Objective To evaluate the efficacy and safety, particularly eye safety of hydroxychloro-quine(HCQ)treatment in primary Sj(o)gren's syndrome(pSS)patients. Methods Forty pSS patients were en-rolled and treated with HCQ 400 mg/day for 12 months. This is a prospective open-label study.Clinical mani-festations,clinical efficacy, biochemical and immunoserological parameters as well as ophthalmological exami-nations were investigated every three months to assess the safety and tolerability. Results There were signifi-cant decrease in the erythrocyte sedimentation rate(ESR),immunoglobulin G(IgG),immunoglobulin M (IgM)and rheumatoid factor(RF)level after 6 months treatment with HCQ(P<0.01 or P<0.05).No changewas detected in serum antinuclear antibody(ANA),anti-SSA/SSB antibodies after treated for 12 months.Somepatients had partial improvement in symptoms such as dry mouth, dry eyes and arthralgia.During the treat-ment,no significant effect on serum alanine aminotransferase (ALT),blood urea (BUN),serum creatinine (Cr),whole blood count(WBC)or hemoglobin(Hb)could be discovered.Central semus retinopathv(CSR)was found in one patient after 6 months treatment with HCQ.However, its association with HCQ could not be confirmed since it was not compatible with the usual HCQ retinopathy. Conclusion HCQ can improve svmp-toms of some pSS patients and can significantly decrease ESR,IgG,IgM and RF level. The safety profile of HCQ is generally good. However, ophthalmological examination before and after a 6-month interval may be necessary in long term HCQ treatment.

Parenteral & Enteral Nutrition 2004;0(05):-
Objective: To evaluate the feasibility, the safety and the effect of early postoperative enteral nutrition in patients with esophageal cancer. Methods: On the first postoperative day,40 patients were randomly divided into two groups. One group was given isocaloric total parenteral nutrition through central vein(TPN group) and the other group was fed with enteral nutrition through nasoenteric tubes or jejunostomy tubes which were inserted during the operation(EEN group). All variables were measured before operation and on postoperative day 1 and 8.Blood was drawn at different time points to assess albumin, prealbumin and transferrin.Immune response was determined by immunoglobins, total lymphocytes,T-lymphocyte subsets and NK cells. Inflammatory response were determined by C-reactive protein,IL-6 and TNF-?.Morbidity,mortality and length of hospital stay were also evaluated. Results: Tolerance of enteral nutrition was excellent.Neither mortality nor serious morbidity occurred in all patients during the period of study. After the study, the levels of serum prealbumin, total lymphocytes,CD3,CD4 and CD4/CD8 ratio in the EEN group were significantly higher than the levels in the TPN group. The concentrations of IL-6 and the length of hospital stay in the EEN group were significantly lower than in the TPN group. Conclusion: Early postoperative enteral nutrition in patients with esophageal cancer is safe, and also has the benefit effects of enhancing the body status of nutrition and immune function, and reducing the inflammatory response and the length of hospital stay.

Chinese Journal of Medical Education Research 2016;15(1):17-20
The research data at home and abroad show that the overall burnout rate of clinical professional graduate students was high. They are vulnerable to burnout because of long duty hours, learn-ing pressure, intense and overloading work and especially the significantly reduced personal accomplish-ment in the situation of the contradiction between doctors and patients in our country. Burnout not only re-sults in psychological distress and physical symptoms, but also has negative effect on the quality of graduate medical education during residency training. Therefore, educators need to develop an active awareness of burnout and ought to perform interventions such as formulating the appropriate learning goals, improving the work efficiency, reducing work hours, positive psychological counseling and stress management training to prevent such occurrences.

Objective To develop a multiplex PCR-based reverse line blot(mPCR/RLB) hybridization assay to detect, simultaneously, seven genes encoding AR-erm(A/TR),erm(B),mef(A/ E),tet(M),tet(O),aphA-3,aad-6 and two AR-related genes,int-Tn and mreA in group B streptococcus. Methods Nine pairs of specific primers and Oligonucleotide probes targeting erm(A/TR), erm(B),mef(A/E),tet(M),tet(O),aphA-3,aad-6 int-Tn and mreA respectively were modified according to former studies or designed in this study. The primers and probes were labeled with biotin and amino, respectively. The nine genes were amplified simultaneously in the same tube.PCR product hybridized with the probes labeled in the BiodyneC nylon membrane to detect the nine genes. To detect the sensitivity and specificity of the method developed,PCR with single pair of primer targeting each gene were tested in 318 isolates tested and the results were compared with the one abtained by RLB.Results The nine resistance-related genes could be successfully detected by mPCR/RLB assay developed in this study.Based on sequencing,21 of 22 isolates with mef had mef(E)and eight of 353 with int-Tn had an atypical sequence.Except for the above 29 genes, all the others corresponded well with the results obtained by single pair primer PCR.Conclusion The mPCR/RLB assay developed in this study is simple, rapid and suitable for surveillance of antibiotic resistance in GBS.
