Journal of Chinese Physician 2015;17(11):1650-1653
Objective To investigate the carotid plaque conditions and clinical commonly used test events in patients with type 2 diabetes mellitus (T2DM) and to explore the risk factors affecting the stability of plaques in carotid artery. Methods According to the results of cervical vascular color Doppler ultrasound examination, 125 patients with type 2 diabetes mellitus were divided into unstable plaque group (n=21), stable plaque group (n =54), and non-plaque group (n =50).Analysis related results including age, gender, systolic blood pressure (SBP), diastolic blood pressure (DBP), intima-media thickness (IMT), fasting blood glucose (FBG), triglycerides (TG), total cholesterol (TC), low-density lipoprotein (LDL), high-density lipoprotein (HDL), uric acid (UA), fibrinogen (Fbg), D-dimer (D-D), free triiodothyronine (FT3), free thyroxine (FT4), and thyroid stimulating hormone (TSH) were analyzed. Results Age, gender, SBP, DBP, TG, HDL, D-D, and TSH were without significant differences among groups.Glucose and Fbg were significant different (P < 0.05) between unstable plague group and stable plaque group/no plaque group. No significance difference was found between stable and no plaque groups.IMT, TC, LDL, and FT4 were significant different (P < 0.05) between unstable and stable plaque groups, and between stable and no plaque groups.Logistic multiple regression analysis suggested that IMT and TC might be independent risk factors for the stability of plaques in carotid artery of patients with type 2 diabetes mellitus (P < 0.05).Conclusions The factors affecting formation of plaques in carotid artery include glucose level, TC, and LDL in patients with type 2 diabetes mellitus. The factors affecting its stability include thickness of IMT, TC, LDL, and FT4.IMT and TC were the independent risk factors for the stability of plaques in carotid artery.

ABSTRACT:Objective To analyze the stress changes of tooth tissues in the medial-occlusal (Class Ⅱ)cavity restored with different kinds of inlay materials by the three-dimensional (3-D)finite element. Methods The inlay restored 3-D finite element model of medial-occlusal (Class Ⅱ)cavity was established by CBCT scanning method and reverse engineering software Mimics,Geomagic Studio,Pro/E5.0 software, and finite element analysis software. The Von-mises stress and change tendency of tooth tissues and four different kinds of inlay materials were compared after taking vertical loading and tongue 45°to loading. Results There were different stress levels of tooth tissues among different inlays of medial-occlusal (MO)Class Ⅱ cavity. The stress level of the cobalt chromium alloy inlay was the largest, the stress level of the gold alloy inlay was lower, and the stress level of the composite inlay was the lowest.Dental tissues stress distribution was similar for the four different restorative materials, and focused on hole bottom of dentin near the pulp in dentin. Conclusion Compared with ceramics, gold alloy and cobalt chromium inlay restoration, restoration with composite resin inlay can reduce the organization stress of the remaining tooth in the medial-occlusal (Class Ⅱ)cavity type.

Objective:To analyse the stress distribution of the tooth and inlay with the restoration of resin, porcelain, gold alloy,co-balt chromium materials respectively.Methods:3-D finite element models of mandibular first molar with meiso-occlusal (class II) cavity and different inlays were established.Von-mises stress distribution on the tooth and inlay with vertical load and lingual load at 45°were analyzed. Results:After restoration peak stress of high elastic module materials and dental inlays was higher than that of the low elastic module material inlays, however, the restorations of the different elastic module materials had the similar stress distribution trend. The stress under of lateral load(lingual at 45°)to the teeth inlays was significantly higher than that under vertical load.Peak stress concentration of tooth was on the bottom of the cavity near the pulp chamber dentin;inlay peak stress distribution is mainly in its corresponding to the gingival wall.In the tooth tissues stress level of the contact surface of inlay restoration, the strength was as the fol-lowing:Composite resin inlay >ceramics inlay >the gold alloy ceramics inlay >cobalt chromium alloy inlay. The stress level of the inlay of the four kinds of inlay restoration materials was just opponent with the tooth tissues. Conclusion:Gingival wall is the weakest part of meiso-occlusal(class II)cavity inlay restoration, while near the pulp chamber at the bottom of the cavity is the weakest part of the tooth.Among the 4 materials Under the same load condition, composite resin inlay restoration has minimal tooth stress and uniform stress distribution, and can reduce the posibility of odontoschism and microleakage.

BACKGROUND: As a cytokine highly expressed in internal organs, visfatin could be used as a biomarker of systemic inflammation response for chronic obstructive pulmonary diseases, but few studies have reported the use of visfatin in severe pneumonia. The present study was undertaken to determine the plasma levels of visfatin in patients with severe pneumonia. METHODS: A total of 70 patients, including 40 patients with severe pneumonia (group A) and 30 patients with non severe pneumonia (group B) who had been admitted to the ICU from June 2009 to June 2010, were enrolled in this prospective study. And another 30 healthy physical examinees served as healthy controls (group C). Patients were excluded if they suffered from severe diseases of the heart, brain and kidney, cancers, autoimmune diseases, or received special treatment in the latest month. The plasma levels of visfatin, IL-6, IL-8 and TNF-α were measured by ELISA, while the level of CRP was determined by immuneturbidimetry, and the routine blood test was performed. Blood gas analysis and Acute Physiology and Chronic Health Evaluation II (APACHE II) were performed in patients with pneumonia. Comparisons between the groups were conducted by Student's t test, ANOVA or nonparametric test. Correlation analysis was carried out by Pearson's correlation test or Spearman's rank-order correlation test. RESULTS: The plasma level of visfatin in group A was significantly higher than that in groups B and C (P<0.001), and the level of visfatin in group B was significantly higher than that in group C (P<0.001). The plasma level of visfatin was positively correlated with CRP, TNF-α, APACHE II and PMN% in patients with severe pneumonia (rho=0.653, r=0.554, r=0.558, r=0.484, respectively, P<0.05 for all), while it was negatively correlated with PaO2 and PaO2/FiO2 (rho=?0.422, r=?0.543, respectively, P<0.05 for all). CONCLUSION: Visfatin may be involved in the systematic inflammation response in patients with severe pneumonia as a pro-inflammatory cytokine, and it is valuable in assessing the severity of pneumonia..
