Objective: Chlamydia pneumoniae is a kind of common pathogenic bacteria in respiratory passage which cause persistent and chronic infection cell mediated immunity results mainly followiny the infection.The infection kinetics and local in vitro-restimulated cell mediated immunity(CMI) responses in the lungs and spleens of Icr mice during Chlamydia pneumoniae(Cpn) primary infection and reinfection will be analyzed in this article.Methods: Icr mice were inoculated intranasally with 105 IFU of Cpn(PT group).The same dose was given in the same way as rechallenge 4 weeks after the primary challenge(ST group).Control groups were inoculated with 0.05 ml PBS.The mice were sacrificed at predetermined days,and the pulmonary mononuclear cells and splenic lymphocytes were isolated and stimulated with Cpn in vitro for 3 days.The proliferative response was measured by MTT;IL-4,IL-10 and IFN-? were detected by ELISA.Results: The PT group showed a weak proliferative response and no secretion of IFN-? in the lungs after Cpn stimulation,while the ST group presented a reinfection characterized by IFN-? production both in the lungs and in the spleens(P

3.Relationships between survival and expressions of estrogen receptorαandβin non-smoking non- small cell lung cancer patients

Background and purpose:The involvement of estrogen signaling in lung cancer is controversial. But the findings showed that ERβ might play an important role in neoplastic lung biology. Several studies have evaluated expression of ERβin both normal and neoplastic human lung tissues with variable conclusions. We aimed to evaluate the expressive of estrogen receptors (ER)α,βin non-smoking non-small cell lung cancer (NSCLC) and investigate the relationships between the survival and expressions of ERα,β. Methods:Immunohistochemical assay was used to detect the expression ERαand ERβin 144 NSCLC patients, and analyzed the expressions status of ERα,βand survival. Results:The positive rate of ERαwas 7.6%, only in adenocarcinoma cancer. The positive rate of ERβwas 52.1%, and the expressions of stageⅠandⅡwas signiifcantly higher than that of stageⅢandⅣ(P=0.020). The median survival time (MST) of ERβpositive expression was better than that of negative expression (43 months vs 38 months, P=0.028). The MST of ERβpositive expression of female patients was better than that of negative expression (45 months vs 37 months, P=0.033). The MST of ERβpositive expression of adenocarcinoma were 46 months and better than negative 38 months (P=0.021). Conclusion:The expression of ERβwas signiifcantly higher in early-stage non-smoking NSCLC. It indicated that the positive expression of ERβwas related to the better MST, especially in female adenocarcinoma patients.

Objective To evaluate the prognostic significance of MMP-2 , MMP-9 and Cathepsin-D expression in lymph node-negative human breast carcinoma. Methods Specimens from the primary breast tumors of 270 axillary lymph node negative breast cancer patients were studied for MMP-2 , MMP-9 and Cathepsin-D protein expression by immunohistochemical staining. Results The positive expression of MMP-2, MMP-9 and Cathepsin-D was 56.7%(153/270),59.6%(161/270) and 55.6%(150/270), respectively. In univariate analysis, MMP-2, MMP-9 and the co-expression of MMP-2 and MMP-9 were significantly associated with tumor free survival. Objective:To search for a method for radical resection of pancreatic and duodenal malignancy involving the mesentery root and for the long post-operation survival of patients.Methods:From Jan.2004 to Aug.2006,a total of 26(16 male and 10 female. aged 27-70)patients with pancreatic and duodenal malignancy involving the mesentery root were treated in our department.The patients included 3 with duodenal malignancy and 23 with pancreatic malignancy.Curative resection was performed by the extended pancreaticoduodenetomy(Whipple procedure)combined with mesentery root resection(MRR)for all patients.The outcomes,safety and the post-operation survival rate were analyzed retrospectively.Results:Thirteen patients were treated with Whipple procedures combined with MRR,9 were treated with partial portal vein/superior mesenteric vein(PV/SMV)and reconstruction of the vessel,and 4 patients received pre-shunt between PV and SMV with artificial vessel graft before the extended Whipple and MRR procedures.The operation time was 2.5 to 7(4.4?1.1)hour,and blood loss was 300 to 5 000(1892?1414)ml with the blood transfusion of 0 to 5 600(2 100?1 586)ml.There was no death in our group and 7(27%)had post-operation complication.The post-operation hospital stay was 10 to 30 days.The pathologic examination showed negative surgical margins for all specimens.The tumor size was 4 to 10 (6.17?2.03)cm.After a follow-up of 9 to 38 months,the pain was relieved in all patients.One of the 3 patients with duodenal adenocarcinoma had liver metastasis at 10 months after operation,and the other 2 survived 10 months and 27 months without evidence of tumor reccurence.The patient with pancreatic micro-adenocarcinoma died of local reccurence 9 months after operation.The patient with neuroendocrine carcinoma died of organ failure 24 months after operation.The patient with lymphoma have survived for 24 months after operation.The 1-year and 2-year accumulated survival rates in the 20 cases with pancreatic ductal cancer were 86.6% and 45.6%. respectively.Conclusion:The extended Whipple procedure with MRR is safe and effective.It can obtain R0 resection in patients with malignant tumors(over 5 cm in diameter)in the head,neck and uncinate process of the pancreas and duodenal.

7.Research of hepatic pathology and clinical characteristics of patients chronic infected with hepatitis B virus in immune tolerant stage.

OBJECTIVETo investigate the characteristics of the hepatic pathological and clinical features of patients with hepatitis B virus (HBV) in immune tolerant stage and find the better measure of diagnosing patients chronic infected by HBV in immune tolerant phase.
METHODS135 patients with HBV chronic infection and persistently normal serum alanine aminotransferase (ALT) levels were involved in this study, whose serum HBeAg and HBV DNA were positive. Statistical analysis included the ages, sex, serum levels of HBVDNA, ALT and histological grade. The grades of inflammation and fibrosis were obtained through hepatic biopsy performed on all the patients.
RESULTSMean age in those patients was 22.61 +/- 8.95 years old. All those patients were divided into two groups according to histological grade: low- histological grade group, G < or = 1 and S < or = 1; and high-histological grade group, G > or = 2, S > or = to 2. Levels of histological grade were low in most of patients (99/135). Patients of low-histological grade had no difference in age, sex and family history of HBsAg carriers. Compared with low-normal ALT (ALT less than 30U/L), those with high-normal ALT (ALT > or = 30U/L) had a greater frequencies of high-histological grade. Compared with high HBVDNA (HBVDNA > or = 10(7) copies/ml), those with low HBVDNA (HBVDNA less than 10(7) copies/ml) had a greater frequencies of high-histological grade.
CONCLUSIONLevels of histological grade were low in most of patients with HBV chronic infection, serum HBeAg and HBVDNA positive, persistently normal serum ALT levels, but some of them were high-histological grade. It showed those patients were not all in immune tolerant stage of chronic HBV infection. Examination of ALT and HBVDNA are helpful to evaluate hepatic pathological damage for them.

8.Clinical study on hybrid bioarttficial liver supporting system for acute on chronic liver failure patients

Objective To construct an hybrid bioartificial liver supporting system,and observe its effectiveness and safety on patients with acute on chronic liver failure.Methods Hybrid bioartificial liver supporting system (HBALSS) was constructed using bioreactor with HepG2 cells transfected with human augmenter of liver regeneration (hALR) gene.12 acute on chronic liver failure patients were divided into 2 groups randomly.The treatment group was treated with the hybrid bioartificial liver support system.The group underwent plasma exchange was used as control.Results In the treatment group,four patients recovered,one patient died of hepatic encephalopathy,one patient died of hepatorenal syndrome,one patient recovered,but died of gastrointestnal bleeding after 1 year.In control group,two patients recovered,one patient underwent orthotropic liver transplantation,and three patients died of liver failure.Conclusion The hybrid bioartificial liver supporting system with HepG2 cell line was established successfully and have certain safety and effectiveness on acute on chronic liver failure patients.
9.Clinical characteristics of hepatitis B virus-related acute-on-chronic liver failure patients with familial aggregation

Objective To study the clinical characteristics of hepatitis B virus-related acute-on-chronic liver failure patients with familial aggregation.Methods 275 patients with hepatitis B virus - related acute-on-chronic liver failure were investigated.The patients were divided into familial aggregation and nonfamilial aggregation group basis on their epidemiological features.Clinical data and biochemical indicators between the two groups were analyzed statistically.Results 93 of 275 patients( 33.82% ) case were family aggregation.There was no significant difference compared with chronic hepatitis B patients( 38.3% ).The mean age of the two groups was 45.98 and 43.61 years old,respectively (P > 0.05 ).The rates of liver cirrhosis in family aggregation group were significant higher than non - familial aggregation group( 73.91% vs 58.24%,p < 0.05 ).Serum total (TBil) and prothrombin activities ( PTA ) were no significant difference between the two groups,but ALT level in familial aggregation group was much higher (407.80U/L vs 256.45 U/L,P0.05 ).Conclusion Familial aggregation were not related to acute-on-chronic liver failure in chronic HBV hepatitis patients. But the rate of liver cirrhosis were higher in patients with familial aggregation.
10.Staging treatment for complex tibial metaphyseal fractures with external fixator.

OBJECTIVETo observe the clinical effects of combined type external fixator in treating complex tibial metaphyseal fractures.
METHODSFrom January 2007 to July 2012, 34 patients with complex tibial metaphyseal fractures were treated with combined type external fixator in different stagings. There were 23 males and 11 females, with a mean age of 41.3 years (ranged, 16 to 63), and the course of disease were from 1 h to 8 d. In the patients, 31 cases were open fractures, 11 cases with type II, 13 cases with type III A, 7 cases with type III B according with Gustilo classification; 19 cases were tibia plateau fractures, 6 cases with type II, 1 case with type IV, 5 cases with type V, 7 cases with type VI according to Schatzker classification; 15 cases were distal tibial fractures (one were bilateral fractures), 2 fractures with type A2, 1 fracture with type A3, 1 fracture with type C1, 5 fractures with type C2, 7 fractures with type C3 according to AO classification. Rasmussensn scoring system and AOFAS Ankle Hind-foot Scale were respectively used to assess the joint function of knee and hip.
RESULTSWound surface of 19 patients obtained at phase I healing and 15 patients obtained at phase III healing. Superficial wound infections occurred in 2 cases and bone non-union necessitated reoperation occurred in 2 cases (final fractures obtained bone healing after the second operation). All patients were followed up from 6 to 38 months with a mean of 14.3 months. At the final follow-up,according to Rasmussensn scoring system, 5 fractures got excellent results, 11 good, 3 fair, the mean Rasmussen score was 23.58 +/- 3.98; according to AOFAS Ankle Hind-foot Scale, 5 fractures got excellent results, 8 good, 3 fair, the mean AOFAS Ankle Hind -foot Scale was 80.75 +/- 14.21.
CONCLUSIONCombined type external fixator can well maintain the stability of the fractures, had advantages of low incidences of soft tissue complications and less influence to joint motion in treatment of complicated tibial metaphyseal fractures. However there were some limitations in long-term use.
