Objective To explore the correlation analysis of plasma TNF-?,D-lactate levels and serious case score and clinical significance.Methods Plasma TNF-?,D-lactate levels and results from serious case'score was contrastingly studied in 120 cases of serious sick children during hospitalization in our PICU.Results The study findings showed that the plasma TNF-? and D-lactate levels had produced an obviously influence on score.Conclusions The more high plasma TNF-? and D-lactate levels,the lower score and the more dangerous condition in children with serious disease.

3.Study of Isolating Anti-Endotoxin Monomer Component from Radix Paeoniae Rubra Biosensor by Biosensor Technique

OBJECTIVE:To isolate anti-endotoxin monomer component from Radix Paeoniae Rubra by means of biosen?sor technique.METHODS:The surface of biosensor cuvette was embedded by Lipid A;the screening target was established,tracking the silica gel column chromatogram and the binding ability of effluent component from HPLC with Lipid A with the ultraviolet scan result of the reclaimed material from biosensor as reference;anti-endotoxin monomer component was isolated;the component of monomer and the synthetic action of extrinsic lipopolysaccharides were also assayed by LAL test method.RESULTS:Components binding to Lipid A was reclaimed from cuvetee by biosensor technique,with the wavelength of UV absorption peak at194nm,215nm and275nm respectively.Anti-endotoxin monomers of higher binding activity with Lipid A isolated by HPLC method were1,2,3,4,6—O—pentagalloyl—?—D—glucose(PGG).PGG at concentration of8,4,2?g/ml respectively neutralized68.8%,43.7%and31.4%of LPS at an activity of0.1EU/ml respectively.CONCLUSION:It is fea?sible to isolate anti-endotoxin monomer component from Radix Paeoniae Rubra by means of biosensor technique,which is a fast,accurate and efficient and can be used to isolate anti-endotoxin monomer component from Radix Paeoniae Rubra on a large scale.
4.Correlation analysis of thromboelastogram and routine coagulation test in the detection of cerebrovascular diseases

【Objective】 To explore the changes of parameters of thromboelastogram (TEG) detection in patients with cerebral infarction and cerebral hemorrhage, and their correlation with routine coagulation indicators, and compare the two methods in detecting coagulation function and status in patients with cerebrovascular disease. 【Methods】 The detection data of 34 cases of cerebral infarction and 102 cases of cerebral hemorrhage, who underwent TEG and routine coagulation test in parallel, in Department of Neurosurgery of our hospital from January 1, 2018 to March 26, 2019 were collected. Fifty healthy subjects, who underwent TEG and routine coagulation tests in the same period, served as the control. The results of TEG and routine coagulation test of the 3 groups were compared and analyzed. 【Results】 Compared with cerebral hemorrhage (group) and the control, R value of cerebral infarction group decreased significantly, while the Angle angle, MA distance, CI value, Fib and D-D levels increased significantly(P<0.05); the K value, Fib, TT, and D-D of cerebral hemorrhage group increased significantly while Angle angle and MA distance decreased significantly compared with other two groups(P<0.05). The R value, K time (min), TT (s), D-D (mg/L), Angle angle (°), MA distance, CI value, and Fib (mmg/L) of cerebral infarction and cerebral hemorrhage group were 5.06±1.71 vs 6.95±4.34, 1.19±0.68 vs 3.43±1.46, 13.89±1.05 vs 18.63±1.25, 1.93±0.25 vs 2.63±0.45, 69.34±10.37 vs 56.54±10.48, 68.34±9.37 vs 54.35±7.48, 2.20±0.31 vs 0.78±0.41, and 380±70.02 vs 354±71.56 (P<0.05). In cerebral infarction group, the R value, APTT, Angle angle, MA, and CI values were positively correlated with Fib (P<0.05); while K time was negatively correlated with Fib ; MA distance with PT/TT; CI value with PT/APTT(P<0.05). In the cerebral hemorrhage group, the R value was positively correlated with PT/APTT; K time with TT; Angle with Fib (P<0.05); R value/K time were negatively correlated with Fib; Angle with TT; CI value with PT (P<0.05). 【Conclusion】 For patients with cerebrovascular disease, there is no correlation between TEG indicators and the D-D level of routine coagulation tests; the Angle angle of patients with cerebrovascular disease is positively correlated with the elevated Fib; and MA distance is more useful for diagnosing cerebral infarction than cerebral hemorrhage. TEG is complementary to routine coagulation test, and can be used as a laboratory index for assessing the severity of cerebrovascular disease.
5.Chemical constituents of Psidium guajava and their antitumor and antifungal activities.

Twenty-six compounds, including sixteen meroterpenoids(1-16), a triterpenoid(17), four terpenoid derivatives(18-21), and five aromatic compounds(22-26), were isolated from the leaves of Psidium guajava. Their structures were identified by spectroscopic analyses including NMR and MS. Compounds 21-26 were obtained from plants of Psidium for the first time. Based on the structure,(R)-2-ethylhexyl 2H-1,2,3-triazole-4-carboxylate(24 a), an α-glucosidase inhibitor recently isolated from Paramignya trimera, should be revised as compound 24. Meroterpenoids 1-16 were evaluated for their antitumor and antifungal activities. Meroterpenoids psiguajadial D(4), guapsidial A(5), 4,5-diepipsidial A(7), guadial A(14), and guadial B(15) showed cytotoxicities against five human tumor cell lines(HL-60, A-549, SMMC-7721, MCF-7, and SW-480), among which 5 was the most effective with an IC_(50) of 3.21-9.94 μmol·L~(-1).

6.Clinical analysis of 163 patients with remnant gastric cancer.

OBJECTIVETo discuss the clinical characteristics of remnant gastric cancer (RGC).
METHODSClinical data of 163 patients with RGC admitted to our department from January 2005 to January 2015 were collected and reviewed.
RESULTSThe time duration of all the RGC patients between primary gastrectomy and occurrence of RGC ranged from 9 to 46 years (median 27.5 years). Among 163 patients, 112(68.7%) patients manifested as discomfort in the upper abdominal region, 47(28.8%) patients had hematemesis and melena, 38(23.3%) had vomiting, 17(10.4%) had swallowing difficulty and 12 (7.4%) had decreased weights. In pathology, 127 (77.9%) cases were median or low grade adenocarcinoma; 114(69.8%) patients were Borrmann type III( gastric cancer under endoscope; 145 (88.9%) patients were TNM III(-IIII( gastric cancer. A total of 106 patients underwent operation, including 75(70.8%) patients of radical resection and 31(29.3%) patients of palliative resection. Patients mainly received Billroth II( anastomosis (77.4%) as digestive tract reconstruction, while 57 cases did not receive surgical operation. The overall 1-, 3-, and 5-year survival rates of the patients were 76.1%, 43.4%, 25.8% respectively. The median survival time of patients receiving radical resection and those receiving palliative resection was 34.6 months and 16.4 months respectively, whereas patients who did not receive surgical operation died 3-8 months after initial diagnosis.
CONCLUSIONSRegular gastroscopy is necessary for patients undergoing gastrectomy for over 10 years. Radical resection is of great importance in the treatment of remnant gastric cancer.

7.Research progress of Lgr4 in gastrointestinal carcinomas.

Gastrointestinal carcinomas, whose incidence increases in recent years, are among the malignancies with highest morbidity and mortality. Many patients are always advanced stages when having diagnosis. So far, there are no any reliable tumor biomarkers which are able to screen people with high risk of developing cancer. Studies have shown that the expression level of Lgr4 is high in gastrointestinal carcinomas and Lgr4 is useful in diagnosis, differential diagnosis and prognosis judgment of these carcinomas. Lgr4 belongs to the G-protein-coupled receptors family. Its function is to potentiate Wnt signaling and plays an important role in the development of multiple organs. Recent studies find that Lgr4 has close relationship with the development of many gastrointestinal carcinomas, such as gastric cancer and colorectal cancer(CRC). In this article we review recent progress in understanding the relationship between Lgr4 and gastrointestinal carcinomas.
8.Differences of tacrolimus' dosage and concentration of individuals in morning periods after renal transplantation

Objective To study the influential factors of tacrolimus'dosage and concentration differences between individuals in morning periods after renal transplantation.Methods The clinical data consisted of 118 receptors in morning periods after renal transplantation,whose immune suppressions were tacrolimus,mycophenolate and hormone.At 3,7,14 and 30 d after operation,all the receptors'weight,dosage of tacrolimus,dosage of hormone,diarrhea,blood fat,liver function,renal function,albumn and erythrocrit were recorded respectively,and at the same time their concentrations of tacrolimus and genetic polymorphisms of CYP3A5,MDRl 3435,MDR1 2677 and MDRl 1236 weredetected.Multiple linear regressions were performed.Results The fitting degrees of stepwise regression equations were low.At 3,7,14 and 30 d after operation,the adjusted R2was 0.284,0.267,0.417 and 0.324,respectively.From the aspect of pharmacogenomics,the main factors rela-ted to the differences of tacrolimus'dosage and concentration included MDR1 2677,MDRl 1236 and MDR13435,which varied intensively.Age,albumn,renal function,blood fat and liver function were important factors too.Conclusions The main reasons of the differences of tacrolimus'dosage and concentration between individuals in morning periods after renal transplantation are medicines and changes of internal environment after operation.The genetic polymorphisms of MDR1,age,albumn,renal runetion.blood fat and liver function are important factors too.
9.Effect and mechanism of mastoparan-1 antagonizing lipopolysaccharide in vitro

Objective To investigate the mechanism of mastoparan-1 (MP-1) antagonizing lipopolysaecharide (LPS) in vitro. Methods The affinity of MP-1 for lipid A was assayed by biosensor, and the neutralization of MP-1 on LPS (2 μg/L) was detected by kinetic turbidimetric limulus test. After exposing fluorescin isothiecyanate (FITC) labeled LPS (FITC-LPS) to MP-1 at different concentrations (5, 10, 20, 40 μmol/L), the binding of FITC-LPS to murine RAW264.7 cells was analyzed by laser scanning confocal microscopy. The influence of MP-1 on TLR4 expression in RAW264.7 cells stimulated by LPS (100 μg/L) was detected by immunoeytochemieal staining. The expressions of TLR4, TNF-α and IL-6 at the gene and protein level were detected by RT-PCR and ELISA after exposing LPS (100 μg/ L) stimulated RAW264.7 cells to MP-1 at different concentrations. The effect of MP-1 on the viability of RAW264.7 cells was detected by MTT assay. Results MP-1 had high affinity to lipid A and could neutralize LPS. MP-1 at 10 μmol/L significantly inhibited not only binding of FITC-LPS to RAW264.7 (P < 0.05), but also protein and gene expressions of TLR4, TNF-α and IL-6 in LPS stimulated RAW264.7 cells in a dose-dependent manner (P < 0.05). No toxic effect of MP-1 on the viability of RAW264.7 cells was found (P > 0.05). Conclusions MP-1 inhibits cell viability mediated by LPS, which may be related to its neutralization of LPS and inhibition of binding of LPS to RAW264.7 cell membrane receptors.

Expressions of interleukin-6 (IL-6) and its receptor (gp80、gp130) in 8 patients with Graves' disease (GD) were compared with those in 8 euthyroid patients with nodular goiter or benign thyroid adenoma. The thyroid tissues of GD expressed significantly higher IL-6 mRNA, gp130 mRNA and IL-6 protein than those of the control group, suggesting that activated IL-6/gp130 signal pathway in the thyroid tissue may contribute to the pathogenesis of GD.
