BACKGROUND: The etiology and pathogenesis of Tourette syndrome(TS)is still unacknowledged, and related studies on the relationship between TS and hypoironemia are unavailable. Since hypoironemia has been found existed in children with TS who showed better outcomes after receiving adjuvant ferralium, thereby this study was designed to investigate the relationship between TS and serum iron.OBJECTIVE: To study the relationship between TS and serum iron.SETTING: At a nursing science department of a university-affiliated hospital, pediatric department of a university affiliated hospital and sectional hospital.PARTICIPANTS: Between June 1997 and December 2000, 45 children with TS received treatment at pediatric clinic of Chenghai sectional people' s hospital, Shantou city, who accorded with the diagnostic standards of the second edition of Chinese Psychopathy Category and Diagnostic Standards, those who were confirmed as brain organic diseases by CT and MIR were excluded.METHODS: Serum iron, hemoglobin and the number of red blood cells were determined in 45 TS children and compared with that in 38 controls.MAIN OUTCOME MEASURES: Difference of the level of serum iron and the incidence of hypoironemia between two groups.RESULTS: The level of serum iron was(12.79 ±0.67) μmol/L in experimental group, obviously lower than(19.26 ± 5.38) μmol/L in control.group( P < 0. 005), the incidence of hypoironemia was 42% significantly higher than13% of control group( P < 0. 005).CONCLUSION: Quite a lot of children with TS have hypoironemia that might be one of the pathogenesis factors, the possible mechanism is, as the result of hypoironemia, decreased activity of monoamine oxidase led to abnormal monoamine neurotransmitter.

3.Sympathetic skin response in different regions of the palm after median or ulnar nerve injury
Chinese Journal of Physical Medicine and Rehabilitation 2010;32(11):832-834
Objective To explore sympathetic skin response (SSR) in the region innervated only by the median or ulnar nerve. Methods Thirty healthy volunteers, ten patients with median nerve injury, ten patients with ulnar nerve injury and three patients with both median and ulnar nerve injury were involved in the study. SSR was elicited by electrical stimulation and the latency and amplitude of the SSR were recorded in the center of the palm and on the hypothenar, and the pulps of the index and little fingers. Results After median nerve injury the SSR of the index finger pulp disappeared, the SSR amplitude of the palmar center was reduced, while the SSR amplitude of the little finger pulp was normal. In contrast, after ulnar nerve injury the SSR of the little finger pulp disappeared, the SSR amplitude of the hypothenar was reduced, while the SSR of the index finger pulp was normal. When both nerves were injured, no SSR could be elicited anywhere on the palm. Conclusions The SSR of the palmar center and hypothenar are affected by both the median and ulnar nerves. The index and little finger pulps are sites for recording SSR which represent the SSR of the median and ulnar nerve distribution, respectively. Index and little finger pulps are ideal sites for detec t ing efferent functioning of sympathetic nerves from the median and ulnar nerve paths, respectively.

ObjectiveTo investigate the distribution of human immunodeficiency virus-1 (HIV-1)subtypes and the relationship with transmission routes in Shenzhen city. MethodsThe epidemiological survey was conducted in 429 subjects with HIV-1 infection.Vein blood was collected and prepared with anticoagulation.Thereafter, the viral nucleic acid was extracted and subsequently amplified for HIV-1 env,gag genes by reverse transcription nested polymerase chain reaction. The viral sequences were analyzed using Mega software and compared with those in HIV database. The HIV-1 subtypes were confirmed and evolutionary tree was established. The gene discrete rate of different subtypes was calculated.ResultsFour hundred qualified samples were identified as CRF01_AE (n=207),CRF07_BC (n=115),CRF08 _BC (n=14),CRF02_AG (n=1),B (n=49) and C(n=14),respectively.CRF01_AE was the predominant subtype that was detected in the subjects with different transmission routes, such as homosexual contact, heterosexual contact, intravenous drug use and hospital-acquired infection. Among these, sexual contact was the most common route. There were nine cases infected with subtype B HIV-1 in 10 case who were blood borne. The gene discrete rate was more significant in subtype B HIV-1 compared to the other subtypes.According to the evolutionary tree analysis, the viruses under the same subtype formed the relatively independent clusters by different transmission routes. But some overlaps were observed as well. Conclusions Tbere are many HIV-1subtypes in HIV-1 infected subjects in Shenzhen, while the recombinant viruses present as the predominant form.Transmission routes are various with sexual contact as the most common route.Every subset distributes unevenly and forms its own characteristic.Overlap can be observed among the gene distribution of different HIV-1 subtypes.

The incidence of breast cancer increased rapidly in recent years. Breast cancer has become the most frequent malignant tumor of female especially in the developed regions. Traditional Chinese medicine (TCM) is effective in treating breast cancer, but its theories appear hysteretic, restricting the progress in clinical practice, teaching and research of TCM in the treatment of breast cancer. This article described the significance and urgency to work out the standardization of syndrome differentiation based on stages for breast cancer and put it into practice. It also analyzed the foundations, ideas and approaches of the research of standardization of syndrome differentiation based on stages for breast cancer in light of the changes of spectrum of diseases, the weaknesses of modern medicine in treating breast cancer, and the existed problems in the update clinical practice.

The study on use of traditional Chinese medicine in perioperative management of breast cancer is still in the beginning phase. With the emergence of new understanding about the biological characteristics of breast cancer, the concept of treatment has changed. For instance, the resection extent is tending to be narrower, large doses of radiotherapy may be adopted during the operation, and early use of adjuvant chemotherapy is advocated after the operation. These have bought about changes to the intervention factors in the perioperative period for breast cancer. Some related problems about the participation of traditional Chinese medicine in perioperative management of breast cancer are discussed in this article, so as to make the perioperative management perfect.

Objective To determine the anatomical and visual outcome of indocyanine-green (ICG)-assisted internal limiting membrane (ILM) peeling for idiopathic macular holes. Methods Thirty-one eyes of 31 patients with 3- (14 eyes, 45.2%) and 4-staged (17 eyes, 54.8%) primary idiopathic macular holes were analyzed. All the patients underwent the subtotal pars plana vitrectomy with removal of the posterior vitreous. ICG solution with the concentration of 1.25 mg/ml was injected into vitreous cavity. The ILM was stained and removed in a circular fashion of 2 to 3 disc-diameter from the edge of the hole. At the end of the surgery, 14% C 3F 8 mixed gas was used and the patients were required to maintain a prostrate posture for two weeks postoperatively. The mean follow-up duration was 9.1 months. Results The preoperative median visual acuity was 20/200. In the final follow-up, 28 eyes (90.3%) had anatomical restoration of the macular holes, 21 eyes had improvement of two lines or more of visual acuity. There was no direct complication or toxicity related to ICG-assisted ILM peeling except one patient with retinal detachment caused by peripheral retinal hole. Conclusion ICG-assisted retinal ILM removal appears beneficial and safe for primary idiopathic 3 and 4-staged macular holes.
