1.Bacterial contamination on uniforms of health care workers in a general hospital

Objective To realize the bacterial contamination on uniforms of health care workers(HCWs)in a gen-eral hospital,and put forward the corresponding management measures.Methods In May-October 2012,a total of 360 specimens of 120 uniforms of HCWs in departments of respiratory internal medicine,general surgery,gynecolo-gy,and pediatrics were taken on the first,third and seventh day of wearing,bacterial counts on uniforms were mo-nitored,compared and analyzed.Results Bacterial counts of uniforms at different wearing time were statistically differ-ent.The longer time of uniforms were worn,the more bacteria could be detected.Bacterial contamination of nurses’uni-forms was more serious than doctors ([0.65±3.38]CFU/cm2 vs [7.68±2.99]CFU/cm2 ),contamination of uniforms of HCWs in surgical departments was more serious than non-surgical departments([10.43±4.12 ]CFU/cm2 vs [8.60± 3.01]CFU/cm2 )(U=5.06,2.78,respectively,both P<0.01),over standard rate of different sites of HCWs’uniforms were significantly different(χ2 =33.12,P<0.01));over standard rates of bacteria on the cuffs,abdomen and chest sites was 73.33%,58.33% and 36.67% respectively.Conclusion The management of cleaning system of HCWs’uniforms needs to be strengthened,the change cycle of uniforms is suggested twice a week,and the frequency needs to be increased in high contamination departments.
2.Bacterial Endotoxins Test for Ilex Pubescens Compound Injection

Objective To establish a method for the determination of bacterial endotoxin in Ilex pubescens compound injection(LPCJ) .Methods According to China Pharmacopoeia (second part,2000),interference test was carried out to detect the endotoxins in LPCJ with limulus lysate agents (?was 0.25 EU/mL).Results Inhibition action was found in bacterial endotoxin test for LPCJ and in the 10-,20-,30-and 40-fold diluted liquid of LPCJ.It was proved that the maximum noninterference concentration was the 50-fold dilution.The inspection results are the same as that of rabbit pyrogen test.Conclusion The 50-fold diluted LPCJ is suitable for the determination of bacterial endotoxin and can be used as the quality control standard for its preparation.
3.Determination of Bacterial Endotoxin in Maodongqing Compound Injection by Kine tic Turbidimetric Assay

Objective To develop an assay for determining bacterial endotoxin(BE T) content in Maodongqing Compound Injection (MCI). Methods The dilution radio o f Maodongqing Compound Injection was optimized and BET content was determined by turbidimetric kinetic method. Results Dilution ratio of Maodongqing Compound In jection at 1 ∶80 did not interfere with the Limulus test. The average recovery of BET was between 50 %and 200 %. Conclusion The method is proved to be accura te,sensitive and suitable for the determination of BET content in Maodongqing C ompound Injection.
4.Effect of Esomeprazole,Clarithromycin,Metronidazole Triple Therapy for Helicobactor Pylori Infection in Children

Objective To study the effect of esomeprazole,clarithromycin,metronidazole triple therapy for helicobactor pylori(Hp) infection in children.Methods Ninty-eight cases of children,identified by ~ 13 C-urea breath test(~ 13 C-UBT) Hp infection,deparded into therapy group(66 cases) and control group(32 cases).Therapy group were given Esomeprazole[0.8 mg/(kg?d),1 time/d],clarithromycin[15 mg/(kg?d),2 times/d],metronidazole[30 mg/(kg?d),3 times/d]triple therapy.Control group were given the same treatment except ameprazole.The course was 1 week.They were followed up 4 weeks later after the course and re-tested by ~ 13 C-UBT.Results The recurrent abdominal pain of the two groups recovered in different degrees,and the efficacy rate was 100%.The eradication rate of Hp in therapy group and control group were separately 90.9%(60/66) and 87.5%(28/32).There was no significance difference of the eradication of Hp.Conclusions The trearment of esomeprazole,clarithromycin,metronidazole triple therapy on Hp infection in children is quite effective and safe.The side effect is moderate.
5. Comparative analysis of prognosis between surgical or non surgical treatment for non small cell lung cancer with great vessel invasion in chest

Objective: To compare the prognosis induced by conventional surgery and chemotherapy and radiotherapy for non-small cell lung cancer (NSCLC) with great vessel invasion in chest. Methods: Clinical data from 102 NSCLC patients with great vessel invasion in chest, treated from 2000 to 2002 in our hospital, were reviewed retrospectively. There were 55 patients who received surgical resection and 47 patients who received chemotherapy and radiotherapy. The multiple factors affecting the survival rate and prognosis of patients were analyzed and compared. Results: The median survival time (MST) was 13 months and the 1-, 3-, and 5-year survival rate was 58.18%, 10.91%, and 3.9% for 55 patients who received surgical treatment. The MST was 16 months and the 1-, 3-, and 5-year survival rate was 65.96%, 22.46%, and 4.29%, respectively, for 47 patients who received non surgical management. There was no significant difference in survival rate between surgical and non surgical management (P > 0.05). Univariate analysis showed that clinical staging, PS score, chemotherapy, and radiotherapy were related with prognosis. The multivariate analysis demonstrated that clinical staging, chemotherapy, and radiotherapy served as independent survival factors (P < 0.05). Conclusions: There was no statistical difference in the survival rate of NSCLC patients at T4 stage with heart and great vessel invasion who received conventional surgery and chemotherapy and radiotherapy. Correct patient selection and skilled surgical technique assured complete tumor resection and increased the survival rate. Adjuvant chemotherapy or radiotherapy helped to prolong the post-operative survival time of patients.

8.Organ dose evaluation for CT scans based on in-phantom measurements

Objective To explore the organ doses and their distributions in different projections of CT scans.Methods The CT values were measured and the linear absorption coefficients were derived for the main organs of the anthropomorphic phantom to compare with the normal values of human beings.The radiophotoluminescent glass dosimeters were set into various tissues or organs of the phantom for mimic measurements of the organ doses undergoing the head,chest,abdomen and pelvis CT scans,respectively.Results The tissue equivalence of the phantom used in this study was good.The brain had the largest organ dose undergoing the head CT scan.The organ doses in thyroid,breast,lung and oesophagus were relatively large in performing the chest CT scan,while the liver,stomach,colon and lung had relatively hrge organ doses in abdomen CT practice.The doses in bone surface and colon exceeded by 50 mGy in a single pelvis CT scan.Conclusions The organ doses and their distributions largely vary with different projections of CT scans.The organ doses of colon,bone marrow,gonads and bladder are fairly large in performing pelvis CT scan,which should be paid attention in the practice.
9.The effects of rat transplantated tumor capillary permeability under microbubble enhanced diagnostic ultrasound exposure

Objective To investigate the effects of transplantated tumor capillary permeability under microbubble enhanced diagnostic ultrasound exposure.Methods Fourty eight rats were randomly divided into four groups after Walker 256 carcinomas were subcutaneousely implanted,named the control group (A),microbubbles group (B),ultrasound group (C),microbubbles and ultrasound group (D).Using Evans blue(EB) as the indicator for tumor capillary permeability,the microbubbles were injected through the vein with diagnostic ultrasound frequency as 1.75 MHz,mechanical index as 1.4.The spillover of EB was estimated under confocal laser microscope,meanwhile the contents of EB in the tumor tissues were calculated according to the standard curve and spectrophotometry.Results EB spillover was observed around the capillaries in the group D,while EB spillover was only observable on capillary in group A,B and C.The EB content was also significantly higher than those in group A,B and C.Conclusions Exposure to both diagnostic ultrasound and microbubbles results in increased capillary permeability of rat tumor tissues,which might become a potential therapeutic method on clinical chemotherapy.
