1.Deep venous thrombosis after spinal surgery: prevention and treatment

BACKGROUND: The incidence of deep venous thrombosis of the lower extremity after spinal surgery remains controversial, and its prevention is still under discussion.OBJECTIVE: To summarize the progress of the prevention of the deep venous thrombosis after spinal surgery.METHODS: The domestic and foreign related literature about the deep vein thrombosis after the spinal surgery, which was filtrated from CNKI and PubMed databases, were searched for the articles concerning the prevention of the deep venous thrombosis after spinal surgery published from January 1993 to January 2016.Finally, 41 eligible articles were included, and the prevention methods, risk factors and incidence were summarized.RESULTS AND CONCLUSION: (1) With the technology and medical equipment development, more novel technologies and biomaterials are extensively applied in spinal surgery; meanwhile, complex and difficult surgeries,big surgical trauma and prolonged operation time all increase the risk of deep venous thrombosis of the lower extremity;thereafter, the incidence is on a rise. (2) The prevention methods of deep venous thrombosis have been improved, and the grading of prevention becomes more sufficient. (3) During venipuncture and injection, upper extremity should be selected firstly, so as to avoid damage to the lower extremity veins. (4) The following studies should concentrate on understanding the epidemic characteristics of deep venous thrombosis of the lower extremity after spinal surgery, and confirming the best prevention time, as well as effective drugs and physical measurements.
2.Hydrops of subcutaneous scalp after repairing skull defect using silicon-polyester fiber network

To analyze the cause and therapy of hydrops in subcutaneous scalp after repairing skull defect by using silicon-polyester fiber network, a total of 91 patients who were treated with silicon-polyester fiber network to repair skull defect from June 1996 to October 2000 in the Department of Neurosurgery, Nanshan People's Hospital were analyzed retrospectively. And 56 cases of them had hydrops of subcutaneous scalp and treated with suction and pressure dressing, including 11 cases of less than 20 mL hydrop quantity, 27 cases of 20-50 mL, and 18 cases of more than 50 mL. They were observed for 6 weeks averagely after operation and were healed within 4 weeks by simple management. No recurrence of hydrops was found in subcutaneous scalp. It is indicated that silicon-polyester fiber network in repair of skull defect may complicate with hydrops of subcutaneous scalp usually. However, the prognosis will be good by means of drainage, so it can be used as a therapy for skull defect.
3.Comparative study of the effects of Gypsum Fibrosum and Gypsum Fibrosum Preparatum in promoting granulation

OBJECTIVE: To investigate the effects of Gypsum Fibrosum and Gypsum Fibrosum Preparatum in promoting granulation. METHODS: The wounds of muscle layer were produced in rats by using surgical operation. Two round wounds, with diameter about 1.5 cm, were cut at the depilatory area of two sides of the back of each rat, with an interval of 2 cm, deep to muscle layer, and the thickness of the knife wound of muscle layer was about 0.15 cm. Forty SD rats with the wounds were randomly divided into 4 groups: untreated group, beifuji-treated group, Gypsum Fibrosum-treated group and Gypsum Fibrosum Preparatum-treated group, with 10 rats in each group. Then the wounds were sprinkled with powders of Gypsum Fibrosum and Gypsum Fibrosum Preparatum, or sprayed with beifuji solution, respectively. The healing state of granulation tissues of the wounds was observed at the eighth and fourteenth day respectively. RESULTS: The number of fibroblasts, the number of capillary tubes and the area of capillary tubes in granulation tissue of wounds in the Gypsum Fibrosum Preparatum-treated group were significantly higher than those in the untreated group and Gypsum Fibrosum-treated group (P<0.01). There were no statistical differences between the Gypsum Fibrosum Preparatum-treated group and the beifuji-treated group. However, Gypsum Fibrosum-treated group showed no obvious differences compared to the untreated group (P>0.05). CONCLUSIONS: Gypsum Fibrosum Preparatum can accelerate the formation of collagenoblast and micrangium in wounds, and the proliferation of granulation tissues, thus promoting the skin wounds to healing. The effect of Gypsum Fibrosum is changed after being calcined, and Gypsum Fibrosum Preparatum has obvious effect in promoting granulation.
4.Clinical observation on three-dimensional conformal radiotherapy combined with concurrent chemotherapy in the treatment of multiple primary carcinoma of esophagus

Objective To analysis of the efficacy and toxicity of multiple primary cancer of esophagus in three-dimensional conformal radiotherapy and concomitant chemotherapy.Methods Fifteen cases of multiple primary carcinoma of esophagus who refused the operation were collected.The Elekta linac 6 MV/10 MV X ray was used to conformal irradiation in three-four coplanar wild,2 Gy/time,1 time/day,5 times/week.The dose to the target area,CTV 40-50 Gy/4-5 weeks,GTV 60 Gy/6 weeks.TP scheme (paclitaxel+cisplatin) was used in the first day and 29th day,three-dimensional conformal radiation therapy initiated in the second day after chemotherapy.Results 1,3 year local control and overall survival were 86.7 % (13/15),33.4 % (5/15) and 73.3 % (11/15),26.6 % (4/15).Class 1-2 and class 3-4 acute radiation esophagitis and leukopenia occurred rates were 86.7 % (13/15),13.3 % (2/15) and 80 % (12/15),20 % (3/15).Conclusion Radiation therapy is the better treatment method for multiple primary carcinoma of esophagus when proximal lesions located in upper thoracic and cervical high.Toxic and side-effects can be tolerated of three dimensional conformal radiation therapy combined with chemotherapy in the treatment of multiple esophageal cancer.
5.Pattern analysis of tempo-spatial distribution of schistosomiasis in marsh-land epidemic areas in stage of transmission control

Objective To investigate the tempo?spatial patterns of schistosomiasis in Jiangling County,Hubei Province,so as to identify the risk areas and provide the scientific evidence in following intervention plans for marshland epidemic areas in the stage of transmission control. Methods The schistosomiasis epidemiological data in Jiangling County from 2009 to 2013 to?gether with the related geographical information were collected and analyzed. The tempo?spatial distribution patterns were ana?lyzed by the spatial autocorrelation analysis and spatial clustering analysis. Results The human infection rate was decreased from 2.15%in 2009 to 0.63%in 2013,which was the historically low level. The results of tempo?spatial analysis showed that there were spatial clustering effects in human schistosomiasis infection for each of the years. The values of spatial autocorrelation index Moran’s I were statistically significant. Eighteen and thirty?five clusters were detected by using SatScan and FlexScan soft?ware,respectively. Conclusions From 2009 to 2013,the schistosomiasis endemic situation in Jiangling County presented a decline trend and reached the historical low level. The identified spatial clustering areas should be targeted as the prioritized ar?eas for schistosomiasis control.
6.Dynamic analysis of distribution of Oncomelania hupensis in mountainous regions

Objective To study the distribution of Oncomelania hupensis snails in mountainous regions and the dynamic charaeteristics of the distribution.Methods An environment calledLanbaoclosed to Puge County.Sichuan Provinee WaS selected as the study field.Random sampling was designed to determine the investigation sites.The snails were collected and the hying snaila were identified by the method of dissection in the laboratory.The distribution of snails was analyzed by some statistical indices,such as mean,variance and so on.Then the negative binomial distribution.log-normal distribution and exponential distribution were fitted to the snail data by the method of maximum likelihood estimation to explore the snail distribution in different time.Results The negative binomial distribution was fitted well to the snail data in April,May,July,August,September,November in 2008,and no distribution was fined to the snail data in June,October.December in 2008 and February in 2009.Conclusions The distribution of Oncomelania hupensis in mountainous regions is not simple negative binomial distribution,but pwbably a dynamic process and an uncertain distribution.
7.Prognostic analysis of 169 patients with unresectable esophageal squamous cell carcinoma treated by three dimensional conformal radiation therapy

Objective To study long-term outcome and prog nostic factors of esophageal squamous cell carcinoma patients treated by three dimensional conformal radiation therapy (3DCRT). Methods 169 patients with unresectable esophageal squamous cell carcinoma treated by 3DCRT were enrolled in the study. The survival rates of 1 year, 3 years and 5 years were estimated by life-table method. Univariate prognostic factor was tested by Log-rank method. Multivariate prognostic factor was analyzed by Cox model. Results The 1 year, 3 years and 5 years survival rates were 63.2%, 34.1%and 21.3%, respectively. Univariate analyses showed that the length of tumor, the site of lesion, chemotherapy, the dose of plan gross tumor volume (PGTV) and the short-term outcomes after treatment were the important prognostic factors for the long-term survival (P< 0.05), and multivariate analyses showed that the length of tumor, chemotherapy and the short-term outcomes after treatment were the independent prognostic factors for the long-term survival (P< 0.05). Conclusions The patients with unresectable esophageal squamous cell carcinoma treated by 3DCRT have a good long-term prognosis. The length of tumor, chemotherapy and the short-term outcomes after treatment are the important prognostic factors for the long-term survival of the patients. Chemotherapy can improve the long-term prognosis significantly.
8.Influence of the Three Gorges Dam on schistosomiasis control in the middle and lower reaches of the Yangtze River

This paper summarizes the factors affecting schistosomiasis transmission in the middle and lower reaches of the Yangtze River before and after the completion of the Three Gorges Dam and analyzes schistosomiasis prevalence trends to provide a basis for the application of the Three Gorges project methodologies in other areas.The Three Gorges Dam has demonstrated a positive effect on schistosomiasis control.Hubei,Hunan,Jiangxi,Anhui,and Jiangsu in the middle and lower reaches of the Yangtze River are currently regions with a high prevalence of schistosomiasis.These five provinces contained 97.62% of known snail areas and 93.66% of calculated schistosomiasis cases with 90.07% of counties (cities,districts) not yet meeting the criteria for schistosomiasis transmission interruption by the end of 2016.After the Three Gorges Dam was built,the prevalence of schistosomiasis in the middle and lower reaches of the Yangtze River decreased.By the end of 2016,the estimated number of cases decreased by 92.75% and the snail habitat areas decreased by 6.56% compared to 2004.No acute schistosomiasis cases have occurred for two consecutive years since 2015.
9.Application of a digital three-dimensional reconstruction technique in the diagnosis and treatment of hepatic alveolar echinococcosis

ObjectiveTo investigate the value of a digital three-dimensional reconstruction technique in the treatment of hepatic alveolar echinococcosis (HAE).MethodsThe computed tomography scan data for 13 patients with HAE who were admitted to the First Affiliated Hospital of Xinjiang Medical University from February 2011 to October 2011 were reconstructed and analyzed by a three-dimensional reconstruction system to assess resectability,and to facilitate surgical planning and individualized virtual surgery.The results of preoperative analysis were compared with the results of actual operations.ResultsThe three-dimensional models of the liver were reconstructed successfully,and intrahepatie lesions and vessels were clearly displayed.One patient received an autologous liver transplantation,10 underwent hepatectomy,and 2 received percutaneous transhepatic cholangial drainage.Virtual operation planning was carried out for 11 patients using the three-dimensional reconstruction system.The mean volume of the liver to be resected was predicted to be 920 ml (range,339-2678 ml),and the mean percentage of liver to be resected to the total liver volume was predicted to be 45% ( range,23% -68% ).The mean volume o[ the actual liver resection was 834 ml (range,315-2250 m[),and the mean percentage of actual liver resected to the total liver volume was 42% (range,22% -70% ),which was consistent with the results of preoperative three-dimensional reconstruction.All patients were followed up for 2-8 months,and no severe complications such as liver failure,hemorrhage and bile leakage were detected.ConclusionDigital three-dimen-sional reconstruction is helpful in the diagnosis and treatment of HAE and effectively reduces surgical risks.
10.Distribution features of wild feces in schistosomiasis endemic areas in Jian-gling County,Hubei Province

Objective To understand the spatial distribution characteristics of wild feces in schistosomiasis endemic areas of Jiangling County,Hubei Province and further explore the source of infection efficiently,so as to provide the evidence for the development of corresponding monitoring and response technology. Methods In 2011,the fresh wild feces were investigated every two months in the selected 15 villages by the severity of historical endemic in Jiangling County. The schistosome miracidi-um hatching method was used to test the schistosome infection of the wild feces. The descriptive analysis and spatial analysis were used for the description of the spatial distribution of the wild feces. Results Totally 701 wild feces samples were collected with the average density of 0.0556/100 m2,and the positive rate of the wild feces was 11.70%(82/701). The results of the re-gression analysis showed a positive spatial correlation between the positive rate of wild feces and the rate of human infection,the area with infected Oncomelania hupensis and the number of fenced cattle,and the corrected R2 of the model was 0.58. Conclu-sion The infection rate of wild feces is positively correlated with the rate of human infection,area with infected O. hupensis and number of fenced cattle in space in Jiangling County,so the prevention and control measures could be conducted according to the spatial distribution of the positive wild feces.
