2.The dynamic change of Brn-3a expression in retina of chronic high intraocular pressure rat

Background Brn-3a is a newly discovered specificity marker for retina ganglion cells(RGCs).It is well-known that RGCs damage is a important pathological basis of hypertension-visual disorder.But the study concerning expression of Brn-3a in RGCs in glaucoma eye is still rately.Objective The purpose of this work was to investigate the changes of Brn-3a expression in model eye with chronic high intraocular pressure(IOP) and its relation with morphology of retina and the expression of Brn-3a in chronic ocular hypertension rats.Methods Thirty-five clean adult SD rats were randomly divided into normal control group(5 rats) and model group(30 rats).Experimental chronic ocular hypertension models were induced unilaterally in the left eyes of 30 health adult SD rats by cauterizing super-scleral veins,and the conjunctival incision was made in the right eyes as sham operative group.The operated rats were subdivided into 6 groups according to the examination time points and 6 rats for each group.IOP was measured with Tono-Pen tonometer before and after 30 minutes,1,3,7,14,28 day after surgery respectively.The rat models were sacrificed in 1,3,5,7,14,28 days after operation by excessive anesthesia method,and retinal section was prepared for the histopathological examination and the RGCs were counted using Nissl staining method.Expression of Brn-3a in RGCs was detected by immunohistochemistry.This experimental complied with the Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission.Results The IOP was significantly raised from 30 minutes to 28 days after operation in model eyes with the top IOP( 34.46±4.65 )mmHg in the 30 minutes after operation,showing statistically significant differences in various time groups ( F =95.631,P =0.001 ) and different eyes ( F =287.473,P =0.001 ).Compared with sham operative group,the IOP were elevated from 1 day through 14 days after operation ( q =18.418,15.261,10.987,6.931,4.975,2.962,P < 0.05 ).The numbers of RGCs were ( 29.08 ± 1.98 ) in the normal control group and decreased gradually by 3.17%,7.84%,14.60%,22.40% in 1,7,14,28 day after surgery in the model eyes with the considerable differences in comparison with normal eyes ( t =5.943,8.034,15.023,17.004,19.371,P < 0.05 ).Immunochemistry revealed that Brn-3a was specially expressed in the RGCs layer and the positive RGCs for Brn-3a were evidently decreased as the prolong of high IOP duration ( F =127.583,P =0.000 ).Conclusions Chronic glaucoma model can be successfully established using Shareef-Sharma method.The loss of RGCs is associated with the high lOP duration.The expression level of Brn-3a is a reliable index of high-IOP-induced damage of optical nurve.

5.A 1:1 Matched Case-control Study on the Risk Factors for Inguinal Hernia in Children

Objective To explore the risk factors for inguinal hernia in children.Methods A 1:1 matched case-control study which based on hospital group data was carried out.201 cases aged 0~6 years with inguinal hernia and controls were enrolled,and their parents were investigated with questionaire.Conditional logistic regression model was used for single factor and multivariate factoranalysistoestimateoddsratios(OR) and the 95% eonfidence interval(95% CI).Results Inguinal hernia was related to ehildren ' s constipation(0R=3.520,95%CI:1.238~10.015),children's cry(OR=6.532,95%CI:2.651~16.091),mother ate pickles(OR=2.529, 95%CI:1.271~5.031) and maternal anemia(OR=6.809,95%CI:2.530~18.322) during the period of one month before pregnancy and the first 3 months of gestation,the children's family history of inguinal hernia(OR=I 1.250,95%CI:4.766~26.553).Conclusion Children's constipation,children's cry,maternal anemia and eating pickles during the period of one month before pregnancy and the first 3 months of gestation,children ' s family history of inguinal hernia are the risk factors for children' s inguinal hernia.
6.The screening assay of platelet-associated immunoglobulin G can not improve the sensitivity of monoclonal antibody immobilization of platelet antigens assay in immune thrombocytopenic purpura

Objective To evaluate if the assay of platelet-associated immunoglobulin G (PAIgG) can improve the sensitivity of monoclonal antibody immobilization of platelet antigens assay (MAIPA) in immune thrombocytopenic purpura (ITP).Methods Anti-GP Ⅱb/Ⅲa and anti-GP Ⅰb/Ⅸ autoantibodies were detected by a modified MAIPA;PAIgG was detected by competitive ELISA.MAIPA and PAIgG were done simultaneously in 190 patients with thrombocytopenia,in which 132 were diagnosed as ITP according to the diagnostic criteria Kappa test was used to analyze the concordance between MAIPA and PAIgG.Results The sensitivities of PAIgG alone or in parallel with MAIPA to ITP were 32.6% and 37.2% higher than MAIPA alone respectively,while their specifieities were 51.7% and 56.9% lower respectively.The sensitivity,specificity,positive predictive value,negative predictive value of PAIgG in series with MAIPA were all not superior to MAIPA alone.The kappa values between MAIPA and PAIgG in ITP and non-immune thrombocytopenia were 0.129 and 0.012 respectively,which meant the concordance between them was poor.Conclusion The detection of PAIgG should not be used as screening before MAIPA in distinguishing immune from non-immune thrombocytopenia.
7.Effect of Chuanbaiqufeng Capsule on treating primary trigeminal neuralgia in rat

8.Pharmaceutical Care for One Elderly Patient with AECOPD and Baumanii Infection

Objective:To explore the therapeutic regimen and key points in the pharmaceutical care for the elderly patients with a-cute exacerbation of the chronic obstructive pulmonary disease ( AECOPD) . Methods: On the basis of clinical pharmacist work, one typical case was selected. Referring to the COPD treatment guidelines, the treatment rationality was analyzed. Meanwhile, an individu-alized pharmaceutical care plan was established and carried out in whole process of the treatment. Results:The pharmacotherapy was effective and rational. By providing the pharmaceutical care for the AECOPD patient, the related problems in the treatment were solved promptly, and the rational advice on the drug treatment was provided. Conclusion:It is very important to enhance integrated pharma-ceutical care in the elderly patients with AECOPD.
9.Effect of Rehabilitative Ankle-Foot Orthoses on walking function of acute stroke hemiplegic patients

ObjectiveTo study the effect of Rehabilitative Ankle-Foot Orthoses training program on walking function of hemiplegic patients after stroke .Methods95 patients were randomly divided into two groups: observed group (49 cases) and control group (46 cases). The patients of control group were trained by routine rehabilitation training program, the patients of observed group were trained by Rehabilitative Ankle-Foot Orthoses and routine rehabilitation training program.ResultsAfter training, either observed group or control group showed significant improvement at walking function and ADL(P<0.01), but the improvement on observed group were more marked than coutrol group(P<0.01 and P<0.05). ConclusionsThe ability of walking in acute stroke patients were obviously improved, and the degree of the disability of them was decreased by Rehabilitative Ankle-Foot Orthoses.
10.Quantitative Detection of Translocation ETS Leukemia-Acute Myeloid Leukemia 1 Fusion Gene in Children with Acute Leukemia and Its Clinical Application in Minimal Residual Disease Monitoring

Objective To develop a method of real-time quantitative polymerase chain reaction (RQ-PCR) for detection of translocation ETS leukemia-acute myeloid leukemia 1(TEL-AML1) fusion gene in children with acute lymphoblast leukemia(ALL),and explore its clinical application in minimal residual disease (MRD) monitoring.Methods By reverse transcription and RQ-PCR,a quantifying of TEL-AML1 fusion gene was developed,and the expression levels of TEL-AML1 were detected in bone marrow (BM) samples obtained from 24 children with ALL at diagnosis and at the end of induction of remission,as well as at a series of follow-up. Moreover,the results of MRD detection by RQ-PCR were compared with that of detection by routine morphological examine of BM and cells differentiation mark analysis by flow cytometer(FCM),to evaluate the sensitivity of RQ-PCR in MRD monitoring. Results A method of RQ-PCR targeted at TEL-AML1 fusion gene was established. In 11 BM samples,which collected from TEL-AML1 positive children at the end of induction therapy and all of them achieved completely remission (CR) by routine morphological examine,5 samples were found to be MRD positive by RQ-PCR,positive ratio was 45%. There were 15 BM samples collected in maintenance therapy period,and all these samples were CR by routine morphological examine. However,by RQ-PCR,3 out of 15 samples were found to be MRD positive during maintenance therapy period. After intense and maintenance therapy,the MRD levels of the 3 children were declined to negative. In a recrudescent child,the expression of TEL-AML1 fusion gene was rose by a magnitude of 103 copies before relapse,and after induction therapy once again the patient was completely relaxed.Conclusions RQ-PCR targeted at TEL-AML1 has a higher sensitivity than conventional morphologic way and FCM. RQ-PCR can be used in the quantitative detection of MRD,and provide gist for early prognosticating a relapse and instructing clinical therapy.
