1.Systemic Monitoring of Blood Glucose Level in Small for Gestational Age Infants

Objective To investigate the distribution and duration of abnormal blood glucose level in SGA infants. Methods Blood glucose level was systemicly monitored in 37 SGA infants for (133?115) h(range 16~524 h, median 93 h), who were non NICU hospitalized and no feeding or IV infusion when admitted at (2 6?2 4) h (range 0.25~10.0 h, median 2.0 h) of age. Results During monitoring period, there was hypoglycemia in 19 cases (51%),hyperglycemia in 2 cases (5%), and both hypo and hyperglycemia in 3 cases (8%). The median of last time abnormal glucose level appeared was 10 hr after birth, its 95% confidence interval was 5~55hr after birth. Multiple regression analysis shown delivery models bad relationship with glucose level at admition (p= 0.013). The blood glucose level was higher in vaginal delivery babies. 2.Effects of monitored anesthesia care with deep sedation on gastrointestinal endoscopy

Objective To explore effects of monitored anesthesia care (MAC) on gastrointestinal endoscopy.Methods All patients who underwent gastrointestinal endoscopy in Peking University People's Hospital from September 2010 to September 2015 were analyzed retrospectively and divided into two groups:the MAC group and control group by usage of MAC.Success rates of removal of foreign bodies by upper gastrointestinal endoscopy were compared between two groups during this period.Detection rates of ectopia of gastric mucosa in upper esophagus were compared between two groups from September 2014 to September 2015 when esophagogastroduodenoscopy was performed.The monthly cases of gastrointestinal endoscopies were compared between the two groups,which were separately collected from one MAC operating table and one routine operating table from September 2014 to September 2015.Results The success rate of removal of foreign bodies in the MAC group was higher than that in the control group (100.0% VS 89.7%,x2 =11.737,P=0.001).The detection rate of ectopia of gastric mucosa in upper esophagus in the MAC group was higher than that in the control group(4.3% VS 1.7%,x2 =58.751,P<0.001).The monthly cases of gastrointestinal endoscopies in the MAC group was higher than that in the control group(350.2±36.2 cases VS 213.2±27.9 cases,t =17.591,P<0.001).Conclusion The use of MAC for gastrointestinal endoscopy improves operating success rate,accuracy and operating time.MAC in gastrointestinal endoscopy can be recommended.
3.Percutaneous transluminal septal myocardial ablation versus septal myectomy for treatment of patients with hypertrophic obstructive cardiomyopathy: a Meta-analysis

Objective To evaluate the efficacy of percutaneous transluminal septal myocardial ablation (PTSMA) and septal myectomy (SM) for treatment of hypertrophic obstructive cardiomyopathy.Methods We searched all clinical controlled trials of PTSMA and SM in treatment of hypertrophic obstructive cardiomyopathy in Cochrane Library,PubMed,Web of Science,Wanfang Database and CNKI update to August,2012.The selected studies in Chinese and English were used for Meta-analysis with software RevMan 5.0.Results Five studies including 286 cases (PTSMA:149,SM:137) were included in the study.Left ventricular outflow tract pressure was significantly reduced by SM treatment compared with PTSMA (OR =9.07,95% CI:0.96-17.18,P =0.030).There were no significant differences in interventricular septum thickness (OR =1.15,95% CI:-0.79-3.10,P =0.240),left atrial diameter (OR =1.95,95% CI:-0.16-4.07,P =0.070),left ventricular end-diastolic (OR =0.70,95% CI:-0.79-2.19,P =0.360),end systolic diameter (OR =1.14,95 % CI:-1.65-3.93,P =0.420),degree of symptom improvement (chest pain:OR =1.07,95 % CI:0.35-3.23,P =0.910;syncope:OR =0.32,95% CI:0.08-1.23,P =0.100),and postoperative mortality (OR =1.63,95%CI:0.46-5.76,P =0.450) between PTSMA and SM treatment; but the rates of pacemaker implantation (OR =5.91,95% CI:2.04-17.11,P =0.001) and reintervention treatment (OR =6.62,95%CI:1.20-36.62,P=0.030) in patients with PTMSA were significantly higher than those with SM.Conclusions There is no evidence to indicate that PTSMA can completely replace SM,as an alternative choice the therapy can be used according to left ventricular out flow tract pressure,symptoms,anatomy of target vessel and other conditions.
4.The relationship between nuclear DNA content and the expressions of cyclin D1 and P53 proteins in carcinogenesis of breast carcinoma induced by DMBA in rats

Objective To investigate the interrelation between nuclear DNA content and the expressions of cyclin D1 and P53 in carcinogenesis of breast carcinoma induced by 7,12 dimethylbenz(a)anthracence(DMBA) in rats. Methods Animal model of breast carcinoma was replicated by gastric perfusion of DMBA at 20 mg/100 g. Nuclear DNA content was detected with image analysis instrument and the expressions of cyclin D1 and P53 at every stage of carcinogenesis of breast carcinoma induced by DMBA in rats observed by Sp immunohistochemical staining. Results In mammary gland tissue at grade Ⅰ, grade Ⅱ and grade Ⅲ atypical hyperplasia of breast carcinoma, there was overepression of cyclin D1(3/20, 6/20 and 11/20, respectively). The overexpression of cyclin D1 was detected to be present in grade Ⅰ, grade Ⅱ and grade Ⅲ breast carcinoma (7/9, 1/4 and 0/7, respectively). The overexpression of P53 was detected to be present in grade Ⅰ, grade Ⅱ and grade Ⅲ breast carcinoma (1/9, 1/4 and 6/7, respectively). Conclusion Nuclear DNA content reflects the state of cell proliferation. Cyclin D1 overexpression participates in the carcinogenesis of breast carcinoma, which may be the early event in breast carcinoma. P53 expression is associated with the development of breast carcinoma, which may be the late event in breast carcinoma. The expression of cyclin D1 is negatively correlated with P53 overexpression.
5.Clinical and pathological analyses of estrogan receptor and progestin recetor expressions in bilateral breast cancer

Objective To investigate the relationship between clinical and pathological features in order to observe estrogen and progestin receptor(ER and PR) expressions in bilateral primary breast cancer(BPBC). Methods ER and PR expressions in 29 cases of BPBC from 1990 to 2003 and the clinical and pathological features were analyzed retrospectively. Results ER and PR positive expressions were 53 2% and 41 2% in unilateral breast cancer, but 69% and 51% in first primary cancer of BPBC, and 65% and 41 3% in second primary cancer of BPBC, respectively. Invasive lobular carcinoma(ILC) was 11 6% in unilateral breast cancer and 34 4% in first cancer of BPBC and 41.3% in second primary cancer of BPBC. Status of lymph nodes in unilateral breast cancer was similar to that in BPBC. Conclusion There is higher ER expression in BPBC than that in unilateral breast cancer. ILC is significantly related to the carcinogenesis of BPBC. The status of lymph nodes is not related to BPBC.
6.Significance of microsatellite instability and expressions of cyclin D1 and P53 proteins in breast carcinoma induced by DMBA in rats

Objective To investigate the significance of microsatellite instability(MSI) and expressions of cyclin D1 and P53 in 7,12 dimethylbenz(a)anthracene (DMBA) induced breast carcinoma in rats. Methods Animal model of rat breast carcinoma was duplicated in 30 female Wistar rats by gastric irrigation of DMBA at the dose of 200 mg/kg. Eight microsatellite loci(D1Mitl4, D1Mgh2, D1Mgh6, D3Mit3, D3Mgh9, D5Mitll, D5Mgh5 and D7Mgh1) were selected for the examination of MSI of breast carcinoma by PCR analysis. S P immunohistochemical staining was used to study the expressions of cyclin D1 and P53 in DMBA induced breast carcinoma in rats. Results Of the 27 cases of breast carcinoma, there were 2 cases of MSI at 8 loci, 9 cases with P53 overexpression and 12 cases with cyclin D1 overexpression. All these were associated with histological grading. Conclusion MSI is infrequent in breast carcinogenesis in rats. Cyclin D1 overexpression may be the early event but P53 protein overexpression may be the late event in breast carcinoma.
7.The role of Survivin and VEGF in angiogenesis of colorectal tumor

Objective To identify the role of survivin and VEGF in angiogenesis of colorectal tumor by investigating tissue expression of survivin, VEGF and CD34 in human colorectal tumors. Methods Immunohistochemical staining for the paraffin sections by using the polyclonal antibodies of survivin, monoclonal antibodies of VEGF and CD34, was performed by the standard avidin-biotin-peroxidase technique. The percentage of positive cell and quantitative analysis were made by image analysis system, which also helped to calculate the amount of MVD. Results The immunoreactivity of survivin significantly increased in the transition from adenoma with low dysplasia to adenoma with high dysplasia (P <0.01). Similar changes in protein expression were observed for VEGF and MVD. The expression of survivin was closely related with that of VEGF. Both were positively correlated with MVD. Conclusion Survivin promotes angiogenesis of colorectal tumor together with VEGF. VEGF may be the potential causes of reexpression of survivin during colorectal tumorigenesis.

9.Different effects of extrinsic and intrinsic recognition loading on gait in patients with Parkiuson disease

Objective To explore the effects of extrinsic and intrinsic recognition loading on gait in patients with Parkinson disease.Methods Eight patients with Parkinson disease and six control subjects were instrutted to walk with extrinsic and intrinsic recognition loading respectively.The gait parameters in two conditions were measured with three-dimension motion analysis equipment.FAB scale and Stroop Test were used as the evaluation of executive function.Results The velocity(cm/s),the cadence(step/min),single support and swing phase(%)in the audition stimulates condition in the patients group((113.4±14.32)step/min,(78.90±16.35)cm/s,(40.50±2.58)%,(40.50±2.58)%)were significantly better than those in the calculation condition(respectively(91.27±15.54)step/min,(63.79±21.49)cm/s,(37.95±2.61)%,(37.95±2.61)%).In the calculation condition,the coefficients of variability in stride length,swing and sinfle support phase were significantly higher in the patient group(respectively(6.69±3.99),(8.56±5.69),(8.56±5.69))than in thecontrol group(respectively(3.23±1.34),(5.02±2.54),(5.02±2.54));in the audition condition,except that,the coefficients of variability in step length and velocity also were significantly higher in the patient group(respeetively(11.92±5.86),(6.89±4.98))than the control group(respectively(7.35±3.32),(2.5±1.53)).In the patients group,the score of FAB(15.63±1.51)was lower and error rates of Stroop test(0.087±0.056)was higher than those in the control group(respectively(17.67±0.52),(0.027±0.03))significantly.The error rate of stroop-3 was significantly negative correlated with the gait variables of patient group in the calculation condition.Conclusion The extrinsic audition stimulates has lower effect on the gait of patients of Parkinson's disease than the intrinsic recognition loading.
10.Comparative study on negative symptoms of schizophrenia treated with amisulpride and other second-generation antipsychotics: a meta-analysis

Objective To compare the efficacy of amisulpride and other SGAs in treating the negative symptoms of Schizophrenia.Methods The randomized controlled trials (RCTs) about Schizophrenia treated with amisulpride and other SGAs from Jan 1995 to Mar 2013 were searched in The Pubmed,EMbase,Cochrane Library,WanFang Data,CNKI and VIP.Two reviewers independently screened the literatures,extracted the data,and evaluated the methodological quality.Than meta-analyses were conducted by using RevMan 5.1 and Stata 12.0 software.Results The totall3 RCTs were included.Among the 1814 patients involved.The results of meta-analyses showed that the score of PANSS-N was no significant differences between two groups (MD =-0.33,95% CI:(-0.87,0.21),Z =1.20,P =0.23) ; and the score of SANS was no significant differences between two groups (MD =-0.21,95% CI:(-1.51,1.50),Z =0.31,P =0.76).The side effects were more in other SGAs group than those in amisulpride group.Conclusion Amisulpride is as effective as other SGAs for the treatment of schizophrenia with predominantly negative symptoms,and it has more advantage than other SGAs in safety.
