1.Long-term influence of selective consecutive three-level appliance of Solis cage on cervical curvature

BACKGROUND: With the understanding of pathology of cervical diseases, cervical curvature has become one of
the important indexes of long-term clinical outcome. To restore and maintain cervical curvature has a profound impact on cervical long-term stabilization and cervical biomechanical environment.
OBJECTIVE: To evaluate long-term influence of selective consecutive three-level appliance of intervertebral Solis cage on cervical curvature for spondylotic myelopathy.
METHODS: From June 2008 to December 2010, 22 spondylotic myelopathy patients were treated with
consecutive three-level anterior cervical Solis fusion. There were 14 males, 8 females, with an average age of
45.1 years (ranged from 26-73 years). There were two cases of C2/3/4/5, seven cases of C3/4/5/6, 12 cases of C4/5/6/7, and one case of C5/6/7/T1 , total y 66 intervertebral spaces. Al cases were fol owed up for 2 years.
RESULTS AND CONCLUSION: Average blood loss amount was 40 mL (20-80 mL). Average operation time was 121 minutes (100-175 minutes). After fol ow-up for 30.7 months, al segments were fused. There was no implant migration or vertebral body col apse. Pre-operative JOA score was 11.94±3.61, 15.56±1.13 at 6 months post-operation, and 15.21±1.85 at 2 years after implantation. Pre-operative cervical curvature was (1.86±3.24) mm,(4.83±1.78) mm at 6 months post-operation, and (4.44±3.36) mm at 2 years after implantation. There were significant differences between pre-operative and at 6 months post-operation, pre-operative and at 2 years post-operation (P < 0.05). There was no significant difference between 6-month and 2-year post-operation (P > 0.05). Results indicated that selective consecutive three-level appliance of intervertebral cage for spondylotic myelopathy could improve cervical curvature and have long-term favorable clinical outcomes.
2.An analysis of latent tuberculosis infection among patients with rheumatic diseases

Objective To investigate the incidence of latent tuberculosis infection (LTBI) in patients with rheumatic diseases in order to find evidence for the prevention of mycobacterium tuberculosis (MTB) in these patients.Methods From January 2013 to July 2015,759 patients with rheumatic diseases and 38 health controls were enrolled.All of them underwent interferon-gamma release assays(T-SPOT.TB) to screen for LTBI.Incidence of MTB infection was evaluated in different groups and test was used for statistical analysis between groups.Results The incidences of LTBI in patients and health controls were 27.27% (207/759) and 10.53% (4/38),respectively.In 2013,24.66% (73/296) (standardized infection rate 23.37%) patients with rheumatic diseases were positive for LTBI screening test.In 2014 and 2015,the percentages were 32.02% (73/296) (standardized infection rate was 32.15%) and 25.96% (73/228) (standardized infection rate was 28.46%),respectively,which was statistically significant in these 3 groups (P =0.004).the infection rate in 2014 tended to be higher than that in 2013 (P =0.001).There were 30.24% (88/291) male and 25.43 % (119/468) female patients who were considered as LTBI.But the difference was not significant between genders.The infection rates between patients older than 60 years old and less was significantly different,which were 45.65% (42/92) and 24.74% (165/667),respectively (P =0.000).As far as diseases were concerned including rheumatoid arthritis,systemic lupus erythematosus,spondyloarthritis and other rheumatic diseases,the incidences were 33.93% (57/168),22.06% (45/204),25.73% (44/171) and 28.24% (61/216) respectively,without statistical significance.Conclusions The incidence of LTBI is high in patients with rheumatic diseases.Attention should be paid especially to elderly patients and rheumatoid arthritis patients who have relatively higher rates of LTBI.Careful monitoring and prevention measures are suggested to take in these patients.
4.Effect of Rhizoma Coptidis apozem on expression of AMP-activated protein kinase in skeletal muscle of metabolic syndrome rats

Objective:To study the effect of Rhizoma Coptidis apozem on expression of AMP-activated protein kinase(AMPK) in skeletal muscle of metabolic syndrome rats. Methods:The models were established by administering high fat diet. Rats were randomly divided into five group: normal control group, metabriolic syndome group, Rhizoma Coptidis apozem group, Berberine group, Metformin group. To estimate insulin resistance by euglycemic hyperinsulinemic glucose clamp(GC) tecchnique. To estimate expression of AMPK by using Western blot. Results: Compared with metabolic syndrome group, Rhizoma Coptidis apozem group had higher M-value, lower wet weight of innards fat, and higher protein level of p-AMPK-?. Conclusion: Rhizoma Coptidis apozem can improve insulin resistance, decrease innards fat, and regulate up expression of AMPK.

7.A 1:1 Matched Case-control Study on the Risk Factors for Inguinal Hernia in Children

Objective To explore the risk factors for inguinal hernia in children.Methods A 1:1 matched case-control study which based on hospital group data was carried out.201 cases aged 0~6 years with inguinal hernia and controls were enrolled,and their parents were investigated with questionaire.Conditional logistic regression model was used for single factor and multivariate factoranalysistoestimateoddsratios(OR) and the 95% eonfidence interval(95% CI).Results Inguinal hernia was related to ehildren ' s constipation(0R=3.520,95%CI:1.238~10.015),children's cry(OR=6.532,95%CI:2.651~16.091),mother ate pickles(OR=2.529, 95%CI:1.271~5.031) and maternal anemia(OR=6.809,95%CI:2.530~18.322) during the period of one month before pregnancy and the first 3 months of gestation,the children's family history of inguinal hernia(OR=I 1.250,95%CI:4.766~26.553).Conclusion Children's constipation,children's cry,maternal anemia and eating pickles during the period of one month before pregnancy and the first 3 months of gestation,children ' s family history of inguinal hernia are the risk factors for children' s inguinal hernia.
8.TNF-α induces endothelial dysfunction via PKC-ζ-dependent NADPH oxidase activation.

Endothelial dysfunction is implicated in a variety of cardiovascular diseases although the detailed mechanisms are not yet completely understood. A relationship has been suggested to exist between inflammation and endothelial dysfunction. TNF-α serves as one of the most important pro-inflammatory cytokines. The main objectives of the present study were to explore the effect of PKC-ζ on TNF-α-impaired endothelial function as well as the underlying mechanisms. Acetylcholine-induced endothelium-dependent vasodilation of mouse thoracic aorta stimulated by TNF-α was initially determined. PKC-ζ deficient mice and the specific inhibitor of NADPH oxidase were respectively applied to elucidate their roles in TNF-α-induced endothelial dysfunction. In vitro superoxide generation in HAECs was detected by DHE staining after administration of TNF-α. Meanwhile, the regulatory p47(phox) subunit of NADPH oxidase was evaluated by Western blotting and RT-PCR. The results showed that TNF-α conspicuously impaired endothelium-dependent vasodilation and the impairment was attenuated by either depleting PKC-ζ or inhibiting NADPH oxidase. In vitro TNF-α increased superoxide production and p47(phox) expression in HAECs, and such increases could be ameliorated by the specific PKC-ζ inhibitor. Our findings suggest that superoxide over-production triggered by PKC-ζ-dependent NADPH oxidase activation contributes to TNF-α-induced endothelial dysfunction.

10.Protective effect of mycophenolate mofetil to renal tissures in diabetic rats

Objective To assess the renal protective effects of mycophenolate mofetil(MMF) in diabetic model rats and to explore its mechanism. Methods SD rats were divided randomly into there groups: control group,diabetic model group and diabetic group treated with MMF. In 12 weeks, blood glucose(BG), blood urea nitrogen(BUN), serum creatinine(Scr), index number of kidney hypertrophy(kidey weight to body weight, KW/BW),creatinine clearance(Ccr) and 24- hour urinary protein (24Upro) were detected. Kidney tissure were examined by microscopy. The protein exprssion of intercellular adhesion molecule-1 (ICAM-1) and proliferating cell nuclear antigen(PCNA) in renal tissure were determined by immunohistochemical technique. The mRNA expression of ICAM-1 in kidney tissure were semi-quantitatively determined with reverse transcription-polymerase chain reaction(RT-PCR).
