Objective To study the influence of postural intervention on ambulation of patients after video-assisted thoracoscope surgery? Methods One hundred and forty eight patients having undergone chest cardiac surgery were randomly divided into the control group(n = 75)and the observation group(n = 74)? The former received routine nursing intervention and the latter postural intervention besides routine nursing? The two groups were compared in terms of the time for ambulation and the ability in ambulation? Result The ambulation in the observation group was earlier than the control group and the ability in ambulation was better compared to the control group(both P < 0?05)? Conclusion The postural intervention after thoracic surgery is effective in pushing patient’s ambulation and improve their ability?
2.Observation of ankle pump motor in the prevention of deep vein thrombosis after surgical operation

Objective To evaluate the influence of ankle pump motor on deep vein thrombosis after surgical operation.Methods 138 cases of patients after surgical operation who were assessed as high risk patients,were randomized into the study group (70 cases)and the control group (68 cases),routine nursing was adopted in the control group,while in the study group,ankle pump motor was adopted in addition.The incidence of deep vein thrombosis was compared between two groups.Results No deep vein thrombosis was found in the study group,while 1 case of thrombosis was detected in the control group,but there was no statistic difference.Conclusions Ankle pump motor was easy and convenient,and can prevent deep vein thrombosis by promoting the reflux of the lower limbs,improving the recovery of patients after operation,and increasing nursing quality as well.

Pediatric surgery is the important part of the paediatrics,and the complicated content,limited time and many difficulties are the main problems of pediatric surgery teaching.The bed side teaching is the supplement for the class teaching,which could solve those problems.Multi-media technique is used in bed side teaching of Pediatric surgery,which is convenient for preparing lesson and teaching of teachers and review and summary of students.After the technique is used,the teaching efficiency and the quality are obviously improved.
4.The effect of ankle strategy stability limit training on balance and gait in recovering stroke patients

Objective To explore the effect of ankle strategy stability limit training on balance and gait in recovering stroke patients with hemiplegia. Methods Forty recovering stroke patients were randomized into an intervention group and a control group.The patients in the intervention group were given ankle strategy stability limit training using visual feedback on the static long sets of a Smart Equitest Balance Master (SEBM) machine.Those in the control group practiced routine postural balance training using mirror visual feedback in parallel bars.Both groups of patients practiced balance and posture control for 30 minutes,once daily,6 days a week for two weeks. Both groups were also given routine therapy and other rehabilitation.The patients' balance function was evaluated using the Berg Balance Scale (BBS),and their gait was assessed using the walk across technique (WA). Results There was no significant difference between the two groups with regard to general information,BBS scores or WA results before treatment.After 2 weeks of treatment,BBS scores as well as the step length and pace in the WA improved significantly in both groups,but all improved significantly more in the intervention group.There was no significant difference in width of gait. Conclusion Ankle strategy stability limit training can enhance weight-bearing on stroke patients' affected foot as well as their balance and the symmetry of their steps.

6.Lewis lung cancer cells affect CD4~+ CD25~+ Treg cells via TLR9

Objective:To explore the influence of tumor cells on CD4~+ CD25~+ Treg cells via TLRs.Methods:The numbers changes of CD4~+ CD25~+ Treg cells in the co-culture system of Lewis lung cancer cells and splenic lymphocytes were detected by flow cytometry;The expression of Foxp3 and TLR1-9 mRNA after co-culture were detected by RT-PCR;TLR9 expression on Lewis lung cancer cells was blocked by TLR9 receptor antagonist chloroquine.Results:Compared with control group,the number of CD4~+ CD25~+ Treg cells and Foxp3 mRNA expression were significantly increased in the co-culture group (P<0.05).The expression of a variety of TLRs were affected by the co-culture of lymphocytes with Lewis lung cancer cells,and TLR9 mRNA expression was significantly increased compared with that of the control group (P<0.05);Blocking TLR9 of Lewis lung cancer cells significantly reduce CD4~+ CD25~+ Treg cells and Foxp3 mRNA (P<0.05).Conclusion:Lewis lung cancer cells could increase both number and function of CD4~+ CD25~+ Treg cells through inducing TLR9 expression in immunocells.This might be one of mechanisms of tumor-induced immune tolerance,and by which to contribute to the occurrence and progression of tumors.

9.Effects of hyperbaric oxygen on learning and memory ability in VPA autism and on the morphology of pyramidal cells in the CA1 region of the hippocampus

Objective To investigate the effects of hyperbaric oxygen on learning and memory ability in rats with a model of VPA autism and on the pyramidal cells in the CA1 region of the hippocampus.Methods An animal model of autism was established in the offspring of Wistar rats which had received a single intraperitoneal injection of sodium valproate at the 12.5th day of pregnancy.A total of 48 male VPA autism model rats were randomly divided into the hyperbaric oxygen group,a high pressure air group,a normal pressure high oxygen group and a normal pressure air group (each group with 12 rats).A normal control group was obtained by injecting physiological saline.The autism model rats of the hyperbaric oxygen group were treated with high pressure oxygen in an animal experiment cabin which was cleaned with pure oxygen for 10 min,pressurized for 15 min,held at 2.0 atmospheres absolute (ATA) for 45 min and then had the pressure relieved over 15 min.For the high pressure air group the cabin was pressurized with air for 15 min,held at 2.0 ATA for 45 min and the pressure was relieved over 15 min.For the normal pressure,high oxygen group the cabin was cleaned for 10 min with pure oxygen,then pure oxygen was supplied for 1 hour with the cabin door open.The normal pressure air group rats were placed in the open cabin with no pressure or additional oxygen.The testing lasted 7 days for 1 hour per day.The normal control group rats were placed in ordinary cages.Learning and memory were evaluated using the Y electric maze test before and after the interventions.Any changes in the pyramidal cells in the CA1 region of the hippocampus were observed after hematoxylin and eosin (HE) staining.Results The average number of tries of the hyperbaric oxygen group after treatment was significantly less than before treatment and memory retention times were increased.The number was also less in the normobaric hyperoxia group and memory retention times again improved.The number of apoptotic cells was reduced and the number of normal form cells in the CA1 region increased after the hyperbaric oxygen intervention.Conclusions The learning and memory abilities of the autistic rats improved after the hyperbaric oxygen intervention.Pyramidal cells in the CA1 region proliferated and the number of apoptotic cell decreased.This may be the mechanism by which hyperbaric oxygen intervention treats autism.
10.Monitoring and regularity of tacrolimus in patients subject to cadaveric liver transplantation and living-donor partial liver transplantation

Objective To analyze and compare the therapeutic drug monitoring (TDM) and regularity of tacrolimus in deceased donor liver transplantation (DDLT) and living-donor partial liver transplantation (LDLT) the therapeutic drug monitoring (TDM) and regularity of tacrolimus.Method The clinical data of 68 patients undergoing liver transplantation from January 2010 to October 2011 were analyzed retrospectively.Thirty cases underwent LDLT as group A,and the remaining 37 underwent DDLT as group B.Result The time to reach therapeutic window was shorter in group A(3.9 ± 0.7 days) than in group B (6.5 ± 1.0 days,P<0.01).The tacrolimus dosage in group A was significantly less than that in group B during the first 28 days post-transplantation.However,the tacrolimus dosage approached gradually and tended to be consistent after 28 days.On the postoperative day 7,14,21 and 28,the Tacrolimus dosage in group A was 72.37%,79.31%,90.11% and 95.45% of that in group B respectively.Concentration-dose ratio in group B was in a steady state (80-90 g/L).Concentration-dose ratio in group A was decreased with time,culminating at 28th day and close to that in group B.Correlation analysis revealed that graft recipient weight ratio(GRWR) had a significantly positive correlation with the tacrolimus dosage on the first 7 days (r =0.557) and a significantly negative correlation with the tacrolimus concentration/dose ratio (r =-0.578).Conclusion The early tacrolimus dosages in patients subject to LDLT were correlated significantly with the GRWR.The early tacrolimus dosages in patients undergoing LDLT were about 70% of those in patients undergoing LDLT.However,with the regeneration of the liver,they tended to be consistent after 28 days.
