ObjectiveTo explore the clinical diagnostic value of ambulatory EEG (AEEG) in epilepsy and atypical epilepsy.MethodsThe monitoring of 24-hour AEEG were performed in 67 epileptic and 53 atypical epileptic.Results65.7% of epileptic cases presented abnormality, and 34.0% of atypical epilepsy. The 85.5% of epileptic form discharges were found in the stage of sleep, especially in the Ⅰor Ⅱstage of NREM. The main epileptic form discharges was sharp waves.ConclusionThe monitoring of 24-hour AEEG may play important role in the diagnosis of epilepsy or atypical epilepsy, but can do little in the classification and detecting the focus of epilepsy.

Objective To detect the protective mechanism of trehalose in tracheal cryopreservation. Methods Inbred male Sprague-Dawley (SD) rats were sacrificed with intraperitoneal injection of ketamine(150mg?kg -1).The tracheas were removed and immersed immediately in the freezing medium of low potassium dextran (LPD) solution only(Group Ⅰ) ,containing with 10% dimethylsulfoxide(DMSO)(Group Ⅱ), containing with 0.15mol?L -1 trehalose (Group Ⅲ),and containing with 10% DMSO and 0.15mol?L -1 trehalose (Group Ⅳ) respectively. A sterile plastic tube containing a 1-cm-long trachea was filled with the freezing medium, sealed, and frozen to -80℃ at rate of -1℃ per minute in a programmable freezer. Then the tube was stored in liquid nitrogen(-196℃) for 20 days. Then the specimen was thawed in a 37℃ water bath and rinsed with physiologic saline solution 10 times.Histologic changes before cryopreservation and after thawing were examined in each group. After the specimens were embedded in paraffin,5-(m-thick sections were stained with hematoxylin and eosin. The epithelium and cartilage was assessed. We also observed Bcl-2 and Bax gene expression by immunohistochemistry. At last, some tracheas(SD) after cryopreservation were thawed and transplanted into the abdominal cavity of Wistar rats. The transplanted tracheas were retrieved and assessed histologically. Results Microscopic findings of the tracheas in Group Ⅲ and Group Ⅳ showed their structure were intact and Bax gene expression was lower in cartilage after cryopreservation(20d) compared with other groups, especially in Group Ⅳ.The tracheas in Group Ⅲ and Group Ⅳ grew well after they were transplanted into cavity of Wistar rats heterotopically, too. There were no significant differences among 4 groups in Bcl-2 gene expression. Conclusion In tracheal cryopreservation the trehalose can protect the trachea by protecting the tracheal cartilage. It is one of the protective mechanism that the trehalose inhibit the Bax gene expression of cartilage cells. The concomitant use of trehalose and DMSO has a synergistic effect.

Objective To investigate the role of survivin and P53 in apoptosis of gastric adeno- carcinoma,as well as its relationship with the clinicopathologic features and the prognosis. Methods The gas- tric tissue microarrays were composed of those from 100 cases of gastric cancer and 30 controls. At these tissue microarrays, expressions of survivin and P53 were investigated immunohistochemically, and tumor cell apoptosis index was examined by TUNEL method.Of the 100 cases,47 cases were followed-up from 14 months to 13 years,in which the survival was analyzed. Results Two paraffin-embedded gastric carcinoma tissue micro- arrays were successfully constructed, including 114 and 116 tissue spots, respectively.Immunohistochemical analysis showed that survivin was expressed in 78 cases (78%).No expression of survivin was detected in control tissue (P0.05).In the 47 cases with followed-up data, univariant analysis revealed that the survival was correlated with invading of vessel and nerve,TNM stages, and expression of survivin. The histological grades and expression of P53 were not related to prognosis. However,Cox stepwise proportional hazards analysis showed that only TNM staging and survivin status retained significant independently in prospecting prognosis. Conclusions The expression of survivin was associated with the pathologic features,TNM stages and prognosis in gastric carcinoma, indicating that overex- pression of survivin may be a poor prognosis factor for gastric carcinoma.

Abstract: Objective China was certified by World Health Organization as a malaria-free country in 2021. Malaria has become a rare infectious disease, and preventing the re-transmission of imported malaria and reducing deaths are the main challenges facing China after elimination of malaria. To analyze and clarify the characteristics of imported malaria deaths, and to provide prevention and treatment recommendations for overseas workers and health care workers. Methods The data of 17 imported malaria deaths in the analysis of malaria deaths from 2016 to 2020 by the National Severe Malaria Treatment Expert Group were collected, and the relevant clinical epidemiological data and disease course records were analyzed. Results The 17 malaria deaths were all imported from Africa with Plasmodium falciparum infection (malarial cerebral type), with no obvious regularity in the month of onset. Among them, 16 were male patients, 5 cases with underlying diseases such as diabetes mellitus, and 10 patients were first diagnosed in a second-level or lower hospital. Excluding patients who died of respiratory cardiac arrest in ambulances, the mean time difference between first onset and malaria diagnosis in 16 patients was 6.8 days (median 5.5 days), and the mean time between first onset and antimalarial treatment was 7.4 days (median 6 days), the mean time difference from initial onset to death was 10.3 days (median 8.5 days). Excluding cases with onset abroad and unknown time of return, all 14 patients developed the disease within 30 days after returning to China. Conclusion All the fatal cases were infected with Plasmodium falciparum imported from Africa. The patients' awareness of actively seeking medical treatment is weak, and the delay in seeking medical treatment caused by the insufficient diagnosis and treatment capacity of health institutions at the township level and below is the main reason for the deaths. It is recommended to strengthen the self-protection awareness of staff in malaria-endemic areas overseas and raise their awareness of malaria. For returnees from areas with high malaria risk, primary medical institutions should pay attention to the patient's travel history in Africa, improve the awareness of malaria diagnosis, malaria diagnosis and treatment capabilities.

Objective To investigate the effects of propofol on Ca2+ -induced mitochondrial dysfunction after focal cerebral ischemia-reperfusion(IR) injury in rats. Methods Eighty-four male Wistar ratsweighing 250-300 g were randomly divided into 4 groups (n= 21 each):group Ⅰ sham operation (group S); group Ⅱ IR; group Ⅲ CaCl2 and group Ⅳ propofol (group P). In group Ⅱ - Ⅳ focal cerebral IR was induced by 2 h middle cerebral artery occlusion (MCAO) followed by 24 h reperfusion. Neurological function was assessed at 2 h of ischemia and scored (0= no deficit, 5 = death). The animals were decapitated at the end of 24 h reperfusion. Left parietal and frontal cortex were immediately isolated and mitochondria were extracted. In group Ⅲ and Ⅳ mitochondria were incubated with 200 μmol/L CaCl2 for 5 min at 37 ℃, and were pretreated with propofol 200 μmol/L in group Ⅳ for 2 min before CaCl2. Morphological changes of mitochondria were examined by electron microscopy. Mitochondrial permeability transition pore (MPTP) activity was detected by ultraviolet visible absorption spectroscopy. Results Mitochondrial ultrastructure was normal in group Ⅰ . Significant mitochondrial swelling, disrupted cristae and membrane rupture were observed in group Ⅲ. Propofol significantly attenuated the Ca2+ -induced changes. The MPTP activity was significantly increased in group Ⅱ and Ⅲ as compared with group Ⅰ but were significantly decreased in group Ⅳ (propofol group). The decrease in MPTP activity was attenuated in group Ⅳ as compared with group Ⅲ. Conclusion Propofol can improve mitochondrial dysfunction induced by Ca2+ after focal cerebral IR by inhibiting MPTP opening in rats.
