvantages. The study endorses end colostomy stoma over loop colostomy. However, there is not a strong evidence for the superiority of one colostomy over another for colostomy patients. So,large scale RCTs and high quality studies are needed.

Intestinal flora imbalance and abnormal glucose and lipid metabolism are important risk factors and pathological mechanisms of colorectal polyps. "Spleen deficiency and dampness accumulation" is the core pathogenesis of colorectal polyps. The imbalance of intestinal flora is related to spleen deficiency, and the application of Chinese herbs for invigorating spleen is helpful to the recovery of intestinal flora balance. Abnormal glucose and lipid metabolism is related to dampness accumulation, and it is effective to treat it with bitter and spicy herbs or spleen-invigorating and dampness-eliminating herbs. The interaction between intestinal flora imbalance and abnormal glucose and lipid metabolism changes intestinal microenvironment, damages intestinal epithelial cells, causes abnormal proliferation of intestinal stem cells and leads to colorectal polyps, which is consistent with the pathogenesis of spleen deficiency and dampness accumulation in Traditional Chinese Medicine. Thus, we tried to explore the biological connotation of the pathogenesis of "spleen deficiency and dampness accumulation" of colorectal polyps from the perspective of the interaction of intestinal flora and glucose and lipid metabolism, in order to provide reference for identifying high-risk population and analyzing the therapeutic mechanism of compound prescription for invigorating spleen and removing dampness.

World Science and Technology-Modernization of Traditional Chinese Medicine 2024;26(3):634-639
Precancerous lesion of gastric carcinoma(PLGC)is a critical component of the occurrence, development, and metastasis of the"inflammation-cancer transformation".Traditional Chinese medicine(TCM)is characterized by a unique mode of diagnosis and treatment which adopts a holistic concept to differentiate and treat syndromes. With this mode,it can improve symptoms in patients, enhance the quality of their lives and elevate the histopathological performance. The pathogenic factors of PLGC are mostly related to spleen, among which the weakness of spleen and stomach is the pathogenetic basis of it.Over time, dampness, phlegm and blood stasis will accumulate in the body, causing local lesions.Spleen deficiency is associated with various development mechanisms of PLGC,including the gastric mucosal inflammation, intragastric flora, gastric mucosal barrier, immune function, energy metabolism, and gastric mucosal secretion. To treat PLGC from the perspective of the spleen theory, physicians should take steps to reinforce healthy Qi to eliminate pathogenic factors, give equal consideration to deficiency and excess, and harmonize the center to restore the balance in the viscera, which will invigorate the spleen and stomach, resolve dampness, clear heat and regulate qi and blood at the same time. The differentiation of the holistic and local syndromes should be combined and the treatment with drugs should take both diseases and syndromes into consideration.

{L-End}Objective To analyze the correlation between occupational burnout and sleep quality among steelworkers. {L-End}Methods A total of 11 491 steelworkers from a large steel enterprise in Gansu Province were selected as the research subjects using convenient sampling method. The Burnout Questionnaire and the Pittsburgh Sleep Quality Index Scale were used to investigate their occupational burnout and sleep quality. Hierarchical regression analysis was used to analyze the effects of occupational burnout on the sleep quality. {L-End}Results The detection rate of occupational burnout and sleep disorder were 50.4% and 39.0%, respectively. There was a positive correlation between the level of occupational burnout and the total score of sleep quality (Spearman correlation coefficient=0.454, P<0.05). The results of hierarchical regression analysis, adjusting for confounding factors such as gender, age, marital status, education level, alcohol consumption, exercise, weekly working hours, seniority, work shift, noise exposure, dust exposure, and high-temperature work, showed that the score of occupational burnout was positively related to the score of sleep quality(P<0.01), explaining 16.0% of the variance in the score of sleep quality among these steelworkers. {L-End}Conclusion The detection rate of occupational burnout and sleep disorders are relatively high among the steelworkers in this enterprise. Higher levels of occupational burnout are associated with poorer sleep quality. Alleviating occupational burnout among steelworkers may contribute to improving their sleep quality.
