Objective To study the construction of tissue engineering adipose in vitro with silk fibroin scaffold and human umbilical cord mesenchymal stem cells (hUCMSCs) transfected by recombinant human insulin gene lentivirus.Methods hUCMSCs infected with recombinant lentiviral pLenti6.3-insulin-IRES-EGFP (Group A) by the best MOI=10 were seeded in silk fibroin scaffold; hUCMSCs transfected by EGFP gene (Group B)was regarded as negative control; and then the cell-scaf-fold complexes were cultured in adipogenic differentiation medium. MTT test was used to detect whether recombinant lentiviral affected the growth and proliferation of hUCMSCs and the growth activity of hUCMSCs seeded in silk fibroin.Results After 5-7 days for adipogenic culture,the numbers of fat-like cells in group A were significantly more than those in group B (P =0.007,P＜0.01).RTPCR results showed that the expression of PPARγ-2 in group A was much stronger than that in group B.MTT test showed that there was no significant difference in optical density (A)at each time between transfected group and nontransfected group (P=0.056,P＞0.05).And there was also no significant difference in optical density (A)between cell group and cell-scaffold group (P =0.066,P＞0.05).Conclusions Insulin gene could obviously promote hUCMSCs getting into adipose,and carrying recombinant human insulin gene lentiviral vector transfection of hUCMSCs and silk fibroin scaffolds could effectively construct tissue engineering adipose in vitro.

Objective To analyze MRI manifestations of cerebral sparganosis and the diagnostic value of MRI.Methods MRI findings in 8 patients with proven cerebral sparganosis by pathological examination were reviewed.Results The lesions were hypointense on T1WI and hyperintense on T2WI with edema of brain around the lesions.Most of the lesions were markedly enhanced with ring-type,nodules,tortous beaded shape or tortuous linear like worm past-enhanced scans.The location and shape of the lesions in 3 cases were changed on followed-up MRI.Conclusion There are some specific pathological findings in cerebral sparganosis.MRI is of important clinical value in diagnosis and differential diagnosis of cerebral sparganosis.

OBJECTIVE: To explore a drug supply system to meet radiation accident.METHODS: According to the characteristics of drug utilization for radiation disease,a retrospective analysis was conducted on the drug demand and supply in the course of treating two patients with severe radiation disease.RESULTS: The characteristics of drug supply to meet demand of remedy of acute radiation sickness manifested as a great variety and wide scope of drugs involved and purchase without scheme because of uncertainties of dosages and course of treatment,and the time is urgent,and the drug supply is more difficult.Great importance should be attached to the reserving of drugs,information,capability,and pre-draft in building and operating the drug supply system to meet radiation accident.CONCLUSION: To build a drug supply system to meet radiation accident,reasonable organization and utilization of drugs resources is quite necessary.

Based on fundamental theory, literatures of TCM and clinical practice, we have found that there are some errors in the theory of stroke. We put forward several views on it: Firstly, the key reason of stroke is the hidden pathogenic factor which is phlegm and stasis, but not wind and fire. Secondly, the production of wind is due to the liver-blood injury caused by phlegm and stasis; the production of f ire is due to whether the liver-blood injury caused by phlegm and stasis or the f ire stagnated by phlegm and stasis in pericardium. Thirdly, the disease location of stroke is in the level of spirit, not in channels and collaterals. Liver soul injury leads to paralysis and aphemia. Lung soul injury leads to numbness. Kidney will injury leads to sensory aphasia. Pericardium invaded by phlegm and stasis, mind disorder will exists. Fourthly, spirit injury is a def icient syndrome, and supplementing blood and qi is the key of treatment, and expelling phlegm and stasis, balancing yin and yang are also needed. The main treatment principles of liver soul injury, lung soul injury, kidney will injury and pericardium injury are tonifying blood, cultivating qi, replenishing essence and nourishing both qi and blood respectively.

OBJECTIVE:To study the influencing factors of protocol deviation in drug clinical trial implementation,and pro-vide reference for improving the quality of drug clinical trial. METHODS:Quality verification was conducted for the drug clinical trial projects in the First Affiliated Hospital of Chongqing Medical University during 2010-2016,and protocol deviations in each year were retrospectively studied,classified and analyzed. Category,frequency,international and domestic pilot projects and the differences of protocol deviation after full-time research nurse participating in trail management were explored,and the measures were put forward. RESULTS:27 drug clinical trials were implemented in our hospital during 2010-2016,including 949 cases,176 cases with protocol deviation,accounting for 18.55％. Deviation protocol in drug clinical trial was decreased year by year for 7 years. The categories were mainly incompleteness of observation/records (30.11％),checking omission/broaden the window(28.41％),adverse drug events and improper combined medication (14.20％) and omission in drug management (11.93％). The proportion of protocol deviation with full-time research nurse participated was lower than the projects without full-time research nurse(11.11％ vs. 28.67％,P<0.01),and proportion of deviation protocol in international multi-center project was lower than the domestic projects(6.60％ vs. 28.84％,P<0.01). CONCLUSIONS:It is suggested to pointedly strengthen the weak links of drug clinical trial. For example,clinical trial institutions should establish the clinical trial data retention system,electronic data should be timely backed up in a different places,etc. Besides,clinical trial institutions should equip professional full-time research nurses as much as possible,learn the rigorous scientific experimental design,standard operational procedures and the authenticity of data pro-cessing from the international multi-center clinical trial projects to effectively reduce the incidence of deviation protocol and im-prove the quality of drug clinical trials.

Objective To investigate the morbidity,clinical feature,diagnosis and therapy of ovarian disease in children.Methods Eighty-two children with ovarian disease were admitted and treated in Nanjing children's hospital from Jan.1992 to Jan.2007,were analyzed retrospectively with age,emergency admissions or not,dwell,pathology and method of operation.Results The age of 82 patients ranged from 1 day to 14 years old and the mean age was 6.7 years old.Thirty-one cases(37.8%) were emergency admissions and 51 cases(62.2%) were routine admissions.Twenty-seven cases(32.9%) were rural patients and 55 cases(67.1%) were urban patients.Forty-five cases(54.8%) were nontumorous disorder,31 cases(37.8%) were benign tumor and only 6 cases(7.4%) were malignant tumor.About the morbidity,12 patients(14.6%)were admitted from 1992 to 1996,24 patients(29.5%) from 1997 to 2001 and 46 patients(55.9%) from 2002 to 2007.Chemotherapy were carried out in 6 cases with malignant tumor in internal medicine,2 cases with sexual precosity kept observation,the others were cured.Conclusions Ovarian disease can occur at any age in children.The clinical manifestation is characterized mainly by acute abdomen.The incidence of ovarian disease of children in urban areas is higher than that children in rural areas.The morbidity continues to show an upward tendency and the pathologic manifestations are mostly benign,laparoscopic operation has obviously superiority.

OBJECTIVE: To discuss the potential problems caused by the exfoliation of the tunic albuginea after the low-temperature plasma operation. METHOD: To observe the occurrence of the tunic albuginea, exfolliating time, wound healing and its complications by following the patients received the low-temperature plasma operation from November in 2012 through June in 2013 (including 116 cases of tonsillectomy, 98 cases of adenoidectomy, 255 cases of the ablation of the inferior turbinate,8 cases of the ablation of the cyst of epiglottis, and 4 cases of the ablation of the tumor of larynx) together with reviewing the relevant literatures. RESULT: The tunic albuginea exfolliated about two weeks after tonsillectomy, bleeding was found in 3 cases, one of which was related with inappropriate eating; the exfoliation about two or three weeks after adenoidectomy, without the occurrence of the narrow nostril and pharyngeal cavity adhesion; about four weeks after the ablation of the cyst of epiglottis, bleeding was found in 1 case; about three or four weeks after the ablation of the inferior turbinate, massive bleeding was found in one case after two months; and the ablation of the tumor of larynx, one with severe atypical hyperplasia and carcinoma in situ, the theunic albuginea did not completely exfoliate four moths after the surgery and new creatures could be found, the wound looked smooth and healed well nine moths after the surgery. The full exfoliation occur four months after the surgery for one with the neurilemmoma in the left laryngeal ventricle and the new creatures could also be found. It exfoliated eighth days after the surgry for one patient with left subglottic epitheliaoid heman-gio endothelioma and it did not recur even six months after the surgery. Another one with the cyst of the left plica aryepiglottica failed to be followed. The first three patients did not suffer from such complications as bleeding, endogenous in trachea, etc. CONCLUSION Low-temperature plasma surgery has obvious advantages, but the potential problems caused by the surgery need to be taken in accounted, especially the exfoliating time of the tunia albuginea is different after the ablation of the tumor of larynx, which is inconsistent with what the literatures reported, it could partially due to the action time of low-temperature plasma surgery with surrounding tissues.

Objective To explore the clinical characteristics and the remedial measurement of measles complicated with severe pneumonia and the acute respiratory distress syndrome(ARDS) in children.Methods From Jan 2004 to Jul 2007,clinical data of 32 cases with measles complicated with severe pneumonia and ARDS in PICU of Chongqing Children's Hospital were retrospective analyzed.In addition,the ratio of partial pressure of oxygen in arterial blood fractional concentration of oxygen in inspired gas[pa(O2)/FiO2] between the death group and survival group,the mortality rates between the hormone therapy group and control group were performed.Results Sixteen cases were cured and 16 cases were died,with the mortality rate 50%.The ratio of pa(O2) /FiO2 before mechanical ventilation in death group was lower than that in the survival group [(11.93?1.67) kPa,(21.77?1.16) kPa,respectively,P0.05).Conclusions ARDS is a severe complication in children with measles pneumonia and associated with a high mortality,which requires early diagnosis and intervention to improve survival rate and prognosis.The ratio of pa(O2)/FiO2 may provide reference to evaluation for severity and prognosis of ARDS.
