Objective To summarize the surgical skills of the pancreaticoduodenectomy via mesenteric approach for the treatment of duodenal gastrointestinal stromal tumors (GISTs).Methods The clinical data of 1 patient with huge duodenal GIST combined with gastrointestinal hemorrhage and liver metastasis who was admitted to the Second Affiliated Hospital of Harbin Medical University in June 2014 were retrospectively analyzed.The patient had continuous bleeding before the operation.The results of computed tomography (CT) showed that there was a solid tumor below the liver and in front of the right kidney,with the size of 12.2 cm × 8.1 cm,inferior vena cava was squeezed by the tumor,a solid tumor was detected in the right lobe of liver,and the colon was suspiciously invaded by the tumor.The patient received pancreaticoduodenectomy,right hemicolectomy and right lobectomy of liver metastases.The patient was followed up by out patient examination and telephone interview up to August 2014.Results The operation time,volume of blood loss and volume of red blood cell (RBC) transfusion were 420 minutes,800 mL and 2 U,respectively,with a full recovery of patients.The duodenal GIST with metastasis tumor and necrosis located at the colon,pancreatis,liver and renal hilum was confirmed by pathological diagnosis.The life quality of the patient was good by follow-up at postoperative month 3.Conclusion In the pancreaticoduodenectomy via mesenteric approach,surgeons perform vigorously the method of mesenteric approach at the first stage of operation,it could maximally reduce the risk of operation and the volume of blood loss.

Objective To evaluate if the assay of platelet-associated immunoglobulin G (PAIgG) can improve the sensitivity of monoclonal antibody immobilization of platelet antigens assay (MAIPA) in immune thrombocytopenic purpura (ITP).Methods Anti-GP Ⅱb/Ⅲa and anti-GP Ⅰb/Ⅸ autoantibodies were detected by a modified MAIPA;PAIgG was detected by competitive ELISA.MAIPA and PAIgG were done simultaneously in 190 patients with thrombocytopenia,in which 132 were diagnosed as ITP according to the diagnostic criteria Kappa test was used to analyze the concordance between MAIPA and PAIgG.Results The sensitivities of PAIgG alone or in parallel with MAIPA to ITP were 32.6% and 37.2% higher than MAIPA alone respectively,while their specifieities were 51.7% and 56.9% lower respectively.The sensitivity,specificity,positive predictive value,negative predictive value of PAIgG in series with MAIPA were all not superior to MAIPA alone.The kappa values between MAIPA and PAIgG in ITP and non-immune thrombocytopenia were 0.129 and 0.012 respectively,which meant the concordance between them was poor.Conclusion The detection of PAIgG should not be used as screening before MAIPA in distinguishing immune from non-immune thrombocytopenia.

Objective To understand the prevalence and risk factors of non-suicidal self-injury in middle school students.Methods 1312 middle school students of Pengzhou and Santai were selected to fill in a Risky Behavior Questionnaire for Adoluscents (RBQ-A),Family Environment Scale ( FES ),Center for Epidemiological Survey,Depression Scale (CES-D),Adolescent Self-Rating Life Events Check List (ASLEC),Social Support Scale for Adolescents (SSSA) and self-administered questionnaire.In all the research subjects,1288 were qualified for the study in April 2011 before the risk factors for non-suicidal self-injury were identified by logistic regression.Results In 1288 middle school students,22.67％ had a history of non-suicidal self-injury,with 22.70％ in boys and 22.64％ in girls.63.36％ of students had injured themselves through variouslyways,more seen in boys (26.88％) than in girls (11.36％ ) who cut or burnt themselves.The scores of ASLEC and CES-D in non-suicidal self-injury group appeared higher than that in the control group and the score of SSSA was found higher in the control group.The main risk factors for non-suicidal self-injuries were family conflict,depressive emotion,negative life events and receiving less social support.Conclusion The prevalence of non-suicidal self-injury among middle school students in Pengzhou was high,whicn called for more attention.

Objective To investigate the effects of weightlessness on the apoptosis of hepatocytes in rats.Methods Eighty-four adult male Wistar rats were randomly divided into weightlessness group and control group according to the random number table.Tail-suspension was used to simulate the weightlessness model.Six rats were sacrificed in each group at the end of day 1-7.Hepatocyte apoptosis and p53 expression were detected by TUNEL assay and immunohistochemistry PV-6001,respectively.All data were analyzed by variance analysis.Results An increase of apoptosis index and up-regulated expression of p53 were detected in weightlessness group.The level of apoptotic index Was significantly higher in weightlessness group than in control group from day 1 to day 5(F=77.608,P<0.05),and the apoptotic indexes of the 2 groups were close at day 6 and day 7.The p53 proteins with positive expression were mainly detected in the nucleus.The level of positive expression of p53 in liver tissue in weightlessness group was higher than that in control group at the early stage(F=113.063,P<0.05).Few hepatocytes with positive expression were detected in control group and weightlessness group at a later stage.Conclusion There is a close relationship between hepatocyte apoptosis,changes of P53 expression and stress response and tolerance to weightlessness.

Objective To investigate the features of familiar facial palsy,ophthalmoplegia and dysphagia characterized by autosomal dominant inheritance in a family and to discuss the classification and pathogenesis of the disease.Methods Clinical,electrophysiological,pathological examinations were performed and blood samples were obtained from 5 patients and 26 family members.PCR protocol was used to identify a certain gene. Results In the 5 patients receiving physical examination,all had ptosis,external ophthalmoplegia,facial paralysis,dyphagia,hoarseness,decreased pharyngeal reflex;4 had amyotrophy of muscle of tongue,temporal nuscle,masseter and muscles of distal lower limbs;3 had proximal limb asthenia and distal limbs amyotrophy.Compared to those of oculopharyngeal muscular dystrophy(OPMD)with similar symptoms and signs,both electrophysiological manifestation and pathological findings of the family members supported the diagnosis of muscular dystrophy,but the(GCG)6(GCA)3GCG in the first exon of PABPN1 mutated neither in normal family members nor in patients.Conclusions This family presents clinical manifestations somewhat resembling to those of OPMD and distinctive to other disorders,but has a totally different genetic background from OPMD.It may be a new subtype of muscular dystrophy.

Objective To study the changes of serum interleukin(IL)-4,IL-12 and correlation with cellular immunity in children with asthma of different stages.Methods Fifty asthmatic children were randomly selected, including 30 cases in attack stage (group A) and 20 cases in remission stage (group R). Withanolide A is a biologically active secondary metabolite occuring in roots and leaves of Withania somnifera. In the present study, adventitious roots from leaf explants of W. somnifera were induced for the production of withanolide-A by Agrobacterium tumefaciens strain C58C1 to obtain hair roots. Hair roots induction rate reached 30%. The withanolide A was determined by HPLC in different hair roots lines and different parts of W. somnifera. The average content of withanolide A in all hair roots lines were 1.96 times as high as that in wild-plant, the concentration of withanolide A in hair roots (1.783 mg x g(-1) dry weight) were 1.51 times as high as the roots of wild W. somnifera (1.180 mg x g(-1) dry weight), respectively. It is possible to obtain withanolide A from hair roots culture of W. somnifera.

Objective To investigate the application of wall shear stress (WSS) in the carotid sinus of type 2 diabetes mellitus (T2DM) patients,and analyze the spatial distribution of WSS with contrast enhanced flow field (CEFF) quantita tive visualization analysis software.Methods Totally 47 T2DM patients and 25 healthy subjects were investigated.According to the intima-media thickness (IMT) of common carotid artery (CCA),the T2DM patients were divided into IMT normal group (n=21) and IMT thickening group (n=26).WSS in the carotid sinus was calculated by CEFF analysis soft ware,the corresponding spatial distribution maps of WSS were designed.WSS of the posterior wall of internal carotid artery (ICA) was recorded and statistical analysis.Results Both in the carotid sinus of healthy subjects and T2DM patients,there were two low WSS regions and one high WSS region.There were a large region of low WSS at the posterior wall of CCA and ICA,a small region of low WSS at the anterior wall of CCA,and a high WSS region at the anterior wall of ICA.The WSS at the posterior wall of ICA both in the IMT normal group [(3.39 ± 0.60)dynes/cm2] and IMT thickening group [(2.58±0.46)dynes/cm2] were significantly lower than that in control group ([3.74±0.53]dynes/cm2,both P＜0.05).WSS in IMT thickening group was lower than that in IMT normal group (P＜0.05).Conclusion CEFF quantitative visu alization analysis might constitute an approach to assess the presence of early-stage arteriosclerosis by measuring the WSS in the carotid sinus.

Objective To investigate the possible role of interstitial cell of Cajal (ICE)in the pathogenesis of diabetic gastroparesis and the protective effect of"Xiaopi prescription (消痞方)".Methods Fifty healthy male Spregue-Dawley (SD) rats were randomly divided into 5 groups (each n=10):normal, model,and 3 Xiaopi prescription groups:low,middle and high dosages.Diabetes was induced by intravenous injection of alloxan,and equal amount of normal saline was intravenously injected in the normal group.Gastric lavage method was used to administer the traditional Chinese medicine decoction of Xiaopi prescription in corresponding amount (5,10 and 20 g?kg~(-1)?d~(-1)) in respective low,middle and high dosage groups.In the normal control group and diabetic model group,only equal amount of normal saline was administered into the stomach.Gastric emptying rate was measured by method of nutritious semisolid paste;c-kit positive cells of ICC were quantitatively measured with immunohistochemistry assay and computer image analysis system;c-kit mRNA positive cells were quantitatively measured with in situ hybridization and computer image analysis system.Results①Gastric emptying rate:The rate was significantly lower in the model group than that in the normal control group (P0.05),but higher than those in the model group and the low dosage group (all P0.05).②c-kit immmunohistochemistry:c-kit positive cell presented yellow in color,and its membrane was stained yellow,this kind of cells primarily were distributed around the neural plexus in the inter-space between the circular and longitudinal muscular fibers, and around the ganglionic cells forming"sheath-like"structure.The results of numbers of c-kit positive cells in the various groups:the number of the cells in the model group was significantly lower than that in the normal group (P0.05),but the numbers in the former two dosage groups were obviously higher than those in the model group (all P0.05),being significantly lower than that in the normal group,middle and high dosage groups (all P0.05),but obviously higher than those in the model group (all P0.05),being significantly lower than that in the normal,middle and high dosage groups (all P
