3.Clinical effect comparison of different surgical methods for treating diabetic retinopathy

AlM:To compare the clinical results of different surgical methods for diabetic antipathy.
METHODS: Eighty cases ( 102 eyes ) with diabetic antipathy were selected in our hospital from January 2012 to December 2013. Thirty-eight cases (48 eyes) in group A received joint surgical treatment, 42 cases (54 eyes) in group B took staging of surgical treatment. The clinical effect was observed in both groups.
RESULTS: The vision after surgery was improved than that of before surgery in two groups, there were no significant differences ( P > 0. 05 ). A postoperative complication rate was 16. 7% in group A and 22. 2% in group B, showed no significant difference (P>0. 05).
CONCLUSlON: Surgery and staging joint surgery are both feasible for diabetic retinopathy patients, can, improve the visionr. Both of them are worthy of clinical application.
4.Change of choridal thickness and its influence factors in adult high myopic patients

Background Aheration of eyeball wall caused by ocular axial extension is associated with multiple complications of high myopia.However,the study on quantitative analysis of choroidal thickness and axial length in adult high myopic patients is less.Objective This study was to investigate the choroidal thickness in high myopic eyes of adult patients and estimate the correlation of choroidal thickness with axial length,age,and spherical equivalent(SE).Methods A prospective cohort study was designed.Seventy-five eyes of 75 adult patients with high myopia were entrolled from December 2012 to May 2013,and 70 eyes of 70 age-and gendermatched healthy volunteers were included in Renmin Hospital of Wuhan University.Enhanced depth imaging (EDI)on Cirrus spectral-domain optical coherence tomography (SD-OCT) was used to measure the choroidal thickness from the outer border of the retinal pigment epithelium through the inner scleral boarder among the 11 meridians in a 500 μm intervals and range of 2 500 μm for each from fovea toward temporal and nasal lateral.The differences of choroidal thickness and axial length were compared between the high myopia group and normal control group,and the correlation of choroidal thickness with axial length,age,SE were analyzed.Results The subfoveal and mean choroidal thickness values were (146±52) μm and (142±63) μm in the high myopia group,and those in the normal control group were (306±60) μm and (271 ±71) μm,with significant differences between the two groups (t =-17.130,P=0.000; t=-15.890,P=0.000).Choroid was thickest in the temporal and then was subfovea and nasal in the high myopia group,but in the normal control group,it was subfovea,temporal and nasal in turn,and the choroidal thicknesses in various areas were thinner in the high myopia group than those in the normal control group.A negative correlation was found between the choroidal thickness and axial length in high myopia group(r =-0.580,P =0.000),and the regression equation determined a decrease of 17.943 μm per millimeter of axial length.Conclusions SD-OCT determines that choroidal thickness is decreased in highly myopic eyes compared with normal eyes.Choroidal thickness varies with the change of axial length in adult high myopia patient.These findings indicate that abnormalities of the choroids may play a role in the pathogenesis of complication of high myopia.
5.CT evaluation and classification of the Le Fort fracture

Objective To evaluate the CT findings and classification of the Le Fort fracture. Methods Sixty-two cases with Le Fort fractures were studied with thin-slice high-resolution CT scanning and analyzed with three-dimensional(3D)imaging reconstruction.Results Of the 62 patients,10 had Le Fort type Ⅰ fracture,9 had Le Fort type Ⅱ fracture,8 had Le Fort type Ⅲ fracture,and 35 had various combinations of the three types of Le Fort fractures,including 18 Le Fort Ⅰ+Ⅱ fracture,7 Le Fort Ⅰ+ Ⅱ+Ⅲ fracture and 10 Le Fort Ⅱ+Ⅲ fracture.Fifty-five cases had associated multiple fractures in the maxillofacial region.On 2D CT images,Le Fort fracture manifested as multiple and complex fractures. Though 2D image was better than 3D image in accurately defining tiny fractures and fractures of deep structures,the diagnosis of Le Fort fracture could not be correctly made solely on 2D image.3D CT clearly and stereoscopically demonstrated the entire shape and orientation of Le Fort fracture,thus facilitating the correct classification of Le Fort fracture.Conclusion 3D CT image is important in providing information about the space relationship of Le Fort fracture,thus very valuable for the preoperative planning.
6.Application of Color Doppler Ultrasonography by Bed Side in the Early Diagnosis of Hypoxic-Ischemic Encephalopathy in Full Term Neonates

Objective To explore the value of color doppler ultrasonography by bed side in the early diagnosis of HIE in full term neonates.Methods The changes of cerebral parenchymal and cerebral arterial blood stream parameter on 35 cases of neonates clinically diagnosed HIE of mild and moderate degree and 40 cases of normal newborns on the 24,48 and 72 hours after birth were observed by color doppler ultrasonography by bed side.Results 1.The cerebral parenchyma was even echo in normal newborns,but it was maldistributed and reinforced in mild asphyxia neonates and it was more serious in moderate degree.The echo of cerebral parenchyma in mild degree was near normal in 48 hours after birth,while the echo of cerebral parenchyma in moderate degree was still maldistributed and reinforced in 48 and 72 hours after birth.2.There was obvious changes in the cerebral arterial blood stream parameter and hemodynamics of the asphyxia newborns compared with normals.The systolic peak velocity(Vs)and end diastolic velocity(Vd)of the cerebral arteries in mild and moderate degree were obviously lower than that of control group in 24,48 hours after birth(Pa0.05).3.Resistance index(RI)of the cerebral arteries in mild and moderate degree were higher than that of control group in 24,48 hours after birth(Pa0.05).Conclusion Color doppler ultrasonography by bed side is a convenient,noninvasive method for diagnosing HIE.
7.Management Strategy of Lumbar Disc Herniation in Community Health Center United Tertiary Hospital

Objective To explore an economic and effective management strategy for lumbar disc herniation (LDH) in community health center. Methods 62 LDH patients treated in Xujiahui community health center (observation group), 48 LDH patients treated in Huajing community health center (control group A) and 55 LDH patients treated in Shanghai 6th People's Hospital (control group B) were included.The observation group was managed by associate professor from Shanghai 6th People's Hospital and accepted different treatment strategy in mild, medium and heavy grades. The control groups A and B were managed routinely. 3 month later, costs of illness were computed and curative effects were accessed in 3 groups. Results There was no difference in curative effects between observation group and control group B (P>0.05), but better than control group A (P<0.05). The observation group cost the least among the groups (P<0.05). Conclusion Graded treatment strategy united with tertiary hospital is an economic and effective mode for LDH management in community health center.
8.Clinical study on mild moxibustion for knee osteoarthritis

Objective:To observe the clinical effect of mild moxibustion for knee osteoarthritis (KOA) and to explore the role of mild moxibustion in relieving pain,reducing stiffness and improving joint dysfunction in patients with KOA.Methods:Eighty patients with KOA were randomly allocated into either a moxibustion group or a medication group by the random number table,with 40 cases in each group.The moxibustion group used mild moxibustion at Neixiyan (EX-LE 5),Dubi (ST 35),Xuehai (SP 10) and Liangqiu (ST 34),30 min each time,3 times a week;the medication group was given celecoxib capsule (celebrex),0.2 g each time,once a day.Both groups were treated for 4 weeks.The visual analog scale (VAS) and Western Ontario and McMaster Universities osteoarthritis index (WOMAC)scores were evaluated before and after treatment.The efficacy of the two groups was compared after treatment.Results:After treatment,the overall efficacy of the moxibustion group was significantly different from that of the medication group (P<0.05).The VAS and WOMAC scores of the two groups were lower than those before treatment (both P<0.01).The changes in the VAS and WOMAC scores after treatment in the moxibustion group were significantly different from those in the medication group (both P<0.05).After treatment,in single item of WOMAC,the changes in pain and joint dysfunction in the moxibustion group were more statistically significant than those in the medication group (both P<0.05).Conclusion:Mild moxibustion and oral celebrex can reduce the VAS and WOMAC scores of patients with KOA.Mild moxibustion is superior to oral celebrex in relieving pain and improving joint function.
9.RORγt expression in the pulmonary tissue of asthmatic mice and the inhibitory effects of budesonide.

OBJECTIVETo study the effect of budesonide (BUD) on RORγt expression in the pulmonary tissue of asthmatic mice and mechanisms of BUD in the treatment of asthma.
METHODSBlab/c asthmatic mouse model was induced by ovalbumin (OVA). Thirty female mice were randomly divided into three groups: control, asthmatic and BUD-treated. IL-17 levels in bronchoalveolar lavage fluid (BALF) and serum were measured using ELISA. Total and differential cell counts in BALF were measured. Airway inflammation was evaluated by hematoxylin and eosin staining. IL-17 mRNA and RORγt mRNA expression were measured by reverse transcription-polymerase chain reaction (RT-PCR).
RESULTSRORγt mRNA and IL-l7 levels in the asthmatic group were significantly higher than in the control group (P<0.01). BUD treatment significantly decreased RORγt mRNA and IL-l7 levels compared with the asthmatic group (P<0.01). Compared with the control group, total, neutrophil and eosinophil cell count in BALF increased significantly in the asthmatic group (P<0.01). After BUD treatment, counts of total, neutrophil and eosinophil cells in BALF were significantly reduced (P<0.01) and were similar to in the control group. Inflammatory reactions in the respiratory tract were significantly alleviated in the BUD treated group.
CONCLUSIONSRORγt and IL-l7 levels in the pulmonary tissue of asthmatic mice increase and this may be associated with the pathogenesis of asthma. BUD can inhibit RORγt and IL-17 and thus reduces lung inflammation.

Abstract: Schistosomiasis is a serious major parasitic disease that threatens human life and health. A better understanding
of the mechanism of host-schistosome interactions is the key to designing new prevention and control strategies. MicroRNAs
(miRNAs) are endogenous small non-coding RNA molecules, which lead to the degradation of the target messenger RNA
(mRNA) or inhibition of its translation in a sequence-specific manner. Both schistosome and its host produce miRNAs, which
can be secreted by extracellular vesicles (EVs). There is accumulating evidence that miRNAs from schistosome can be taken
up by host cells, and finely manipulate the phenotype of host cells for their survival or pathogenesis in a cross-species manner,
even inhibiting the growth and metastases of hepatoma cells. It is still unknown whether host free miRNAs can be taken up by
schistosome, but this phenomenon is highly probable. miRNA-mediated cross-species regulation has emerged as a novel
mechanism for host-schistosome interactions, and this review summarizes the advances in this regard.
