1.Effect of preconditioning with pioglitazone on ischemia reperfusion/hypoxia reoxygenation-induced mitochondrial structure and membrane potential in rats

AIM:To observe the effect of preconditioning with pioglitazone on ischemia reperfusion/hypoxia reoxygenation-induced mitochondrial ultramicro-structure and membrane potential in rats. METHODS: Sprague-Dawley rats were randomly divided into four groups: sham-operated (SO) group, ischemia reperfusion (IR) group, pioglitazone preconditioning group (Pio-P) and 5-HD+pioglitazone (5-HD+Pio) group. Apart from the SO group, IR, Pio-P and 5-HD+Pio groups were subjected to 30 min ischemia and 4 h reperfusion. The heart was quickly removed for observing the structure of mitochondria and measurement of the apoptosis index (AI) by TUNEL. Primary cultured cardiomyocytes of Sprague-Dawley rats were divided into control, hypoxic reoxygenation (HR) and different concentrations of Pio-P group. JC-1 staining flowcytometry was adopted to examine mitochondrial membrane potential (?m). RESULTS: The injury of mitochondrial structure in IR group was severer than that in Pio-P group, while the difference between 5-HD+Pio group and IR group was not evident. Flameng score in Pio-P group(1.62?0.60) was significantly lower than that in IR group (2.75?1.09), P0.05). CONCLUSION: Pioglitazone protects the heart from ischemia reperfusion/ hypoxia reoxygenation injury evidenced by improving mitochondrial ultrastructure and lessening the loss of mitochondrial membrane potential, and decreasing apoptosis. The cardioprotective effects can be inhibited by the blocker of mitochondrial ATP-sensitive potassium channels.

AIM: To evaluate whether tolerogenic dendritic cells (DC) loaded with heat shock protein 60 (HSP60) inhibit the progression of aortic atherosclerotic plaque in hypercholesterolemic apolipoprotein E (Apo-E) -null mice. METHODS: Bone marrow derived DC of the mice were loaded with HSP60 and co-cultured with rapamycin to generate tolerogenic DC. The tolerogenic DC, DC loaded only HSP60 and PBS were injected into the ApoE-null mice at 8 weeks of age for three times at a one-week interval. 8 weeks after the last injection, aorta were harvested for HE staining and anti-CD4~+T cell immunostaining. Responses of pleenic cells to HSP60 were also evaluated. RESULTS: Compared with DC, DC_ HSP60 expressed higher levels of CD86, and stimulated T lymphocytes to proliferation significantly, while the tolerogenic DC expressed lower levels of CD86, and inhibited T lymphocytes to proliferation. Objective:To discuss effect of nicorandil on unstable angina patients With persistent Weakly positive for troponin I (TnI).Methods:A total of 111 unstable angina patients With persistent Weakly positive for TnI Were randomly divided into control group (received routine treatment,55 cases) and intervention group (received nic-orandil 5mg,3 times/d based on routine treatment,56 cases).The relief of chest pain in one Week,the recurrent hospitalization for chest pain aggravation in 3 months and the cardiac mortality rate in one year betWeen tWo groups Were observed in tWo groups. Results:Compared With control group,the relief of angina pectoris in one Week (63.6% vs. 91.1%,χ2=11.97,P=0.0005)significantly increased,re-hospitalization for chest pain aggravation in three months (56.4% vs.19.6%,χ2=15.91,P=0.0001)significantly decreased in intervention group;but cardiac mortality rate during one year betWeen tWo groups Was no significant difference (5.5% vs. 8.9%,χ2=0.50,P=0.4792).Conclusion:Nicorandil can significantly reduce the unstable angina and re-hospitalization for chest pain aggravation in patients With persistent Weakly positive for TnI,but there Was no significant difference in reducing mortality Within one year betWeen tWo groups.
4.Research progress in late-onset group B Streptococcus infection in infants and its prevention

Group B Streptococcus(GBS)is a major member of the Gram-positive Streptococcus family,which occupies a dominant position in the infection spectrum of newborns and young infants.GBS infections occurring 0?6 days after birth are called GBS early-onset disease(GBS-EOD),and those occurring 7?89 days are called GBS late-onset disease(GBS-LOD).With the use of intrapartum antibiotic prophylaxis,the incidence of neonatal GBS-EOD has declined without a notable impact on GBS-LOD,which brings great pressure and challenge to clinical diagnosis and nursing work.However,the specific mechanisms by which GBS transmission triggers illness in newborns and young infants are not fully elucidated.Some factors,such as maternal GBS colonization,maternal characteristics(age,weight and ethnicity),preterm birth,GBS infection in multiple births,and exposure to human immunodeficiency virus,have been identified as risk factors for GBS-LOD.To prevent maternal and infant GBS infection,some measures,such as vaccination of pregnant women,gut microbiota regulation and lactoferrin supplementation,are developing.In addition,strengthening maternal health care and health education,along with some other conventional infection control strategies(including enhancing hand hygiene awareness among caregivers and making good disinfection of the environment and equipment)is also effective in the prevention of GBS-LOD.This review elaborates the prevalence,transmission,risk factors and prevention of GBS-LOD in infants,aiming to improve the understanding and clinical practice ability of the medical staff.
5.Antiapoptotic Mechanism of Insulin in Reoxygenation-induced Injury in Cultured Cardiomyocytes of Neonatal Rats

To examine the protective effect of insulin on reoxygenation-induced injury and explore the underlying mechanisms, the model of anoxia/reoxygenation (A/R) injury was established by inducing anoxia for 2 h and reoxygenation for 4 h in cultured cardiomyocytes of neonatal rats. The rats were randomized to four groups receiving vehicle, insulin, LY294002, insulin plus LY294002at the onset of reoxygenation after 2 h of anoxia. At the end of reoxygenation of 4 h, activity of lactate dehydrogenase (LDH) and content of malondialdehyde (MDA) were spectrophotometrically determined, apoptosis of cardiomyocytes were detected by using TUNEL and DNA Ladder, and Western blotting was employed to examine the expression of phosphorylated Akt in all groups. Our results showed that compared with vehicle-treated group, activities of LDH, contents of MDA, apoptosis index (AI) were significantly decreased, and expression of phosphorylated Akt was in creased significantly in insulin-treated group. However, changes in LDH, MDA, AI and phospho rylated Akt resulting from insulin were attenuated or abolished by LY294002 (PI3K inhibitor).These data strongly suggest that early administration of insulin at reoxygenation protects cardiomyocytes from reoxygenation-induced apoptosis through PI3K/Akt signaling pathway.
6.Study on Anti-inflammatory Effect and Mechanism of Jingulian Capsule on Inflammatory Model Rats

OBJECTIVE:To study the anti- inflammatory effect and mechanism of Jingulian capsule on inflammatory model rats. METHODS :Totally 48 rats were randomly divided into blank control group ,model group ,Jingulian capsule low-dose , medium-dose and high-dose groups (0.66,1.32,2.64 g/kg),dexamethasone group (positive control ,0.945 mg/kg),with 8 rats in each group. Blank control group and model group were given constant volume of water intragastrically ,and other groups were given relevant medicine intragastrocally ,twice a day ,for consecutive 3 days. Thirty minutes after last administration ,model group and administration groups were given lipopolysaccharide (10 mg/kg)intraperitoneally to induce inflammatory model. Six hours after intraperitoneal injection ,the contents of TNF-α,IL-1β,IL-6,PGE2 in serum were detected by ELISA. The wet to dry weight (W/D)ratio of lung tissue were determined. HE staining was used to observe the pathological changes of lung tissue . RT-PCR was used to detect the mRNA expression of TNF-α,IL-6,PGE2 and IL- 1β in lung tissue. Western blot assay was used to detect the phosphorylation of NF-κB p65 protein and the expression of IκBα protein in lung tissue. RESULTS:Compared with blank ; control group ,the contents of TNF-α,IL-1β,IL-6 and PGE 2 in serum ,the W/D ratio of lung tissue ,the expression of TNF-α,IL-1β,IL-6 and PGE 2 mRNA and the phosphorylation level of NF-κB p65 protein in lung tissue of model group weresignificantly increased ,and the expression of IκBα proteinwas significantly decreased (P<0.05 or P<0.01);a large number of alveolar atrophy and collapse ,alveolar wall thickening ,lung consolidation ,and a large number of inflammatory cell infiltration could be seen in lung tissue. Compared with model group ,the contents of TNF-α,IL-1β(except for low-dose group ), IL-6 and PGE 2 in serum ,as well as the expression of TNF-α(except for high-dose group ),IL-1β,IL-6(except for low-dose , high-dose groups )and PGE 2 mRNA in lung tissue were decreased significantly in Jingulian capsule groups (P<0.05 or P<0.01); the W/D ratio of lung tissue was decreased significantly in Jingulian high-dose group (P<0.05 or P<0.01);the expression of phosphorylation level of NF-κB p65 protein in lung tissue of Jingulian medium-dose group were decreased significantly (P<0.05 or P<0.01),while the expression of IκBα protein was increased significantly(P<0.05);the alveolar structure was clear ,the alveolar wall was slightly thickened , and a small amount of inflammatory cell infiltration was seen in lung tissue. CONCLUSIONS:Jingulian capsule has good anti-inflammatory effect on inflammatory model rats ,the mechanism of which may be related to the inhibition of NF-κB signal pathway.
7.Difference Analysis of Volatile Components in Roots ,Stems and Leaves of Gaultheria yunnane nsis by GC-MS

OBJECTIVE:To compare the volatile components extracted from the roots ,stems and leaves of Gaultheria yunnanensis. METHODS :Steam distillation method was used to extract the volatile oils from roots ,stems and leaves of G. yunnanensis. Chemical constituents were analyzed by GC-MS. NIST 2011 standard mass spectral library was adopted to select the chromatographic peak with a matching degree higher than 80,and combined with relevant literatures for identification. Relative percentages of chemical constituents were calculated by peak area normalization. RESULTS :Totally 95 chromatographic peaks were detected in valatile oil from the roots of G. yunnanensis and 54 chemical constituents were identified ,accounting for 82.35% of the total content of root volatile components. The constituents with relatively high content were methyl salicylate (20.30%), n-hexadecanoic acid (19.86%),(Z,Z)-9,12-octadecadienoic acid (9.26%),phenanthrene(4.37%)and so on. Totally 69 chromatographic peaks were detected in volatile oil from the stems and leaves of G. yunnanensis and 46 chemical constituents were identified,accounting for 97.10% of the total content of volatile components in stems and leaves. The constituents with relatively high content were methyl salicylate (86.72%),n-hexadecanoic acid (2.60%),(Z,Z,Z)-9,12,15-octadecatrienoic acid (1.25%) and so on. Both of them contained 16 common constituents such as alkenes esters ,acids and so on. CONCLUSIONS:The chemical constituents of volatile oils extracted from the roots , stems and leaves of G. yunnanensis are mainly esters and acids. The components are similar to each other ,but the contents of acids in the roots and esters in the leaves and stems are higher.
8. Analysis of Pharmacokinetic Behavior of Five Components in Qingkailing (Lyophilized) for Injection in Normal Rats and Cerebral Ischemia Rats by UPLC-MS/MS

Objective: To establish a UPLC-MS/MS analysis method for determination of baicalin, geniposide, chlorogenic acid, cholic acid and hyodeoxycholic acid in Qingkailing (lyophilized) for injection in rat plasma, and to investigate the pharmacokinetic behavior of this preparation in normal and cerebral ischemic rats. Method: Rats were randomly divided into normal group and cerebral ischemia model group. The rat model of cerebral ischemia was established by suture embolization. The rats were given by intraperitoneal injection, and normal saline was used as the solvent. Blood samples were taken at the corresponding time points. After treatment, UPLC-MS/MS was used to determine the blood concentration of five components. The main detection conditions were mobile phase of 0.1%formic acid aqueous solution-acetonitrile for gradient elution (0-0.25 min, 90%A; 0.25-1 min, 90%-75%A; 1-2 min, 75%-50%A; 2-2.6 min, 50%-45%A; 2.6-2.65 min, 45%-90%A; 2.65-4.0 min, 90%A), the flow rate of 0.4 mL·min-1, the column temperature at 40℃, electrospray ionization under negative ion mode. The pharmacokinetic parameters were fitted and the bioavailability was calculated, the differences of treatment process of five components from Qingkailing (lyophilized) for injection in normal and cerebral ischemic rats were analyzed. Result: Compared with the normal group, the area under the curve (AUC0-t) of geniposide in rats from cerebral ischemia model group decreased significantly after intraperitoneal injection of Qingkailing (lyophilized) for injection (P<0.05), and the time to peak (Tmax) of chlorogenic acid in rats from cerebral ischemia model group was significantly earlier than that in the normal group (P<0.01). Pharmacokinetic parameters of baicalin, cholic acid and hyodeoxycholic acid had no significant difference between these 2 groups. Conclusion: Qingkailing (lyophilized) for injection has a certain difference in the treatment process between normal and cerebral ischemic rats, which has certain guiding significance for the clinical treatment of cerebral ischemic diseases with this preparation.
9.Simultaneous determination of eleven volatile components in Cinnamomi Oleum by GC-MS.

A gas chromatography-triple quadrupole mass spectrometry(GC-MS) method was established for the simultaneous determination of eleven volatile components in Cinnamomi Oleum and the chemical pattern recognition was utilized to evaluate the quality of essential oil obtained from Cinnamomi Fructus medicinal materials in various habitats. The Cinnamomi Fructus medicinal materials were treated by water distillation, analyzed using GC-MS, and detected by selective ion monitoring(SIM), and the internal standards were used for quantification. The content results of Cinnamomi Oleum from various batches were analyzed by hierarchical clustering analysis(HCA), principal component analysis(PCA), and orthogonal partial least squares-discriminant analysis(OPLS-DA) for the statistic analysis. Eleven components showed good linear relationships within their respective concentration ranges(R~2>0.999 7), with average recoveries of 92.41%-102.1% and RSD of 1.2%-3.2%(n=6). The samples were classified into three categories by HCA and PCA, and 2-nonanone was screened as a marker of variability between batches in combination with OPLS-DA. This method is specific, sensitive, simple, and accurate, and the screened components can be utilized as a basis for the quality control of Cinnamomi Oleum.
