1.Classification and Pathological Analysis of Immunophenotype of 76 Cases of Non-Hodgkin Lymphoma in Shenyang

Objective: Our aim was to discuss the pathological characteristics and immunophenotype of non-Hodgkin lymphoma (NHL) in Shenyang. Methods: Histopathological observation was performed with immunohistochemical methods (SP method). We used 10 antibodies as B,T markers to analyze 76 cases of NHL according to the new WHO classification. Results: The B-NHL was more prevalent than the T-NHL. In B-NHL, the Diffuse Large B cell Lymphoma (DLBL) was the most common, next was the mucosa-associated lymphoid tissue lymphoma (+/-monocytoid B-cells) and lymphoplasmacytic lymphoma (LPL). In T-NHL, the peripheral T cell lymphoma unspecificly accounted most. The positivity rates of CD79a in B-NHL were 100% and its cross-reactivity was 0. The straining rates of polyclonal CD3 with T cell were 88% and CD3 only reacted with 5% of B-NHL. Conclusion: The NHL in Shenyang was marked by the most common DLBL and peripheral T cell lymphoma, unspecificly. The rates of monocytoid lymphoma and LPL were higher. The CD79a and CD3 were antibodies of B-cell and T-cell markers with high sensitivity and specificity, respectively.
2.Detection of myocardial coronary flow reserve of syndrome X by real-time myocardial contrast echocardiography

Objective To evaluate myocardial coronary flow reserve of syndrom X by real-time myocardial contrast echocardiography(MCE). Methods Ten patients with syndrome X and seven normal subjects were involved in the study. Real-time MCE was performed with acoustic contrast SonoVue,and the peak video density (A), and re-turgor velocity of microvessel (β), and the product of A ×β of quiescent condition and after adenosin loading were detected,and also the coronary flow reserve (CFR, the ratio of A xβ circa-adenosin loading). Results There was no significant difference of A between syndrom X and contrast group in quiescent condition, the β and product of A x β of patients with syndrome X were lower than those of contrast group, the CFR of syndrome X was obviously less than that of contrast group.Conclusions Myocardial microvessel function is abnormal in syndrome X, real-time MCE is useful for evaluate myocardial coronary flow reserve.
4.The value of heart-type fatty acid binding protein in the early diagnosis of acute myocardial infarction in elderly patients

Objective To investigate the diagnostic value of heart-type fatty acid binding protein (H-FABP) versus cardiac trofonin I (cTnI) and creatinekinase-MB (CK-MB) in early diagnosis of acute myocardial infarction (AMI) in elderly patients.Methods 67 patients with acute chest pain were selceted sequentially and divided into AMI group (n=30) and non-AMI group (n=37).Plasma H-FABP level was rapidly detected by using colloidal gold reagent plate and solid phase immunochromatographic assay for qualitative determination within and after 6 hours of AMI onset.Plasma levels of cTnI and CK-MB were determined within and after 6 hours of onset.The diagnositic value of H-FABP,cTnI and CK-MB in AMI was compared within and after 6 hours of onset.Results The sensitivity of H-FABP was better than that of cTnI and CK-MB within 6 hours of onset (93.3% vs.46.6%,23.3%,both P<0.05).The negative predictive value of H-FABP was better than that of cTnI and CK-MB within 6 hours of onset (94.7% vs.69.8%,61.1%,both P< 0.05) While,positive predictive value and specificity were basically the same between H-FABP,versus cTnI and CK-MB.H-FABP and cTnI levels had significant differences between AMI and non AMI group after 6 hours of onset (all P<0.05).Plasma levels of cTnl and CK-MB were higher after 6 hours than within6 hours [cTnI (4.10±1.79) mg/L vs.(1.45±1.31) mg/L,CK MB(180.52± 158.70) U/L vs.(20.02± 7.97) U/L,both P<0.05].Conclusions As compared with cTnI and CK-MB,within 6 hours after AMI onset,H-FABP as a new myocardial necrosis marker has higher sensitivity,specificity,positive and negative predictive values in the diagnosis of AMI.While,after 6 hours of AMI onset,H-FABP has the same diagnostic value as cTnI and CK-MB.
5.Application of six sigma in processing of non-coated surgical instrument

Objective To apply six sigma management method to optimizing flow,enhancing treatment quality,prolonging service life and decreasing purchase cost of the non-coated surgical instrument.Methods The treatment flow of the noncoated surgical instrument was optimized through five steps of six sigma method including defining,measuring,analyzing,improving and controlling.The treatment results before and after flow optimization were compared,analyzed and evaluated.Results The rates for corrosion and rejection were both decreased significantly after flow optimization,while the quality was increased obviously (P<0.05).Conclusion Six sigma management method involved in the treatment of the non-coated surgical instrument saves purchase cost,enhances the efficiency of disinfection supply room and facilitates effective supervision and monitoring.

This article made a summarization and analysis on acupuncture related empirical and clinical research literature in Science Citation Index (SCI) periodicals in 2009.Authors used acupuncture respectively as the key word to retrieve in SCI periodicals in 2009 through Web of Science and got 630 articles.The articles were summarized and analyzed on experimental design, experimental research content, disease category, acupuncture accident and systematic review of clinical effect from native and abroad.

ObjectiveTo analyze literature on acupuncture-related adverse events published in periodicals in Science Citation Index (SCI) and Medllars Online(Medline).MethodsWe searched the Web of Science database and the Medline database to identify articles about the safety of acupuncture therapy.Case reports,case series,surveys and other observational studies were included if they reported factual data,but review articles,translations and clinical trials were excluded.ResultsFindings:The inclusion criteria were met by 232 articles that in total reported on 389 cases of adverse events after acupuncture.Conclusion Acupuncture therapy is relatively safe,most acupuncture-related adverse events can be avoided by standardized clinical practice.Establishment of acupuncture medical safety standard system is extremely urgent.
8.A study on the timing of liver transplantation and factors influencing prognosis in patients of acute liver failure

Objective To explore the optimal timing of liver transplantation for acute liver failure,and analyze relative risk factors affecting patients' prognosis.Methods We retrospectively analyze perioperative clinical data of 50 patients suffering from acute liver failure who underwent liver transplantation between March 2005 and June 2010 in Organ Transplantation Center,the First Affiliated Hospital of Sun Yat-sen University.Patients were divided into survival group and death group.Clinical data before operation were collected and analyzed by univariate and multivariate analysis,respectively.Risk factors influencing the perioperative mortality were selected and were used to direct the operation time.Results There were 50 patients who underwent liver transplantation and 11 cases died during perioperative phase.Five patients died of respiratory infection,three of acute renal failure,one of digestive tract hemorrhage,one of primary allograft failure and one of multiple organ failure.The perioperative,one-year and three-year survival rate were 78%,74% and 72% respectively.In the univariate analysis,age,bilirubin level,prothrombin time,creatinine level,grade of encephalopathy and MELD score were found to be significantly different between survival group and death group.The multivariate analysis revealed that age≥65 y,INR ≥3.5,creatinine level ≥ 2.5 mg/dl and MELD score ≥ 40 were independent risk factors affecting survival.Conclusions Liver transplantation is an effective treatment of choice for acute liver failure.Appropriate timing of transplantation helps achieve a high survival rate.
9.Effect of Molecular Weight and Substitution Degree of Chitosan-poly-arginine on in vitro Transdermal Pene-tration Enhancement

Objective:To study the effect of molecular weight and degree of substitution (DS) of chitosan-poly-arginine (CS-R9) on transdermal penetration enhancement in vitro. Methods:Low molecular CS, medium molecular CS or high molecular CS was respectively used to synthesize CS-R9 with different molecular weight (LCS-R9-1, MCS-R9 and HCS-R9). Low molecular CS was used to synthesize CS-R9 with various degree of substitution by changing the mole ratio between R9 and CS (LCS-R9-1, LCS-R9-2 and LCS-R9-3). The in vitro transdermal penetration enhancement of the different CS-R9 on tinidazole ( TNZ) was studied using Franz diffusion cells. Results:According to the results of FTIR and 1 H-NMR, a series of target CS-R9 were synthesized including LCS-R9-1 with the DS of 2. 30, MCS-R9 with the DS of 2. 17, HCS-R9 with the DS of 2. 20, LCS-R9-2 with the DS of 8. 05 and LCS-R9-3 with the DS of 15. 87. Compared with the blank control group, Azone group, LCS group, R9 group and LCS+R9 group, LCS-R9-1 could enhance the in vitro transdermal penetration of TNZ significantly (P<0. 05). When the DS was unchanged, LCS-R9-1 and HCS-R9 showed similar enhancement in the first 12h, and the effects were both higher than that of MCS-R9 (P<0. 05). The enhancement of HCS-R9 was decreased during 12-24h, while compared with that of LCS-R9-1, the difference was not notable (P>0. 05). When the molecular weight of CS was unchanged, the effect was increased with the rise of DS in the first 21h, however, after that, the effect was decreased with the rise of DS. Conclusion:Molecular weight and DS both have significant effect on the in vitro transdermal penetration enhancement of CS-R9, and it is valuable to further study the in vivo transdermal penetration enhancement of CS-R9 and underlying mechanisms.
10.Dissection of differences and similarities of botanical drugs in European Union, US and Canada.

Because of the unique nature and treatment concept of traditional Chinese medicines (TCMs), TCMs have been respected again since 70s of last century. The regulatory agencies of some developed countries (the FDA, EMA and Health Canada etc) have published new guidance/guidelines/directives in recent years, such as the botanical drug product guidance of the FDA, the evidence for quality of finished natural health products guidance of the Health Canada and the guideline on quality of herbal medicinal products/ traditional herbal medicinal products of the EMA etc. All of the regulatory agencies are willing to scientifically evaluate the herbal medicines and accept it as therapeutic product. This paper analyzed the history of herbal medicine regulation and the similarities and differences of the regulatory requirements of the European Union, the United States and Canada, proposed possible future direction of the international development of Chinese medicine from the perspective of global regulatory affairs.
