1.Changes of blood fentanyl concentration in rats during anhepatic phase treated by large dose of dexamethasone

Objective To observe the changes of blood fentanyl concentration in rats during anhepatic phase by treatment of large dose of dexamethasone and to investigate the extrahepatic metabolism of fentanyl. Methods Thirty rats were randomly divided into 3 groups (n=10 in each group): normal control group (Group A), anhepatic phase group (Group B), anhepatic phase+dexamethasone treatment group (Group C). The hepatic portal of rats was dissociated and closed in Group B. In Group C, 20 mg/kg of dexamethasone was infused 1 h before the occlusion of hepatic portal. After intravenous injection of 20 ?g/kg fentanyl each time, the blood samples were collected. The fentanyl blood concentration was analyzed by HPLC. Results The blood fentanyl concentration of anhepatic phase dropped more slowly in Group B than Group C (P0.05). Conclusion The liver is the main organ for fentanyl metabolism. There is the extrahepatic metabolism for fentanyl and dexamethasone can enhance the extrahepatic metabolism of fentanyl.
2.Clinicopathological analysis for postoperative recurrence in early gastric cancer patients

Objective To clarify the risk factors of postoperative recurrence of early gastric cancer (EGC)patients who underwent radical gastrectomy.Methods We investigated follow-up records of 336 EGC patients on whom curative operations were performed at the First Affiliated Hospital of Dalian Medical University between January 1994 and January 2008 with special reference to 12 cases of cancer recurrence.Results The 5-year survival rate of D1 was 89.0%,D1 + t was 91.4%,D1 + β was 91.8%,D2 was 92.5%,D3 was 90.0%(P =0.981).The recurrence rate was 3.6%(12/336).The recurrence time was 17-58 months,the survival time was 33-68 months.Six cases suffered from hematogenous recurrences,four cases from lymph node recurrences,two cases from remnant stomach recurrences.The recurrence rate of SM was higher than M(P =0.015),lymph node positive was high than negative(P =0.011),D1 was higher than D2(P =0.025).Conclusions The recurrence rate of EGC correlated with depth of invasion,lymph node metastasis and lymph node dissection.
3.Analysis of lost goodwill target proteomic fingerprints drifting from negative to positive in advanced lung adenocarcinoma patients

Objective To analyze the serum related proteomic fingerprints when Lost Goodwill Target (LGT) proteomic fingerprints drifting from negative to positive in the advanced lung adenocarcinoma patients. Methods The serum proteomic fingerprints of 31 advanced lung adenocarcinoma patients whose LGT fingerprints drifted from negative to positive were detected by SELDI and CM10 protein chip. More than 10 % cluster and M/Z values from 11,000+H to 12,000+H was regarded as LGT positive, otherwise as negative. Different fingerprints were screened by Biomarker Wizard 3.1 and Biomarker Wizard software and the decision tree model was established. Results There were 16 statistically different protein peaks when LGT fingerprints drifting from negative to positive, including 10 up-regulation proteomic fingerprints (M/Z:11531, 11483, 11686, 11394, 11822, 11323, 11911, 12450, 5811 and 5709) and 6 down regulation proteomic fingerprints( M/Z: 4126, 13927, 13784, 7001, 1959 and 2741). Conclusion By SELDI and CM10 protein chip detection, up-regulating fingerprints of M/Z 11531, 11483, 11686, 11394, 11822 and 11323 were regarded as the subtype of LGT when it drifting from negative to positive, while up-regulation of M/Z 11911,12450, 5811 and 5709 and down-regulating of M/Z 4126, 13927, 13784, 7001 and 1959 were regarded as the related fingerprints when LGT drifting from negative to positive. The above different fingerprints are constituted of the fingerprints library when LGT fingerprints drifting from negative to positive and it will provide a platform for studying the LGT proteins.
4.Study on treatment of tibial avulsion fractures of posterior cruciateligament combined with anterior cruciate ligament

Objective To explore the clinical efficacy of one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of posterior cruciate ligament(PCL) under posterior direction version and in treatment of tibial avulsion fracture at the anterior cruciate ligament (ACL)under arthroscopy.Methods From January 2010 to June 2015,25 patients with tibial avulsion fractures of the posterior cruciate ligament combined with the anterior cruciate ligament in our hospital were retrospectively analyzed.Firstly,arthroscopic exploration was conducted in all patients in their horizontal position,aiming to observe whether combined injures existed or not and to confirm fracture fragments and the degree of tibial avulsion fractures of the cruciate ligaments.All patients recieved arthroscopic fragment fixation by using cannulated screws,then,were treated with a minimally posteromedial incision in the prone position by cannulated screw fixation.All knee joints were protected by adjustable orthopedic brace for 4-6 weeks.The clinical knee joint function of patients were measured by using International Knee Documentation Committee (IKDC) scoring scale and Lysholm scoring scale.The X-ray or CT image were used to evaluate healing process of the fractures.Results All patients were followed up for 6-24 months,the median follow-up time was 15 months.All fractures healed in one stage,time of fracture healing ranged from 9 to 24 weeks,and the median time was 12 weeks.The average IKDC scores at the finial follow-up were as follows:claudication (4.88±0.29)points,support (4.65±0.36)points,pain (4.78±0.49)points,swelling (4.88±0.52)points,squat (4.85±0.35)points,all were higher than those before operation,there were statistically significant differences when compared with those assessed before operation (P<0.05).The average Lysholm score at the finial follow-up was (93.4±2.8)points,the rate of excellent and fine was 96.0%,no statistically significant difference was found compared with that at the healthy side (P>0.05).Conclusion The one-stage cannulated screw fixation in treatment of tibial avulsion fracture at the insertion of PCL under posterior direction version and in treatment of tibial avulsion fractures at the ACL under arthroscopy is easy and convenient to operate,which has the advantages of reliable fixation and satisfactory recent curative efficacy.
6.A family-based association study of the autism and PTEN genes polymorphisms in children

Objective To explore the correlation of single nucleotide polymorphism (SNP) with PTEN gene involving in mammalian target of rapamycin (mTOR) C1 signaling genes polymorphisms and autism in children.Methods A total of 97 cases with autism were enrolled from Mar.2011 to Dec.2012 in this study,who came from the child and adolescent out-patient department in Shanghai Mental Health Center of Shanghai Jiaotong University School of Medicine.Single SNP association and haplotype association analysis were performed using the family-based association test and Haploview software.Results 1.In a family-based association test,two SNPs showed significant association with autism(rs17107001 G:Z =2.982,P =0.003 ; rs2299941 G:Z =2.524,P =0.012).After the correction of false discovery rate,they all remained significant.2.Haplotype association analysis showed significant transmission disequilibrium in haplotype T-T-G and C-T-A generated from rs532678-rs17562384-rs2299941 (block2) in LD Block,and haplotype T-T-G was over transmitted to offspring(Z =-2.986,P =0.003) while haplotype C-T-A was the opposite (Z =-2.197,P =0.028).Conclusion The SNPs of PTEN genes might have a correlation with autism in children.
7.Clinical study of laparoscopic hepatectomy for hepatolithiasis

Objective To explore the feaibility of laparoscopic hepatectomy(LH) for hepatolithiasis.Methods Eight patients with hepatolithiasis were treated by laparoscopic cholecystectomy(LC) and common bile duct exploration(LCBDE) and LH.Laparoscopic resection of left lateral lobe of liver was performed in 7 cases,and left hemihepatectomy in 1 case.Results Procedures were all successful with operation time of(285.00?37.42) minutes,and bleeding volume(306.25?29.73)mL.The postoperative hospital stay was(7.88?1.36) days.No complications occurred.No residual stone was found in any patient.Conclusions LH was safe and effective for hepatolithiasis,and gives better results when combined with choledoscopic stone removal.
9.Pylorus-preserving gastrectomy for early gastric cancer

Objective To discuss the effect of pylorus-preserving gastrectomy for early gastric cancer(EGC). Methods Between August 1995 and December 2005, 52 cases of EGC underwent pyloruspreserving gastrectomy(PPG) and 159 cases of EGC underwent distal gastrectomy(DG), Clinicopathlogic data and follow-up results of the two groups were analyzed retrospectively, and gastric emptying and gallbladder function of 15 cases PPG and 17 cases DG were compared at the same time.Results Compared with DG group, patients in PPG group maintain the body weight, gastric emptying and gallbladder function. There was no significant difference between PPG group (92. 3% ) and DG group (93.1% ) in overall 5-year survival rate ( P = 0. 881 ). The 5-year survival rate of the the PPG group with lymph node dissection was D1 100%, D1+α 92. 3%, D1+β 88.9%, D2 87. 5% respectively.Conclusions For early gastric cancer, the pylorus-preserving gastrectomy is effective for maintaining the postoperative function with similar long term survival as that of distal gastrectomy.
10.Kinematics Modeling and Analysis of Central-driven Robot for Upper Limb Rehabilitation after Stroke.

The present paper proposed a central-driven structure of upper limb rehabilitation robot in order to reduce the volume of the robotic arm in the structure, and also to reduce the influence of motor noise, radiation and other adverse factors on upper limb dysfunction patient. The forward and inverse kinematics equations have been obtained with using the Denavit-Hartenberg (D-H) parameter method. The motion simulation has been done to obtain the angle-time curve of each joint and the position-time curve of handle under setting rehabilitation path by using Solid Works software. Experimental results showed that the rationality with the central-driven structure design had been verified by the fact that the handle could move under setting rehabilitation path. The effectiveness of kinematics equations had been proved, and the error was less than 3° by comparing the angle-time curves obtained from calculation with those from motion simulation.
