2.Comparison of students' learning curves between Macintosh laryngoscope and Airtraq laryngoscope in endotracheal intubation

Objective To compare medical students' learning curve between Macintosh laryngoscope and optical laryngoscope in endotracheal intubation and to decide which one is more suitable for novice personnel.Methods Totally 26 interns after being trained by experienced anesthesiologists were enrolled in this study.They performed intubation on 6 patients by using both Macintosh laryngoscope and optical laryngoscope ( each laryngoscope for 3 patients) under the supervision of experienced anesthesiologists.The sequence of laryngoscope was determined by computer-generated random number list.Results Totally 149 patients who were ready to receive surgery under general anesthesia were enrolled in this randomized crossover controlled study.Duration of intubation was significantly shorter in Airtraq group (78 -± 33 ) s than in Macintosh group (114 ± 32) s,P < 0.001.Intubation success rate was significantly higher in Airtraq group than in Macintosh group ( 87.8% vs.66.7%,P < 0.05 ).Conclusions With the advantages of rapid learning curve,higher intubation success rate and shorter intubation duration,Airtraq laryngoscope is easier to master for novice personnel.
3.Evaluation on implementation effect of Malaria Elimination Project support-ed by Global Fund in Shaanxi Province

Objective To evaluate the implementation effect of Malaria Elimination Project supported by the Global Fund in Shaanxi Province so as to provide the evidence for the scientific implementation of Malaria Elimination Action Plan and the exami-nation and evaluation work. Methods The data of malaria prevention and control work were collected and analyzed statistically in 27 counties from 2010 to 2012 in order to evaluate the measures and effects of malaria control. Results There were 64 reported cases in the project areas from 2010 to 2012,and the average annual incidence rate was 0.15/10 million with a declining trend. The infected area sources of patients were mainly imported overseas accounting for 79.69%of the total cases. The blood completion rate of febrile patients was 94.14%,and the scope of the blood smears testing work had expanded to 88 counties(including 61 third-class counties). The training coverage rate of malaria prevention knowledge and skills had achieved to 100%. The awareness rates of malaria prevention and control were 91.68%and 89.44%in the students and local residentsrespectively. The case reporting with-in 24 h after diagnosed,epidemiological case study and outbreak disposal,laboratory testing and confirming and the other case management capabilities had also improved significantly. Conclusions The implementation of the Global Fund Malaria Project in Shaanxi Province has accelerated the process of Malaria Elimination work and improved significantly the malaria control ability in the project areas.
4.Transferring Novel Gene into Human Hematopoietic Cells by Adeno-Associated Virus

Objective: To investigate the expression of MDR1 and GFP in the human hematopoietic cells mediated by adeno-associated virus. Methods: The GFP gene was transferred into the human hematopoietic cells by AAV vectors and created strong visible fluorescence by purely molecular biological means. Using adeno-associated virus vectors, we have transferred human mdr-1 gene into human hematopoietic cells and investigated the drug resistence of human hematopoietic cells modified with mdr-1 gene. PCR analysis confirmed that mdrl cDNA had been successfully transferred into the human hematopoietic cells. An assay of MTT proved that the human hematopoietic cells modified by mdrl gene had resistance to colchicine. Results: It was about 30% of the hematopoietic cells that expressed the green fluorescent proteins. The resistance of hematopoietic cells was increased parently when the cells were infected by the crude virus stocks. Conclusion: It is conducted that the AAV vector could successfully transfer the foreign gene into the human hematopoietic cells. The cells modified with mdrl gene have increased the resistance to drugs.
5.Investigation on extract process for total saponins in root of Ophiopogon japonicus

Object To optimize the best extraction of the total saponins in the root of Ophiopogon japonicus (Thunb.) Ker-Gawl. (RO). Methods The extract condition of total saponins in RO was observed by orthogonal design with ophiopogonis D as index. Results The optimum extract condition of RO was established. The more effective way was to smash RO to 5-10 mesh and extract total saponins in RO with ten times amount of 80% alcohol thrice by refluxing, one hour each time. Conclusion The total saponins in RO could be fully extracted in this condition.
6.Identification of Dongfengju Oral Liquid by TLC

Objective To develop a quality standard for Dongfengju oral liq ui d (DOL). Methods Atalantia buxjfolia, Radix Scrophulariae, Fructus Aurantii in DOL was identified by TLC. Results The obtained TLC spots of Atalantia buxj folia, Radix Scrophulariae, Fructus Aurantii in DOL occurred at the correspond ing positions compared to the controls. Conclusion The method is proved to be simple, sensitive, precise and reproducible and can be used for the quality con trol of DOL.
7.Application of chilly-cycle microwave ablation in treatment of hepatocelluar carcinoma

Objective To study the effectiveness of microwave ablation in treating HCC.Methods A total of 98 nodules in 56 cases of HCC were treated via appropriate puncture approach ac-cording to the volume, amount and site of tumor nodules. Results Only 1 patient failed to finish treatment since he could not suffer the pain. The other 55 patients finished without a hitch. Clinical and imaging findings were analyzed for change of physiology. Conclusion The microwave ablation is effective for the treatment of HCC. Such therapy can be important supplement of direct surgical man-agement. Its effect mainly depends on accurate puncture approach and enough diametric range. Mean-while, reasonable puncturing method should be selected.
8.Effect of flurbiprofen axetil and fentanyl of postoperative analgesia on T-lymphocytes subtypes in patients undergoing esophagectomy

Objective To compare the effects of flurbiprofen axetil and fentanyl of postoperative analgesia on T-lymphocytes subtypes in patients undergoing esophagectomy.Methods Forty patients undergoing esophagectomy were randomly divided into two groups(20 cases each):group C (group fentanyl) was given fentanyl 20μg/kg plus tropisetron 5 mg diluted to 100 ml via PCIA after surgery,group F (group flurbiprofen-fentanyl) was administrated flurbiprofen axetil 50 mg at 30 min before the end of surgery,fentanyl 10μg/kg and flurbiprofen axetil 100 mg plus tropisetron 5 mg diluted 100 ml was administrated via PCIA after surgery.The PCIA rate was 2 ml/h,bolus 0.5ml,lock time 15 min.The VAS score was recorded at 12,24,48 h after surgery.Blood samples 2 ml were obtained from peripheral vein for determination of CD3+,CD4+,CD8+ and CD4+/CD8+ at 30 min before surgery(T0),24 h(T1)and 72 h(T2)after surgery.Results Patients in two groups did not show any significant difference in the VAS scores(P>0.05).At T1 CD3+,CD4+ T-lymphoeytes were significantly lower than those at T0 in two groups(P<0.05).At T2,CD3+,CD4+ T-lymphocytes in group F were significantly higher than those in group C(P<0.05).In group C,CD3+,CD4+ T-lymphocytes at T2 were significantly decreased than those at T0(P<0.05).CD8+ T-lymphocytes Was no significantly changed in two groups and at each time point determined (P>0.05).Conclusion Postoperative analgesia by using flurbiprofen axetil and fentanyl can diminish the using dose of postoperative opioid drug,and it can'improve patient's cellular immune function.
9.Clinical analysis of hepatectomy in combination with portal azygous disconnection for primary hepatocellular carcinoma complicated with severe portal hypertension

Objective To evaluate the safety and efficacy of hepatectomy in combination with portal azygous disconnection for primary hepatocellular carcinoma (HCC) complicated with severe portal hypertension (PHT). Methods Clinical data of 30 cases of HCC complicated with PHT treated in our hospital from April 2005 to April 2008 were retrospectively analyzed. All 30 cases were randomly divided into the single operation group (group Ⅰ ) and combined group (group Ⅱ ). Results After operation, there were no significant statistical differences in rates of complications such as hepatic encephalopathy, ascites, stress ulcer, etc. and recovery of liver function between the 2 groups. The WBC and PLT counts in the blood samples of group Ⅱ were higher than those in group Ⅰ after operation. The postoperative 1- and 2-year hemorrhagic rates were lower in group Ⅱ than in group Ⅰ .Conclusion Hepatectomy in combination with portal azygous disconnection is safe and feasible for treatment of HCC complicated with PHT.
