1.Influence of excipients on the mechanical properties of microcapsules

AIM: To use the increasing rate of hygroscopic weight added to represent the mechanical properties of microcapsules, and to study the influence of excipients on the mechanical properties of microcapsules. METHODS: To develop a method of processing the microcapsules, which simulates the rubbing, shearing effect on the microcapsules during manufacturing, and calculate the increasing rate of hygroscopic weight added to study the influence of excipients on the mechanical properties. RESULTS: The excipients could affect the mechanical properties of microcapsules. CONCLUSION: The mechanical properties of microcapsules is the important quality standard, We can use it for selecting excipients used for microcapsulation.
2.Logistic regression analysis of fetal malnutrition and its affected factors

Objective To investigate the incidence of fetal malnutrition( FM), its affected factors and their relationship with small - for gestational - age (SGA) fetus. Methods The intrauterinal growth of fetus was evaluated by the body weight of neonates and FM was detected by clinical assessment of nutritional status ( CANS ). Results ①33 out of 204 cases were found to be FM (16% ). FM in SGA group was significantly higher than that in AGA group ( 5.9% ) (x2 = 69.9, P < 0. 01 ).② When ot = 0. 05, the body weight and head circumference of neonates, the education level of mothers were included into logistic regression model, and their regression coefficient were negative. Conclusion SGA was supposed to be two of the risk factors in developing FM. Fetus with low body weight, small head circumference and not - so - well educated mother were prone to suffer from FM.
4.The influence of the sedation based on remifentanil analgesia on the occurrence of delirium in critically ill patients

ObjectiveTo investigate the influence of the midazolam sedation based on remifentanil analgesia on the occurrence of delirium in critically ill patients in intensive care unit (ICU).Methods A single-center prospective randomized controlled trial was conducted. 140 consecutive critically ill patients admitted to ICU of Peking University People's Hospital, undergoing mechanical ventilation longer than 24 hours, with the need of sedation, from February 2014 to January 2015 were enrolled. They were randomly divided into two groups by computer generated random numbers table, eachn = 70. The patients in observation group received midazolam 1μg·kg-1·min-1 for sedation, and 1 mg/mL remifentanil for analgesia with 0.05 mg/kg intravenous bolus, then continuous infusion of 0.02-0.10 mg·kg-1·h-1. The patients in control group received midazolam for sedation only. The data were recorded as follows: the main indices for observation included the occurrence of delirium and its duration; the second item for observation was consumption of drug for sedation, followed by the mean arterial pressure (MAP) before and after sedation, the time of wake-up, duration of mechanical ventilation, the length of ICU stay, and 28-day fatality rate. The 28-day survival was analyzed by Kaplan-Meier survival curve.Results The dosage of remifentanil used in observation group was (98.6±24.9) mg/d, the dosage of midazolam was significantly lower than that of the control group (mg/d: 160.6±33.3 vs. 178.9±43.4, t = 2.829,P = 0.005), the incidence of delirium was obviously lower than that of the control group [22.9% (16/70) vs. 57.1% (40/70),χ2 = 15.700,P< 0.001], and the time of delirium was slightly shorter than that of the control group (hours: 162.9±78.0 vs. 194.8±117.3,t = 0.947,P = 0.348). Among the patients with delirium, the dosage of dexmedetomidine used in observation group was significantly less than that of the control group (mg/d: 0.54±0.11 vs. 0.64±0.14,t = 2.112,P = 0.041). The MAP before sedation was similar as the MAP after sedation in both groups, and there was no significant difference between observation group and control group [mmHg (1 mmHg = 0.133 kPa), before treatment: 84.7±16.2 vs. 89.5±37.7, after treatment: 82.3±10.7 vs. 80.8±13.9, bothP> 0.05]. There was no significant difference in the time of waking-up between observation group and control group (hours: 2.3±0.9 vs. 2.4±0.8,t = 0.487,P = 0.627). The duration of mechanical ventilation (hours: 143.4±138.3 vs. 163.9±158.9, t = 0.812,P = 0.418), the length of ICU stay (days: 8.8±7.7 vs. 10.0±7.8,t = 0.917,P = 0.361) and 28-day fatality rate [11.4% (8/70) vs. 20.0% (14/70),χ2 = 1.941,P = 0.245] in observation group were slightly lower than those of the control group without significant difference. Kaplan-Meier survival curve showed that the cumulative 28-day survival rate in observation group was slightly higher than that of control group (χ2 = 1.647,P = 0.199). ConclusionAnalgesia based on sedation may reduce the occurrence of delirium and its severity, furthermore, even if delirium occurs, it may be less severe.
5.ALLOZYME-BASED GENETIC VARIATION WITHIN AN UNSTABLE"POPULATION" OF CHINESE ONCOMELANIA HUPENSIS(GASTROPODA: RISSOACEA: POMATIOPSIDAE)

[Objective] To answer the following questions:① For Oncomelania snails collected two years apart from the same locality,has there been genetic divergence?②How much experimental error has there been in studying subsets of these populations? ③As this is an unstable population,what has the net effect been on Hardy-Weinberg equilibrium(Hwe)?[Methods] Allozymes were studied using horizontal starch gel electrophoresis.Data collected from numbers of experiments were conapiled.Data from each collection were divided into two equal subsets based on chronology of the experiments.Thirty-four loci were studied using 72 to 180 snails per subset.[Results] The mean number of alleles per locus ranged frcra 1.5~1.9.With each consecutive subset,the 96 polymorphic loci dropped from 38.2 to 17.6.The mean heterozygosity was very low:0.033 to 0.049 and not significantly different from Hardy-Weinberg expectations.Ten loci and 11 alleles exclusive to the first group were eliminated from the overall study reducing the number of polymorphic loci from 19 to 10.There were significant departures from Hwe at five loci having a substantial number of individuals for each allele.Nei's and Wright's D were 0.003±0.001 and 0.054±0.006 respectively.[Conclusion] ①There were significant errors seen primarily in the results scored' in the earliest experiments.②These earlier errors involving scoring difficult to resolve loci,and interpretation of rare alleles that were not found in later experiment had no significant effect on overall genetic distance.③The use of Wright's D for closely related populations is explained.Results with Nei's D indicated no significant difference among the four subunits; Wright's D yielded significant difference between the collections made two years apart,attributed to the annual flooding of the Yangtze River mixing snails from different localities.④ Major polymorphic loci were not in Hwe as predicted using the unstable population model.⑤One must study 25 or more individuals to find relatively rate alleles and study population genetics.
6.Analysis of failed total hip arthroplasty

Objective To analyse the failed total hip arthroplasty(THA) and to improve its long term results. Methods Thirty five patients(36 hips) undergone revision THA from November 1996 to April 1999, were grouped according to the different causes of the primary failed THA. Results Statistical analysis failed to show the factors related to the failures. In any one failed case, the causes were usually more then one item. The most common causes of failed surgery found in 24 cases were aseptic loosening and peri prosthetic osteolysis, wear out of acetabulum was seen in 7, malpositioned prostheses in 6 and postoperative dislocation in 3. Infection and fracture of femoral stem happened in 3 and 2 patients respectively. Conclusion THA has been extensively used as an effective procedure for the reconstruction of hip joint. Long term excellent clinical results are related to strict selection of indications, appropriate prosthesis, and finally the experience and surgical technique of the surgeon are also of importance to the final outcome.

Objective:To observe the expression of P70 S6 kinase(P70 S6K) in parotid acinic cell carcinoma. Methods:P70 S6K expression was examined by means of SP immunohistochemical technique and Western blot test in 30 cases of parotid acinic cell carcinoma. Objective To investigate the efficacy of ultrasound?guided iliohypogastric∕ilioinguinal nerve block and transversus abdominis plane block for analgesia after cesarean section. Methods Ninety parturients, aged 20-40 yr, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, scheduled for elective cesarean section, were divided into 3 groups ( n=30 each) using a random number table:con?trol group (group C), ultrasound?guided iliohypogastric∕ilioinguinal nerve block group (group IH∕II) and ultrasound?guided transversus abdominis plane block group (group TAP). In IH∕II and TAP groups, bilat?eral ultrasound?guided iliohypogastric∕ilioinguinal nerve block and transversus abdominis plane block were performed after surgery, respectively, with 0.5% ropivacaine 1.5 mg∕kg ( the maximal dose 100 mg) plus dexamethasone 5 mg for each side. All the patients received patient?controlled intravenous analgesia with morphine after surgery, and numeric rating scales scores at rest and during movement were maintained<4 within the 48 h after surgery. The cumulative consumption of morphine was recorded at 6, 12, 24, 36 and 48 h after surgery. The occurrence of adverse reactions such as nausea, vomiting, pruritus, over?sedation and respiratory depression was observed and recorded in the analgesic period. Results Compared with group C, the cumulative consumption of morphine was significantly decreased at each time point in IH∕II and TAP groups ( P<0.01) . Compared with group IH∕II, no significant change was found in the cumulative consumption of morphine at 6 and 12 h after surgery, and the cumulative consumption of morphine was sig?nificantly increased at 24, 36 and 48 h after surgery in group TAP ( P<0.05) . Conclusion For par?turients, ultrasound?guided iliohypogastric∕ilioinguinal nerve block and transversus abdominis plane block both can provide analgesic efficacy after cesarean section, and the efficacy of the former one is better.
