1.Correlation study of serum interleukin 6,tumor necrosis factor α and C reaction protein levels with smoking,body mass index in elderly patients with stable chronic obstructive pulmonary disease

Objective To investigate the relationship between serum interleukin 6( IL-6 ),tumor necrosis factor α( TNF-α)and C reaction protein( CRP)levels and smoking and body mass index( BMI)in the elderly patients with stable chronic obstructive pulmonary disease( COPD ). Methods Elderly participants including 50 cases smokers with stable COPD,45 cases ex-smokers with stable COPD and 40 cases healthy ex-smokers were recruited in this study,who were hospitalized in the People's Hospital of Gansu Province from Dec. 2012 to Feb. 2014. Serum IL-6,TNF-α and CRP levels were detected. Correlation analysis was performed between serum IL-6,TNF-α,CRP levels and smoking index( SI),BMI in COPD smokers. Results The levels of serum IL-6,TNF-α and CRP in smoking group were(45. 9 ± 12. 1)mg/L,(58. 2 ± 15. 8)ng/L,(12. 2 ± 4. 1) mg/L,significantly higher than those in stop-smoking group((38. 1 ± 9. 6)mg/L,(45. 9 ± 11. 2)ng/L,(8. 6 ±3. 2)mg/L respectively),and healthy controls group((17. 0 ± 9. 9)mg/L,(27. 3 ± 13. 2)ng/L,(6. 3 ±5. 2)mg/L),and the differences were significant(F=84. 934,57. 224,23. 023;P﹤0. 01). In patients with smoking index≥400,the levels of serum IL-6,TNF-α and CRP were(50. 1 ± 12. 1)mg/L,(64. 2 ± 12. 6) ng/L,(13. 4 ± 3. 7)mg/L,significantly higher than those in patients with SI ﹤400 group((41. 0 ± 10. 2) mg/L,(47. 8 ± 14. 0)ng/L,(10. 8 ± 4. 2)mg/L respectively),and the differences were significant( t=2. 845, 4. 343,2. 347;P ﹤0. 01 or P ﹤0. 05 ). The levels of serum IL-6,TNF-α and CRP in smoking group were positively correlated with SI(r=0. 458,0. 438,0. 313;P﹤0. 01 or P﹤0. 05). The levels of serum IL-6,TNF-αwere negatively correlated with BMI,and the correlation coefficient were - 0. 358,- 0. 319( P ﹤0. 05). Conclusion The increase of serum IL-6,TNF-αand CRP caused by smoking may play an important role in the pathogenesis of COPD. And chronic smoking is the reason of the decline of BMI in COPD patients,and the decline of BMI is related to the high levels of serum IL-6 and TNF-α.

3.Thoracoscopic Nuss procedure for the correction of pectus excavatum in 21 patients

Objective To summarize the experiences of Nuss procedure with thoracoscope for repairing pectus excavatum.Methods 21 cases of pectus excavatum were treated by Nuss procedure with thoracoscope.In these patients,10 had symmetric pectus excavatum and 11 had asymmetric one.Results The operation in all patients were successful.The mean operating time wag(70.7±9.6)min and the mean intraoperative blood loss was(20.3±10.9)ml.The average hospital length of stay was(5.3±1.1)days,the average durante dolors was(6.9±4.0)days.The exceUent rate was 81.0%(17/21),including 90.0%(9/10)in symmetric pectus excavatum and 72.7%(8/11)in asymmetric pectus cxcavatum.Conclusion Nuss procedure is easy to perform,with minimal invasion,short operating time,and less blood loss.Nuss procedure is suitable for the symmetric pectus excavatum.
5.Survey on nutrional status and health of the old people in two urban homes for the old

It is a dangerous module which affects the health of the old. Thus, the diet for the old should be improved for their health.
6.System evaluation of health status and intervention in US Armed Forces

Objective To systematically evaluate the health status and intervention measures in the US Armed Forces, and to provide reference for the development of health promotion strategies in our army.Methods The PubMed, Medline, Springer, Elservier, HighWire and CNKI Database were searched electronically, with assigned search strategy for American military health status and intervention measures published from Jan.1997 to Dec.2015.Results There were 25 of pieces literature about training injury, low back pain, posttraumatic stress disorder, depression and cardiovascular disease that were screened.Conclusion The main health problems facing the US Armed Forces and the focus of the intervention measures involved training injuries, low back pain, mental illness and cardiovascular disease.Protection of military health is shifting from the single disease prevention to diversified comprehensive maintenance in the new era.
7.Course Construction of Physical Therapy for Higher Vocational Education

Ubiquitin-proteasome pathway (UPP) can degrade specific proteins in eukaryotic cells. Targeting specific proteins for ubiquitination and degradation technique, based the theory of UPP and using certain chimeric E3s or protein-targeting chimeric molecules, can specifically knock down the targeted proteins. It is a promising new method for exploring the functions of genes or proteins and, together with gene-silencing technique, may play an important role in basic and clinical research.

9.Value of trauma and injury severity score in predicting mortality of trauma patients in different rank hospitals

ObjectiveTo evaluate the trauma care effect and the value of trauma and injury severity score (TRISS) in prediction of the mortality by using TRISS to calculate the survival probability of trauma patients in five hospitals from Zhejiang province in 2009.MethodsA retrospective study was done on trauma patients (study group) firstly admitted to Emergency Department of five hospitals from Zhejiang province in 2009.The relevant information was collected,including demographic data,trauma types and injury causes.The TRISS score was obtained through calculating injury severity score (ISS) and revised trauma score (RTS) on admission into emergency department.With the major trauma outcome study (MTOS) as control group,M value,standardized Ws value and 95% confidence interval (CI) were calculated to compare actual survival rate and anticipation survival rate.ResultsA total of 2 193 patients at mean age of 44.39 years were enrolled in the study,including 1 661 male patients (75.74%).Traffic accident injury was the most common,followed by fall injury.The mortality rate according to TRISS was 13.22%,but the actual mortality rate was 9.75%.For all the patients,M =0.80 indicated that the injury severity of the study group was significantly different from that of the control group.At the same time,Ws =2.15,95% CI for Ws:1.54-2.77 showed that the actual survival rate of the study group was significantly higher than that of the control group.Besides,the survival rate of trauma patients in the affiliated hospitals and three hospitals at class A grade was significantly higher fian that of the control group,but there was no significant difference between three hospitals at class B grade and control group. ConclusionsTRISS overestimates the mortality of the study group,which is probably associated with the rapid development of traumatology and the old coefficients of TRISS.Setting up local trauma database and renewing coefficients of TRISS may improve the ability of TRISS in predicting mortality of the trauma patients.
10.Effect of trauma emergency care related factors on length of ICU stay

ObjectiveTo investigate the effect of factors correlated with trauma emergency care system on the length of ICU stay and figure out independent risk factors of prolonged ICU stay. Methods A total of 1 361 trauma patients admitted to the ICU of five tertiary hospitals in Zhejiang province in 2009 were retrospectively studied.Demographic data,time of ICU stay and variables related to trauma care were collected.Logistic regression was performed to determine the independent risk factors of prolonged ICU stay ( ≥ 15 days). ResultsOverall,192 trauma patients ( 14.1% ) had a prolonged ICU stay ( ≥15 days).Single factor analysis indicated that ISS≥ 16 points,GCS≤7 points,blunt trauma,prehospital emergency care,length of emergency department stay ≥4 hours,mechanical ventilation and central venous pressure monitoring were associated with the prolonged ICU stay.Multivariate analysis showed that pre-hospital emergency care was a protective factor for the prolonged ICU stay( ≥ 15 days) and that mechanical ventilation,length of emergency room stay≥4 hours and ISS≥ 16 points were the independent risk factors for the prolonged ICU stay ( ≥ 15 days).Conclusions Pre-hospital emergency care and ICU care show significant influence on the length of ICU stay.Furthermore,shortened length of emergency department stay is also contributive to reduced length of ICU stay.
