1.Changes in neural stem cells and their derived supporting factor in injured spinal cord tissues of rats

AIM:Neural stem cells(NSCs)and their supporting factors are related to nerve regeneration after central nerve injury, but there are few evidences for the changes of NSCs and their derived supporting factors.This study explores the change of NSCs and gene expression of stem cell-derived neural stem/progenitor cell supporting factor(SDNSF)in the injured spinal cord tissues of rats,and investigates the relation between the NSCs and expressions of SDNSF. METHODS:The experiment was carried out in the laboratory of Nantong University Neuroscience Institute between September 2005 and March 2006.①A total of 27 SD rats,cleaning grade,were offered by the Animal Experimental Center of Nantong University.Twelve rats used for immunohistochemistry assay were divided into sham operation group and spinal cord injury group,while other 15 rats used for RT-PCR detection were assigned into sham operation group and spinal cord injury group.All the disposals were in agreement with the ethical standard for animals.②The spinal cord contusion model of rats was established according to Allen's falling strike method.Laminectomy was conducted without strike in the sham operation group.The quantity and morphology of NSCs,gila cells and SDNSF-positive cells were observed by immunochemistry to detect expressions of nestin,glial fibrillary acidic protein(GFAP)and SDNSF in the injured spinal cord on days 8 and 16 after injury.The expression of SDNSF mRNA in the injured spinal cord was studied by semi-quantitative RT-PCR on days 4,8,12,16 after injury. RESULTS:①There were nestin-positive cells about central canal of spinal cord in the sham operation group.The nestin-positive cells showed erupted roots,migrated peripherally and proliferation,some GFAP-positive cells emerged and the SDNSF-positive cells were observed,which were similar to neuron on morphology on day 8 after injury.The quantity of nestin-positive cells decreased obviously,there were a number of GFAP-pesitive cells and few SDNSF-positive cells were showed on day 16 after injury.②RT-PCR revealed that SDNSF mRNA expressed in normal rate,and the expression was up-regulated on day 4 after injury,peaked on day 8,and decreased to the normal level on day 16. CONCLUSION:①There are some NSCs about the central canal of spinal cord,which proliferate and soon may differentiate to gila cell after injury in normal rats.②SDNSF is expressed in the normal spinal cord.The expression of SDNSF mRNA in the spinal cord varies with injured time,accompanying the proliferation and differentiation of NSCs. There are intimate relations between NSCs and expression of SDNSF in the repair of spinal cord injury.
2.Effect comparison of keratoconjunctivitis sicca treated by different medicines after phacoemulsification in patients with diabetes

AIM: To analyzed the effect of keratoconjunctivitis sicca treated by different medicines after phacoemulsification in patients with diabetes.METHODS: Totally 78 diabetic patients 78 eyes with keratoconjunctivitis sicca (KCS) after phacoemulsification from March 2015 to March 2016 were selected and divided into two groups using random number table.In Group A, 39 eyes were treated with pranoprofen eye drops + artificial tears + Qiming granules.In Group B, 39 eyes were treated with pranoprofen eye drops + artificial tears.the two groups were observed for clinical symptoms, tear film break up time(BUT), corneal fluorescein staining and adverse reaction and so on before and after treatment.RESULTS: Total effective rate of Group A was 97%, significantly higher than that of Group B (64%)(P<0.05).After treatment, symptoms and signs scores of the two groups were significantly lower, BUT were significantly prolonged.At 1, 2wk and 1mo after treatment, symptoms and signs scores of Group A were significantly lower the BUT was longer than those of Group B (P<0.05).Corneal fluorescein staining of Group A at 2wk was lower than that of Group B (P<0.05).CONCLUSION: The combined therapy, pranoprofen eye drops + artificial tears + Qiming granules, is able to quickly and effectively improve the clinical symptoms and signs, with no serious adverse reactions for keratoconjunctivitis sicca after phacoemulsification in patients with diabetes.
3.Study on the involvement of microtubule-associated protein l light chain 3B in the inlfammatory response of Kawa-saki disease

ObjectiveTo investigate the relationship between microtubule-associated protein l light chain 3B (LC3-II) and the inlfammatory response of Kawasaki disease (KD).MethodsThirty-nine cases of acute KD before intravenous admin-istration of immunoglobulin were enrolled. According to the results of echocardiography, the 39 cases were furtherly divided into KD with coronary artery lesion (CAL, 20 cases) group and KD with non-CAL (NCAL, 19 cases) group. At the same time, 12 healthy children were selected as controls. Serum samples were collected and cultured in vitro by human coronary artery endothe-lial cells (HCAEC) for 12 h. The LC3-II protein and mRNA expression of HCAEC were detected by Western-blotting and Q-PCR respectively.ResultsThe LC3-II protein and mRNA expression in CAL group and NCAL group were signiifcantly higher than those in control group, the LC3-II protein and mRNA expression in CAL group was higher than those in NCAL group, and both differences were statistically signiifcant (P<0.05).ConclusionsAutophagy may be involved in the inlfammatory response of KD in acute phase, which may be related to endothelial cell lesions of coronary artery in children with KD.
4.Analysis on minimum alveolar effective concentration of sevoflurane for laryngeal mask airway insertion under general anesthesia in premature infants

Objective To determine the minimum alveolar concentration(MAC) of sevoflurane without body movement during laryngeal mask airway(LMA)intubation in premature infants less than 37 weeks of corrected gestational age undergoing total inhalation general anesthesia induction.Methods Twenty-one ASA Ⅰ or Ⅱ premature infants less than 37 weeks of corrected gestational age undergoing elective inhalation general anesthesia were enrolled in this study.At first,the general anesthesia induction was started by inhaling 6 % sevoflurane.After the premature infant lost consciousness,the end tidal sevoflurane concentration(ET-sev)was adjusted to the predetermined concentration and maintained stable for 15 min.After that,LMA was inserted.The up-anddown sequential allocation was used to determine MAC.The initial ETsev was 2 %,which was increased or decreased by 1 gradient concentration in the next case according to the LMA insertion body movement response.The adjacent concentration gradient was 0.2%.The midpoint from th body movement response to non-body movement response was set as the balance point and the mean value of the concentrations of sevoflurane at all the balance points were calculated as MAC.Results The end tidal sevoflurane con centration without the body movement responses to LMA insertion was 1.71%.Conclusion The MAC of sevoflurane without the body movement responses to LMA insertion in premature infants less than 37 weeks of corrected gestational age is 1.71%,which is lower than that in the normal children and probably because imperfect central nervous system development in premature infants.
5.Effect of ginsenoside total saponinon on regulation of P450 of livers of rats after γ-ray irradiation.

Effect of ginsenoside total saponin (GTS) on the regulation of P450 of livers of rats after γ-ray irradiation was studied. Rats were irradiated by the ⁶⁰Coγ-ray for one-time dose of 5.5 Gy, dose rate of 117.1-119.2 cGy. The cocktail probe, qPCR and Western blot were used to detect expression of enzymatic activites, mRNA and protein of rats. Contrasted with blank group, expression of CYP1A2, 2B1, 2E1, 3A4 of irradiation group showed a up-regulated (P < 0.05). Contrasted with irradiation group, exprression of CYP1A2, 2B1, 2E1, 3A4 of GTS group showed a downward trend. GTS had negative agonistic action against expression of P450 of rats by irradiatied.

6.Influence of video-based teaching of Lamaze method on delivery among primiparae

Objective To explore the effect of video-based teaching of Lamaze method on delivery among primiparae.Methods One hundred and twenty pregnant women were randomly assigned to the intervention group and the control group with 60 in each.The intervention group received additional train-ing by using the video-based teaching of Lamaze method besides routine antepartum education,while the control group received only routine antepartum education.The cesarean section rate,labor course and neonatal asphyxia were compared between the two groups.Results The rate of natural delivery in the in-tervention group was significantly lower,and the rate of cesarean section caused by social factors decreased,which were all better than those in the control group.Neonatal asphyxia showed no difference between the two groups. Conclusions The video-based teaching of Lamaze method can promote natural delivery,shorten Labor course and worthy of wide application in pregnancy school.
7.Evaluation of community rehabilitation program for stroke patients

One hundred and twenty stroke patients were randomly divided into rehabilitation group (n =60) and control group (n =60).Patients were assessed with National Institute of Health Stroke Scale (NIHSS) ,Fugl-Meyer Assessment (FMA),Bathel Index (BI) and World Health Organization Quality of Life Assessment Instrument Brief Version(WHOQOL-BREF) before and 6 weeks after treatment by the same doctor.The scores of NIHSS,FMA,BI and WHOQOL-BREF were improved significantly in rehabilitation group after treatment; while those of control group were no improved.The scores after treatment of rehabilitation group were significantly higher than those of control group ( P < 0.05 ).

9.Clinical application of minimally invasive direct cardiac surgery: 108 cases report

Objective The aim of this study is to summarize primarily the application range and the clinical effect of the minimally invasive direct cardiac surgery (MIDCS).Methods From April 2010 to August 2011,108 operations had been performed in our centre using MIDCS,among of which 44 males and 64 females,mean age (44.3 ± 16.4 ) years old,mean weight (60.0 ± 12.0 ) kg.These operations included 29 MVR operations (replacement with mechanical valve 19,with tissue valve 10),17 AVR operations( replacement with mechanical valve 11,with tissue valve 6 ),5 MVP operations,6 MVR + AVR operations (replacement with mechanical valve 5,with tissue valve 1 ),12 VSD repair operations and 23 ASD repair operations,9 MIDCAB operations,1 MVR + CABG oparation,2 PECD correction,1 repair of ruptruec chordae tendineae of tricuspid septum and 1 Ebstein deformity correction operation.Concomitant procedures included 19 TVP operations and 11 radiofrequency ablation operations.Except MIDCAB operations,other operations are performed with closed type extracorporeal circulation.Conversion to median sternotomy was necessary in only two patients ( 1.9% ).Results Mean cardiopulnonary bypass time and aortic eross-clamp time were ( 104.4 ± 59.3 ) minutes and ( 66.7 ± 52.8 ) minutes respectively,74 patients ( 92.5 % ) recover to beat automatically after heart ceased operation.Median mechanical ventilation time was ( 14.5 ± 11.9 ) hours,Median intensive care unit stay was ( 18.8 ± 15.3 ) hours,median hospital stay post operative was (6.9 ± 2.9) days.Median incision length was (5.3 ± 1.2 )cm.Median draining volume was( 337.6 ± 240.9 )ml in the first day after operation,No transfusion occurred in 72 patients(66.7% ).Hospital mortality was 0.No re-exploration for bleeding and sternal wound infection.When leaving hospital,94 being of heart functional class Ⅰ,8 of class Ⅱ,6 of class Ⅲ.Conclusion MIDCS was associated with good operative effect in the near future,superior safety and broad application range.Furthermore MIDCS has minimally invasive,less postoperative bleeding,fewer blood transfusions,good cosmesis,as well as the absence of sternal wound infection.It may be safely performed on selected patients with low postoperative mortality and morbidity.
10.Genetic analysis of UGT1A1 gene in a case and her family members with Crigler-Najjar syndrome type Ⅰ

Objective To test the mutation locus of uridine diphospho-glucuronosyltransferase gene (UGT1A1) in a Chinese patient with Crigler-Najjar syndrome type Ⅰ and her family members,analyzing the genetic characteristics of the pedigree.Methods Genomic DNA was extracted from the patient and her family members and other 50 full-term infants with normal serum bilirubin as a healthy control group.Fifty cases of full-term newborn whose serum bilirubin level were nomal were study as controls.The promoter and all exons of UGT1A1 gene were amplified by the method of polymerase chain reactions (PCR),and mutations were identified by direct sequencing.Results The propositus and her miscarriage sister were homozygous for a nonsense mutation at nucleotide number 715 (715C > T) in exon 1 of gene UGT1A1,substituting of stop codon (TAG) for glutamine (CAG) at position 239 (Q239X).The other 5 members were heterozygous in the same mutation locus.A TA insertion mutation and a G71R mutation in exon 1 were observed in the family members.The patient and her sister were homozygous of A(TA)7TAA mutation while other four were heterozygous.Propositus,grandmother,mother and her younger brother were heterozygous of G71 R mutation.No mutation was found in exons 2-5.No mutation was found in other fifty healthy cases in the healthy control group.Conclusions Q239X homozygous mutations is considered to be the lethal gene in this Crigler-Najjar syndrome family.Collaborative G71 R and A(TA)7TAA mutations may further reduce the enzyme activity of UGT1A1,causing varying degrees of bilirubin disorder.
