1.Evaluation of Plasma Concentration Monitoring Method for Sodium Valproate by Fluorescence Polarization Immunoassay

OBJECTIVE:To explore quality control of plasma concentration monitoring for sodium valproate and to improve the quality of pharmaceutical monitoring.METHODS:The statistical and continuous analysis of sodium valproate control data from our hospital of 2008 were conducted using fluorescence polarization immunoassay(FPIA).RESULTS:Mean recovery and RSD were 98.37% and 1.94% for low control samples,99.33% and 2.88% for medium control samples,98.17% and 4.10% for high control samples.The RSD of sample was lower than 5% and in line with the requirement for biological sample determination in Chinese Pharmacopoeia.CONCLUSION:FPIA is a comparatively accurate and stable method for plasma concentration monitoring of sodium valproate.

3.Research on the bacteroides of patients with AIDS and the healthy control

Objective To investigate the bacteroides of patients with AIDS and the healthy control by real-time quantitative PCR in order to reveal the role and significance of gut microflora in the AIDS-associated molecular pathogenesis. Methods The feces of the preoperative AIDS patients (n = 30) and the healthy control (n = 30) were collected. According to the sequences of target genes, specific PCR primers were designed. Bacterial genome DNA extracted from fecal samples was quantified by real-time quantitative PCR to analyze the bacterial amounts. Results In the patients with AIDS, the level of B.fragilis (3.23 ± 1.59; 4.05 ± 1.65), B.uniformis (5.69 ± 0.95;6.70 ± 2.18), B.thetaiotaomicron (5.01 ± 1.61; 6.41 ± 2.34), B.ovatus (5.78 ± 1.03; 7.07 ± 1.75), B.distasonis (4.21 ± 1.21; 5.53 ± 2.46) and B.vulgatus (2.92 ± 1.30; 4.48 ± 1.32) were significantly higher than those of healthy controls (P < 0.05). Conclusion The amount of fecal bacteroides of AIDS patients are significantly higher than those of the healthy control. These data indicate that the gut microflora of AIDS patients was disordered.
4.Study on Pharmaceutical care mode of the patients with chronic disease

Objective To investigate the medical and pharmaceutical knowledge of patients with chronic diseases and analyse the influence factors of rational administration in patients ,to provid data to support the establishment of pharmaceutical service mode . Methods 386 cases of patients with chronic diseases were asked to finish the questionnaires for the medical and pharmaceutical knowledge ,and factors affecting the rational drug were analyzed by single factor and multiple factors Logistic regression analysis . Results Among the 386 patients ,cardiovascular and celebralvascular disease ratio was the highest(53 .3% ) ,followed by respiratory system diseases(13 .8% ) and the musculoskeletal system diseases (11 .50% );The averaged score of 386 patients was 1 .76 ± 0 .78 , medication knowledge was at a general level;single factor analysis results showed that there was significant difference(P<0 .05) between rational drug-use and abuse of drugs among patients in number ,form of payment ,marital status ,income ,education level , taking drug knowledge lectures ,combined treatment .Multivariate Logistic regression analysis showed that education level ,partici-pation in lectures ,drug combination ,disease species had a significant impact on the rational drug use among patients with chronic disease(P<0 .05) .Conclusion The pharmaceutical knowledge that patients with chronic disease mastered is unsatisfactory ;and unreasonable behavior of medication is common scence .Education level ,participation in lectures ,drug combination ,the number of diseases have great influence on the rational use of drugs in patients with chronic diseases .A kind of effective pharmaceutical service mode should be established for patients with chronic diseases by clinical pharmacists .This is a very meaningful work for rational ad-ministration .
5.Rapid Surface-Enhanced Raman Spectroscopic Detection of Blood Cyanide Based on Pinhole Shell-Isolated Nanoparticleand Online Lysis-Purging and Trapping Approach

A rapid and simple method for the determination of cyanide in blood was developed based on pinhole shell-isolated nanoparticles (pinSHINs)-enhanced Raman spectroscopy and an online lysis-purging and trapping approach.In the online lysis-purging and trapping device, the bound cyanide in blood can be cleaved through sulfuric acid acidification, and transferred into HCN volatile gas, then purged into alkaline solution to form NaCN solution, thus high-efficient liberation and entrapment of cyanide from the methemoglobin-bound form can be achieved.The pinSHINs substrate is quite stable to weaken the gold-dissolution effect caused by cyanide under strong alkaline condition, and therefore the detection window can be prolonged to 1 h comparing with 5 min of AuNPs.A limit of detection down to 10 μg/L and a linear range from 100-2000 μg/L in blood were achieved in this method.This method was further applied to rapid measurement of blood samples of cyanide exposed rats and clinic poisoned patients, which provided a sensitive, selective and reliable way for rapid detection of cyanide poisoning.
6.Revision functional endoscopic sinus surgery for nasal cavity abnormality

OBJECTIVE To discuss the operative plan for nasal cavity abnormality and evaluate the effect of revision FESS. METHODS A retrospective study of 36 revision FESS performed during 2002.3-2005.5 (age from 17 to 55 years old) is presented. Patients were evaluated with endoscopic examination of the nasal cavities and computed tomography of the sinuses and nasal cavities. Information was also collected during the revision surgery. The plan was designed according to nasal cavity abnormality. RESULTS The reasons of revision FESS: Among 36 cases there are 24 ethmoid sinusitis, 16 cases with recurrence of polyps,13 middle meatal stenosis, 11 middle turbinate adhesion, 10 septal deviation, 5 frontal recess stenosis, 5 sphenoid sisusitis, 3 maxillary ostium obstruction,3 inferior turbinate hypertrophy. Follow up for 6 month~43 month after revision FESS, 22 cases were successful, 10 satisfied and 4 inefficacy. CONCLUSION The primary FESS failure was associated with nasal cavity structure diseases. Meticulous attention in these areas and correcting it during surgery may reduce recurrence and ensure operative effect after revision FESS.
7.Investigation on safety limits of traditional Chinese medicine containing endogenous cyanide glycosides as toxic ingredients

Objective To evaluate the safety limits of amygdalin in the Chinese patent medicines and Chinese herbal medi-cines,so as to provide reference for establishing the standard in the safety limits of endogenous cyanide glycosides as toxic ingredients. Methods The toxicity differences between D-amygdalin and L-amygdalin were explored by CCK-8 assay. The daily safe dose of amyg-dalin was deduced by the no-observed-adverse-effect level(NOAEL)of cyanide. Based on the deduced results,the safety limits of amygdalin in thirty-six Chinese patent medicines and three different kinds of Chinese herbal medicine were calculated. Results The half maximal inhibitory concentration of D-amygdalin and L-amygdalin was(3.5±2.0)and(6.6±0.4)mmol/L,respectively,and the daily safe dose of amygdalin was 104.30 mg. Conclusion The toxicity of D-amygdalin was approximately two times that of L-amygda-lin. To establish the safety limits of amygdalin,the sum content of the epimer could be regarded as the D-amygdalin itself to enlarge the safety scope. The content of amygdalin of three Chinese patent medicines was determined to exceed the maximum safety limit for all measured batches,indicating some kind of safety risk. Moreover,in the researches on safety limit establishment,the toxicity-alleviat-ed processing effect should be included.
8.Nutrition Therapy in Crohn’s Disease

Nutrition therapy is an important part of the treatment of Crohn’s disease(CD),which plays an important role in correcting malnutrition,inducing disease remission,and for the perioperative management. Nutrition therapy, especially enteral nutrition(EN),has been studied for its effect on CD. We have long been focusing on the use of nutrition therapy in the treatment of CD,especially the use of EN in the perioperative management of CD. Here,we reviewed the indications,effects,endpoints and the underlying mechanisms of preoperative nutrition therapy,through which we hope can shed light on improving the optimization of preoperative management as well as the standardization of preoperative nutrition therapy in CD.
9.Risk Factors for Postoperative Intra-abdominal Septic Complications in Crohn ’s Disease

Background:As postoperative intra-abdominal septic complications( IASCs)in Crohn’s disease( CD)are difficult to manage,it is of great importance to prevent this condition in CD patients after surgery. Till now,there are no large sample studies on risk factors for postoperative IASCs in CD in China. Aims:To determine the risk factors for postoperative IASCs in CD for guiding the formulation of preventive strategies. Methods:This retrospective study was based on a computerized database of CD patients who had undergone surgery for CD complications between 1999 and 2014 at Nanjing General Hospital of Nanjing Military Command,PLA. Patients were divided into IASCs group and non-IASCs group. Thirty potential variables were selected,and both univariate and multivariate( Logistic regression)analyses were performed to identify the risk factors for IASCs after surgery. Results:Seven hundred and sixteen operations were reviewed,and IASCs occurred in 41 cases(5. 7%). By univariate and multivariate analyses,IASCs were significantly associated with one stage anastomosis(OR=1. 656,95% CI:1261-3. 279),preoperative low albumin level( <30 g/L)(OR=1. 457,95% CI:1. 152-2. 368),preoperative high CRP level( >10 mg/L)(OR=8. 641,95% CI:3. 376-16. 364),preoperative steroids use ≥3 months(OR=3. 785,95% CI:1. 237-4. 671)and presence of intra-abdominal abscess or infection at the time of surgery(OR=1. 784,95% CI:1. 155-3. 826). However,enterostomy(OR =0. 125,95% CI:0. 062-0. 561)and preoperative enteral nutrition ≥ 1 month( OR =0. 147,95% CI:0. 078-0. 781 ) were found to be the independent protective factors. Conclusions:Malnutrition,active CD and preoperative long-term steroids use increase the risk of postoperative IASCs in CD. Patients with these risk factors should not receive immediate surgery. If surgery is inevitable, enterostomy instead of resection and anastomosis should be the first choice. Preoperative enteral nutrition is helpful for reducing the occurrence of IASCs after surgery.
10.Effect of aescuven forte treatment on the postoperative complications of breast cancer

Objective To investigate the treatment effect of aescuven forte on the postoperative complications of breast cancer.Methods One hundred and twenty patients with breast cancer radical surgery were randomly divided into control group(n =60) and treatment group(n =60).Patients in control group were given postoperative routine treatment,while in treatment group were administrated aescuven forte pills at 300 mg orally,2 times/day for 4 weeks beside the conventional treatment.Results (1)After the 1st,2nd weeks therapy,the flap congestion disappear rate in the treatment group were 80.0% (48/60) and 93.3% (56/60),better than that in the control group 60.0% (36/60) and 71.6% (43/60),the difference was statistically significant (P < 0.05).However this trend was not seen in third weeks treatment (P > 0.05).(2) The total efficiency of remission of upper limb edema was 93% (56/60),higher than that in control group 77% (46/60),the difference was statistically significant(x2 =5.17,P < 0.05).(3) Visual analogue scale(VAS) pain score in treatment group were decreased form (8.87 ±0.74) in before treatment to (3.21 ±0.92) at after treatment.And the VAS score in control was from (8.91 ±0.85) down to (4.87 ± 1.34),the difference was statistically significant (P < 0.05).Moreover VAS score in treatment group was lower than that in control group (P < 0.05).(4) There was no adverse effect of the medication process.Conclusion Aescuven forte showed a ability to reduce congestion disappear time of breast cancer and shorten the recovery time of upper limb swelling and pain and other symptoms.
