Objective To compare the clinical efficacy between reformed laparoscopic-assisted vaginal hysterectomy with sutureless of cardinal and uterosacral ligament(LAVH)and trans abdominal hysterectomy(TAH)for large uterus.Methods A retrospective study was performed using 106 reformed LAVH patients(observation group)and 100 TAH patients(control group)whose uterus were big(≥ 12weeks of gestation),the information was observed in perioperative period.Results Anal exsufflation time,hospitalization time after operation and postoperative pain time in observation group[(25.4 ± 3.3)h,(6.5 ±1.5)d,(16.9 ± 2.4)h]were significantly shorter than those in control group[(42.4 ± 5.3)h,(8.5 ± 1.5)d,(30.6 ± 5.3)h](P ＜ 0.05),the rate of complication in observation group[5.7％(6/106)]waa lower than that in control group[10.0％(10/100)](P＜ 0.05).Conclusion As to large uterus,reformed LAVH has several advantages over TAH,the technique is easy to learn,operation time is short,and the rate of complication is low.

Objective To evaluate the hematologic effects of linezolid in advanced aged patients with hospital onset of infection.Methods The clinical characteristics and complete blood routine examination results of 45 elderly patients aged(92.9±5.2)years treated with linezolid over 600 mg of lowest dose everyday for more than 72 hours were retrospectively analyzed.Results Among the 45patients,20 patients(44.4％)had substantially lower platelet,6 patients(13.3％)lower haemoglobin,1 patient(2.2％)lower absolute neutrophil,and the linezolid treatment in 19 patients were discontinued due to hematological system adverse reactions.The significant drop in platelet occurred at(10.2±3.3)days after treatment.The significant drop in hemoglobin occurred at(9.8±3.0)d after treatment.The lowest count of platelet and hemoglobin took place at(13.1±3.9)d and (10.5±3.5)d after treatment,respectively.The drops in platelet and hemoglobin of some patients took place after cease of treatment.Stepwise logistic regression analysis revealed that treatment duration(OR =1.407,P＜0.05)and creatinine clearance rate(OR=0.732,P＜0.05)were the risk factors for thrombocytopenia.Conclusions The elderly patients using linezolid more likely suffer from adverse reactions of hematological system,particularly thrombocytopeniawhich is significantly more common in patients with renal insufficiency and prolongation of treatment time.The blood routine examination should be monitored closely when elderly patients are treated with linezolid.

Objective The purpose of this study was to improve the clinical results of the treatment firearm injury to maxillofacial-cervical region, so as to raise the life quality of these patients. Methods Based on the study of the anatomical study of this region, different methods were adopted to reconstruct various maxillofacial-cervial firearms injuries. Three methods were adopted to reconstruct the mandibular region by early bone transplantation to the defects of the mandible in diffferent regions. In patients with combined defects of the mandible and soft tissue, the defects were repaired by transplantation of vascular pedicle graft with muscle and nerve or free tissue transplantation to restore function. Results The survival-rate of early bone transplantation was 91.52%(54/59). The rate of function restoration was 90.72%(88/97) in cases of tissue flap grafting. Conclusions Applied anatomy is the basic of design new surgery method. Tne new developed surgery methods can improve clinical cured effect of maxillofacial-neck region firearms injure.

Objective To assess the feasibility and advantage of systematic transperineal ultrasound guided template prostate biopsy. Methods In a prospective study, a total of 1270 patients(26 -90 year old,mean age 70. 4) who met the inclusion criteria underwent 11 regions systematic transperineal ultrasound guided template prostate biopsy. The median PSA level was 12. 96 ng/ml(range 0. 25000. 0 ng/ml) and the mean prostate volume was 53. 6 ml(range 7-200 ml). Results Prostate cancer was detected in 486 of 1270(38. 3%). The prostate cancer detection rate in group with PSA 0-4.0, 4. 1-10. 0, 10. 1-20. 0, 20. 1-40.0, 40. 1-60.0, 60. 1-100.0、and> 100. 0 ng/ml groups was 21.9%(16/73 )、16.6%(67/ 403)、30.7% (122/397)、46. 7%(77/165)、68. 3%(41/60)、86.2% (46/54) 、99.2% (117/118), respectively . While cancer detection rate in group with prostate volumes less than 20, 21-40, 41-60, 61-80 and great than 80 ml was 72. 1%(49/68 )、55. 6%(234/421) 、32.5%(136/418)、23.3%(48/206)、12. 1%、(19/157). The mean positives for the cancer of regions 1- 10 and region 11 (the apical region) were 35.3% vs. 44.8% in patients whose PSA＜20ng/ml(P＜0. 05). The positives for cancer within the right lobe and left lobe were 70. 8% vs. 74.2%(P>0.05)and the positives for cancer within the anterior and posterior parts were 79. 4% vs. 86. 8%(P>0. 05). No serious complication occurred during the procedure. Conclusions Systematic transperineal ultrasound guided template prostate biopsy could be accurate and safe. Prostate carcinoma foci are more frequently localized in the apical region in patients with PSA ＜20 ng/ml. Special attention should be paid to the apical region during the process of biopsy.

Objective: To establish a GC method for the determination of resorcinol, borneol, glycerol and DMSO in compound fluocinonide tincture.Methods: The analysis was performed on an HP-INNOWax column (30.0 m×0.32 mm,0.25 μm).The inlet temperature was 280℃ and the detector temperature was 290℃.The split ratio was 10∶1.With nitrogen as the carrier gas, the flow rate was 2.0 ml·min-1.The temperature was as follows: the initial temperature was maintained at 100℃ for 5 min and raised to 240℃ at a rate of 20℃·min-1, and maintaining for 8 min.The flow rate of hydrogen, air and tail blows was 35, 350 and 25 ml·min-1, respectively.Chennai was used as the internal standard.Results: The concentrations of DMSO, resorcinol, borneol and glycerol were within the range of 99.20-793.60 μg·ml-1 (r=0.999 9), 507.25-4 058.00 μg·ml-1 ,respectively(r=0.999 9), 102.20-817.60 μg·ml-1 (r=1.000 0) and 316.20-2529.60 μg·ml-1 (r=1.000 0) with a good linear relationship.The average recovery was 98.58%, 98.34%, 98.19% and 102.29%, and RSDs were 3.80%, 3.93%, 2.87% and 3.65% (n=9), respectively.Conclusion: The method is simple, accurate and reliable, and can be used for the quality control of DMSO, resorcinol, borneol and glycerol in compound fluocinonide tincture.

The Chinese acupuncture doesn't only belong to China but rather to the world.The boom of overseas acupuncture education and scientific research as well as the satisfactory working environment compels us to reflect the Chinese traditional medicine education in our country.Especially in higher education specialty courses of acupuncture should not be subdivided;meanwhile,much attention should be paid to elementary education of western medicine.

[Objective]To evaluate the effect of treatment of unstable intertrochanteric fracture with dynamic condylar screw.[Method]Fifty-six cases of unstable intertrochanteric fractures were treated with typical ORIF (open reduction and internal fixation) with dynamic condylar screw from 2000 to 2004. According to the Evan's classification systems, there were 19 cases of Evan's Ⅲ fracture, 27 cases of Evan's Ⅳ fracture and 10 cases oblique reverse Evan'sⅤ fracture. The average period of follow-up was 10 months (8～15 months).[Result]All cases had union with average union time of 4 months, no coxae inversion, fixation failure and fracture nonunion. The satisfactory rate of function was 89%. [Conclusion]For unstable intertrochanteric fracture,including Evan's Ⅲ, Evan's Ⅳ and oblique reverse Evan'sⅤ fracture, using DCS is a good method to improve the clinical results.
