1.Effects of obstructive sleep apneas on endothelial function and autonomic modulation in adult man.

Objective To study the effects of obstructive sleep apneas on endothelial function and autonomic modulation. Methods From June 2009 to June 2011, male patients with obstructive sleep apnea hypopnea syndrome (OSAHS) were consecutively enrolled in this study. Patients with an apnea/hypopnea index (AHI) of greater than 15 and without previous treatment for OSAHS were included as Group OSAHS and obese subjects with an AHI of less than 5 were included as non-OSAHS controls (Group Control). Electrocardiography and beat-to-beat blood pressure were continuously recorded from the radial artery by applanation tonometry which was synchronized with polysomnography recording. Endothelial function was measured by arterial augmentation index (AAI). Spectral analysis of heart rate variability (HRV) and blood pressure variability (BPV) were computed for cardiac parasympathetic modulation (high frequency power, HF); sympathetic modulation (low frequency power, LF), sympathovagal balance (LF/HF power of R-R variability, LF/HF) and BPV sympathetic modulation (BPV LF) in normalized units [total power of the components/(total power-very LF power)×100]. Results Finally, 27 moderate-severe OSAHS patients and 22 non-OSAHS obese controls were recruited in the Group OSAHS and Group Control, respectively. In Group OSAHS, the age was 43.3±9.3 year-old, body mass index (BMI) was 36.8±8.7 kg/m2; in Group Control, the age was 42.9±8.6 year-old, BMI was 34.4±7.9 kg/m2; there were no significant differences in age and BMI between the Group OSAHS and Group Control (all P>0.05). The baseline AAI (12.5%±2.2% vs. 8.2%±2.1%) and BPV LF (68.3%±13.5% vs. 61.1%±11.7%) of the Group OSAHS were significantly higher than those of the Group Control (all P<0.05). And after overnight sleep, systolic BP (143.7±14.2 vs. 132.8±13.3 mm Hg), diastolic BP (87.7±7.7 vs. 78.6±5.5 mm Hg), HRV LF (69.7%±14.4% vs. 64.3%±12.1%), HRV LF/HF (3.7±2.0 vs. 2.3±1.3) and BPV LF (77.8%±15.6% vs. 68.3%±13.5%) of the Group OSAHS were significantly increased (all P<0.001), while HRV HF was significantly decreased (21.1%±9.3% vs. 27.5%±10.3%, P<0.05) from baseline.

2.Repair of facial scar with tubed pattern skin flap from the cervical cutaneous branch of transverse cervical artery

Objective To summarize the clinical experience of repairing facial scar with tubed pattern skin flap from the cervical cutaneous branch of transverse cervical artery.Methods Firstly,a skin flap in the cervical cutaneous branch of transverse cervical artery was designed at chest according to the size of facial scar.The chest skin would be expanded if it was not large enough.Secondly,a skin flap was cut,and the proximal end was rolled and sutured as a tube,whereas the remote end was formed an interface.Thirdly,the scar was removed from the facial scar area and the wound was covered with the tubed pattern skin flap interface.After a blood circulation was established between the face skin and the flap,the flap was trained so that it had plentiful blood supply.Lastly,the tubed pattern skin flap was cut off and open,the remaining facial scar was removed,and the wound was covered and sewed up with the sectioned tubed pattern skin flap.Results The facial scar 35 patients were repaired with tubed pattern skin flap from the cervical cutaneous branch of transverse cervical artery.The survival rate of flap was 100%.Twenty patients received thinning and shaping on flaps in 0.5 to 1 year after surgery.All patients were satisfied with this repair within 1 ~ 5 year follow-up.Conclusions The tubed pattern skin flap in the cervical cutaneous branch of transverse cervical artery is of highly similar to the facial skin.In addition,the supply of the skin is enough.Therefore,it is an ideal flap to repair large facial skin defects.
3.Develepment of Hand Function Rehabilitation Evaluation and Training System Based on Virtual Reality Technol-ogy

Objective To develop a system for hand function evaluation and training based on virtual reality. Methods 5DT Data Glove 5 Ultra, Visual Studio 2010 were integrated as the development environment and DirectX 9.0 as components, and the system was developed based on the MFC programming framework. Results and Conclusion The system can assess the fingers function, recognise gestures and can be used as a 3D environment for virtual training, which may guide the patients to take the training actively.
4.Chemiluminescence in Whole blood and its Clinical application Ⅱ. Detection of phagocytic function of leucocytes from patients with various diseases

Measurement of whole blood CL was carried out to evaluate phagocytic of function of PMNL and opsonic activity of plasma from children (22 cases), adults (31), newborn infants (umbilical cord blood, 21), patients with bacterial infection (31), viral infections (21),and mild anemia (10). The following results were obtained: first, phagocytic function of PMNL and opsonic activity of plasma from normal children was compared to adults; second, phagocytic and/or metabolic activity of PMNL and opsonic ativity of plasma from newborn infants were lower then normal, third, PMNL of the patients with bacterial infections had been preactivated in vivo, their phagocytic function being enhanced, but, blood from the patients with viral infections showed almost no abnormalities; fourth, phagocytic activity of PMNL from the patients with mild anemia was similar to normal human. The significance of the results was also discused in the paper.
5.Therapeutic Effect of Transcutaneous Electrical Point Stimulation on Scapulohumeral Periarthritis of Adhersion Type

Objective To observe clinical therapeutic effect of transcutaneous electrical point stimulation (TEPS) on scapulohumeral periarthritis of adhersion type. Methods Criteria of standardized diagnosis and assessment of therapeutic effects were adopted, various treatment parameters were fixed, and pain of shoulder and activity of shoulder joint were taken as observed indexes, TEPS and electroacupuncture (EA) were applied respectively in the treatment of 194 cases of scapulohumeral periarthritis of adhersion type. ResultsTEPS and EA had a similar good therapeutic effect, with the total effective rat 96.9% and 97.9% separately. TEPS could improve both pain index and activity of shoulder joint. Conclusion TEPS is a better therapy for scapulohumeral periarthritis of adhersion type, with ease of administration.
6.Relationship of serum ANGPTL4 to obesity and type 2 diabetes mellitus

Objective To investigate the level of serum angiopoietin-like protein 4 (ANGPTL4) in normal subjects and patients with obesity and newly-diagnosed type 2 diabetes mellitus (T2DM),and to explore the relationship of serum ANGPTL4 with parameters of body fat,glucose-lipid metabolism,and insulin resistance index (HOMA-IR).Methods A total of 160 subjects with normal glucose regulation(NGR) and newly-diagnosed T2DM,were divided into 4 groups:NGR with normal weight group (NGR-NW),NGR with overweight/obese group (NGROW/OB),T2DM with normal weight group (T2DM-NW),and T2DM with overweight/obesity group (T2DM-OW/ OB) according to their body mass index(BMI).ELISA was used to determine the level of serum ANGPTL4.The weight,height,waist circumference,hip circumference,waist hip ratio(WHR),and BMI,fasting insulin(FINS),lipid metabolism parameters,and insulin resistance index (HOMA-IR) were measured.Results Compare with the NGR group,the level of ANGPTL4 was significantly decreased in the T2DM group [(25.75 ±2.85 vs 31.08 ±3.35)ng/ml,P<0.01],the level of ANGPTL4 in OW/OB group was significantly lower than that in the NW group [(27.27± 3.05 vs 29.04 ± 4.64) ng/ml,P<0.01].Partial correlation analysis showed that serum ANGPTL4 was negatively correlated with BMI,WHR,FPG,2hPG,HbA1C,FINS,HOMA-IR,AST,ALT,and TG levels(P<0.01),but positively correlated with the HDL-C(P<0.01).Multiple stepwise regression analysis showed that HbA1C,FINS,and HDL-C were independent risk factors of serum ANGPTL4 (β =-0.825,-0.119,3.301,P<0.01).Conclusion Serum ANGPTL4 level in T2DM and OW/OB group was lower than that in NGR and NW group.Serum ANGPTL4 is associated with FINS and lipid metabolism,suggesting that it may play an important role in the pathophysiology of obesity and metabolic syndrome.
7.Treatment of Early-stage Adhesive Shoulder Periarthritis with Transcutaneous Electric Stimulation on Acupoints

To observe the therapeutic effects of transcutaneous electric acupoint stimulation on early-stage adhesive shoulder periarthritis. Methods:by using the research approach of evidence-based medicine such as multi-center,large sample and randomization,the 163 cases of early-stage adhesive shoulder periarthritis were treated with transcutaneous electric point stimulation and electric acupuncture respectively to observe the therapeutic effect of transcutaneous electric acupoint stimulation and compare its effect with electric acupuncture.Results and Conclusion:The total effective rate of transcutaneous electric point stimulation on early-stage adhesive shoulder periarthritis reached 96.5%,showing no significant difference with the electric acupuncture group;the transcutaneous electric acupoint stimulation could not only relieve pain,but also improve the shoulder joint movement. As a result,the transcutaneous electric acupoint stimulation is an easy and more effective therapy for shoulder periarthritis.
8.Comparison of components in Coptis chinensis processed with various quantity of Evodia rutaecarpa

Object To investigate the berberine in Coptis chinensis Franch. processed with various quantity of Evodia rutaecarpa (Juss.) Benth. and the components absorbed from E. rutaecarpa. Methods Taking berberine, evodiamine and rutaecarpine as targets, HPLC method was used to determine the components of C. chinensis, E. rutaecarpa, C. chinensis processed with 10%, 20%, 30%, 40%, 50% E. rutaecarpa. Results The content of berberine in C. chinensis processed with E. rutaecarpa decreased, and that of C. chinensis processed with 20% and 30% E. rutaecarpa was higher than the rest. C. chinensis processed with E. rutaecarpa absorbed the components of E. rutaecarpa really. Conclusion The suitable quantity of E. rutaecarpa is 20% when processing for C. chinensis.
9.Ophiopogon samponin Ⅵ?Release rate of ophiopogon saponin enteric microsphere

AIM: To explore the influencing factors in the release rate and give the application to the preparation. METHODS: Preparing ophiopogon saponin enteric microsphere by spray drying process,the accumulative release rate of acid and buffer solution were detected by colorimetric analysis. RESULTS: With the increase of Eudragit Ⅱ concentraction,the accumulative release rate tended to decrease.But ratio of drug and Eudragit Ⅱ,increased with the decrease in delay time on the break point. CONCLUSION: The concentration of Eudragit and the ratio of drug and material are the rey factors in the accumulative release rate in acid and buffer solution.
10.Prescription design of Ophiopogon japonicus saponin enteric microsphere by spray drying technique

Objective To explore the prescription factor on Ophiopogon japonicus saponin enteric microsphere by spray drying technique. Methods Observing the type and content of enteric coating material, the type of plasticizer, the type and dosage of antistickiness material by single factor. Optimizing the prescription by orthogonal test design. Results Both Eudragit Ⅱ and micronization silica gel made in China could meet the need of the preparation. The best prescription included the proportion between drug and enteric coating material (1∶4), the dosage of castor oil (1%), and the dosage of micronization silica gel (1.5%). Conclusion O. japonicus saponin enteric microsphere accorded with the expecting demand. The kind of medical subsidiary material made in China will be the main raw material in producing the enteric microsphere. The study of prescription design will provide the basis for realizing microencap-sulation in Chinese materia medica.
