1.Mechanisms of fluconazole resistance in clinical and experimental induced isolates of Candida glabrata

Objective To investigate the mechanisms of fluconazole resistance in clinical and experimental induced isolates of C.glabrata.Methods Efflux of rhodamine 6G was performed to evaluate the effects of efflux pumps.The expression levels of transporter genes CDR1,CDR2,SNQ2 and ERG11 were examined by real-time RT-PCR.Meanwhile,sequence of PDR1 was determined by PCR based DNA sequencing.Results Efflux pumps of all fluconazole-resistant isolates had stronger effects than that of susceptible isolates,consistently with significant upregulation of CDR1,but no obvious difference was found in CDR2 or SNQ2.Also,no notable change in the expression level of ERG11 between susceptible and resistant isolates.PDR1 mutations existed in both clinical and experimental induced isolates of C.glabrata,among which P927S,L543P and S947L haven't been reported previously.Conclusion Mutations of PDR1 were induced by fluconazole both in vivo andin vitro,which will result in overexpression of CDR1 and strengthen the effect of efflux pump.
2.Endocytosis and exocytosis of gold nanochain attaching Hep-2 cells of human laryngeal carcinoma

BACKGROUND: The gold nanoparticles have a killing effect on tumor cells.OBJECTIVE: To investigate the influence of gold nanochain on Hep-2 cells proliferation of human laryngeal carcinoma.METHODS: The gold nanochain was prepared by a glucose synthesis method and added into the culture cells with different concentrations (10%, 25%, 50%, 75%, 95%) to test the influence on proliferation of in vitro cultured Hep-2 cells. The endocytosis and exocytosis of transmembrane when gold nanochain attached to Hep-2 cells were observed by electron microscopy.RESULTS AND CONCLUSION: The gold chain at high concentrations (75%, 95%) exhibited inhibitory effects on the proliferation of Hep-2 cells, but the influence was not increased with increasing concentration, belonging to a range of non-toxic. Gold nanchain can enter into Hep-2 cells after 8 hours of co-culture and leave cells after 48 hours, indicating gold nanoparticles chain can enter and leave Hep-2 freely.
3.Mutation of P927S in PRD1 gene mediates azole resistance in Candida glabrata

Objective To investigate the role of PDR1 gene in azole-resistant Candida glabrata (C.glabrata).Methods Thirty-eight clinical isolates of C.glabrata were collected from five different hospitals.The minimal inhibitory concentrations (MIC) of azole antifungals including fluconazole,itraconazole and voriconazole against C.glabrata were determined by broth microdilution.Sequencing and amplification of PDR1 gene was achieved by real-time quantitative polymerase chain reaction (PCR).The mutation was cloned into an expression plasmid and then transferred into C.glabrata.The efflux of rhodamine 6G and drug sensitivity test were performed,and expressions of CDR1 and CDR2 were examined to verify function of mutation.Results Among these 38 isolates of C.glabrata,17 were resistant to at least one of azole antifungals.Moreover,mutations of PDR1 gene existed in every resistant isolates.Results of phenotyping test showed that in the isolate that expressed PDR1P927S,the expression of CDR1 and CDR2 were increased by 20.53 and 4.03 fold,respectively.And the fluorescence intensity of rhodamine 6G was decreased to 0.62 in efflux experiment.Conclusion P927S mutation of PDR1 gene could induce azole resistance of C.glabrata by increasing the expressions of CDR1 and CDR2,which results in drug resistance due to enhanced effect of efflux pump.
4.Genotyping of candida glabrata clinical isolates by repetitive sequence-based PCR and multilocus sequence typing

Objective To assess the application value of REP-PCR in genotyping of candida glabrata strains in clinical pratice. MethodsFrom 2009 to 2010, thirty-eight candida glabrata strains were isolated from Shanghai Ruijin Hospitals, Shanghai Renji Hospital, Shanghai Huashan Hospital, Anhui Medical University Hospital, Shenzhen People's Hospital. Six loci in housekeeping genes (FKS, LEU2,NMT1, TRP1, UGP1 and URA3 ) were amplified and sequenced. The sequences were compared with the MIST database and allele profile and sequence type (ST) were obtained. With primers Ca21, Ca22 and Com21 used to amplify the adjacent variable gene regions,the amplicons were analyzed through electrophoresis to generate different REP-PCR types. Finally, the results of these two genotyping methods were compared. ResultsFor REP-PCR, Ca22-Com21 has the best genotyping effect. REP-PCR and MLST have the same genotyping results. Five REP-PCR types were found in 38 candida glabrsta isolates. Type A,B, C, D and E strains from REP-PCR were genotyped as ST 7, 3, 19, 45 and new type respectively byMIST. REP-PCR saves time compared with MIST. Conclusions REP-PCR offers a simple and rapid method for molecular typing, with similar discriminatory power with MIST. Therefore, REP-PCR can be the preferred choice in laboratory, especially for a large number of isolates.
5.Randomly Amplified Polymorphic DNA in Epidemiological Study of Pseudomonas aeruginosa

OBJECTIVE To establish a fingerprinting method by randomly amplified polymorphic DNA.Epidemiological study was carried out on Pseudomonas aeruginosa in Ruijin Hospital. METHODS To obtain optimum scheme on reaction system for randomly amplified polymorphic DNA(RAPD) of P.aeruginosa. RESULTS P.aeruginosa strains isolated from the same ward shared the same RAPD fingerprint type,except for pulmonary ward.Different ward was with different fingerprint type. CONCLUSIONS Prevalent strain was not found in the whole hospital,but within ward exists hospital-acquired infection phenomenon.
6.Effects of intravenous immunoglobulin on immunity and cytokine levels in children with EV71 infection associated high-risk pulmonary hemorrhage

Objective To investigate the influence of IVIG on immunologic function and cytokines levels in children with EV71 infection associated high-risk pulmonary hemorrhage. Methods According to the inclu-sion criteria , 64 children were enrolled and randomly divided into two groups: 39 cases in the IVIG treatment group and 25 cases in the general treatment group. The alternations of blood and immune cytokine markers before and after treatment were detected in the patients. Results (1) Before treatment, the peripheral blood T cells, TH and B cells in the IVIG group were higher than those in the general group , but the peripheral blood IgA was lower than that in the general group(P < 0.05); (2) In the IVIG group, the NK cells and IgG increased and T cells, TH cell and B cells significantly decreased after receiving IVIG therapy (P < 0.05); (3)No significant difference was observed in the levels of IL-6 and IL-10 in the two treatment groups before treatment. However , after IVIG treatment, IL-6 and IFN-γ levels reduced, IL-10 level increased (P < 0.05). After the general treat-ment, no significant difference was found in levels of IL-6,IL-10 and IFN-γ in peripheral blood (P > 0.05). Conclusion Disorders of cellular immunity and humoral immunity appeared in children with EV71 infection-re-lated high-risk pulmonary hemorrhage. It has clinical value to use IVIG timely to regulate the immune disorder.

According to the Implementing Regulations of Standard Examination for Schistosomiasis Control in Jiangxi Province and the Criteria for Control and Elimination of Schistosomiasis, the endemic situation of schistosomiasis in Nanchang City was assessed by listening reports, checking up data, field survey, ect. Results showed that the indexes of endemic situation for both human and cattle reached the criteria for schistosomiasis control.

Objective:The 11?-hydroxysteroid dehydrogenase(11?-HSD) plays an important role in tumor biological behavior,and the 11?-HSD inhibitor Glycyrrhetinic acid(GA) may have an anticancer effect,although its mechanism remains unkown.This study aimed to observe the expressions of 11?-HSD 1 and 2 in colorectal cancer and the effects of glucocorticoid and GA on colorectal cancer cells.Methods: The mRNA expressions of 11?-HSD1 and 2 in the human colorectal cancer cell line HCT-8 were detected by reverse transcription polymerase chainreaction(RT-PCR),and the protein expressions of 11?-HSD1 and 2 were detected by Western blot.The inhibition of the growth of the HCT-8 cells was determined by MTT assay after treated with glucocorticoid,cosubstrate,GA only or their combination.Results: The expression of 11?-HSD2 mRNA and proteins but not that of 11?-HSD1 mRNA and proteins was expressed in the HCT-8 cells.The rate of cell growth inhibition was(40.87 ? 1.47)% after a 12-hour treatment with cortisol,and it was(5.79 ? 0.20)% in the cortisol + NAD group,with statistically significant difference(P

ObjectiveTo analyze the effect of lowering the upper limit of normal fasting glucose on the distribution of impaired gheose regulation(IGR)in the population in Wuhan urban area.MethodsThe health profiles of 1896 people after physical examinations were analyzed by X2 test.ResultsThe low limit for impaired fasting glucose(IFG)was decreased from 6.1 to 5.6 mmol/L,IFG increased 271 patients.the prevalence of IFG increased from 13.1%to 27.4%(X2=62.68,P<0.05),and the value of normal glucose test decreased from 60.3%to 47.5%(X2=120.03,P<0.05).Overweisht or obesity,hypetention,fatty liver,hyperlipidemia,ECG abnormality were significantly different(P<0.05)in groups of fasting glucose 5.6-6.0 mmol/L to<5.6 mmol/L and to 6.1-6.9 mmol/L ConclusionThe new cut point of IFG has significantly influenced the distribution of IGR in the population of Wuhan urban areas.In the newly IFG.increased subjects,metabolic abnormalities of glucose and lipid occur,and an intervention of life-style is needed for them.
10. Bibliometric analysis of the literature related to rapid rehabilitation nursing of Orthopedics in PubMed and Wanfang

Objective:
To understand the status and progress of rapid rehabilitation nursing on orthopedics, to discuss the future direction, in order to grasp the development trend.
Methods:
PubMed and Wanfang database were searched for the statistical materials. According to the quantity, age, country, subordinates of orthopedics and nursing strategies, all the papers were analyzed.
Results:
Until February 2017, PubMed and Wanfang collected a total of 60 papers on the rapid rehabilitation nursing of Orthopedics, the number of papers in Chinese occupied the main position, 35 articles accounting for 58.3%.
Conclusions
Researchers in this field are focusing on health education, psychological comfort, rehabilitation training and pain control. The mainly research methods include observation, improvement and summary ofclinical nursing, so the conclusions of descriptiveness are more than quantitative conclusions which are from rigorous randomized controlled trials (RCTs).
