1.Prevention and treatment of short-term complications in 8 heart transplantations and 1 heart lung transplantation

Objective To summary the experience of management for orthotopic homologous heart transplantation,heart-lung co-transplantation and to discuss the postoperative treatment of short-term complications.Methods Orthotopic homologous heart transplantation and heart-lung co- transplantation on 9 cases were performed from May 2004 to February 2006.The patients were traced closely and all complications were treated in time.Results During hospital stay,hypertension oc- curred in one case,right heart insufficiency in one case,infection of mycetes and cytomegalovirus in 2 cases after the heart transplantation,and in the patient undergoing heart-lung co-transplantation,renal inadequacy,bronchial anastomosis stenosis and fungus infection occurred.All patients obtained symp- tomatic relief before discharge.Conclusion Proper perioperative management,immunosuppressive therapy and management of postoperative complications could reduce mortality and obtain satisfactory therapeutic effect in the near future.
2.Transport of aripiprazole across Caco-2 monolayer model.

This study aimed to investigate the transport characteristics of aripiprazole. A human intestinal epithelial cell model Caco-2 cell in vitro cultured had been applied to study the transport of aripiprazole. The effects of time, concentration of donor solutions, pH, temperature and P-glycoprotein inhibitor on the transport of aripiprazole were investigated. The determination of aripiprazole was performed by HPLC. It is concluded that aripiprazole is transported through the intestinal mucosa via a passive diffusion mechanism primarily, coexisting with a carrier-mediated transport. The transport of aripiprazole is positively correlated to transport time, pH, and temperature. Papp increased with donor concentrations up to 10 microg x mL(-1), and then decreased for higher concentrations. The P-glycoprotein inhibitor cyclosporine A significantly enhanced the transport amount of aripiprazole.

3.Detection of anti-tyrosinase IgG antibody and anti-tyrosinase-related protein-1 IgG antibody in sera of patients with vitiligo

Objective To investigate relationships between serum levels of anti?tyrosinase IgG antibody(TYR IgG)as well as anti?tyrosinase?related protein?1 IgG antibody(TRP?1 IgG)and vitiligo. Methods Enzyme linked immunosorbent assay(ELISA)was performed to detect serum levels of TYR IgG and TRP?1 IgG in 260 patients with vitiligo and 50 health controls. The threshold for defining a positive test result was set at 3 standard deviations above the mean serum level of TYR IgG or TRP?1 IgG in the healthy controls. Results The positive rate of TYR IgG and/or TRP?1 IgG in the vitiligo group was 57.31%(149/260). The positive rates of TYR IgG and TRP?1 IgG were both significantly higher in the vitiligo group than in the control group(TYR IgG:37.3%[97/260]vs. 0,χ2=25.441, P<0.01;TRP?1 IgG:33.5%[87/260]vs. 0,χ2=21.630, P<0.01). The positive rate of TYR IgG was not associated with that of TRP?1 IgG in the vitiligo group(r=-0.032, P>0.05). Among patients with vitiligo, the positive rate of TRP?1 IgG was significantly higher in females than in males(χ2=5.811, P<0.05), as well as in patients aged≤20 years than in those aged>20 years(χ2=6.498, P<0.05), while the positive rate of TYR IgG didn′t differ between females and males, or between patients aged ≤ 20 years and those aged > 20 years (both P >0.05). Conclusion Detection of TYR IgG and TRP?1 IgG may provide some evidence for the diagnosis and treatment of vitiligo.
4.Study on the correlation of interleukin-1β, interleukin-6, tumor necrosis factor-α and bone marrow edema with knee osteoarthritis

ObjectiveTo investigate the correlation of the serum and synovial fluid interleukin (IL)-1β,IL-6,tumor necrosis factor(TNF)-α and bone marrow edema with osteoarthritis of the knee (KOA).MethodsThe clinical data of 331 KOA patients were included and all patients had knee MRI.Bone marrow edema were detected in 172 cases and 159 cases had non-bone marrow edema.The clinical symptoms,WOMAC score,and the serum and synovial fluid IL-1β,IL-6,TNF-α levels were compared using One-way ANOVA analysis and Spearman's correlation analysis.Results① The serum and synovial fluid IL-1β,IL-6,TNF-α in osteoarthritis was positively correlated(serum r=0.24,0.38,0.31; synovial fluid r=0.20,0.29,0.33 ; all P<0.05) ; ② The serum and synovial fluid IL-6 and TNF-α levels of osteoarthritis with bone marrow edema were significantly higher than those of the osteoarthritis without bone marrow edema(serum F=8.139,7.172; synovial fluid F=9.201,7.001; all P<0.05); ③ The serum and synovial fluid TNF-α,IL-6 levels was associated with osteoarthritis with bone marrow edema in volume (serum r,=0.27,0.26; synovial fluid rs=0.29,0.32; all P<0.05) and severity(serum rs=0.29,0.27; synovial fluid rs=0.29,0.31; all P<0.05); ④ The serum and synovial fluid IL-1β,IL-6,TNF-α of osteoarthritis with synovitis was significantly higher than that of osteoarthritis without synovitis group (group of bone edema:serum F=3.931,5.866,5.514; synovial fluid F=4.211,5.202,5.972; all P<0.05.non-bone edema patients:serum F=3.513,3.114,7.112; synovial fluid F=3.722,3.965,8.891; all P<0.05).ConclusionIL-1β,IL-6,TNF-α are elevated in osteoarthritis with synov-itis.IL-6 and TNF-α are elevated significantly in knee osteoarthritis with bone marrow edema in particular.
5.Peripheral blood leukocyte double strand RNA-dependent protein kinase gene expression in patients with systemic lupus erythematosus

Objective To investigate the expression of the double-stranded RNA-dependent protein kinase (PKR) gene in the peripheral blood leukocyte of patients with systemic lupus erythematosus (SLE),and to evaluate the relationship between the gene expression and the disease activity.Methods The clinical data of 100 SLE patients,40 non-SLE patients with rheumatic diseases,and 40 normal controls were collected.Total RNA was extracted from the peripheral blood and then reverse transcribed into cDNA.Sybr green dye based real-time quantitative PCR method was used to compare the expression levels (indicated as 2-△Ct value) of PKR in the three groups.Results (1) The 2-△Ct value of PKR expression level in the SLE patients was (14.69 ± 7.62),which was significantly higher than those in the non-SLE patients (5.09 ±4.73,P =0.012) and normal controls (4.79 ± 3.49,P =0.005).(2) The 2-△Ct value of PKR expression level in the SLE patients with severe activity was (22.57 ±2.61),which was significantly higher than those in the SLE patients with mild activity and no activity(12.94±2.41,P =0.000 ;8.85 ± 2.17,P =0.000).(3) The 2 △Ct value of PKR expression level in the SLE patients with lupus nephritis was significantly higher than that in the SLE patients without lupus nephritis (16.85 ± 7.32 vs 8.35 ± 2.04,P =0.034).(4) The 2-△Ct value of PKR was correlated with the systemic lupus erythematosus index(SLEDAI) scores(r =0.32,P =0.000),WBC (r =0.46,P =0.000),Hb (r =-0.22,P =0.035),the quantitation of urine protein in 24 hours (r =0.21,P =0.000),HDL-C (r =0.21,P =0.022),and anti-RNP antibody (rs =-0.21,P =0.025).Conclusions The expression of PKR in the SLE patients is up-regulated,especially in those with severe activity.The expression level of PKR gene is associated with SLE disease activity.
6.The in vitro kinetics of uptake, transport and efflux of 9-nitrocamptothecin in Caco-2 cell model.

AIMTo study the kinetics of uptake, transepithelial transport and efflux of 9-nitrocamptothecin (9-NC).
METHODSA human intestinal epithelial cell model Caco-2 cell in vitro cultured had been applied to study the kinetics of uptake, transport and efflux kinetics of 9-NC at small intestine. The effects of time, pH, temperature and P-glycoprotein inhibitors on the uptake of 9-NC were investigated. The determination of 9-NC was performed by HPLC.
RESULTSThe uptake and absorption of 9-NC were passive diffusion as the dominating process. The uptake of 9-NC is positively correlated to uptake time, and negatively correlated to pH and temperature. The inhibitors, cyclosporine A and verapamil, significantly enhanced the uptake amount of 9-NC (P < 0.05). P(app) of Basolateral to Apical was much more than that of Apical to Basolateral (2.6-6.9 fold). The efflux of 9-NC was fitted to apparent two-order process. The m0 [(148.0 +/- 2.2) pmol x cm(-2)] and the efflux rate (41.1 pmol x cm2 min(-1)) on Apical side were higher than the m0 [(121 +/- 7) pmol x cm(-2)] (P < 0.05) and the efflux rate (29.2 pmol x cm2 x min(-1)) on Basolateral side (P < 0.01).
CONCLUSIONThe uptake and absorption of 9-NC were passive diffusion as the dominating process. P-glycoprotein had strong efflux effects on the uptake and transepithelial transport of 9-NC.

7.Advance in research on chemical constituents from Notopterygii Rhizoma et Radix.

This paper summarized the recent 30 years research progress of the chemical constituents from Notopterygii Rhizoma et Radix. The chemical constituents from Notopterygii Rhizoma et Radix mainly consist of coumarins, polyene-polyacetylenes, sesquiterpenes, phenolic acids, while steroids and flavonoids were less reported. All constituents were confirmed and corrected through SciFinder. We also checked the Chinese name and English name and listed the CAS number of each compound. It can provide some guidelines for the research, development and utilization of Notopterygii Rhizoma et Radix in the future. Whether there is columbianin in the Notopterygii Rhizoma et Radix need to be further researched.

8.Electrocardiogram Minnesota codings from 30 000 adult cases with Kazakh ethnicity in Xinjiang,China

Objective To observe the abnormal Minnesota code (MC) distribution and interrelated characteristic on electrocardiograms (ECGs) of the adult Kazakh population.Methods Resting ECGs and blood press of randomly sampled 30 000 adult Kazakh people in three Northern regions of Xinjiang were continuously examined and analyzed,using Minnesota code recommended by WHO as the classification of ECG.Results The overall rate of abnormal ECG findings was 248.60‰,and the main abnormality in males was 146.83‰,compared to 157.71‰ in females.The prevalence rates of abnormal ST-T changes,the total arrhythmia and atrial fibrillation (AF) were 100.03‰,71.17‰ and 2.83‰ respectively.There were statistically significant differences among the main abnormities from the three regions.Conclusion The ECGs abnormalities of adult Kazakh people were high.There was significant relation found between the main abnormalities and hypertension.The prevalence of AF was different from the domestically reported literature that calls for further study.
9.Effects of HO-1 on Lipopolysaccharide-induced Endoplasmic Reticulum Stress of Rat Hepatocytes.

OBJECTIVE:
To investigate effects of antioxidant stress protein heme oxygenase-1 (HO-1) on lipopolysaccharide (LPS)-induced endoplasmic reticulum stress (ERS) of rat hepatocytes.
METHODS:
The BRL cells (rat hepatocyte cell line) were cultured. The hepatocytes were treated with LPS, LPS+HO-1 siRNA, HO-1 siRNA and PBS solution, respectively. The cell viability was measured by trypan blue exclusion test. The apoptosis cells were detected by the fluorescent dye Hoechst 33258. Expressions of GRP78, CHOP, caspase-12 and HO-1 were detected by Western blotting.
RESULTS:
LPS caused an increase of HO-1 protein expression of rat hepatocytes in a dose-dependent and time-dependent manner, a up-regulation of GRP78, CHOP and caspase-12, a decrease in cell viability, and an increase in apoptosis rate of hepatocytes. Pretreatment of HO-1 siRNA inhibited the up-regulation of LPS-induced HO-1, however, aggravated ERS and cellular injury.
CONCLUSION
HO-1 inhibites ERS-mediated cellular injury of rat hepatocytes induced by LPS.

10.The pharmacokinetics and pharmacodynamics of intranasal preparation of Panax notoginseng Saponins.

AIMTo investigate the pharmacokinetic course of intranasal powders of Panax notoginseng Saponins (PNS) in a rat model and its protective effects against cardio-cerebrovascular diseases administrated in the form of its suspension.
METHODSAfter administration, Rgl concentration in the serum was analyzed by HPLC and the absolute bioavailability was calculated. The protective effects against cardia-cerebrovascular diseases were studied on actue myocardial infarction model in rats built by occlusion of left coronary artery and cerebral ischemia-reperfusion model in gerbils built by occlusion of bilateral common carotid artery (CCA).
RESULTSThe in vivo course of Rgl in rats conformed to two-compartment model after intranasal administration of PNS suspension and the absolute bioavailability was 103.56%. The suspension significantly reduced myocardial infarct size induced by occlusion of the left coronary artery, alleviated cerebral edema and the stroke symptoms induced by occlusion of bilateral common carotid artery (CCA). And the effects were dose-dependent, the higher dose, the better effects.
CONCLUSIONThe results of pharmacokinetics and pharmacodynamics demonstrated that PNS intranasal preparation has a pretty prospect to develop.
