Objective:To observe effects of Fuzheng Huoxue Recipe on blood growth hormone (GH) and insulin growth factor(IGF-1) in chronic renal failure(CRF) rats.Methods:Platt method was adopted to make 5/6 kidney excision CRF model rat,and the rats were divided into model group,Huangqi group,Enalapril group,Danshen group,Fuzheng Huoxue Recipe group and normal control group according to the blood creatinine level after modeling.Effects of Fuzheng Huoxue Recipe on GH and IGF-1 levels in CRF rats with large quantity of urinal protein were investigated.Results:GH and IGF-1 levels in the model control group were significantly higher than that in normal control group,and after treatment of Fuzheng Huoxue Recipe,blood GH and IGF-1 levels were significantly decreased.Conclusion:Fuzheng Huoxue Recipe can decrease blood GH and IGF-1 levels in CRF rats,and can delay advances of CRF.

Objective: To establish a GC method for the determination of resorcinol, borneol, glycerol and DMSO in compound fluocinonide tincture.Methods: The analysis was performed on an HP-INNOWax column (30.0 m×0.32 mm,0.25 μm).The inlet temperature was 280℃ and the detector temperature was 290℃.The split ratio was 10∶1.With nitrogen as the carrier gas, the flow rate was 2.0 ml·min-1.The temperature was as follows: the initial temperature was maintained at 100℃ for 5 min and raised to 240℃ at a rate of 20℃·min-1, and maintaining for 8 min.The flow rate of hydrogen, air and tail blows was 35, 350 and 25 ml·min-1, respectively.Chennai was used as the internal standard.Results: The concentrations of DMSO, resorcinol, borneol and glycerol were within the range of 99.20-793.60 μg·ml-1 (r=0.999 9), 507.25-4 058.00 μg·ml-1 ,respectively(r=0.999 9), 102.20-817.60 μg·ml-1 (r=1.000 0) and 316.20-2529.60 μg·ml-1 (r=1.000 0) with a good linear relationship.The average recovery was 98.58%, 98.34%, 98.19% and 102.29%, and RSDs were 3.80%, 3.93%, 2.87% and 3.65% (n=9), respectively.Conclusion: The method is simple, accurate and reliable, and can be used for the quality control of DMSO, resorcinol, borneol and glycerol in compound fluocinonide tincture.

Objective To study the impact of pregnancy on differentiated thyroid cancer. Methods Retrospective comparative analysis were made between pregnancy-related thyroid cancer patients ( including nine months before delivery and one year after delivery) and the age-matched non-pregnancy-related thyroid cancer patients treated in the Department of head and neck, Fudan University Shanghai Cancer Center. Results There were 125 cases of pregnancy-related thyroid cancer patients from 1998 to 2008, including 28 cases (22.4%) of neck recurrence and metastasis after the surgery of thyroid cancer, meanwhile there were 691 cases of the agematched non-pregnancy thyroid cancer patients, including 90 cases ( 13% ) of neck recurrence and metastasis after the surgery of thyroid cancer. The rate of neck recurrence and metastasis after the surgery of thyroid cancer in the pregnancy-related group was higher than that in the non-pregnancy related group ( P ＜ 0.05). The maximum diameter of tumor in the pregnancy-related group and the non-pregnancy group is 2. 1 cm and 2.6cm respectively.There were 19 cases of multi-focal cancer ( 19.6% ) in the pregnancy-related group and 58 cases (9. 6% ) in the non-pregnant group, P ＜0.05. There were 23 patients (23.7%) of extrathyroid invasion in the pregnancy-related group and 65 patients ( 10.8% ) in the non-pregnant group, P ＜0.05. There were 67 cases (53.6%) and 35 cases (28%) of the central and lateral cervical lymph node metastasis in the pregnancy-related group respectively and 296 cases (42.8%) and 117 cases (16. 9% )in the non-pregnant group, P ＜0.05. There were 19 cases (15.2%) of extralymph invasion in the pregnancy-related group and 32 cases (4.6%) in the non-pregnant group, P ＜ 0.05. Conclusions The incidence of Extrathyroid invasion, multi-focal cancer, cervieal lymph node metastasis and extralymph invasion of the differentiated thyroid cancer is higher in pregnancy-related patients.Pregnancy may deteriorate the prognosis of thyroid cancer. For patients after the surgery of thyroid cancer, pregnancy may increase the risk of neck recurrence and metastasis. However, pregnancy does not affect the stage of thyroid cancer and the prognosis is still good.

Objective To summarize the experience of diagnosis,treatment and clinical effects of Ni-Ti shape memory alloy embracing fixator for severe chest trauma complicated with multiple rib frac-tures.Methods From January 2009 to June 2013,the clinical data of 148 cases with embracing fixator and 47 cases with non-internal fixation were retrospectively analyzed and compared.Results Internal fix-ation was superior to non-internal fixation in hospital stay,pain perception,mechanical ventilation time, pulmonary function recovery and complications(P<0.05).Conclusion Ni-Ti shape memory alloy em-bracing fixator is better than other conservative therapies in the treatment of multiple rib fractures and it's worthy of spreading.

Objective:To observe the clinical effect of different background volumes of ropivacaine in continuous tibial nerve block of postoperative analgesia after calcaneal surgery.Methods:This study was a prospective,randomized,controlled study.Sixty cases of calcaneal visual analogue scale (ASA)ⅠorⅡundergoing elective surgery were selected and randomly assigned to two groups,thirty cases in each group.The patients received popliteal fossa posterior tibial nerve block using ultrasound guided.The con-tinuous stimulation catheter was inserted after successful position and the 0.2%ropivacaine was injected. The background volumes of the A and B groups were 5 mL/h and 3.2 mL/h.The VAS score,the senso-ry block and motor block of tibial nerve and common peroneal nerve,and the satisfaction of the patients at h 12,h 24 and h 48 were recorded after catheter insertion.Results:The VAS scores at the three time points (h 12,h 24 and h 48)on the two groups of the patients were compared,and the difference was not statistically significant (P >0.05).The difference of the sensory block and the motor block of the tibial nerve at the three time points (h 12,h 24 and h 48)on the two groups of the patients were also compared,and the difference was not statistically significant (P>0.05).The difference of the sensory block and the motor block of common peroneal nerve at h 48 hs was statistically significant,group A was higher than the group B (P<0.05).The difference of the patient satisfaction at the three time points (h 12,h 24 and h 48)on the two groups was not statistically significant (P>0.05).Conclusion:The use of 0.2% ropivacaine with the background volume of 3.2 mL/h in continuous tibial nerve block can pro-vide good analgesia and reduce the incidence of the sensory block and motor block of the common perone-al nerve.

From the view of medical professionals, this paper explored the standards of residents′ competency cultivation in Zhuhai using Delphi method. The research outcome that consisted of 53 factors and 6 dimensions was obtained after two rounds of expert consultation. There are 6 first-level indexes including professionnalism and body-mind quality, clinical practice skill, the competency of lifelong learning and self-improvement, humanistic medicine practice skill, medical knowledge, and the competency of promoting the medical system to develop. As well, there are 18 second-level indexes and 29 third-level indexes. The standards stated that the clinical prac-tice skill would not be the main part of residents′competency cultivation. Qualified residents should reach the na-tional standard of the clinical practice skill but also the other five indexes.

Objective To evaluate retroperitoneal laparoscopic adrenalectomy. Methods From June of 1999 to January of 2001, retroperitoneal laparoscopic adrenalectomy was performed on 52 patients with adrenal diseases, including 34 cases of aldosterone-preducing adenoma,3 nodular hyperplasia bilateral in 1, 5 Cushing's syndrom,1 Cushing's disease, 3 adrenal pheochromacytoma (bila- teral in 1), 4 nonfunctional adrenal adenoma, 1 myelolipoma and 1 metastasis carcinoma. Results 54 sessions of retroperitoneal adrenalectomy have been carried out for 52 patients (2 on both sides) with success in 51 procedures. The procedure was converted to open operation in 3 occasions because of bleeding or adhesion. The mean operation time was 135 minutes (40～270 min), and the estimated blood loss 45 ml (5～150 ml) with no need of transfusion. The postoperative hospital stay was 6 d (3～14 d) and the mean analgesia consumed 8.5 mg (0～50 mg) of morphine equivalents with no need of any analgesic at all in 17 patients. Conclusions Retroperitoneal laparoscopic adrenalectomy was less traumatic to the patients, with less postoperative discomfort and quicker recovery. The procedure should be considered as the first choice of therapy for benign adrenal diseases.
