1.Preparation and Clinical Application of Compound Levofloxacin Eye Drops

OBJECTIVE:To study the preparation of compound levofloxacin eye drops and its quality control,then observe its clinical effect.METHODS:Levofloxacin was used as basic remedy mixed with dexamethasone sodium phosphate and phos?phate was adoped as buffer,while EDTA-2Na as stabilizer.Finally the content of levofloxacin was determined by UV-spec?trophotometric method.RESULTS:The preparations was stable and within the range of2~6?g/ml,the concentration of lev?ofloxacin had a linear relationship with absorbance(C=26.1780A—1.0838,r=0.9997).The average rate of recovery was100.34%and RSD was0.44%(n=5).CONCLUSION:This preparation has a good effect with simple preparing technique and reliable quality control.
2.Effects of dexmedetomidine on activity of nuclear factor kappa B in neutrophil granulocytes during one-lung ventilation in patients undergoing pulmonary lobectomy

Objective To evaluate the effects of dexmedetomidine on the activity of nuclear factor kappa B (NF-κB) in neutrophil granulocytes during one-lung ventilation in the patients undergoing pulmonary lobectomy.Methods Thirty-eight ASA physical status Ⅰ or Ⅱ patients,aged 40-64 yr,weighing 50-75 kg,scheduled for elective pulmonary lobectomy,were randomized into 2 groups (n =19 each) using a random number table:control group (group C) and dexmedetomidine group (group D).In group D,dexmedetomidine 1.0 μg/kg was infused intravenously over 15 min before induction of anesthesia,followed by infusion at a rate of 0.5 μg· kg-1 · h-1 until 30 min before the end of operation.The equal volume of normal saline was given in group C.At 5 min after intubation,0,30 and 60 min of one-lung ventilation,0 and 30 min of two-lung ventilation,and 30 min after operation,blood samples were taken from the radial artery for blood gas analysis and for measurement of plasma concentrations of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6).The oxygenation index and respiratory index were calculated.The nuclear protein of neutrophil granulocytes was extracted for measurement of NF-κB DNA-binding activity.Results Compared with group C,the respiratory index,plasma concentrations of TNF-α and IL-6 and NF-κB DNA-binding activity were significantly decreased,and no significant difference was found in the oxygenation index in group D.Conclusion Dexmedetomidine can inhibit the activation of NF-κB in neutrophil granulocytes and is helpful in reducing the systemic inflammatory responses during one-lung ventilation in the patients undergoing pulmonary lobectomy.
3.Effects of Blood-Circulation Promoting and Phlegm Removing Treatment on Mitochondria membrane in Spontaneous Hypertensive Rats

Objective To explore the effects of blood-circulation promoting and phlegm removing treatment on the structure, activity of Ca 2+ -Mg 2+ ATPase and malondialdehyde(MDA) content of mitochondria membrane in spontaneous hypertensive rats (SHR). Methods Spontaneous hypertensive rats (SHR) were classified into 14-week-old model group(Group C),28-week-old model group(Group D) and treatment group (treated with blood-circulation promoting and phlegm removing Chinese herbal medicine,Group E).Normal Wistar rats 14 weeks old (Group A)and 28 weeks old(Group B) served as the controls.The structure of myocardial mitochondria membrane were analyzed quantitatively by transmission electron microscope,and the activity of Ca 2+ -Mg 2+ ATPase and MDA content were also detected. Results Specific surface area of mitochondria was smaller, the activity of Ca 2+ -Mg 2+ ATPase was lower and MDA content was higher in SHR than the controls.MDA content increased as the duration of disease prolonged. Chinese herbal medicine could counteract the above changes. Conclusion Blood-circulation promoting and phlegm removing herbs can partially counteract the progress of left ventricular hypertrophy in SHR.
4.Expression of Low Molecular Weight Proteins in Serum of Ischemic Stroke Patients

Objective To explore the differential expression of low molecular weight proteins in serum of ischemic stroke patients.Methods Serum samples obtained from ischemic stroke patients (n=33) and normal controls (n=39) were analyzed by Matrix Assisted Laser Desorption/ionization Time-Of-Flight Mass Spectrometry (MALDI-TOF MS). The severity of stroke was assessed with the National Institutes of Health Stroke Scale (NIHSS) and functional outcome was assessed with modified Rankin Scale (mRS). Results 55 peaks were significantly different between ischemic stroke patients and the controls (P<0.05). There were 7 peaks between 1000~2000 Da, 4 increased peaks and 3 induced peaks in serum of ischemic stroke patients. One induced peak at 1864(m/z) had highest peak intensity in controls and induced significantly in ischemic stroke patients. While induce degree of this peak didn't correlate with the NIHSS, mRS, stroke risk factors and laboratory data. Conclusion The application of this potential biomarker is not restricted to certain subgroups of ischemic stroke patients,so it may serve as one reverse direction biomarker and provide support for early diagnosis and treatment of ischemic stroke.
5.Correlation between Anxiety of Gastric Cancer Patients and Their Resilience and Hope Levels

Objective To explore the relationship between anxiety ,resilience and hope levels among gastric cancer patients ,so as to provide evi?dence for psychological intervention. Methods Cluster sampling design was used to select 324 gastric cancer patients. The anxiety self?assess?ment scale,resilience scale and hope scale are used to measure the related indicators. Results The comparative analysis showed that patients with different genders,education levels and marital status had different scores in anxiety level and resilience level(P<0.05),and those with differ?ent genders and marital status got different hope levels(P<0.05). Correlation analysis showed that anxiety was negatively correlated with resil?ience and hope levels(P<0.05),the coefficient of correlations ranged from-0.556 to-0.124. Multiple linear regression analysis showed that edu?cation level,marital status,income level,resilience,hope levels explained portion of the variance of anxiety. Conclusion Clinical nursing staff should reduce the gastric cancer patients′anxiety by improving their resilience and hope levels.
6.Rapid screening of 28 alkaloids in food poisoning samples by liquid chromatography-tandem mass spectrometry

Abstract: Objective To investigate a poisoning incident caused by eating eight treasure congee, and establish liquid chromatography (LC)-mass spectrometry (MS)/MS screening method of 28 alkaloids to provide references for disposal of similar poisoning incidents. Methods LC-MS/MS was used for screening 28 alkaloids in the urine, eight treasure congee and food raw material, and the detected alkaloids were quantified. Samples were extracted with 0.4% formic acid aqueous solution and separated by a Acquity UPLC BEH C18 column (1.7 μm, 100 × 2.1 mm). Acetonitrile-0.2% formic acid aqueous solution was used as the mobile phase and gradient elution was adopted. The ionization mode was electrospray positive ionization mode, and the detection method was multi-reaction monitoring (MRM). Analytes were quantified with the external standard method. Results In the concentration range of 0-100 ng/mL, the linear correlation coefficient r were greater than 0.999 for 28 alkaloids. The recovery of 28 alkaloids in urine sample ranged from 63.0% to 105.0%, and the relative standard deviations (RSDs) were between 5.8% and 8.6%. The recovery of 28 alkaloids in eight treasure congee sample ranged from 72.0% to 109.0%, and the RSDs were between 6.3% and 9.7%. The recovery of 28 alkaloids in semen sesami nigrum sample ranged from 60.0% to 95.0%, and the RSDs were between 4.8% and 8.2%. Hyoscyamine (2 380.0 ng/mL), scopliamine (3.6 ng/mL) and rac-anisodamine (4.7 ng/mL) were detected in the patient's urine. Hyoscyamine (63.3 μg/g), scopliamine (5.7 μg/g) and rac-anisodamine (2.1 μg/g) were detected in eight treasure congee. Hyoscyamine (901.0 μg/g), scopliamine (80.0 μg/g) and rac-anisodamine (30.1 μg/g) were detected in the seed of Datura stramonium L. The ratio of scopliamine and hyoscyamine in the seed of D. stramonium was 1∶11, which complies with the characteristics of D. stramonium L. In urine sample, the proportion of scopliamine and rac-anisodamine was 0.15% and 0.20%, and hyoscyamine accounted for 99.65%. Conclusion Seed morphology, the content range and proportion of three alkaloids are all in accord with the characteristics of D. stramonium. Combined with the clinical symptoms of atropine poisoning, it can be deduced that this incident is a family food poisoning caused by accidental consumption of seed of D. stramonium L. The method can provide technical support for the clinical diagnosis and treatment of alkaloid poisoning patients, and also provide a basis for emergency detection and disposal of alkaloid poisoning events.
7.Endoplasmic reticulum stress mediates lipopolysaccharide-induced apoptosis in rat hepatocyte.

OBJECTIVE:
To investigate the role of endoplasmic reticulum stress (ERS) in lipopolysaccharide (LPS)-induced hepatocyte apoptosis.
METHODS:
Cells of the rat hepatocyte line BRL were cultured. The hepatocytes were treated with LPS, ERS inducer thapsigargin (TG), and ERS inhibitor 4-phenylbutyric acid (4-PBA), respectively or in their different combination. The cell viability was measured by MTT assay. The cyto-nuclear morphological changes of apoptosis cells were detected by the fluorescent dye Hoechst 33258. The apoptosis rate was assessed by flow cytometry with Annexin V-FITC/PI double-staining. Expressions of GRP78 as ERS marker protein, CHOP, caspase-12 and cleaved-caspase-3 as ERS related protein were detected by Western blotting.
RESULTS:
LPS could cause a decrease in cell viability and an increase in apoptosis rate in a dose- and time-dependent manner. The expression of GRP78, CHOP, caspase-12 and cleaved-caspase-3 proteins were significantly increased with LPS treatment. TG led to a marked decrease in cell viability and an increase in apoptosis rate, which aggravated the hepatocyte injury induced by LPS; whereas 4-PBA alleviated LPS-induced apoptosis.
CONCLUSION
ERS mediates LPS-induced hepatocyte injuries, indicating that ERS may play a vital role in the pathogenesis of LPS-induced hepatocyte injuries.

9.Determination of ferulic acid in Fuan oral liquid by high-performance capillary electrophoresis.

OBJECTIVETo establish a quality control method of Fuan oral liquid.
METHODSHigh-performance capillary electrophoresis (HPCE) was used to determine the content of feruild acid in Herba taraxaci, and thin-layer chromatography (TLC) was performed to identify Herba taraxaci and Bupleurum chinense. The condition of HPCE was optimized with a fuse silica capillary tube (70 microm x 60 cm) and 20 mmol/L sodium tetraborate buffer (pH9.18) at a constant voltage of 12 kV and temperature at 25 degrees celsius;, with the detection wavelength at 313 nm.
RESULTSClear spots were displayed on TLC. The calibration curve was linear in the range of 8-40 microg/ml for ferulic acid (Y=360.5-207.4, r=0.9997, n=5). The average recovery rate exceeded 95% with a RSD<3% (n=3).
CONCLUSIONThis method is simple and specific with a good reproducibility for quality control of Fuan oral liquid.

10.Chemical constituents from Callicarpa nudiflora and their cytotoxic activities.

The chemical consitituents from cytotoxic fraction of the Callicarpa nudiflora extract were isolated and purified by a combination of HP-20 macroporous resin, silica gel and Sephadex LH-20 column chromatographies. The structures were elucidated on the basis of the spectroscopic data and comparison of their spectroscopic data with reported data. The cytotoxicity was evaluated by the MTT assay. The 50% and 70% EtOH elutions of EtOH-extract showed significant cytotoxic activities, leading to the isolation of twelve compounds, which were identified as luteoloside(1), lutedin-4'-O-β-D-glucoside(2), 6-hydroxyluteolin-7-O-β-glucoside(3), lutedin-7-O-neohesperidoside(4), rhoifolin (5), luteolin-7, 4'-di-O-glucoside (6), forsythoside B (7), acteoside (8), alyssonoside (9), catalpol(10), nudifloside(11), and leonuride(12). Compounds 3-6, 10 and 12 were isolated from this genus for the first time, and compound 9 was isolated from this plant for the first time. The cytotoxicity assay demonstrated that flavonoids 1-6, in various concentrations, showed monolithic proliferation inhibitory activities against Hela, A549 and MCF-7 cell lines. Compounds 3, 5 and iridoid glycoside 11 possessed higher cytotoxicacivities. In short, flavonoids are the main components of cytotoxic extract from C. nudiflora, while phenylethanoid glycosides are the predominant ingredient but inactive to cancer cell lines. In addition, the minor iridoid glycoside expressed weak cytotoxic activity.
