3.Cranioplasty and analysis of related problems after craniocerebral trauma

Objective To analyze the choice of implant materials,key points of operative proce- dures,prevention and management of postoperative complications in patients treated with cranioplasty. Methods Two hundred and twenty six patients with craniocerebral trauma underwent cranioplasty with different implant materials.Attention should be paid for the purpose of not tearing or injuring the dura ma- ter during operation.To take the dura mater up to the implant material and the edge of the bone flap,and put drainage under the scalp flap before closure of the incision.Results The clinical symptoms and neural function were improved in 146(64.6%)patients postoperatively.Postoperative complications mainly were infection,fluid collection and hematoma in the operative region.Follow-up showed the occur- rence of displacement or collapse of the implant materials in some patients.Conclusion Timely cra- nioplasty can not only resolve cosmetic problems but also improve clinical symptoms and neural function. The causes of postoperative complications are mainly related with the operative procedures and the materi- als used.
4.Technique analysis of microsurgical treatment of large and huge sphenoid wing meningiomas

Objective To investigate and evaluate the technique of microsurgery,for the treatment of the large and huge meningiomas of the sphenoid wing. Methods Fifty-eight patients were finally diagnosed by CT and/or MR in this series. The sphenoid wing meningiomas were from 4 cm×3.5 cm×3.5 cm to 11 cm×10 cm×8 cm in volume,and were located at outside sphenoid wing in 28 and at the inner in 30,and were spherical in 53 and en plaque in 5.The microsurgery for resection of the tumors was done through a cranio-orbito-zygomatic or craniozygomatic approach in 10, and through a modified pterional approach in 48.Results Total resection was achieved in 52.No death occurred in the group.Follow-up of 6 months to 9.5 years showed obvious nervous function impmvement in 49 and regrowth of the tumor in 8. Conclusions Based on the volume and development of the tumor,it is very imprtant for improving merapeutic Outcome to choose appropriate operative approach, get rid of the tumor piece by piece,and protect the nerves and vessels intraoperatively.
5.Transumbilical laparoscopic pull-through for children with hypoganglionosis.

OBJECTIVETo investigate the surgical outcomes after on transumbilical laparoscopic pull-through procedure for pediatric hypoganglionosis(HYP).
METHODSTwelve children with HYP had received transumbilical laparoscopic pull-through procedure from June 2009 to June 2010. Specially designed curved and elongated laparoscopic instruments were used during the procedures. All the patients were followed up over 10 months. Data were collected and analyzed. The diagnosis of hypoganglionsis was pathologically confirmed.
RESULTSNo conversions to laparotomy or traditional laparoscopic surgery were required and there were no damages to the abdominal blood vessels, intestine, ductus deferens, or ureters. The average duration of operation was 140 min. The mean intraoperative blood loss was 45 ml. The mean length of specimen was 40 cm. Postoperatively there were no complications such as anastomotic leak, anastomotic stricture, constipation, seepage, or fecal in continence. The average hospital stay after surgery was 9 days. During 10 to 22 months of follow-up(median 16 months), no postoperative recurrence was noticed. No obvious scar was seen 1 months after surgery.
CONCLUSIONIt is safe and effective for children with hypoganglionosis to undergo transumbilical laparoscopic pull-through procedure.

6.Comparison of embryonic nature orifice transumbilical endoscopic surgery with conventional laparoscopic surgery treatmenting Hirschsprung disease

Objective To compare the therapeutic effects of embryonic nature orifice transumbilical endoscopic surgery(ENOTES) with conventional laparoscopic(CL) surgery in Hirschsprung disease(HD).Methods A total of 111 children from Sep.2009 to Dec.2010 were included in the retrospective study.The patients were divided into 2groups:ENOTES group (66 cases) and CL group (45 cases).The general conditions of children were reviewed operative situation,postoperative complications and defecation 1 year after operation.Results In the left colectomy,there was no difference between 2 groups in age,weight,operative blood loss and postoperative stay (all P > 0.05),but ENOTES took up less time than CL(P < 0.05) ;in the subtotal colectomy,no significant difference existed in all series.All children underwent primary radical surgery,neither transfer to open surgery nor dead case.In ENOTES group,there were 9 cases suffering from enterocolitis postoperatively (13.6%),while 7 cases (15.6%) in CL group.Twenty-eight patients in ENOTES group and 12 in CL group were followed up for 1 year.Less abdominal distension postoperatively was noted in ENTOES group than CL group(P < 0.05),but no other difference in other available data.In addition,no obvious operative scar was noted on the abdomen in ENOTES group,what meant better cosmetic benefit than CL.Conclusion HD treated with ENOTES can gain the same outcomes with CL with better cosmetic effect.
7.Antivirus effects of extract from gardenia.

OBJECTIVETo observe the effect of the extract from gardenia on influenza viral pneumonia in mice and virus-induced cytopathic effect.
METHODThe mice were infected by influenza virus in nasal, the lung inflammation, mortality rate and life elongation rate were observed respectively. The anti-viral activity of the extract from gardenia was accessed by cytopathic effect (CPE) in vitro and 0% toxicity concentration (TC0), 50% toxicity concentration (TC50), 50% inhibitor concentration (IC50), therapeutic index (TI) were determined by Reed-Muench method.
RESULTThe pneumonia induced by influenza virus in mice was inhibited significantly by the extract from gardenia, as the mortality rate decreased and the life elongation rate increased remarkably. Meanwhile the NO content in serum decreased significantly; The cytopathic effect induced by six kinds of viruses was inhibited remarkably.
CONCLUSIONThe six kinds of viruses were inhibited significantly by the extract from gardenia which inhibitory effect on mice influenza viral pneumonia was related to the NO content decreased.

8.Colonization of B.bifidum ATCC 29521 In Mice Intestinal Tract

Objective:Effect of gut microbiota on human health has become a hotspot in recent years with the emergence of new microbial technology.B.bifidum is a traditional probiotics and has been widely used in food and medicine.But the metabolism and function of B.bifidum ATCC 29521 has not been well documented.To investigate colonization and growth of B.bifidum ATCC 29521 in mice intestine,fecal B.bifidum concentrations were analyzed by real-time fluorescent quantitative PCR.Methods:Female C57BL/6 mice were orally gavaged with eigher a single dose of 1 × 109 CFU B.bifidum ATCC 29521 or continuous doses of 1 × 109/day for 3 weeks.Feces were collected at 0,2,4,6,8,10,12,16,20,24 h and at 10,7,14,21,24,28 d,respectively.Total DNA was isolated from the feces using the Qiagen DNA stool extraction kit according to the manufacturer's instructions.The linear standard curve was established by each dilution degrees of B.bifidum ATCC 29521 and their corresponding CT values.The curve equation was y =0.1835x + 9.0628 and R2 was 0.9994.The concentration of B.bifidum ATCC 29521 in mice feces were calculated by substituted CT values obtained by RT-PCR into the curve equation.Results:The curves of B.bifidum ATCC 29521 concentration rose at first and then reduced gradually.In single dose group,the concentration of B.bifidum ATCC 29521 began to increase at 4h after oral gavage,reaching its peak 6.0 × 107 CFU/g at 10h and then decreased gradually.The biggest drop occurred at the period between 12 h and 16 h after B.bifidum treatment.In successive administration group,the concentration rose at the highest rate in the first week when it achieved 2.0 × 107CFU/g and kept on inceasing to 1.0 × 108CFU/g in the next week.However,the concentration did not rose up significantly in the third week.It means that the concentration of B.bifidum ATCC 29521 in mice intestine reached platform in the second week after oral gavage.The concentration of B.bifidum ATCC 29521 significantly decreased at 24h and one week after B.bifidum treatment course in two group repectively and was still higher than baseline before oral gavage.Conclusion:Once the B.bifidum ATCC 29521 supplement was discontinued,the concentration fell down in short time.B.bifidum ATCC 29521 could not sustain colonization and growth in mice intestine without external supplement.It is necessary to provide daily supplemention for at least two weeks and to keep on in order to maintain sufficient concentration.
9. Prognostic value of myeloperoxidase concentration in patients with acute coronary syndrome

Objective:
To evaluate the difference of plasma myeloperoxidase (MPO) level in different types of acute coronary syndrome (ACS) patients, and the value of baseline MPO level in predicting short-term and long-term outcomes in patients with ACS.
Methods:
The study cohort was derived from "the 12th Five-Year" National Science and Technology Support Program Project "Study on Comprehensive Intervention and Prognosis of Acute Coronary Syndrome" . We enrolled all hospitalized ACS patients who were enrolled in "the 12th Five-Year" cohort from January 1, 2011 to December 31, 2013. A total of 630 patients from 20 centers were enrolled. According to the diagnosis, the patients were divided into two groups: ST-segment elevation myocardial infarction (STEMI) group and non-ST-elevation acute coronary syndrome (NSTE-ACS) group. Plasma levels of MPO were measured by ELISA method. Cardiovascular events in the hospital were recorded. All patients were followed-up by telephone, follow-up ended December 31, 2015. The occurrence of major adverse cardiovascular events (MACE, defined as cardiac death, recurrent myocardial infarction, unscheduled coronary revascularization procedure and stroke) and all-cause death were recorded. Logistic regression analysis and Cox regression analysis were used to evaluate the predictive value of baseline MPO levels obtained during hospitalization and the long-term outcomes of ACS patients.
Results:
A total of 597 ACS patients were enrolled in final analysis. Level of plasma MPO in STEMI patients was significantly higher than that of NSTE-ACS patients (34.02(19.31, 67.87) μg/L vs. 27.25(16.69, 52.92) μg/L,

BACKGROUND AND PURPOSE: It is essential to develop a reliable predictive serum biomarker for Parkinson's disease (PD). The accumulation of alpha-synuclein (αSyn) and up-regulated expression of Rab35 participate in the etiology of PD. The purpose of this investigation was to determine whether the combined assessment of serum αSyn and Rab35 is a useful predictive biomarker for PD. METHODS: Serum levels of αSyn or Rab35 were determined in serum samples from 59 sporadic PD patients, 19 progressive supranuclear palsy (PSP) patients, 20 multiple system atrophy (MSA) patients, and 60 normal controls (NC). Receiver operating characteristics (ROC) curves were calculated to determine the diagnostic accuracy of αSyn or/and Rab35 in discriminating PD patients from NC or atypical parkinsonian patients. RESULTS: The levels of αSyn and Rab35 were increased in PD patients. The serum level of Rab35 was positively correlated with that of αSyn in PD patients. Compared to analyzing αSyn or Rab35 alone, the combined analysis of αSyn and Rab35 produced a larger area under the ROC curve and performed better in discriminating PD patients from NC, MSA patients, or PSP patients. When age was dichotomized at 55, 60, 65, or 70 years, the combined assessment of αSyn and Rab35 for classifying PD was better in the group below the cutoff age than in the group above the cutoff age. CONCLUSIONS: Combined assessment of serum αSyn and Rab35 is a better biomarker for discriminating PD patients from NC or atypical parkinsonian patients, and is a useful predictive biomarker for younger sporadic PD patients.
