Background Greater variability in blood pressure and the disappearance of circadian rhythm are frequently found in the very elderly with hypertension.Blood pressure variability may be closely related to the target organ damages.Objective To assess the relationship between nocturnal blood pressure fall(NBPF)and 24 h blood pressure variability(BPV)by using BP weighted standard deviation as a new index for computing 24 h BPV,and to verify the clinical value of this new marker in the very elderly with hypertension.Methods We analysed retrospectively 1441 ambulatory blood pressure(BP)recordings,and computed:①the standard deviation(SD)of 24 h BP directly from all individual readings;②weighted mean value of daytime and night-time SD(wSD);③the magnitude of nocturnal BP fall(NBPF).Left ventricular mass(n=345)and carotid intima-media thickness(n=216)were determined by echocardiography.Results The 24 h SD of BP was significantly greater than the 24 h wSD(P

Objective To compare the effects of neutral posture training (NPT) and gas resistance training on balance and motor func-tion in stroke patients. Methods From June, 2014 to February, 2016, 44 hemiplegic patients were randomly divided into control group (n=22) and experimental group (n=22). The control group received conventional decubitus trunk muscle training and sitting gas resistance train-ing. The experimental group received NPT. They were assessed with Fugl-Meyer Assessment-Upper Extremities (FMA-UE), Fugl-Meyer Assessment-Lower Extremities (FMA-LE), and Berg Balance Scale (BBS) before and after training. Results There was no significant differ-ence in the scores of FMA-UE, FMA-LE and BBS between two groups (t<0.962, P>0.05). The scores significantly increased after training in both groups (t>12.248, P<0.001), and were higher in the experimental group than in the control group (t>1.972, P<0.05). Conclusion The NPT could improve the motor and balance function of stroke patients, rather than gas resistance training.

ObjectiveTo investigate the relationship between carotid intima-media thickness (IMT)and plaque extent of carotid artery with coronary heart disease (CHD).Methods 131 inpatients were divided into 3 groups,in which 54 cases of coronary heart disease group,46 cases of risk group with coronary heart disease,31 cases of control group.The carotid wall IMT,plaque length and thickness was measured by Color Doppler ultrasound.The obtained data using SPSS 17.0 software for statistical processing.ResultsCarotid intimal thickening and the incidence had no significant difference between the group of coronary heart disease and risk group with coronary heart disease risk( all P ＞ 0.05),carotid artery IMT and incidence rate between the above two groups and the control group had statistical differene( t =3.26,3.48,all P ＜ 0.05 ),and the three groups of carotid artery plaque score and plaque classification were statistically significant( F =4.28,P ＜ 0.05 ).ConclusionCarotid IMT and plaque formation was the independent risk factor of CHD,and it could predict the occurrence and development of CHD,especially in carotid artery plaque specificity is higher,in the primary hospital could be used as the auxiliary examination method of CHD.

The bladder meridian of foot-taiyang is considered as key of six meridians and the yang of the yang, which is the pivot of transportation for qi and blood in the meridians and zang-fu. The running route and treatment characteristic of bladder meridian is closely related with chronic fatigue syndrome (CFS). The bladder meridian belongs to brain and connects with governor vessel, which has a close relationship with zang-fu function, quality of sleep and fatigue. Besides, the running route of bladder meridian is highly consistent with the surface projections of important anatomical structures such as muscle, nerve and sympathetic trunk, etc. Therefore, regulating the meridian-qi of bladder meridian can harmonize five-zang and calm the mind, but also effectively relieve physical and mental fatigue in CFS.

OBJECTIVE:To prepare Baicalin proliposomes (PBA) containing sodium deoxycholate (SD) with optimized for-mulation,and to study pharmacokinetics of it in rats in vivo. METHODS:PBA were prepared by spray drying method. Response surface method based on Box-behnken design was adopted to optimize the formulation of PBA with the amount of HSPC,cholester-ol and SD as factors using entrapment efficiency of PBA as index. The particle size,morphology,leakage rate and stability of the optimal PBA were evaluated along with the pharmacokinetics of it(compared to raw materials)in rats after ig administration of 15 mg/kg. RESULTS:The optimal formulation of PBA was that the amounts of baicalin,HSPC,cholesterol and SD were 100,214, 68 and 53 mg,respectively;the predicted and practical values of entrapment efficiency were 86.42% and 84.32%,respectively, and particle size of the optimal PBA was 358.4 nm. The leakage rate of reconstituted liposomes was low and the stability of PBA was good. Compared with baicalin raw material,t1/2,tmax,MRT,cmax and AUC0-t of PBA were all increased significantly(P<0.05 or P<0.01 or P<0.001). CONCLUSIONS:PBA were prepared successfully using the spray drying method. This method is simple and easy,and the optimized formulation is feasible and can improve the oral bioavailability of baicalin.

Objective:To explore the impact of clinical pharmacists involved in the treatment team on the reasonable drug use rate in medical orders. Methods:Totally 782 medical records including hypertension and 1-3 kinds of hypertension associated diseases in our hospital from January 2011 to December 2011 were used as the pre-intervention group, and 1 061 ones with the same conditions from September 2012 to August 2013 were used as the post-intervention group. The patients of the two groups were divided into 8 dis-ease subgroups, clinical pharmacists performed timely intervention on the medical orders in the post-intervention group, and the rational drug use rate between the two groups was compared. Results:In hypertension, hypertension associated coronary heart disease, hyper-tension with heart failure and hypertension with diabetes subgroups,the difference in the rational drug use proportion before and after the intervention was statistically significant(P<0. 01), in hypertension with coronary heart disease and heart failure, hypertension associ-ated coronary heart disease and diabetes, hypertension with heart failure and diabetes subgroup, the difference in the rational drug use proportion before and after the intervention was statistically significant(P<0. 05), while in hypertension with coronary heart disease, heart failure and diabetes subgroups, the difference was not statistically significant(P>0. 05). After the medical records in the 8 sub-groups were aggregated, the difference in the rational drug use proportion before and after the intervention was statistically significant( P<0. 01). Conclusion:Clinical pharmacists should involve in the clinical treatment to intervene irrational drug use,which can signifi-cantly improve the medical record rationality, avoid the health risks as much as possible and ensure the medication safety and treatment effectiveness for patients.

Objective To explore the effect of pre-hospital nursing intervention in the new regional cooperative rescue model on treatment delay and the therapeutic effect in patients with myocardial infarction.Methods From January 2012 to May 2014,158 patients with acute myocardial infraction (AMI) were selected.Patients were divided into two groups,intervention group and control group,The first medical contact to balloon(FMC-to-B) time,referral time,cardiac function were analysed.Results Mean FMC-to-B time [(94±21)min vs.(102±23) min],referral time in nursing intervention [(5±3) min vs.(9±4) min)] were significantly shorter than those in control group (t=2.14,6.67,P＜0.05).After a month compared with control group,LVEF was increased [(54.8±6.9)％ vs.(48.8±6.9)％],and LVED was deceased [(50.1±8.2) mm vs.(50.5±5.6)mm] in intervention group.Conclusions Pre-hospital nursing intervention can decrease the FMC-to-B time,which could improve the cardiac function.

OBJECTIVETo research the preparation technology and dissolution of Blumea volatile oil suppository.

METHODIn order to establish the content determination and methodology inspection method of Blumea volatile oil plug, the extraction process of Blumea volatile oil was optimized by using orthogonal test. Optimization on the investigation to the suppository matrix by melting time, appearance and dissolution was carried on. The best prescription craft was determined by determining the best molding temperature, dosage of the matrix and complementary makings. The determination method of dissolution was established by investigating different dissolution method and its impact on the preparation of dissolution.

RESULTThe best conditions of steam distillation extracted Blumea volatile oil was as followed, the ratio of gardenia to liquor 1:6, 2.5% drug amount of sodium, 8 hours of extracting time. The optimum temperature for mold was 60-65 degrees C. Preparation technique of Blumea volatile oil suppository was stable, which after 45 minutes and 3 h in pH 4.5 PBS released at least 70% and 90%.

CONCLUSIONBlumea volatile oil suppository with rational prescription, simple preparation and good stability.
