1.Diagnostic value and influencing factors of 11C-PIB in mild cognitive impairment and Alzheimer′s disease

Objective:To investigate the diagnostic value of 11C-Pittsburgh compound B (PIB) in patients with mild cognitive impairment (MCI) and Alzheimer′s disease (AD) and explore the factors that may affect the binding of 11C-PIB. Methods:From January 2017 to December 2019, the 11C-PIB uptake of 6 patients with normal cognitive (NC; 3 males, 3 females, age: (64.5±12.3) years), 11 patients with MCI (4 males, 7 females, age: (64.5±9.8) years) and 21 patients with AD (7 males, 14 females, age: (68.1±9.1) years) from Daping Hospital, Army Medical University were retrospectively analyzed. Regional 11C-PIB binding was assessed by using standardized uptake value ratio (SUVR) and visual reading of 11C-PIB scan. Clinical data, including age, gender, education level, cognitive impairment, neuropsychological scale score, vascular risk factors (VRF), apolipoprotein E (ApoE) gene, were collected and differences among groups were analyzed by using one-way analysis of variance, least significant difference t test or Fisher exact test. Factors that affected the 11C-PIB binding were analyzed by multiple linear regression. Results:SUVR of cerebral lobe among NC, MCI and AD groups were significantly different (range of mean SUVR: 1.16-1.26, 1.19-1.35 and 1.40-1.61; F values: 5.331-9.279, all P<0.05). For positive PIB patients, SUVR of posterior cingulate and precuneus were increased in MCI group compared with NC group (1.20±0.15 vs 1.50±0.12, 1.18±0.15 vs 1.59±0.13; F values: 6.389 and 10.668, t values: -2.33 and -3.10, both P<0.05), and there were no significant differences in all lobes between MCI and AD group ( t values: from -1.29 to -0.51, all P>0.05). Visual analysis showed that the positive rates of PIB in frontal lobe (85.7%(18/21)), posterior cingulate (85.7%(18/21)), precuneus (81.0%(17/21)), temporal lobe (81.0%(17/21)) and occipital lobe (47.6%(10/21)) in AD were higher than those in MCI (4/11, 4/11, 4/11, 3/11 and 1/11, respectively; all P<0.05). Multiple linear regression showed that the degree of cognitive impairment were independent risk factors for SUVR of all lobes ( b values: 0.377-0.536, all P<0.05). The ApoE ε4 gene was independent risk factor for SUVR of precuneus ( b=0.290, P<0.05). Conclusion:11C-PIB is helpful for clinical diagnosis of MCI and AD patients and the degree of cognitive impairment and ApoE ε4 gene may be independent risk factors for increasing 11C-PIB binding.

Obesity is associated with many respiratory diseases, especially sleep-related breathing disorders including obstructive sleep apnea-hypopnea syndrome, obesity hypoventilation syndrome, and overlap syndrome. This article reviews the association between obesity and these sleep-related breathing disorders.

3.Research on the mechanism and security of MDR1 gene transfection into the bone marrow mononuclear cells of rabbit with adenovirus vector promoted by ultrasonic microbubble

Objective To explore the mechanism and safety of miltidrug-resistance gene 1(MDR1)transfection into the bone marrow mononuclear cells of rabbit in vitro with the adenovirus vector promoted by ultrasonic microbubble.MethodsBone marrow mononuclear cells of rabbits were collected and divided into 5 groups after cultured in the 6-well plate according to the different experimental conditions(MDR1 gene was transferred into the cells with or without ultrasound irradiation and microbubbles):conventional culture group (A),Ad5-MDR1 group(B),Ad5-MDR1+ultrasound irradiation group(C),Ad5-MDR1+microbubbles group(D),Ad5-MDR1+ultrasound irradiation+microbubbles group(E).The positive transfection rate of MDR1 gene in mononuclear cells of different groups were tested by flow cytometry,and the survival rate of cells in different periods were tested by trypan blue exclusion method.Moreover,the appearance and ultramicrostructure of cells were observed by electronmicroscope.Results①The transfection rate of MDR1 gene in different groups were 0.39%±0.11%,5.03%±0.35%,4.93%±0.38%,5.25%±0.80%and 19.93%±1.51%respectively.The transfection rate of MDR1 gene in group E was higher than those in other groups(P<0.05).②Compared with those in control groups(group A,B,C and D),the transfection rate in group E was significant raised by ultrasound irradiation and microbubbles.However,there were no significant difference in survival rate of cells between the five groups(P>0.05).③After ultrasonic irradiation,there were transient holes on the cell membrane,which could disappear after irradiation by ultrasound for 24 hours.And the temporary swelling of organelles was reversible.Conclusions Microbubbles irradiated by ultrasound can cause small transient holes on eell membrane and increase permeability of it,and enhance the transfection of MDR1 gene in bone marrow mononuclear cells with the adenovirus vector,which is safe and available.
4.Relationship of tumor necrosis factor gene polymorphism and acute graft-versus-host disease after unrelated ailogeneic hematopoietic stem cell transplantation

Objective To explore the relationship between tumor necrosis factor (TNF) gene polymorphisms in donors and recipients and the incidence and severity of acute graft-versus-host diseases (aGVHD) after unrelated allogeneic hematopoietic stem cell transplantation (alIo-HSCT). Methods Single nucleotide polymorphisms (SNPs) of TNFα-238 (G/A), TNFα-857 (C/T), TNFα-863 (C/A), TNFα-1031 (T/C), TNFβ + 252 (A/G) were analyzed by Multiplex SNaPshot analysis in 76 pairs of donors and recipients. Results Transplantation involving donors with TNFα-857 CC genotype resulted in a higher incidence of grade Ⅱ-Ⅳ aGVHD than donors with CT genotype (91.3% vs 8. 7% , P =0. 039). In the 23 patients with grade Ⅱ-Ⅳ aGVHD, no patients had TNFβ +252 AA genotype, 19 (82.6%) had GA genotype and 4 (17.4%) had GG genotype. There was a significant difference in the distribution pattern of the TNFβ +252 (AA, GA and GG) genotypes in these patients (P =0.03). There was no significant association of TNFα-238 (G/A), TNFα-863 (C/A) and TNFα-1031 (T/C) polymorphisms with the risk of aGVHD. Conclusion These results suggest donor TNFα-857 CC genotype is related to a higher incidence of grade Ⅱ -Ⅳ aGVHD, and patients with TNFβ +252 AA genotype have protection against the risk of grade Ⅱ -Ⅳ aGVHD.
5.Comparison of subgluteal and sub-subgluteal-fold approach for ultrasound-guided sciatic nerve block

Objective To compare the effects of subgluteal(SG) and sub-subgluteal-fold(SSGF)approach for ultrasound-guided siatic nerve block. Methods One hundred forty-eight patients undergoing lower limb surgery were randomly divided into two groups to receive SG approaches and SSGF approaches to sciatic nerve block under real time ultrasound guidance. A combined posterior lumbar plexus block under ultrasound guidance was performed for sufficient surgery anesthesia. 20 ml of 0. 5% ropivacaine was used for sciatic nerve and lumbar plexus block separately. Measurements included skin-to-nerve distance,reorientation of the needle during block and execution time,rates of sensory and motor blockade after 15 min and 30 min of injection, quality of surgery blockade, duration of the sensory and motor block, and postoperative complications related to sciatic nerve block. Results In SSGF group, execution time and reorientation of needle for sciatic nerve block was significantly less than those of the SG group( P <0.01).But motor blockade in the SG group was quicker when compared with SSGF group ( P <0.01). There were no significant differences in the quality and duration of blockade between the two groups. Conclusions Both SG and SSGF approach can be used for sciatic nerve block with equal sensory and motor block rate,whereas sciatic nerve block via SSGF approach was faster and easy to perform than the SG one.
6.Recombinant human endostatin improves tumor vasculature and alleviates hypoxia in Lewis lung carcinoma

Objective To investigate whether recombinant human endostatin can create a time window of vascular normalization prior to vascular pruning to alleviate hypoxia in Lewis lung carcinoma in mice. Methods Kinetic changes in morphology of tumor vasculature in response to recombinant human endostatin were detected under a confocal microscope with immunofluorescent staining in Lewis lung carcinomas in mice. The hypoxic cell fraction of different time was assessed with immunohistochemical staining . Effects on tumor growth were monitored as indicated in the growth curve of tumors . Results Compared with the control group vascularity of the tumors was reduced over time by recombinant human endostatin treatment and significantly regressed for 9 days. During the treatment, pericyte coverage increased at day 3, increased markedly at day 5, and fell again at day 7. The vascular basement membrane was thin and closely associated with endothelial cells after recombinant human endostatin treatment, but appeared thickened, loosely associated with endothelial cells in control tumors. The decrease in hypoxic cell fraction at day 5 after treatment was also found. Tumor growth was not accelerated 5 days after recombinant human endostatin treatment. Conclusions Recombinant human endostatin can normalize tumor vasculature within day 3 to 7, leading to improved tumor oxygenation. The results provide important experimental basis for combining recombinant human endostatin with radiation therapy in human tumors.
7.Simultaneous determination of 3 kinds of components in Gardenia by high-performance liquid chromatography under different UV-vis wave length.

AIMTo determine 3 kinds of biologically active components (9 compounds) in Gardenia, namely geniposidic acid, gentiobioside, gardenoside, genipin geniposide, chlorogenic acid, crocin 1, crocin 2, crocin 3 and crocetin simultaneously and quantitatively.
METHODSAn HPLC method detected by 3 different UV-vis waves--240 nm for geniposidic acid, geniposide, gardenoside, genipin gentiobioside, 330 nm for chlorogenic acid, 440 nm for crocin 1, crocin 2, crocin 3 and crocetin respectively, has been developed: Phenomenex Luna C18 column(250 mm x 4.6 mm ID, 5 microns) with linear gradient elution using methanol-acetotrile (9:1) and 0.3% aqueous formic acid.
RESULTSThis method successfully determined 9 compounds in Gardenia (from 5 different areas) simultaneously and quantitatively.
CONCLUSIONThis is a more reasonable and credible quality control method for the Chinese traditional medicine.

8.Pattern recognition applied to the fingerprint of traditional Chinese medicine characterized by two-dimension information data.

AIMTo establish the two-dimension information fingerprints chromatograph of traditional Chinese medicine and investigate via pattern recognition.
METHODSHierarchical cluster was applied to fingerprints described by one-dimension information data and by two-dimension information data respectively and corresponding results was compared.
RESULTSThe gross classes, in which the samples denoted by two-dimension information data, classified by all kinds of hierarchical cluster methods had less differences and more robustness than the methods containing the samples expressed by one-dimension information data. The classes to which the unknown samples would be belonged were determined by hierarchical cluster analysis and artificial neural network (ANN), and the same results were obtained.
CONCLUSIONThe fingerprints characterized by two-dimension information data could provide more overall and special information as compared with the methods indicated by one-dimension information data.

9.Research on bone marrow mesenchymal stem cells in the treatment of severe acute pancreatitis

Objective To investigate the mechanism of acute inflammatory response and tissue repair when rats accepted transplanted bone marrow mesenchymal stem cells (MSC) in severe acute pancreatitis (SAP).Methods A total of 40 rats were randomly divided into 4 groups which included the normal group (n=10),the model severe acute pancreatitis group (n=10),the transplanted bone marrow mesenchymal stem cells group (n =10),and the combination of bone marrow mesenchymal stem cells and granulocyte colony-stimulating factor (G-CSF) group (n=10).To cure the acute severe pancreatic injury caused by SAP,rats were injected with EDU-labeled MSCs and granulocyte colonystimulating factor (Gr-CSF).The conversion rate of MSCs to pancreatic cells or MSCs to endothelial cells was detected to assess the role of MSCs in tissue regeneration and repair.The expression of amylase,interleukin-6 (IL-6),and interleukin-10 (IL-10) in serum was detected to assess the role of MSCs in an acute inflammatory response.Results The concentrations of amylase and IL-6 were reduced and the concentration of IL-10 was increased in MSC and MSC+G-CSF groups after the onset of SAP.Flow cytometry showed a small amount of MSCs converting to endothelial or pancreatic cells,but the conversion rate was relatively low.Conclusions MSCs can play an important role in the antipre-release of inflammatory mediators,reducing acute immune response to control the acute inflammatory response in SAP.Moreover,MSCs can repair a damaged pancreas by converting into both pancreatic and endothelial cells.
10.Endovascular graft exclusion of aortic dissection

Objective:To explore the operative indications, methods, curative effects and clinical prospect of Endovascular Graft Exclusion (EVGE) for descending thoracic aortic dissection (TAD). Methods:Under dynamic supervision of DSA, an EVGE with a stent graft complex of 130 44 mm was performed successfully on a 75 year old man with an aortic dissection (DeBakey ⅢB type). Results:Following the operation, DSA revealed that the intimal tear of the dissection was excluded by the endoluminal stent graft complex and the false lumen disappeared. The postoperative one week and one month, Duplex scan and CTA revealed that the stent graft complex was patent and had an inner diameter of 36 mm, without any migration and torsion. The false lumen of dissection was full of thrombi. The patient was discharged in a week. One month postoperative follow up showed that the patient was recovered to his usual normality. Conclusion:The indications for EVGE are DeBakey ⅢB type aortic dissections with the intimal tear, EVGE is a minus invasive method with a firmly curative effects, its clinical prospect will be very bright.
