1.Introduction of “Bar Code Checking Program”for the Drug Dispensing in Our Hospital

OBJECTIVE:To reduce errors in drug dispensing so as to improve the accuracy of drug use.METHODS:The application of bar code technology in both domestic and foreign counties was overviewed,the flow-sheet and the chief function of the “bar code checking program”for the drug dispensing developed in our hospital were introduced.RESULTS &CONCLUSION:Connecting “bar code checking program”to hospital information system in the drug dispensing and adding machine judgment procedures can reduce the occurrence of error while improving quality of drugs dispensing.
2.Tirofiban in patients with non-ST elevation acute coronary syndrome undergoing percutaneous coronary intervention

Objective To explore the safety of tirofiban and short-term outcomes in elder patients (≥70 years old) with non-ST elevation acute coronary syndrome (NSTEACS) undergoing percutaneous coronary intervention (PCI). Methods One hundred and eight patients with NSTEACS were enrolled in the study with 42 patients older than 70 years (Group A) and another 66 patients younger than 70 years (group B). Basic characteristics, immediate success rate of PCI, the incidence of bleeding complications and thrombocytopenia were recorded. Clinical main adverse cardiac events(MACE) including cardiac death, recurrent angina pectoris, myocardial infarction(MI) and heart failure at 30-day were recorded and analyzed. 3.Efficacy of rotational atherectomy with stents for heavily calcified lesion

Objective To assess the immediate and mid-term outcomes of the combination of rotational atherectomy(RA)with stents on heavily calcified lesions.Methods Twenty-one patients with severely calcified lesions were treated with RA followed by stenting.Six cases had intravascular ultrasonic guidence during the procedure.The success rate of the procedure and the 6-month-follow-up results were analyzed.Results The burrs passed through the lesions successfully in all cases.All the lesions were either type B2 or C calcified lesions under angiography.Seventeen cases chosed the 1.25 mm bur while 2 cases used 2 burrs in the proceudre.All patients received PTCA after RA and stenting was performed in 19 cases.One patient had severely coronary vasospasm and 2 cases showed no-flow after PCI.None of the cases needed emergence bypass graft surgery and the mortality was null.In-stent restenosis was found in 2 cases at 6 months follow-up angiogram.Conclusion The combination of rotational atherectomy with stents therpy can selectively ablate calcific plaque and may increase the success rate of the procedure.

5.Twenty six cases of renal artery stenosis treated with angioplasty and stents

Objective To assess the short-term outcome of percutaneous transluminal renal angioplasty (PTRA) and stents for renal artery stenosis. Methods Twenty-six patients (17 males and 9 females) were diagnosed as renal stenosis by renal angiography and underwent simultaneous or selective PTRA and/or renal stenting. Changes in vessel diameter and perioperative complications were recorded. The blood pressure level, prescription of pressure-lowering drugs and the serum creatine level before and after the operation were analyzed. Results Unilateral artery stenosis was found in all 26 cases. Ostial stenosis were found in 18 cases (69%) and non-ostial stenosis in 8 cases (31%). The degree of stenosis was 70%-99%. All the 26 cases received renal stenting and 11 of them also received PTRA. The mean lumen diameter increased from 2.0 mm to 6.0 mm. The renal function was improved in 2 cases with impaired renal function before operation. The blood pressure leveal decreased or returned to normal level after stenting in 20 cases (76.9%) out of all the 26 hypertension patients. The were no perioperative complications recorded. Conclusion PTRA and/or renal stenting are effective and safe in treating renal artery stenosis.
6.Expression of survivin and P53 detected by tissue microarray and its significance in gastric adenocarcinoma

Objective To investigate the role of survivin and P53 in apoptosis of gastric adeno- carcinoma,as well as its relationship with the clinicopathologic features and the prognosis.Methods The gas- tric tissue microarrays were composed of those from 100 cases of gastric cancer and 30 controls.At these tissue microarrays,expressions of survivin and P53 were investigated immunohistochemically,and tumor cell apoptosis index was examined by TUNEL method.Of the 100 cases,47 cases were followed-up from 14 months to 13 years,in which the survival was analyzed.Results Two paraffin-embedded gastric carcinoma tissue micro- arrays were successfully constructed,including 114 and 116 tissue spots,respectively.Immunohistochemical analysis showed that survivin was expressed in 78 cases (78%).No expression of survivin was detected in control tissue (P0.05).In the 47 cases with followed-up data,univariant analysis revealed that the survival was correlated with invading of vessel and nerve,TNM stages,and expression of survivin.The histological grades and expression of P53 were not related to prognosis.However,Cox stepwise proportional hazards analysis showed that only TNM staging and survivin status retained significant independently in prospecting prognosis.Conclusions The expression of survivin was associated with the pathologic features,TNM stages and prognosis in gastric carcinoma,indicating that overex- pression of survivin may be a poor prognosis factor for gastric carcinoma.
7.Genetic analysis of UGT1A1 gene in a case and her family members with Crigler-Najjar syndrome type Ⅰ

Objective To test the mutation locus of uridine diphospho-glucuronosyltransferase gene (UGT1A1) in a Chinese patient with Crigler-Najjar syndrome type Ⅰ and her family members,analyzing the genetic characteristics of the pedigree.Methods Genomic DNA was extracted from the patient and her family members and other 50 full-term infants with normal serum bilirubin as a healthy control group.Fifty cases of full-term newborn whose serum bilirubin level were nomal were study as controls.The promoter and all exons of UGT1A1 gene were amplified by the method of polymerase chain reactions (PCR),and mutations were identified by direct sequencing.Results The propositus and her miscarriage sister were homozygous for a nonsense mutation at nucleotide number 715 (715C > T) in exon 1 of gene UGT1A1,substituting of stop codon (TAG) for glutamine (CAG) at position 239 (Q239X).The other 5 members were heterozygous in the same mutation locus.A TA insertion mutation and a G71R mutation in exon 1 were observed in the family members.The patient and her sister were homozygous of A(TA)7TAA mutation while other four were heterozygous.Propositus,grandmother,mother and her younger brother were heterozygous of G71 R mutation.No mutation was found in exons 2-5.No mutation was found in other fifty healthy cases in the healthy control group.Conclusions Q239X homozygous mutations is considered to be the lethal gene in this Crigler-Najjar syndrome family.Collaborative G71 R and A(TA)7TAA mutations may further reduce the enzyme activity of UGT1A1,causing varying degrees of bilirubin disorder.
8.Study on expression of transforming growth factor?1 in human colorectal carcinoma and its correlation with prognosis

Objective To investigate the expression of transforming growth factor?1(TGF-?1)in human colorectal carcinoma and its value for predicting the prognosis.Methods The expression of TGF-?1 and vascular endothelial growth factor(VEGF)was measured in specimens of 52 coloreetal cancers by immunohistoehemistry.The features of clinical pathology were analyzed and the follow-up of all patients were conducted.The correlation between the expression of TGF-?1 and the survival time was studied with Log-rank test.Results Of 52 patients,no expression of TGF-?1 and VEGF was observed in 11 and 14 patients,and the expression was noticed in 41 and 38 patients,respectively.There was a signifi- cant positive correlation between expression of TGF-?1 and expression of VEGF(x~2=0.633,P<0.01). Furthermore,the expression of TGF-?1 was significantly correlated with Dukes staging(x~2=19.866,P<0.01)and metastasis of lymph nodes(x~2=13.152,P<0.01).The 3-year overall survival rates(OSR)in all patients was 49.1% and the 3-year OSR of patients with and without expression of TGF-?1 were 20.5% and 69.2% respectively(x~2=11.64,P=0.0006).Conclusion The expression of TGF-?1 could be served as an important predicator for prognosis of coloreetal carcinoma.

10.Bacteriology of adenoids and tonsils in children with sleep-disordered breathing

OBJECTIVE This study was conducted to analyze the underlying bacterial pathogens of the tonsils and adenoids in children with sleep-disordered breathing(SDB).METHODS The core tissue from the tonsils and/or adenoids of 163 SDB children was cultured aerobically. Of the 163 cases, 120 children underwent adenoidectomy and tonsillectomy simultaneously(A+T), 39 children underwent adenoidectomy(A) and 4 tonsillectom(T) only. 124 children who underwent tonsillectomy were subdivided into two groups based on history(with or without a history of recurrent tonsillitis). 71 children with the history were enrolled in the 'recurrent tonsillitis group' and 53 children without the history were enrolled in 'non- recurrent tonsillitis group'.RESULTS Of the total 120 cases who underwent A+T, 114(95.00%) cases had same distribution of bacteria detected in both sides in the same patient. Besides this, 17 cases in whom mixed organisms were identified in both sites shared common pathogen. No significant difference in the detection rates of staphylococcus aureus and haemophilus influenzae were found when we compared seasons(Tonsil:χ2=8.538,P=0.201; Adenoid:χ2=5.427, P=0.490). No significant difference in the type and detection rate of essential bacteria was found when we compared between recurrent tonsillitis group and 'non-recurrent tonsillitis group' (χ2=3.028,P=0.387).CONCLUSION The bacterial isolates from the tonsils and adenoids are virtually identical in type and detection rate in the same SDB patient. The bacterial distribution of the tonsillar and adenoidal core is unaffected by the seasonal variation and history of recurrent tonsillitis.
