1.Interventional treatment of iliac and femoral vein stenosis concomitant with thrombosis

Objective To study the effect of interventional treatment of iliac and femoral vein stenosis concomitant with deep venous thrombosis. Method Fifty-three patients were divided into 5 groups. In group A after placing into inferior vena cava a filter,11 patients adopted Amplatz Trombectomy Device for thrombolysis or ORSIS thrombolysis and persistent thrombolysis through popliteal vein. In group B thrombus was taken out through guiding catheter and then persistent thrombolysis through popliteal vein after placing into inferior vena cava filters in 9 cases. In group C 13 patients adopted persistent thrombolysis through femoral arteries. In group D 8 patients received persistent thrombolysis through popliteal vein. In group E persistent thrombolysis through foot veins was carried out in 12 patients. Seventeen patients received implanted stents and balloon-expansion in iliac and femoral veins. Results Symptoms disappeared in 26 patients(49.0%), significantly improved in 21 patients (39.6%), improved in 3 patients (5.7%), did not improve in 3 patients (5.7%), respectively. The repatency of iliac and femoral vein was achieved in more than 80% of the 17 patients. Complications developed in 3 cases in the course of thrombolysis. Conclusion The effect of mechanical removal of thrombus, persistent thrombolysis through catheter and transluminal angioplasty is safe and satisfactory.

Integration of "5+3" training mode is to accelerate the construction of standardization of clinical medical personnel training system,and the establishment of the new training mode.The training goal is to make the students become the high-level clinicians with the ability of clinical thinking and clinical practice,and scientific research and teaching,who can independently and pro-fessionally prevent and treat the related common disease.Therefore,guided by the general training goal of our students,we designed and practiced a teaching mode centered on "immune disease mechanism analysis" in the teaching of medical immunology,aiming at improving students'ability of clinical thinking,self learning and team coorperation.
5.Therapeutic efficiency of amniotic extraction for dry eye rabbit model

AIM: To investigate therapeutic efficiency of amniotic extraction on dry eye in rabbit model induced by topical benzalkonium chloride (BAC).
METHODS: Totally 26 rabbits (26 right eyes) with dry eye model were studied and divided into two groups:group A (control group with PBS eye drops, n = 13) and group B ( amniotic extraction group, n = 13). Another two rabbits were chosen as normal control. The SchirmerⅠ tests ( S Ⅰ t) and corneal fluorescein staining ( FL) were made, and the tear total protein content, amylase activity, lactoferrin, lysozyme contents, goblet cell density were performed in two groups before treatment and 1, 2, 4 and 8 wk after treatment.
RESULTS: There were significant differences in SIT, FL scores, lysozyme activity and goblet cell density among different groups at different time points (P<0. 05). But, there was no significant differences in SⅠt, FL scores, lysozyme activity and goblet cell density between two groups before treatment (P>0. 05). After 8wks' treatment with PBS, the mean differences of the group A showed great changes in SⅠt, lysozyme and goblet cell density compared with those before treatment ( P < 0. 05); but there was no significant differences in FL scores compared with those before treatment (P>0. 05). As for group B, 8wks after treatment, there were statistical changes in SⅠt, FL, lysozyme (P<0. 05); but there was no significant differences in goblet cell density compared with those before treatment ( P > 0. 05). It was evident that statistical differences were observed in S Ⅰ t, FL scores, lysozyme activity and goblet cell density between two groups at each time point (P<0. 05). However, there were no significant differences in total protein, lactoferrin, amylase activity at different time points (P>0. 05). Meanwhile there was no significant differences in total protein, lactoferrin, amylase activity between two groups before treatment ( P > 0. 05 ). But there were significant differences in total protein, lactoferrin, amylase activity between two groups after 4 and 8 wks'treatment (P<0. 05).
CONCLUSION: Amniotic extraction has significant therapeutic effect on the dry eye in rabbit model.
6.The function and drug development progress of free fatty acid receptor 1 in chronic inflammatory diseases

As a member of G protein coupled-receptors superfamily, free fatty acid receptor 1 (FFAR1), is also known as GPR40, has been shown to regulate numerous pathophysiological processes in a variety of tissues and organs. The activated FFAR1 has a variety of biological functions. For instance, it can not only regulate metabolism of fatty acids and glucose, but also play an important role in immune inflammatory response, it may be a potential drug target for the treatment of various chronic inflammatory diseases. In this review, we focus on the recent researches of FFAR1's action in the regulation of pathophysiological processes, its molecular mechanism and new agonists development. At the same time, this review will take the discovery of series FFAR1 agonists as examples, and display the applied prospects of FFAR1.
7.Prediction of hemorrhagic transformation in acute ischemic stroke using permeability surface of CT perfusion

Objective To investigate the value of permeability surface (PS) in predicting hemorrhagic transformation (HT) in acute ischernic stroke (AIS) using CT peffusion (CTP).Methods The study included 31 consecutive patients who presented symptoms suggestive of an AIS for 3-9 h. All patients underwent CT examination (noncontrast CT,CTP).HT was determined by follow-up CT images.According to presence of HT,the AIS was divided into HT group (PSHT,11 patients) and non-HT group(PSNo-HT,20 patients).PS,cerebral blood flow (CBF),cerebral blood volume (CBV) and mean transit time (MTT) on both sides of brains were measured.The relative PS(rPS),relative CBF (rCBF),relative CBV(rCBV) and relative MTT(rMTT) were obtained by calculating the ratio of the values of bilateral regions.The rPS between PSHT and PSNo-HT was compared with an exact Wilcoxon signed-rank test. The rCBF,rCBV,rMTT and the PS of the ischemic side between PSHr and PSNo-HT were compared with independent-sample t test.Meanwhile,Spearman rank correlation analysis was conducted to analyze the relationship between the CTP parameters and HT.ResultsThe PS value of ischemic side was (1.61±0.77) ml · min - 1 · 100 g-1 for the PSHT group,and the value was (0.91 ± 0.49) ml · min - 1 · 100 g- 1 for the PSNo-HT group.For the PSHT group,rPS,rCBF,rCBV,rMTT were 2.76 ±0.78,0.32 ±0.18,0.66 ±0.31,2.67 ±0.71,and for the PSNo-HT group,rPS,rCBF,rCBV,rMTT were 1.35 ±0.19,0.50±0.21,0.91 ±0.28,2.62 ± 1.31.Compared with PSNo-HT,PSHT had higher rPS and PS value,and there were significant statistical differences (U =0.000,t =3.070,P <0.01).But rCBF and rCBV values were lower in the PSHT group compared to the PSNo-HT group,and there were significant statistical differences (trCsF =2.343,trCBV =2.210,P < 0.05).There was no significant statistical difference in rMTT between the two groups(t =0.118,P > 0.05).Significant positive correlations were detected between the rPS and PS with HT(r=0.496,0.821,P <0.01).ConclusionsThe value of rPS is helpful in predicting HT in AIS.And it can be used as a predictor in determining clinical personalized treatment and thus reduce the incidence of adverse events.
8.Long-term follow-up and outcome of patients who survived more than 7 years after liver transplantation:a single-center analysis

Objective To investigate the occurrence and prevention measures of long term complications in long term survival recipients after liver transplantation.Methods In the recipients undergoing liver transplantation from Sept. 2003 to Dec. 2004,by Nov. 30,2011,there were 62 cases with the survival time more than seven years.The clinical data and follow up examination results of these 62 cases were retrospectively,including weight,blood pressure,blood sugar,blood lipids,and liver and kidney functions. The incidence of long-term complications was statistically tested.Results Postoperative metabolic complications including overweight or obese occurred in 21 cases (33.9%), new onset diabetes in18 patients (29%), hyperlipidemia in 17 cases (27.4%),hypertension in 9 cases ( 14.5 % ),and kidney dysfunction in 12 patients ( 19.4% ).The incidence of diabetes and hyperlipidemia in the patients with overweight and diabetes (respectively 52.4% and 42.9%) was significantly higher than in the normal weight group (respectively 17.1 % and 19.5 %)(P<0.05).In 58 recipients with primary diseases of hepatitis B-related liver diseases,one case had hepatitis B virus reinfection. In 17 recipients with primary disease of primary liver cancer,tumor recurrence occurred in 2 cases.During the follow up period,4 patients received liver re-transplantation due to hepatic artery stenosis (1 case) or biliary complications-induced loss of the transplanted liver function (3 cases).Conclusion The major complications of the long term survival recipients after liver transplantation are metabolic complications and primary disease relapse. Postoperative long-term follow up and monitoring of recipients is recommended to prevent and treat a variety of long-term complications.
9.Relationship between calcified aortic stenosis and peripheral blood inflammatory cells and lipids in the elderly

Objective To investigate the relationship between calcific aortic valve stenosis and inflammatory cells in peripheral blood and dyslipidemia in elderly patients(age ≥65 years).Methods A total of 76 elderly patients(≥65 years old)diagnosed as calcific aortic stenosis in our hospital from June 2015 to June 2017 were selected as the case group,and 78 seniors(excluding valvular heart disease)hospitalized due to chest discomfort were selected as the control group.The white blood cell count (WBC), neutrophil ratio (N%), neutrophil count (N), lymphocyte count (L), neutrophil lymphocyte ratio (NLR),hypersensitive C-reactive protein(hs-CRP),N-terminal B-type natriuretic peptide(NT-pro BNP),total cholesterol (TC),triglyceride(TG),apo-α,high-density lipoprotein(HDL),low-density lipoprotein(LDL),very low-density lipoprotein (VLDL)and other indicators were tested on admission.All these indicators were compared between the two groups. Results The levels of WBC, N%,N,NLR, hs-CRP, NT-pro BNP,VLDL were higher in the case group than those in the control group.The levels of L and HDL were significantly lower in the case group than those of the control group(P<0.05).Multiple regression analysis showed that smoking, the increased hs-CRP and NT-pro BNP levels were independent risk factors for senile calcific aortic stenosis.Conclusion Senile calcific aortic stenosis is not a simple degenerative disease.It is closely related to systemic inflammatory response and abnormal lipid metabolism.
10.Anti corruption during high-value consumables purchasing in medical institutions

Object To standardize hospital high-value consumables purchasing procedure to eliminate corruption.Methods The deficiencies of some tertiary Grade A hospital were analyzed in high-value consumables purchasing, and some suggestions based on foreign countries' achievements were proposed including completing high-value consumables purchasing procedure and making anti-corruption efforts in medical institutions.Results It's suggested that high-value consumables management system be completed, admission qualification management and purchasing procedure be standardized and supervision mechanism be established in the hospital.Conclusion Improved high-value consumables purchasing procedure contributes to anti corruption in medical institutions.
