1.Study of the complement 1q and its autoantibodies and their relationships with systemic lupus erythematosus and lupus nephritis

Objective To detect the serum level of complement 1q (C1q) and anti-C1q autoantibodies (C1qAb) in systemic lupus erythematosus (SLE) patients to analyze the correlation of serum level of C1q and C1qAb with renal lesion and disease activity in SLE. Methods The serum level of C1q and C1qAb were detected by single radial lmnmnodiffusion and enzyme-linked immunosorbent assay respectively. Results The serum level of C1q in SLE patients was significantly lower than that of the control groups. The serum level of C1qAb in SLE patients was significantly higher than that of the control groups. There was a strong correlation between the serum level of C1q and C1qAb in SLE patients. SLE patients in flare stage showed a significantly higher level of serum C1qAb and a lower level of serum C1q than stable patients. Lupus nephritis(LN) patients showed a significantly higher level of serum C1qAb and a lower level of serum C1q than non-LN patients. Conclusions Low level of serum C1q and the high level of serum C1qAb are correlated with SLE. The serum level of C1q and C1qAb are significantly correlated with renal lesion and disease activity of SLE patients.
2.Application of single port laparoscopy in classification and treatment of Meckel's diverticulum in children

Objective To explore the feasibility of single port laparoscopy in classification and treatment of Meckel's diverticulum in children and its guiding treatment.Methods The clinical data in 75 children cases of Meckel's diverticulum with symptoms treated in our hospital from Aug.2011 to Aug.2015 were retrospectively analyzed.Meckel's diverticulum was classified under single port laparoscopy.The operation modes were selected according to different classifications.The excised materials were submitted to the pathologic examination.Results Among 75 children cases,50 cases were the simple type of Meckel's diverticulum and 25 cases were complex type of Meckel's diverticulum.The average operative time in the simple type and complex type was (38.93±8.75) min and(55.64 ± 13.27) min respectively,average bleeding amounts were (46.58 ± 15.81) mL and (50.12 [16.90) mL respectively,average postoperative hospitalization time was (7.33±1.41)d and (7.52 ± 1.68)d respectively,the operative time in the simple type was less than that in the complex type(P<0.05),the other two indexes had no statistical difference between the two groups(P>0.05).The ectopic gastric mucosal pathological change was only seen in the simple type,while the inflammatory manifestation in the complex type had higher proportion.The main clinical manifestations were lower gestational tract bleeding and infection.The two groups all obtained follow up.One case of simple type appeared the symptoms of abdominal pain and hematochezia and was cured after the second operation.Conclusion Meckel's diverticulum can be divided into the simple type and complex type under single port laparoscopy.The operation mode can be selected according to different types.This method is safe and reliable and is worthy of being clinically promoted.
3.Diagnostic value of 4 kinds of serum autoantibodies in nasopharyngeal carcinoma

Objective To analyze the diagnostic value of 4 serum autoantibodies in nasopharyngeal carcinoma patients (anti-Rho-GDI-2, -HSP70, -CK19 and -LAP3 antibodies) using a serological proteomic approach. Methods The conditions for blocking, antigen concentrations and serum dilution were optimized to establish the ELISA method for detection of serum autoantibodies for Rho-GDI-2, HSP70, CK19 and LAP3. The serum concentrations of these autoantibodies were detected by the established method in 36 NPC patients, 20 other cancer patients and 20 healthy individuals. The statistical analysis was used to evaluate the diagnostic value of the 4 serum autoantibodies. Results The concentrations of 4 serum autoantibodies in the NPC patients were significantly higher than those in the healthy individuals(A450 values in NPC group of CK19, Rho-GDI-2, HSP70, LAP3 were 0. 188 8 ±0.047 3, 0.240 5 ± 0.024 7, 0. 116 8 ±0.025 3, 0.276 9 ±0.044 2 repectively. A450 values in healthy control group were 0.010 5 ±0.004 4, 0. 105 3 ±0. 016 9, 0. 098 6 ± 0. 014 0, 0. 149 8 ± 0. 033 1 respectively. F values were 4. 869, 15. 919, 10. 331,6. 369, respectively. P values were 0. 010, 0. 000, 0. 000, 0. 003, respectively). The sensitivities of antiRho-GDI-2, anti-HSP70, anti -CK19 and anti -LAP3 autoantibodies for diagnosis of NPC was 85.0%,75.0%, 75.0% and 75.0%, respectively, and the specificities were 58.9%, 80. 4%, 58. 9% and 57. 1%, respectively, anti-Rho-GDI-2 and anti-HSP70 achived the highest sensitivity and specificity respectively when single marker was analyzed. The sensitivity and specificity were 94. 4% and 95. 0%respectively when 4 markers was combined for analysis. Conclusion Detection of serum anti-Rbo-GDI-2,anti-HSP70, anti-CK19 and anti-IAP3 auotantibedios by ELISA may be of help in the screening and diagnosis for NPC.
4.Acupuncture at neck Jiaji acupoints combined with moxibustion for clinical study of cervical vertigo

Objective To observe the effect of acupuncture at neck Jiaji acupoints combined with moxibustion to cervical vertigo.MethodsIn Wuhan Chinese and Western Union Hospital from August 2015 to November 2016, 80 patients with cervical vertigo were randomly divided into two groups, the treatment group was treated by needling cervical Jiaji points, at the same time with moxibustion on neck Jiaji acupoints, the control group was given drug treatment.ResultsThe total effective rate in the treatment group was 95%, but the control group total effectiveness was 72.5%, the effect of the two groups had significant difference(P<0.05).ConclusionAcupuncture at Neck Jiaji points combined with moxibustion treatment had the functions of relieving rigidity of muscles and activating collaterals,dispelling cold and dredging channel blockage, relieveing local muscle spasm,improveing blood circulationand improve the function of the blood supply to the brain to vertigo.
5.Intravenous transfusion of endothelial progenitor cells reduces neointima formation

AIM: To investigate the feasibility of transplanting endothelial progenitor cells(EPCs) obtained from spleen in vascular endothelium repairmen after vascular injury.METHODS: EPCs were isolated by using a Ficoll density gradient centrifugation,and were cultured in plate.The endothelial characteristics of EPCs were identified by immunochemical staining and fluorescent labeling.Dil-Ac-LDL labeled spleen-derived EPCs were transplanted into the rats by intravenous injection directly after induction of arterial injury and again 24 hours later.Rats received FITC-labeled lectin intravenously before euthanasia.The distribution of fluorescent labeled EPCs was traced.The morphology of arterial intima and media was studied by optical microscopy and image analysing system.RESULTS: The adherent cells were considered EPCs that showed spindle shape and form blood-siland-like structures during development.The adherent cells had many endothelial characteristics.Fluorescent labeling showed that the intravenously injected EPCs specifically restricted to the vascular injury site,and lectin binding confirmed the endothelial phenotype.The ratio of neointimal/media area in EPCs transplantation group was obviously reduced than that in injury group and M199 group(0.82?0.09 vs 1.52?0.21,1.48?0.19,P
6.Magnetic resonance imaging combined with diffusion tensor tractography can improve prognosis after ische-mic stroke

Objective To explore the pattern of functional reorganization in the cortex after corticospinal tract ( CST) injury and its relationship with the recovery of upper limb motor function. Methods Fifteen patients with complete paralysis on one side after acute cerebral infarction were studied. Within 1 week after the onset, func-tional magnetic resonance imaging ( fMRI ) and diffusion tensor tractography ( DTT) were performed in parallel with timed finger flexion and extension movements in all subjects. The number of nerve fibers in corticospinal tract ( CST) in the affected and healthy sides was measured by using Dtv.Ⅱ. R2 software. One and three months later, fMRI was performed while the affected fingers were flexed and extended passively and any cortical activation was observed. In addition, Fugl-Meyer arm motor function scores were assessed one week, one month and three months after the stroke. Results According to the reconstructed nerve fiber number in CST on the affected side, the patients were classified into three types. Type I:the number of newly-built CST nerve fibers is more than 2/3 of that on the healthy side;type II:the ratio is between 1/3 and 2/3;and type III:the ratio is less than 1/3. For typeⅠpatients, blood oxygenation level-dependent fMRI ( bold-fMRI) showed initial activation of the bilateral sensorimotor cortex ( SMC) and the sup-plementary motor area ( SMA) on the affected side. That was followed by a gradual decrease in the activity in the healthy SMC and an increase in the affected SMC at 1 and 3 months. Among the type II patients bold-fMRI indicated activation of the SMC and SMA on the affected side initially, significant activation of the bilateral SMC and SMA one month later and then stronger activation in the SMC on the healthy side and a weakening of activation in the SMC on the affected side. For typeⅢpatients, initially the SMA and the posterior parietal cortex were found to be slightly ac-tivated. One month later SM1 on the unaffected side was slightly activated, and 3 months later neither the SMC nor the SMA on either side was activated. One week after the onset, the average upper extremity FM scores of the three types of subjects were not significantly different. After one month the three groups′averages were all significantly dif-ferent from one another. But after three months the averages for types I and II were again not significantly different, but significantly better than the average of the type III patients. Conclusion Different CST injuries induce different modes of cortical reorganization. The reorganization is a dynamic process, and different activation patterns are closely correlated with clinical prognosis.
7.Expression and clinical significance of Rho GDP dissociation inhibitor 2 in pancreatic cancer

Objective To investigate the expression and clinical significance of Rho GDP dissociation inhibitor 2 (Rho-GDI2) in pancreatic cancer.Methods Immunohistochemistry and RT-PCR were used to evaluate the expression of Rho-GDI2 in 60 pairs of pancreatic cancer tissues and adjacent pancreatic tissues,and its correlation with clinicopathological parameter of pancreatic cancer was also analyzed.Results The expressions of Rho-GDI2 mRNA were 0.661 ± 0.021 and 0.199 ± 0.023 in pancreatic cancer tissues and adjacent pancreatic tissues,and the positive rate of Rho-GDI2 protein expression were 73.3% (44/60) and 41.7% (25/60),the positive rate of Rho-GDI2 expression in pancreatic cancer tissues was higher than that of adjacent normal tissues,and the difference between the two groups was statistically significant (P < 0.01).The expression of Rho-GDI2 was strongly correlated with tumor size,differentiation,staging,lymph node metastasis,vascular invasion (P<0.05),but it was not associated with gender,age,tumor location (P >0.05).Conclusions Rho-GDI2 expression is up-regulated in pancreatic cancer,and is strongly correlated with the malignant biological behavior of pancreatic cancer.

9.Identification of tetracenomycin X from a marine-derived Saccharothrix sp. guided by genes sequence analysis.

The crude extracts of the fermentation broth from a marine sediment-derived actinomycete strain, Saccharothrix sp. 10-10, showed significant antibacterial activities against drug-resistant pathogens. A genome-mining PCR-based experiment targeting the genes encoding key enzymes involved in the biosynthesis of secondary metabolites indicated that the strain 10-10 showed the potential to produce tetracenomycin-like compounds. Further chemical investigation of the cultures of this strain led to the identification of two antibiotics, including a tetracenomycin (Tcm) analogs, Tcm X (1), and a tomaymycin derivative, oxotomaymycin (2). Their structures were identified by spectroscopic data analysis, including UV, 1D-NMR, 2D-NMR and MS spectra. Tcm X (1) showed moderate antibacterial activities against a number of drug-resistant pathogens, including methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococci (VRE) pathogens, with the MIC values in the range of 32-64 microg x mL(-1). In addition, 1 also displayed significant cytotoxic activities against human cancer cell lines, including HL60 (leukemia), HepG2 (liver), and MCF-7 (breast) with the IC 50 values of 5.1, 9.7 and 18.0 micromol x L(-1), respectively. Guided by the PCR-based gene sequence analysis, Tcm X (1) and oxotomaymycin (2) were identified from the genus of Saccharothrix and their 13C NMR data were correctly assigned on the basis of 2D NMR spectroscopic data analysis for the first time.
10.Case-control study on locking plates fixation for the treatment of Neer 3-and 4-part proximal humerus fractures.

OBJECTIVETo compare therapeutic effects of locking plates for the treatment of Neer 3-and 4-part proximal humerus fractures.
METHODSFrom January 2009 to June 2011, 64 patients with Neer 3-and 4-part proximal humerus fractures were treated with locked plate fixation. There were 39 patients in the 3-part group including 16 males and 23 females, with an average age of (55.12 +/- 12.52) years old; and 25 patients in the 4-part fractures group including 9 males and 16 females,with an average age of (57.92 +/- 13.14) years old. The American Shoulder and Elbow Surgeons score (ASES), visual analogue scale (VAS) and complications were documented for analysis before and after treatment.
RESULTSAll the patients had incision healing at the first stage. All the patients were followed up, and the duration ranged from 12 to 30 months, with a mean of 16.5 months. Comparably better shoulder function recovery was achieved in the 3-part fractures group with regard to the ASES (76.14 +/- 14.10 in the 3-part fractures group vs. 65.93 +/- 11.82 in the 4-part fractures group, P < 0.05). Moreover,a statistical difference (P < 0.05) was observed regarding the VAS pain score (2.12 +/- 1.63 in the 3-part fractures group vs. 3.90 +/- 2.21 in the 4-part fractures group). For the complications rate,no statistical difference was noted between 3-part fractures group and 4-part fractures group (20.51% vs. 36.00%).
CONCLUSIONThe clinical outcomes of the 3-part proximal humerus fractures is better than the 4-part fractures proximal humerus fractures treated with locking plate. Complex proximal humeral fractures treated with locking plates can be achieved a satisfactory outcome when attention is paid to anatomic reduction, stable fixation, proper screws and plate placement, and reasonable functional exercise postoperative.
