1.The expression and clinical significance of ICAM-1 and VCAM-1 in colorectal carcinoma

Objective To investigate the expression of ICAM-1 and VCAM-1 in colorectal carcinoma and the rela-tionship between ICAM-1, VCAM-1 and the occurrence, the metastasis of colorectal carcinoma, and to analyze the significance. Methods Immunohistochemistry staining(S-P method) was used to detect the expression of ICAM-1 and VCAM-1 on consecutive section in 78 cases of colorectal carcinoma and normal mucosa and 42 cases of colorec-tal adenomas. Plasma was obtained from 78 patients with colorectal carcinoma and 40 healthy volunteers prior sur-gery, by an enzyme-linked immunosorbent assay (ELISA). Results There was significant difference between the high expression of ICAM-1 and VCAM-1 in colorectal carcinoma and the low expression in normal mucosa and ade-noma(P < 0. 01). Serum ICAM-1 and VCAM-1 were significantly higher in patients with colorectal carcinoma than those in normal control group(P < 0. 01). The ICAM-1 and VCAM-1 levels were not related to sex, age, tumor lo-cation and histological grades(P > 0. 05),but had significantly statistical difference between lymph node metastasis and Dukes stage(P < 0. 01). Conclusion The increase expression of ICAM-1 and VCAM-1 in colorectal cancer suggests that the ICAM-1 and VCAM-1 may play an important role in evaluation of malignant degree and judging the progress as well as the mechanism of colorectal cancer.
2.The design and application of fully automatic X-ray exposure control

Objective To realize fully automatic X-ray exposure controlling,simplify operating processes,and improve the quality of images.Methods We designed 5 or 7 X-ray detectors with silicon batteries,improved film-holding board of the main engine of the X-ray machine,then assembled them by mechanical gearing,and thus made multi-measured-limit,automatically-changed X-ray detectors.Results When choosing film boxes according to the radiographed positions,the detectors will change automatically into measured-limits coinciding with the interested positions.The average density was around 1Ⅴ in 9 randomly selected photographs.High quality was achieved in up to 98% of 10 000 radiographed images for clinic radiography.Conclusion The key of X-ray automatic exposure controlling technology is X-ray detecting technology,by which the quality of image can be improved.
3.Effects of human vitreous fluid on the proliferation of cultured human retinal pigment epithelial cells and vascular endothelial cells

Purpose To investigate the effects of human vitreous fluid on proliferation of cultured human retinal pigment epithelial (RPE) cells and vascular endothelial cell lines(VEC304). Methods Human RPE cells and VEC304 were cultured and treated in different human vitreous conditioned medium (VCM) with or without serum, vitreous volume concentrations of VCM were 1∶8, 1∶4 and 1∶2. Cells proliferation was assayed by tetrazolium (MTT) colorimetry at the 2nd, 4th and 6th day respectively. Objective To describe cultured human retinal pigment epithelial (RPE) cells transdifferentiation and investigate the effects of human vitreous fluid on the morphologic and cytoskeleton changes of RPE cells in vitro. Methods Cytoskeleton characteristics in the 2 nd, 5 th, 8 th passage of RPE cells in normal culture, which included cytokeratin 18 (CK18) and ?-smooth muscle actin (?-SMA) were analyzed by Western blot. RPE cells were cultured in human vitreous-conditioned medium (VCM) at the concentration of 1∶4 for 6 days, morphologic changes were examined by light and electron microscopy, and cytoskeleton characteristics were analyzed by imunocytochemistry and Western blot. Results During culture in vitro, RPE cells lost epithelial characteristics and aquired fibroblast-like phenotype. The expression of CK18 was the highest at the 5 th passage, and it decreased in the following passage, but ?-SMA increased gradually. The morphologic transdifferentiation from epithelial to fibroblast-like cells of RPE was accelerated by VCM. Ultrastructural changes such as decreased microvilli and gradually increased rough endoplasmic reticulum and Golgi complex were found during the cultivation. 5.MRI qualitative estimation of cerebrospinal fluid flow in normal volunteers

Objective To evaluate the feasibility of the cerebrospinal fluid (CSF) flow measurement by cine phase contrast MRI. Methods Evaluation of intracranial and intraspinal CSF flow was accomplished by the use of periphery gated cine phase contrast MRI technique. Normal patterns of pulsatile flow within the ventricles, cisterns and cervical subarachnoid space were established by this technique. Results CSF flow within ventricle, cistern and subarachnoid space could be clearly displayed, and the relationships between the cardiac cycle and the CSF pulsations were demonstrated on both magnitude reconstruction and phase reconstruction MR images. Conclusions Cine phase contrast MRI is capable of demonstrating intracranial and intraspinal CSF flow. As a new noninvasive technique, it is really superior than other modalities in studying the flow pattern and direction of CSF.
6.Residual subretina fluid recovery after scleral buckling surgery

Objective By using optical coherence tomography (OCT) to observe the residual subretina fluid changes in fovea that may explain visual recovery after anatomically successful repair of rhegmatogenous retinal detachments (RD). Design Prospective observational case series. Participants 34 patients(35 eyes) with successful retinal reattachment after macula involving RD repair. Methods 34 patients were followed up at 3 days, 15 days, 2 months and 6 months after sclera buckling. The best corrected visual acuity(BCVA), eye fundus status were recorded. The OCT images through the center of the fovea were obtained. Main Outcome Measures The OCT images and BCVA after RD repair. Results The BCVA and OCT images were obtained 3 days to 6 months postoperatively. There were 3 eyes, 5 eyes, 9 eyes and 23 eyes respectively with residual subfovea fluid absorbed totally at 3 days, 15 days, 2 months and 6 months. In the residual subforvea fluid group, the mean visual acuity increased at 0.124?0.013. In the subfovea fluid absorbed group, the mean visual acuity increased at only 0.019?0.006. Conclusions After anatomically successful repair of RD performend with sclera buckling, the minimal residual subforvea fluid will exist for a long time and will be absorbed slowly. This may explain slow increase of postoperative visual acuity after anatomically successful repair.
7.Amniotic membrane transplantation in the treatment of conjunctival rupture, dehiscence and socket contracture after hydroxyapatite orbital implantation

AlM: To investigate the clinical effect of preserved amniotic membrane transplantation in the treatment to conjunctival rupture, dehiscence and socket contracture after hydroxyapatite ( HA) orbital implantation.
METHODS: ln 16 cases of conjunctival rupture and socket contracture after HA orbital implantation, conjunctival tension was release by operation and preserved amniotic membrane was transplanted on conjunctival scleral exposure area.
RESULTS:ln all cases, conjunctiva healing, completely cover the sclera and conjunctiva socket recover ideal depth after operation in 15 cases, 1 case was fail.
CONCLUSlON: Preserved amniotic membrane transplantation is an effective method to treat conjunctival dehiscence and keeping the ideal conjunctival socket depth after orbital implantation.
8.Radioimmunoassay of TNF?, IGF, and TGF? in proliferative vitreoretinopathy

Objective To measure the levels of tumor necrosis factor ? (TNF?), insulin like growth factor (IGF), and transforming growth factor ? (TGF?) in the pathogenesis of proliferative vitreoretinopathy (PVR), traumatic proliferative vitreoretinopathy (TPVR), and proliferative diabetic retinopathy (PDR). Methods The levels of TNF?, TGF?, and IGF in vitreous samples taken from 56 patients and 13 normal subjects were determined by radioimmunoassay (RIA). Results The levels of TNF? and IGF in vitreous samples from patients with PVR, TPVR, and PDR increased significantly, as compared with those of the normal controls ( P 0.05). Conclusion TNF? and IGF participate in the pathogenesis of PVR, TPVR, and PDR.
9.Vitreoretinal surgery for complicated rhegmatogenous retinal detachment

Objective To evaluate the efficacy of vitreoretinal surgery for the treatment of complicated rhegmatogenous retinal detachment and to study the correlated factors affecting the surgical outcomes. Methods Complete follow up data from 103 patients (eyes) with complicated rhegmatogenous retinal detachment treated with vitreoretinal surgery were analyzed retrospectively. Conditions of the eyes included macular hole in 21 eyes [with proliferative vitreoretinopathy (PVR) stage C2 or more serious in 12 eyes], giant hole in 11 eyes, and PVR stage C2 to D3 in 71 eyes. Filling with C3F8 was performed in 82 eyes, and silicone oil in 21 eyes. A follow up for 3-18 months was conducted. Results The reattachment of retina was achieved in 99 eyes (96.1%) and functional success in 68 eyes (66.0%). In eyes with functional success, visual acuity of 0.02-0.08 was achieved in 18 eyes, 0 1-0 4 in 39 eyes, 0.5-0.9 in 10 eyes, and 1.0 in 1 eye. Conclusion Vitreoretinal surgery is an effective way for the management of complicated rhegmatogenous retinal detachment. Early surgery may result in satisfactory surgical outcomes.
10.Clinical significance of monocyte chemoattractant protein-1, tissue factor in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the levels and clinical significance of monocyte chemoattractant protein (MCP)-1 and tissue factor (TF) in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Forty-nine elderly patients with AECOPD(AECOPD group) and 30 healthy controls (control group) were selected,and AECOPD group was divided into procalcitonin (PCT) increased group (PCT ≥ 0.5 μ g/L,19 cases) and PCT normal group (PCT < 0.5 μ g/L,30 cases) according to the level of serum PCT.The levels of plasma TF,serum MCP-1 and PCT were determined by enzyme linked immunosorbent assay.Results The levels of plasma TF and serum MCP-1 were (203.6 ± 92.9),(152.8 ±99.9) ng/L in AECOPD group,and (136.9 ±24.3),(87.5 ±41.5) ng/L in control group.There were significant differences in the levels of plasma TF and serum MCP-1 between AECOPD group and control group (P<0.01).The level of plasma TF was positively correlated with serum MCP-1 (r =0.673,P=0.029).The levels of plasma TF and serum MCP-1 were (215.3 ±71.2),(181.1 ±61.6) ng/L in PCT increased group,and (192.4 ±79.7),(137.3 ±74.4) ng/L in PCT normal group.There were significant differences in the levels of plasma TF and serum MCP-1 between PCT increased group and PCT normal group (P<0.05).Conclusions In patients with AECOPD,hypercoagulability state is activated,and it is more severe in the patients with increased PCT.The level of plasma TF is positively correlated with serum MCP-1 in patients with AECOPD.To monitor the levels of plasma TF and serum MCP-1 is particularly important for elderly patients of AECOPD with hypercoagulability state to prevent cardiovascular,lungs and cerebrovascular thrombotic disease.
