1.Association of Serum Total Cholesterol Level and Prognosis in Patients with Acute Left Heart Failure

[Objective] The present study was to evaluate the association of serum total cholesterol level and prognosis in patients with acute left heart failure and associated mechanisms.[Methods] Sixty-eight patients due to acute episode of left heart failure prospectively enrolled,and baseline data and biochemical parameters were collected.After discharge,patients were follow-up for 1 month and they were divided into two groups (with and without cardiovascular events).Differences between groups were evaluated and the association of serum total cholesterol level and cardiovascular events were analyzed by logistic regression analysis.[Results] The mean age was 57.3 ± 12.6 years old and 52 cases were male patients accounting for 76.5 %.Among these patients,46 had a diagnosis of coronary heart disease (67.6 %),10 rheumatic heart disease (14.7 %),12 dilated cardiomyopathy (17.7%),38hypertension (55.9%) and 24 diabetes mellitus (35.3%).After 1 month's follow up,39 patients (57.4%) had experienced cardiovascular events,36 cases were re-hospitalized,and 3 died from heart failure.Compared to those with cardiovascular events,event free individuals were younger and were less likely smokers (P < 0.05).In addition,event free group had lower serum levels of N-terminal pro-BNP and C-reactive protein (P < 0.05) while serum levels of total cholesterol and albumin were significantly higher (P < 0.05).There was no significant difference in medication between these two groups.After adjusted for age,gender,smoking,systolic blood pressure,serum albumin level,diabetes,hypertension and medications,increased total cholesterol level was independently associated with better prognosis with odds ratio of 0.91 (95 % confidence interval 0.80-0.96).Further adjusted for C-reactive protein,the association was attenuated to non-significance,with odds ratio of 0.97 (95 % confidence interval 0.87-1.09).[Conclusion] Adequate serum total cholesterol level was beneficial for improving short-term cardiovascular outcomes in patients with left heart failure and the potential mechanisms might be related to cholesterol effects on improving nutritional status and anti-inflammation.
2.Analysis of Viral Pathogens in Hospitalized Children with Lower Respiratory Infection in Beijing Area from Dec.2005 to Apr.2006

Objective To explore the characteristics viral pathogens in hospitalized children with lower respiratory infection,and to provide reference data for diagnosis and treatment.Methods Nasopharyngeal secretion(NPS) samples were collected from hospitalized patients with lower respiratory infection(LRI) from Dec.2005 to Apr.2006.The NPS samples were detected for 7 respiratory virus antigens including respiratory syncytial virus(RSV),influenza virus A and B(IVA and IVB),parainfluenza virus 1,2,3(PIV 1,2,3) and adenovirus(ADV) by indirect immunofluorescent assay.Results Nine hundred and thirty-five NPS samples were collected from children(597 boys,338 girls) with LRI.The mean age was 7.5 months(range from 1 day to 6 years).Viral pathogens were identified in 516(55.2%) samples.The positive rate of RSV decreased with increasing of age,whereas the positive rate of IV and PIV increased.ADV was only detected in children less than 3 years of age,accounting for 0.6%-6.2%.Conclusions Viral pathogens are the main etiology of LRI in young children in Beijing area from Dec.2005 to Apr.2006.RSV is the most frequent viral pathogens,followed by IV and PIV.
3.Genotype of wild-type measles viruses circulating in Beijing in 2003.

OBJECTIVETo investigate genotype of wild-type measles viruses circulated in Beijing in 2003.
METHODSThroat swabs specimens were collected from patients seen during an outbreak of measles and from clinically suspected sporadic measles patients in 2003. Vero/SLAM cell lines recommended by WHO were used to isolate measles virus. Four hundreds and fifty nucleotides of COOH-terminal of nucleoprotein (N) genes were amplified by using PR-PCR. The amplified products were sequenced and the sequences were compared with references viruses from GeneBank.
RESULTSEight strains of measles viruses were isolated from throat swabs of patients who came from seven districts and counties of Beijing. Sequence analysis of the 450 nucleotides of COOH-terminal of nucleoprotein (N) genes indicated that these 8 strains belonged to H1a genotype. The average genetic distances of these 8 strains to H1a genotype, Chin9322, H1b genotype, Chin9475 and H1c genotype, Chin9427, were 0.004 - 0.011, 0.026 - 0.031 and 0.015 - 0.022, respectively. The average genetic distances of these 8 strains to H1a genotype, Anhui 01 - 1/Anhui 02 - 2, were 0.000 - 0.009 (0 - 5 nucleotide variation).
CONCLUSIONSMajor genotypes of wild-type measles viruses circulated in Beijing in 2003 were H1a genotype. The genotypes H1c, H1b and H2 may have disappeared in Beijing.

4.Acupuncture for moderate to severe allergic rhinitis: A non-randomized controlled trial.

OBJECTIVETo evaluate the effect and safety of acupuncture therapy on patients with moderate to severe allergic rhinitis.
METHODSA non-randomized controlled design was used to compare between the acupuncture group and the medication group. The acupuncture group received 8-week acupuncture therapy, and the medication group received budesonide nasal spray with cetirizine tablets for 8 weeks. The clinical symptoms and signs were analyzed before treatment, at 4 and 8 weeks after the start of treatment, and at 12 weeks after the end of treatment. Furthermore, the clinical efficacy and safety indicators were compared between the two groups.
RESULTSA total of 76 participants consisting of 38 in each of the two groups were enrolled. The scores of each clinical symptom and sign, including sneezing, runny nose, stuffy nose, nasal itching, and turbinate edema, and the total scores decreased over time in both groups (all P<0.05); and no difference was found in the scores between the two groups (P>0.05). There was no statistically significant difference in the effective rates of the acupuncture group at 4 and 8 weeks after the start of treatment as well as at 12-week follow-up compared with those of the medication group (83.3% vs. 91.2%, and 94.4 % vs. 85.3%; and 80.6 % vs. 82.4%, all P>0.05). Experimental items including blood routine, urine routine, aspartate transaminase, alanine aminotransferase, urea nitrogen and creatinine were all in the normal reference ranges during the treatment in the acupuncture group.
CONCLUSIONSAcupuncture therapy has a comparable effect to the medication treatment on patients with moderate to severe allergic rhinitis, and it is safe with no severe adverse effects.

Objective: To investigate the mechanism of Zuoguiwan in treating ovariectomy-induced osteoporosis rats by receptor activator of nuclear factor kappa-B ligand (RANKL)/osteoprotegerin (OPG) signaling pathway mediated by β2-adrenergic receptor (β2AR). Method: Forty Sprague-Dawley female rats were randomly divided into Sham-operated group (Sham) and four ovariectomized (OVX) subgroups. Rats in Sham and OVX groups were treated with 17β-estradiol (50 μg·kg-1·d-1), and low and high-dose ZGW (2.3,4.6 g·kg-1 lyophilized powder) for 3 months, respectively. Enzyme-linked immunosorbent assay (ELISA) was used to measure the serum markers of bone turnover. Micro-CT was used to evaluate and measure trabecular bone microarchitecture and bone mineral density (BMD) of the right distal femur. Western blot analysis and Real-time PCR were used to measure mRNA and protein expressions of β2AR, OPG and RANKL. Result: After 12 weeks of treatment with Zuoguiwan, the level of serum β-cross-linked c-telopeptide of type Ι collagen (β-CTX) (P<0.01) was lower, while the level of serum bone-specific alkaline phosphatase (BALP) was higher (P<0.01) than those in the OVX group. Moreover, it could prevent the OVX-induced bone loss, and alleviate the trabecular bone microarchitecture of distal femur. Furthermore, Zuoguiwan could up-regulate the mRNA and protein expressions of OPG in tibia of the Zuoguiwan groups(P<0.01), reduce the mRNA and protein expressions of β2AR in the hypothalamus (P<0.01), and down-regulated the mRNA and protein expressions of RANKL (P<0.05) in the tibia, compared with those in the OVX group. Conclusion: The mechanism of Zuoguiwan in alleviating BMD and trabecular bone microarchitecture in ovariectomy-induced osteoporosis rats might be related to the regulation of RANKL/OPG Pathway mediated by β2AR.
7.Effects of Body Weight and Waist Circumference on Balance Function in Overweight and Obese Adults

ObjectiveTo explore the effects of body weight and waist circumference on static and dynamic balance function in overweight and obese adults. MethodsParticipants (n=103) were selected by cluster random sampling from the Hypoglycemic Weight Loss Clinic of the Endocrinology Department of the Third Affiliated Hospital of Sun Yat-sen University. All participants were assessed for basic data collection, biochemical test, body weight, waist circumference and height measurement, static balance function assessed by balance error scoring system (BESS) and dynamic balance function assessed by functional reach test (FRT). The BESS score and FRT distance of all participants were compared among groups according to different BMI grades and WC grades. Multivariate linear regression was used to analyze the influencing factors of participants' dynamic and static balance functions. Results① With the increase of BMI grading, the BESS score showed an upward trend, and the difference between groups was statistically significant (P = 0.004). The BESS score of normal WC patients was lower than that of central obesity patients (P < 0.001), which indicated that compared with normal BMI, overweight and obese people had poor dynamic and static balance ability; ② With the BMI grading, the FRT distance increased, showing a downward trend (P < 0.001). The FRT distance in normal WC patients was significantly higher than that in central obesity patients (P < 0.001), which indicated that the static and dynamic balance ability of central obesity patients was worse than that of normal WC patients; ③ In BMI overweight group, the FRT distance decreased significantly with the increase of WC (P = 0.02). The results showed that under the condition of no difference in BMI, Compared with normal WC, the dynamic and static balance ability of central obese patients was worse; ④ The influence of BESS score on BMI(B=4.12,P =0.027, 95% CI=0.48-7.75)and WC(B = 3.47,P = 0.046, 95% CI = 0.07 - 6.88)was significant. The influence of FRT distance on BMI(B = -5.68,P = 0.001, 95% CI = -8.95 - 2.41)and WC(B = -4.71,P = 0.003, 95% CI = -7.83 to -1.61)was significant, which indicated that the static and dynamic balance ability of obese people was worse with the increase of BMI, and the ability of dynamic and static balance of central obesity was worse than that of normal WC. ConclusionWaist circumference is an independent factor affecting the dynamic and static balance function of overweight and obese people. Under similar BMI, the dynamic balance function of central obese people is worse than that of people with normal waist circumference, leading to higher risk of falling.
8.Single Nucleotide Polymorphisms of Paclitaxel-induced Peripheral Sensory Neuropathy in Chinese Han Population.

Objective To study the single nucleotide polymorphisms (SNPs)that predict a patient's risk of grade 2-3 paclitaxel-induced peripheral sensory neuropathy (PSN) in Chinese Han populations.Methods Totally 216 patients received paclitaxel in Peking Union Medical College Hospital from May 2014 to December 2016 were enrolled.DNA was isolated from peripheral blood.Genotyping for eight candidate SNPs was performed on Sequenom-MassARRARYiPLEX platform.Patients were followed up and PSN was assessed by trained physicians according to National Cancer Institute-Common Terminology Criteria for Adverse Events v4.03.Results A total of 209 patients entered the final analysis.Among the candidate SNPs,only rs4141404:A>C(LIMK2) was significantly associated with grade 2/3 PSN (OR:4.32,95%CI:2.37-7.89,P<0.0001).In multivariate logistic regression analysis,both rs4141404:A>C(LIMK2) and history of receiving platinum compound (OR:2.70,95%CI:1.32-5.51,P=0.007) were associated with grade 2/3 PSN.Conclusion rs4141404:A>C(LIMK2) may be the markers of risk of grade 2/3 PSN.
9.Deaths attributable to alcohol use and its impact on life expectancy in China, 2013.

Objective: To analyze the deaths attributable to alcohol use and its impact on people's life expectancy in China in 2013. Methods: The mortality data from the Disease Surveillance Points System and alcohol use data from China Chronic Disease Surveillance (2013) were used. The deaths attributed to alcohol use and its impact on the life expectancy of Chinese residents were estimated based on the principle of comparative risk assessment by calculating population attributable fraction. Results: In 2013, alcohol use resulted in 381 200 deaths, including 97 100 hemorrhagic stroke deaths, 88 200 liver cancer deaths, 61 400 liver cirrhosis deaths and 48 700 esophageal cancer deaths, and prevented 76 500 deaths, including 68 500, 4 900 and 3 100 deaths which might be caused by ischemic heart disease, hemorrhagic stroke and diabetes respectively. If risk factor of alcohol use is removed, the people's life expectancy would rise by an average of 0.43 years, especially in western China by 0.52 years, which was 0.12 years higher than that in eastern and central China, and the life expectancy of the population in rural and urban areas would rise by 0.48 years and 0.31 years respectively. Conclusions: Although alcohol has a protective effect on reducing ischemic heart disease, stroke and diabetes deaths, alcohol use is still a risk factor influencing the mortality and life expectancy of residents in China. It is necessary to take targeted measures to reduce the health problems caused by harmful use of alcohol.
