Journal of Shanghai Jiaotong University(Medical Science) 2006;0(07):-
The use of flaps for plastic and reconstructive surgery is very popular today.More and more surgeons have realized that the blood supply plays an important role in the design and survival of flaps. After Ian McGregor and Ian Jackson first introduced the axial flaps into clinics,Mathes divided the flaps into two types in 1981,the fasciocutaneous perforator flaps and musculocutaneous perforator flaps. In 1989,the concept of perforator flap was first brought into clinical practice. With the development of microsurgery and anatomy, the epidermis of human skin is divided into areas based on the vascular anatomy.A new century of musculocutaneous perforator flaps has been established. The terminology, classification, characteristics and application of musculocutaneous perforator flaps are described in this paper.

OBJECTIVEThe aim of the present study was to prepare uniform-sized silybin loaded poly (lactic-co-glycolic acid) (PLGA) microspheres in study of silybin with stainless steel membrane.
METHODSilybin PLGA microspheres were prepared by stainless steel membrane emulsification. The preparation conditions were optimized by single-factor test and orthogonal experiment, and evaluating the mean diameters, the particle size distribution, drug loading, entrapment efficiency and morphology of microsphere.
RESULTPrepared microspheres were round and surface was smooth. The mean diameter was (4.961 +/- 0.56) microm. The span was (1.75 +/- 0.18). The entrapment efficiency was (54.997 +/- 4.05)% and the average drug loading was (23.6 +/- 1.70)%.
CONCLUSIONThe stainless steel membrane emulsification can be used to prepare the silybin PLGA microspheres. The mean diameters of the silybin PLGA microspheres can be controlled in certain level. Stainless steel membrane emulsification has great potentiality exploitation and utilization.

OBJECTIVETo explore the theory of "Fei and Dachang being interior-exteriorly related" and the pathogenesis of lung injury by observing changes of inflammatory cytokines and oxygen free radicals in ulcerative colitis (UC) rats.
METHODSTotally 50 healthy male Wistar rats were randomly divided into two groups, the normal control group and the model group, 25 rats in each group. The UC model was established by allergizing colon mucosa combined with TNBS-alcohol (50%) enema. Another 25 rats were recruited as the normal control group. At week 2 and 4 after modeling, the pathomorphological changes of the lung were observed. Furthermore, the contents of tumor necrosis factor alpha (TNF-alpha) and IL-1beta were determined by ELISA. The activities of superoxide dismutase (SOD) and malondialdehyde (MDA) were evaluated with colorimetry.
RESULTSCompared with the normal control group, the pathomorphology of the lung tissue in the model group appeared abnormal at week 2 and 4. Compared with the normal control group, levels of TNF-alpha, IL-1beta, and MDA in the lung tissue significantly increased in the model group (P < 0. 01) and the activities of SOD significantly decreased (P < 0.05, P < 0.01).
CONCLUSIONTNF-alpha, IL-1beta, SOD, and MDA might be common material bases for the large intestine involved in lung disease of UC patients, thus providing a modern scientific basis for the theory of Fei and Dachang being interior-exteriorly related.

Journal of Zhejiang University. Medical sciences 2015;(2):145-153
Objective: Based on cell model and HPLC-MS technology , to screen myocardial protection active compounds from traditional patent medicine Tongmai Yangxin pill ( TMYXP ) . Methods: Fractions of TMYXP were prepared by high performance liquid preparation technology . The cardioprotective effects of prepared fractions were tested on H2O2 oxidation-damaged H9c2 myocardiocytes. The active components were analyzed by high performance liquid chromatography ( HPLC) coupled with high resolution mass spectrometry .The possible active compounds were putatively identified by comparison of their MS ions and molecular weight with literatures . Results:Ten TMYXP components presented significant myocardial protective activities , 5 of which were investigated and presented good dose-effect relationships .Their median effective concentrations ( EC50 ) were respectively 11.66, 17.44, 13.10, 7.332, 15.15 μg/mL.Totally, 11 potential active compounds were analyzed and identified , including Glycyrrhizic acid , Glycycoumarin , Licoisoflavone , Ophiopogonin D′, Licoricon, Gancaonin L, Neoglycyrol, Emodin, Angeloylgomisin H, Angeloylgomisin Q and Glyasperin A .Conclusion:The myocardial protection active compounds of TMYXP were screened successfully .

In order to clarify material basis of effective parts of liangxue tongyu prescription, blood-heat and blood-stasis rat model induced by dry yeast was established. The changes of rectal temperature, blood viscosity and plasma viscosity were used to evaluate the cooling-blood and activating-blood effects of liangxue tongyu prescription and its parts. Compared with the model group, the extract from liangxue tongyu prescription, its volatile oil and n-butanol part could significantly reduce rectal temperature (P<0.01), and also reduce blood viscosity and plasma viscosity to various degrees (P<0.01 or P<0.05). So volatile oil and n-butanol part were primarily identified as effective parts of liangxue tongyu prescription. By using GC-MS with normalization method of area to analyze volatile oil of liangxue tongyu prescription, 70 compounds were identified, accounting for about 92.54%, mainly as β-asarone, paeonol, α-asarone and shyobunone. 42 compounds such as peony glycosides, tannins, and iridoid glycosides were identified by HPLC-MS techniques and standard comparison. The study determined the effective parts of liangxue tongyu prescription and clarified the chemical composition providing the foundation for further studies on material basis of liangxue tongyu prescription.

The UHPLC-LTQ-Orbitrap high resolution mass spectrometer was used to explore the chemical compositions in safflower. The rapid separation of the compositions was conducted by the UHPLC, following by high resolution full scan and MS2 scan, under the positive and negative ion mode. The chemical formula of compositions were deduced by full scan data in less than 5, then the potential structures were confirmed by the MS2 data. Forty-nine compounds were detected, of which 26 was identified, and 5 compounds was validated by the standard substances.

Journal of Peking University(Health Sciences) 2015;(1):104-108
Objective: To identify nasal width changes occurring after Le Fort Ⅰosteotomy and to compare prospectively the effect of three surgical techniques for controlling postoperative nasal width . Methods:In the study, 79 patients (22 male and 57 female, mean age 23.2 ±3.4 years), who re-ceived Le Fort Ⅰosteotomy at Peking University Hospital of Stomatology from 2011 to 2014, were ran-domly divided into three groups .Group 1 was treated with traditional intraoral alar base cinch suture ( ABCS);Group 2 with extraoral ABCS, and Group 3 with traditional ABCS plus an extra intraoral suture at points G.lat.All the patients had taken 3D photos using 3dMD camera before operation , and 3, and 6 months after operation. The nasal widths, which were indicated as distances between Sbal-Sbal, G.lat-G.lat and Al-Al, were measured by two examiners in the 3D photos three times with a time-interval of one week .SPSS 13 .0 was used to do the statistic analysis .Results: At the end of the postoperative 6 months, the nasal widths lessened as compared with the postoperative 3 months. No significant diffe-rences were found between the three groups 6 months after the operation .The degree of the postoperative nasal width widening had positive correlation with that of the intraoperative nasal width widening , and had negative correlation with the initial nasal width and the amount of post-suture narrowing .Conclusion:There is no difference between three suturing techniques for controlling nasal width widening after Le FortⅠosteotomy. The postoperative nasal width-widening can’t be totally avoided, and the alteration might last at least 6 months after the operation .For patients with narrow nasal width and need to move maxilla forward , more overcorrection of ABCS is needed to control the postoperative nasal base widening .
