[Objective]To discuss the efficacy of medial wedged proximal tibial osteotomy for treating genu varus deformity.[Methods]From July 1998 to October 2007,49 patients with genu varus deformity were treated by medial wedged proximal tibial osteotomy combined with internal fixation.Full-length anterior-posterior radiographs were taken preoperatively at 8 weeks and 1.5 years postoperatively. The parameters including the femorotibial angle and the medial joint space were measured on the radiographs. The pre-and postoperative function of knee were evaluated according to the HSS score system of the knee.[Results]All the 49 patients were followed up from 18 to 128 months. The mean postoperative femorotibial angle decreaced from 187.5??5.3? to 172.6??3.6?.The mean postoperative HSS score significantly improved from 83.2?15.3 to 47.2?17.6 points after 1.5 years of operation. The medial joint space increased from(2.4?1.2) mm to(4.3?1.2) mm.There were complications in 5 cases: 3 cases of intra-articular fracture during operation and 2 of postoperative superficial wound infections. There were no delayed union,no recurrence of varus deformity, and no blood vessels or nerves injury in all cases. The pain relieved and walking function improved significantly postoperativly.[Conclusion]Medial wedged proximal tibial osteotomy combined with internal fixation is an effect approach to treat genu varus deformity.

Objective To investigate factors influencing islet isolation and purification in dogs and to explore how to obtain massive purified viable islets from canine pancreas. To provide experimental evidence for pancreatic islet transplantation in clinical practice. Methods Islets were isolated from pancreas of 22 dogs by modified automated techniques, followed by purification using continuous density gradients. The purity of islets was determined by dithizone (DTZ) staining, and the function of islets was evaluated by glucose stimulating insulin release test. Results The crude islets yield was ( 155 040 ±310) IEQ/pancreas while the yield was (74 200 ±185) IEQ/pancreas after purification. The average purity was (89.4 ±2.6) %.The recovery rate was(47.8 ±1.3) %.Islet viability was(93.0 ± 1.7)%.The modified techniques improved both the yield and the quality in islet isolation and purification. The islets were morphologically intact after purification.Glucose stimulating insulin release test showed that the difference of the secreted insulin concentration had statistical significance ( P <0.001 ) between low concentration glucose group and high concentration glucose group. Conclusion It is of great value for clinical research to obtain enough islets through islet isolation and purification in dogs.

Objective To explore a method to improve the nutritional state and immune function of patients after radical gastrectomy, and to decrease postoperative complication rate and accelerate recovery from operation. Methods In line with the concept of fast tract surgery (FTS),a new perioperative nutritional schedule was made for patients prior to radical gastrectomy, ehich included preoperative nutritional risk screen and prophylactic enteral nutrition, early postoperative light enteral nutrition, early ambulation and perioperative nursing care.Patients with gastric cancer were randomly assigned into fast track group or a simple control group.New perioperative schedule was executed in the patients of fast track group and traditional nutrition support in control group. After operation, serum albumen was measured to evaluate the patient's nutritional state and serum IgA to evaluate gastrointestinal immune function. In addition, the time to passage of flatus, postoperative recovery time and complication rate after operation were compared. Results Patiens in FTS group (n=35) showed higher serum albumin and serum IgA on d1 and d3 after operation than those in the control group (n=25) (P

ObjectiveTo observe the effect of reduced glutathione(GSH) on expression of malondialdehyde(MDA),glutathione peroxidase(GSH-PX) and superoxide dismutase(SOD) after focal cerebral infarction in rats.MethodsRat models of middle cerebral artery occlusion(MCAO) were estabilished with thread after 2-hour ischemia and 6-hour reperfusion.Rats were divided at random into three groups,i.e.,sham-operated,control and treatment group(with GSH 1200 mg/kg) respectively. After the rats model was performed, neurology deficit score, the size of brain infarct region and the change of brain tissue pathologic were evaluated.Contents of MDA and activity of SOD and GSH-PX were detected with spectrophotometer.ResultsCompared with the control group,GSH can ameliorate neurological deficit score and decrease infarct volume induced by MCAO.GSH may reduce contents of MDA and improve activity of SOD and GSH-PX in brain tissue.ConclusionGSH may reduce contents of MDA and improve activity of SOD and GSH-PX so as to enhance capability of eliminating oxygen free radical, and play a neuroprotective effect after cerebral focal ischemia reperfusion.

Objective To study clinicopathologic significance of microvessel density(MVD)and the expres- sion of CD34 in nasopharyngeal carcinoma. Methods Using Elivision Plus immunohistochemistry method.50 cases of nasopharyngeal carcinoma and 15 cases of inflammation nasopharyngeal tissues were stained with CD34.Results In comparison with inflammation nasopharyngeal tissues MVD(9.23?1.84),the MVD in nasopharyngeal carcino- ma(21.92?7.80)was significantly higher(P

Objective To investigate the role of survivin and P53 in apoptosis of gastric adeno- carcinoma,as well as its relationship with the clinicopathologic features and the prognosis. Methods The gas- tric tissue microarrays were composed of those from 100 cases of gastric cancer and 30 controls. At these tissue microarrays, expressions of survivin and P53 were investigated immunohistochemically, and tumor cell apoptosis index was examined by TUNEL method.Of the 100 cases,47 cases were followed-up from 14 months to 13 years,in which the survival was analyzed. Results Two paraffin-embedded gastric carcinoma tissue micro- arrays were successfully constructed, including 114 and 116 tissue spots, respectively.Immunohistochemical analysis showed that survivin was expressed in 78 cases (78%).No expression of survivin was detected in control tissue (P0.05).In the 47 cases with followed-up data, univariant analysis revealed that the survival was correlated with invading of vessel and nerve,TNM stages, and expression of survivin. The histological grades and expression of P53 were not related to prognosis. However,Cox stepwise proportional hazards analysis showed that only TNM staging and survivin status retained significant independently in prospecting prognosis. Conclusions The expression of survivin was associated with the pathologic features,TNM stages and prognosis in gastric carcinoma, indicating that overex- pression of survivin may be a poor prognosis factor for gastric carcinoma.

OBJECTIVETo analyze the relationship between Chinese medical constitutions and chemotherapy-induced leucopenia (CIL) of primary breast cancer patients.
METHODSTotally 306 breast cancer patients undergoing adjunctive chemotherapy for the 1st time, and effective 291 breast cancer patients were recruited in this study.Nine Basic Constitutional Scale was used before first chemotherapy. Chinese medical constitutions were classified and quantitatively scored. The highest grading for any item of adverse reactions in each case during the whole chemotherapy course was recorded after chemotherapy. Data were statistically analyzed using SPSS16.0.
RESULTSThere was no significant difference in CIL between different chemotherapy regimens and various Chinese medical constitutions of breast cancer patients (P > 0.05). Yang deficiency constitution is one risk factor for CIL. The higher the score of yang deficiency constitution, the more severe the CIL.
CONCLUSIONSYang deficiency constitution was correlated with the degree of CIL. The higher the score of yang deficiency constitution, the greater the risk of III-IV grade CIL in breast cancer patients.
