1.Correlation between serum lipoprotein(a) level and carotid artery atherosclerosis in young and middle-aged patients with ischemic stroke: a retrospective case series study

Objective To investigate the risk factors for carotid artery atherosclerosis in young and middle-aged patients with ischemic stroke and the effect of serum lipoprotcin(a) [(Lp (a)] level on young and middle-aged patients with ischemic stroke.Methods Patients with ischemic stroke between the ages of 18 to 55 were enrolled.Carotid artery ultrasonography was used to evaluate the degree of carotid artery atherosclerosis,and the concentrations of serum total cholesterol,triglycerides,high density lipoprotein cholesterol,low density lipoprotein cholesterol,very low density lipoprotein cholesterol,apolipoprotein A1,apolipoprotein B and Lp(a) were detected.According to the results of carotid ultrasound,the patients were divided into groups of without atherosclerosis,plaque without stenosis,and carotid artery stenosis.The demographic and clinical features were compared among the 3 groups.Multivariate logistic regression analysis was used to identify the independent risk factors for carotid artery atherosclerosis in young and middle-aged patients with ischemic stroke.Results A total of 106 patients with ischemic stroke (n =50 in the without atherosclerosis group,n =44 in the plaque without stenosis group,n =12 in the carotid artery stenosis group) were enrolled in the study.The constituent ratios of age (45.98 ±7.12,50.07 ±4.79,and 50.92 ± 1.83 years,respectively; F =7.169,P =0.001),hypertension (26.0%,47.7% and 58.3%,respectively; x2 =6.862,P =0.032),diabetes mellitus (22.0%,45.5% and 66.7%,respectively; x2 =10.729,P =0.005),hyperlipidemia (24.0%,40.1% and 75.0%,respectively; x2=11.372,P=0.003) and smoking (34.0%,61.4% and 75.0%,respectively;x2 =10.93,P=0.006),as well as the serum high-density lipoprotein cholesterol (1.03 ± 0.26,0.95 ± 0.26 and 0.76 ± 0.08 mmol/L,respectively; F=5.882,P =0.004) and Lp (a) (0.108 ± 0.044,0.155 ± 0.028 and 0.200 ± 0.011 g/L,respectively; F =41.556,P =0.000) levels had significant differences in the 3 groups.Multivariate logistic regression analysis showed that age > 48 years (plaques without stenosis:odds ratio [OR] 2.89,95% confidenee interval [CI] 1.20-6.96,P =0.018; carotid artery stenosis:OR 4.43,95% CI 1.19-16.57,P =0.027),hypertension (plaque without stenosis:OR 2.60,95 % CI 1.09-6.18,P =0.031; carotid artery stenosis:OR 3.99,95% CI 1.08-14.77,P =0.039),diabetes (plaque without stenosis:OR 2.96,95% CI 1.21-7.23,P=0.018; carotid artery stenosis:OR 7.09,95% CI 1.79-28.02,P =0.005),hyperlipidemia (plaque without stenosis:OR 2.19,95% CI 0.91-5.31,P =0.082; carotid artery stenosis:OR 9.50,95% CI 2.21-40.86,P =0.002),smoking (plaque without stenosis:OR 3.08,95% CI 1.33-7.16,P =0.009; carotid artery stenosis:OR 5.82,95% CI 1.39-24.38,P =0.016),and Lp (a) (plaque without stenosis:OR 4.38,95% CI 1.76-10.90,P=0.001; carotid artery stenosis:OR 12.80,95% CI 2.73-52.67,P =0.001) were the independent risk factors for carotid artery atherosclerosis in young and middle-aged patients with ischemic stroke.Conclusions Age,smoking hypertension,diabetes and Lp(a) were the independent risk factors for carotid artery atherosclerosis in young and middle-aged patients with ischemic stroke.
2.Fasting plasma preptin levels and relevant factors in the subjects with different glucose tolerances

Fasting plasma preptin level was assayed by RIA in the patients with different glucose tolerances. Regarding fasting plasma preptin levels, it was higher in females than in males. Fasting plasma preptin level was increased in patients with type 2 diabetes as compared with subjects of impaired glucose tolerance and normal controls, and there were positive correlations with diastolic blood pressure, blood triglyceride, total cholesterol, high-density lipoprotein-cholesterol, free fatty acids, 2 h plasma glucose after glucose load, HbA1C and HOMA-IR, suggesting a potential link between preptin and glucose-lipid metabolism and insulin resistance.
3.Expression of nuclear factor kappaB, MCP-1 and RANTES concentration in peritoneal fluid in women with endometriosis

Objective To compare the concentration of nuclear factor-kappa B (NF-κB), MCP-1 and RANTES in peritoneal fluid in women with endometriosis and normal peritonal fluid ,To explore the relationship between these cytokines with pathogenesis of endometriosis. Methods There were 2 groups:the endometriosis group and the control group.Double-antibody sandwich ABC-ELISA technique was adopted for determinating concentration of NF-кB、MCP-1 and RANTES in peritoneal fluid. Results Peritoneal fluid level of NF-κB MCP-1 and RANTES in endometriosis patients was higher than the control group (P < 0.05). NF-κB and MCP-1, NF-κB and RANTES expressed positive correlation by linear correlation analysis in both EMs group and control group , correlation coefficients were 0.385, 0.569(EMs group), 0.474, 0.388(P < 0.05). Conclusions NF-кB、MCP-1 and RANTES are mutual affection and promote endometriosis in the pathogenesis.
4.Analysis of the mutations of BRCA1 in 9 familiar breast cancer patients

Objective: To analyze the mutations of BRCA1 in 9 Chinese familiar breast cancer patients. Methods: Peripheral blood samples were obtained from 9 patients enrolled from 9 breast cancer families, one normal control, 32 sporadic breast cancer patients and 33 normal donors. DNA extracted from lymphocytes was amplified by polymerase chain reaction (PCR). The 22 exons and partial introns of BRCA1 were screened by PCR denaturing high performance liquid chromatography (PCR-DHPLC) and confirmed by direct sequencing. Results: Among these 9 familiar breast cancer patients, a deleterious mutation was detected in one case in exon 11 (3870delTGTC) which was a 4 base deletion and caused a frameshift in turn. One novel and unique amino acid substitution (E867R) was detected in one case. Eight patients were detected to have a known variation in intron 18 (IVS18+65G→A), and the ratio of this variation detected was 88.9%(8/9). The ratio of this variation was 37.5%(12/32) in sporadic breast cancer patients or 33.3%(11/33) in normal control. This variation was found to be accompanied all the time with a known missense variation in exon 11 (P871L) and a polymorphism in intron 9 (IVS8 57delT). Those three variants were also detected in homozygous in one case, which implies the linkage of the 3 sites. The linkage had not been reported. Two patients had been found with a known polymorphism in exon 13 (S1436S). Another known polymorphism was found in one case (L771L). In addition, intronic variants (IVS2+48C→T, IVS2+133C→T, IVS12+112C→A) were detected. Conclusion: The mutations of BRCA1 in Chinese familiar breast cancer patients are different from the hot spots reported in Caucasian and Jewish. It is important that further study be conducted to seek for specific mutations of this gene or other possible relevant genes in Chinese familiar breast cancer patients.

Objective To investigate whether Ki-67 and DNA topoisomerase Ⅱ α (Topo Ⅱ α) are effective prognostic markers in patients with primary hepatocellular carcinoma (HCC) after liver transplantation.Methods This retrospective cohort study included 105 patients with HCC who underwent liver transplantation in a single center from 2001 to 2012.The demographic features,clinicopathological data,expressions of Topo I c and Ki-67 as detected by immunohistochemistry.The long-term survival and the potential prognostic factors,together with standard histologic parameters,were analyzed by univariate and multivariate analyses.Results A positive correlation was found between Topo II α and Ki-67 levels in HCC (r = 0.469,P < 0.01).Multivariate analyses showed that Ki-67 was an independent prognostic risk factor of recurrencefree survival (HR = 2.296,P < 0.05).The 5-year overall survival rate was related to tumor size (HR = 1.743,P < 0.05),AFP (HR = 2.291,P < 0.05),histological grade (HR = 0.283,P < 0.01),and high expressions of Ki-67 (HR = 1.977,P < 0.05) and Topo Ⅱ α levels (HR = 1.883,P < 0.05).The KaplanMeier analysis showed that there was a significant difference in the 5-year recurrence-free survival rate (40.4% vs.57.6%) between patients with high and low expressions of Ki-67,which were significantly lower in the high Topo Ⅱ α expression patients (13.5% vs.63.8%) (P <0.01).The 5-year overall survival rates were significantly lower in the high Ki-67 expression patients (12.7% vs.61.1%,P <0.01) when compared with the low Ki-67 expression patients,which were significantly lower in the high Topo Ⅱ α-and Ki-67 expression patients (10.7% vs.54.5%,P <0.01) than the low Topo Ⅱ α-or Ki-67 patients.Conclusions Ki-67 was associated with recurrence and metastasis in patients with primary hepatic carcinoma after liver transplantation.High expression of both Ki-67 and Topo Ⅱ α were associated with poor prognosis in these patients.
7.Analysis of the effect and prognosis of multiple channels percutaneous renal ureteral calculi removal in the treatment of patients with complex renal calculi

Objective To study the effect and prognosis of multiple channels percutaneous renal ureteral calculi removal in the treatment of patients with complex renal calculi.Methods One hundred and forty-four cases patients with complex renal calculi accepted treatment in Dongda Hospital of Shanxian County from June 2012 to March 2015 for research.According to the random number table method,the patients were divided into the observation group and the control group,72 cases in each group.The single channel type was used in the control group,while multi-channel type operation was use in the observation group.Indicators related to the operation,stones clearance rate,changes in the degree of hydronephrosis,and the major complications after treatment of the two groups were compared.Results The operation time of the observation group was (104.82 ±8.54) min,significantly longer than that of the control group ((90.23 ±7.68) min),but the duration of hospital stay,fistulation tube indwelling time and the residual stone surface area were (6.84±1.25) d,(5.23±2.16) d and (223.16± 113.49) mm2 respectively,significantly less than that of the control group ((8.33±2.13) d,(6.74 ± 2.19) d and (429.54 ± 107.88) mm2 respectively),the differences were statistically significant(P =0.000).The stone removal rate of the observation group was 86.11% (62/72),significantly higher than that of the control group(72.22% (52/72)),the difference was statistically significant (P =0.040).There was no significant difference in terms of the total stone clearance rate and the degree of kidney water after treatment between the two groups (P>0.05).The total complication rate of the observation group was 11.11%(8/72),of the control group was 9.72% (7/72),and the difference was not statistically significant(P >0.05).Conclusion The efficacy of multi-channel operation for complex renal calculi is better,safe and feasible,but should reduce the number of channels as much as possible,so as to reduce the damage to patients effectively.

The uptake and retention of ~3H-HPD in.human malignant cells(cell line MGC 80-3,SGC-7901,BEL-7402,Os-732,Detroit-6 and HeLa)and normal fibroblasts were studied by liquid scintillation counting method.The order of cell lines which took up ~3H-HPD after 30 min.incubation were fibroblasts,HeLa, BEL-7402,Detroit-6,SGC-7901,MGC 80-3 and Os-732 respectively according to the amount of ~3H-HPD in cells.Due to the variety in the amount of excretion, however,those cells which retained ~3H-HPD after 24 hours cultivation were Os-732, Detroit-6, BEL-7402,HeLa,SGC-7901,MGC 80-3 and fibroblasts respectively in respect to the ~3H-HPD level in cells.The uptake was reduced by raising extracel- lular pH and lowering temperature as well as adding higher concentration of serum. In order to test the equilibrium solubility of puerarin in different solvents and solubilizer,cilia toxicity and irritation of these excipient, the balance method, toad in the ciliary body toxicity and rat nasal mucosa irritation were used respectively. Results showed that puerarin solubility was 56.44 g x L(-1) in combined solvent of 30% PEG200 and 10% Kolliphor HS 15. With normal saline solution as negative control and sodium deoxycholate as positive control, the effects of 30% PEG200, 30% PEG 400, 10% Kolliphor HS 15 and combination of 30% of PEG200 and 10% Kolliphor HS 15 on toad palate cilium were observed and cilia movement duration was recorded. The results indicated that there was no significant difference in cilia movement duration among 30% PEG200, 10% Kolliphor HS 15 and normal saline group. The rats long-term nasal mucous membrane irritation of 30% PEG 400, 10% Kolliphor HS 15, which had no cilia toxicity, was studied, with normal saline solution as negative control. There were no significant difference revealed on rat nasal mucosa epithelial thickness among 30% PEG 400, 10% Kolliphor HS 15 and normal saline. Above researches showed 30% PEG 400, 10% Kolliphor HS 15 was ideal for solubility of puerarin nasal drops and showed a lower cilia toxicity and irritation, and can be used as the solvent and solubilizer of puerarin nasal drops.

10.Studies on release behavior of sustained release tablets of extracts of Gardenia by antioxidant activity.

Using sustained release tablets of gardenia extract as model drug and DPPH radical scavenging capacity as antioxidant index, the feasibility of using pharmacodynamics index was explored to evaluate sustained release tablets. Applying the established quantifiable method of DPPH radical scavenging to the dissolved liquid of model drug, release profiles and biological effects profiles were drawn, and their correlation was discussed. A good correlation was observed by linear regression and f2 actor, suggesting that the indicator could be used to evaluate sustained release tabletsofextracts of gardenia in which iridoids were mainly involved.
