1.A Clinical Study of 14 Cases of Sarcoma of the Uterus.

Uterine sarcoma whieh is originated from uterine muscle and/or connective tissues, is very rare malignant tumor and is the most lethel of all primary uterine tumors. This study was undertaken to correlate the clinieal findings, diagnoses, managements and ultimate outcome of each particluar grouy of uterine sarcoma at Depart,ment of Obstetrics and Gynecology in Korea University Hospital. The reaults were as follows, 1. The distribution of uterinesarcomaby histologic type was 5 cases (35.7%) for leiomyosarcoma, 5 cases (35.7%) for endometrial strornal sarcoma and 4 eases (28.6%) for mixed Mullerian tumor, 2. The mean age and yarity were 50.8 years and 3.1. 3. The most common syrrlptorn was irregular vaginal bleeding (64.3%), and lower abdominal pain (21.4%), abdominal palpable mass (14.3%) in order of frequency. 4. The distribution by YIGO clinical atage was 35.7% for stage I, 35.7% for stage II, 7.2% for stage IE and 21.4% for stage lV. The average survival time of each stage of disease was decreased with increasing stage. 5. The mean survival time was decreased with inereasing numbers of mitotic figure per 10 high power fields. 6. The mean survival time according to histologc type was 14.5 months for leiomyoaarcoma, 21.5 months for endometrial stromal marcoma, 5.8 months for malignant mixed Mullerian tumor, respectively.

3.Efficacy of Haloperidol and Clonidine Combination in the Treatment of Chronic Schizophrenia.

OBJECTIVE: This study was designed to investigate the efficacy in the combination of haloperidol and clonidine with chronic schizophrenia. Moreover, we evaluated the effects of clonidine to the plasma haolperidol and reduced haloperidol concentration 36 patiens were entered on 4 weeks by the double blind trial. METHOD: The patients (male=11, female=25) were randomly divided into two groups of eighteen patients each. Their antipsychotic medication was fixed for 2 weeks before double blind trial with haloperidol and clonidine or placebo. Clonidine was started at 0.15 mg/day and then raised 0.6mg/day during 4 days, and the placebo was applied by same method as clonidine. The clinical assessment of the patients was assessed with BPRS and NOSIE. The plasma concentrations of haloperidol and reduced haloperidol were measured by HPLC (UV detector). RESULTS: 1) There were no statistical differences in sex distribution, age, duration of illness, dosages of haloperidol, plasma concentration of haloperidol and reduced haloperidol of each groups before clonidine or placebo trial. 2) On total and subcluster scores of BPRS and total score of NOSIE, there were decreased tendencies over the time, but there was no statistical significance in both groups. 3) Plasma concentrations of haloperidol and reduced haloperidol after 4 weeks of clonidine trial were revealed statistically significant increasement as compared to baseline. CONCLUSION: The combination of haloperidol and clonidine for 4 weeks did not reveal the improvement. In our opinions the reasons of those results were that our samples were not made of paranoid type of schizophrenia, the duration of clonidine trial was not good enough. So we should suggest that more specifying design will be needed.

4.Characteristics of 10 MV X-ray Beam from a Mevatron KD Linear Accelerator.

The beam characteristics and dosimetric measurements of the 10 MV X-ray beam from a Mevatron KD linear accelerator are examined. The Percent Depth Dose (PDD) table and the Tissue Maximum Ratio (TMR) table are taken from measurement as a function of the field size and the depth. The calculated TMR table from PDD table is compared with those from measurement. Other beam characteristics such as output factor, beam profile (including flatness, symmetry and penumbra), wedge, and the variation of Dmax are presented.

5.The Factors Affecting Physical Fitness Level of Female Flight Attendants.

PURPOSE: This study is aimed at obtaining the information on the physical fitness level of female flight attendants at a local airline. METHODS: The subjects are 2,409 female flight attendants who have had medical examination and physical fitness tests at an airline medical center from January 2 to December 30, 2008. The independent variables include socio-demographic & behavioral characteristics, and medical examination results, whereas the dependent variable is their physical strength score representing their fitness level. RESULTS: Factors that affect the fitness strength score are age, flight hours per year, and fasting blood sugar level. It is found that physical fitness level is higher when the subjects are over 35, on board for over 850 hours per year and their fasting blood sugar level is over 100 mg/dL. CONCLUSION: It is concluded that for the improvement of flight attendants' health, more exercise to develop cardio-respiratory endurance and muscular strength is needed, and further study needs to be done to develop fitness enhancement programs considering flight attendants' job characteristics, working years, and flight hours per year, and programs for those returning to their workplace after a long-term leave, and new employees.

A choledochocele is a benign cyst-like herniation of the intramural segment of the distal common bile duct protruding into the duodenal lumen. Cholangiography is essential to demonstrate a choledochocele. Because choledochoceles are often associated with characteristic structual alterations of the papilla of Vater and the peripapillary area, ERCP is helpful in demonstrating a choledochocele. We present the clinical, endoscopic and radiographic (ERCP) findings in a patient with choledochocele.

8.Pityrosporum Folliculitis.

Thirty-nine patients with pityrosporum folliculitis were investigated clinically and histopathologically. On clinical observation there were numerous, chronic, moderately itchy (64.1%), dome-shaped papules (89.7%) and pustules (66.7%). The most frequent sites of the lesions were the upper portion of the chest (76.9%) and back (56.4%). In biopsy specimens, abundant round and budding yeast cells were seen in a dilated hair follicle. The reptured follicle was observed in 19 specimen (48.7%). The accumulation on inflammatory cells were observed in or around the upper part of the follicle in all specimens. The effect of antimycotic treatment was excellent. After 4 weeks of treatment, 36 patients (92%) were cured and 3 (8%) had improved significantly. KOH/Parker Ink direct smear was done in 20 patients. Blue-colored round and budding yeast cells were observed under a light mcroscope in all patients. We suggest that pityrosporum folliculitis is a common disease of young and middle-aged Koreans.

9.Effects of Cofactor Tetrahydrobiopterin and Deletions on the Regulatory Domain of Tyrosine Hydroxylase on the Expression of Tyrosine Hydroxylase.

BACKGROUND: For the treatment of Parkinson's disease, dopamine-producing cells or genes involved in producing dopamine or supporting neurons have been tested to replace conventional chemical therapies. Of these, tyrosine hydroxylase (TH) was the most widely used gene for the therapy. Trials using TH via various vectors yielded behavioral improvements in animal models but the effectiveness did not last long enough. As one of the approaches for solving this problem, the regulation of expression of the protein and mRNA of TH was studied. METHODS: Two approaches for a higher and/or more stable expression of TH were pursued. First, the effect of cofactor tetrahydrobiopterin (BH4) on the expression level of TH and second, the effect of deletion which enables TH protein to escape from protease attack, were examined. RESULTS: Cells producing BH4 showed an approximately 10-fold higher TH expression than cells expressing TH alone. When the in vitro modified TH was expressed in NIH-3T3, mutant THs showed elevated protein (17.5 ~68.6 fold) and mRNA (1.8 ~4.6 fold) expression levels at a steady state. CONCLUSIONS: Results suggest that an addition of BH4 has a more positive effect on mRNA expression levels than protein. However, the deletions seem to have a tremen-dous effect on the translation and/or protein stability, but a small effect on the mRNA level. (J Korean Neurol Assoc 19(5):514~519, 2001)

10.Effect of Thymectomy in Myasthenia Gravis.

This study was performed to observe the effectiveness of thymectomy in the management of myasthenia gravis. Twenty-seven myasthenic patients were undergone thymectomy at Keimyung University Dongsan Hospital between January 1981 and December 1990. The most prevalent age group was the 3rd decade and the myasthenic syrnptom developed below thirty in most. Seven patients had remission and 10 patients fared better after thymectomy. There was no significant effect with age and duration of the disease in the result of thymectomy. Those with less severe group had better response than more severe group and non-thymoma group had higher remission rate without statistical significance than thymoma group. There were better response and more complications in the group with maximal thymectomy than simple thymectomy. This study demonstrated that the effect of thymectomy was better in less severe, nontymoma and maximal thymectomy group in myasthenia gravis.
