1.Clinical effect of point-to-point acupuncture with electric stimulation of direct current induction for facial nerve paralysis

Objective To observe the clinical effect of point-to-point acupuncture with electric stimulation of direct current induction for facial nerve paralysis. Methods A total of 65 patients with facial nerve paralysis were randomly divided into the observation group (n=33) and the control group (n=32). The control group received routine treatment, and observation group was received Dicang (ST4)-to-Jiache (ST6) and Yangbai (GB14)-to-Taiyang (EX-HN5) by acupuncture with electric stimulation of direct current induction combined with conventional western medicine. The treatment of both groups last 3 weeks. The House-Brackmann Scale and the BR indexe at baseline and after 3 weeks treatment were observed. Results After treatment, The clinical effect of observation group was 66.7%, which was significantly higher than control group 43.8% (U=2.175, P=0.013). After treatment, House-Brackmann facial nerve scale (Ⅰ level=15 cases, Ⅱ level=4 cases, Ⅲ level=3 cases) in the observation group was significantly better than that in the control group (Ⅰ level=5 cases, Ⅱ level=7 cases, Ⅲ level=2 cases; U=2.069, P=0.018). After treatment, R1 latency (12.28 ± 2.55 ms vs. 13.84 ± 3.07 ms, t=2.227) and R2 latency (32.28 ± 7.08 ms vs. 35.95 ± 5.11 ms, t=2.396) in the observation group were significantly shorter than those in the control group (P=0.030, P=0.020). Conclusions The point-to-point acupuncture with electric stimulation of direct current induction could reduce latency of R1 and R2 for facial nerve paralysis, and improve the clinical effect.
2.Preparation and dissolution of Wurenchun Dispersible Tablet

AIM:To prepare Wurenchun Dispersible Tablet(Fructus schisandrae Chinensis)and investigate the dissolution of it.METHODS:The optimal formulation of Wurenchun Dispersible Tablets was established by the single factor experiments.The dissolution of deoxyschisandrin from Wurenchun Dispersible Tablets was determined by HPLC.RESULTS:Wurenchun Dispersible Tablets prepared by the mixture of 50 g extraction and 125 g CaHPO_4 plus excipent(MCC∶CMS-Na 3∶1)could disintegrate and disperse well within 3 min in(20?1)℃ water.The dissolution of deoxyschisandrin from Wurenchun Dispersible Tablets was higher than 80% in 15 min.CONCLUSION:Wurenchun Dispersible Tablets were stable,dissolved fast and completely.The preparation could promote the dissolution of total lignans.
3.Determination of ester-alkaloids in Shiwuweirupeng Capsules by RP-HPLC

Objective: To establish the determination method of ester alkaloids in Shiwuweirupeng Capsules. Methods:The chromatographic method was carried out on Aichrom TM C 18 column using CH 3OH 0.05 mol?L -1 KH 2PO 4 CH 3COOH-(CH 3) 2CHOH(67∶173∶4∶4) as a mobile phase. The detection wavelength was set at 230nm. The flow rate was 1mL?min -1 . The column temperature was 35?C. Results: The benzoic acid calibration curves were linear at the ranges of 0.0152~0.076?g( r =0.9998). The average recoveries of aconitine was 93.3% with RSD =1.9%.Conclusion: This method is simple, reliable and provides a reference standard for the quality control of Shiwuweirupeng Capsules.
4.Determination of Tannins in Ershiwuweidatang Capsules

Objective: To determine tannins in Ershiwuweidatang Capsules (Flos Carthami, Fructus Chebulae, Fructus Terminaliae Billericae, Fructus Phyllanthi, etc.). Methods:Tannins were determined by casein method.Results: The contents of tannins in Ershiwuweidatang Capsules were 0.96%, 0.90%, and 0.85%, respectively. The gallic acid calibration curves were linear at the ranges of 0.8 ~4.0 ?g?mL -1 ( r = 0.999 ). The average recovery of tannic acid was 96.2% with RSD =1.4%( n =5).Conclusion: The method was simple and reliable.

6.Effect of early enteral nutrition on mechanically ventilated patients

Objective To observe the effectiveness of early enteral nutrition (EEN) in managing ICU mechanically ventilated patients.Methods Totally 47 patients who had been ventilated for more than one week were randomly divided into EEN group and control group.The EEN group was supplied with enteral nutrition (EN) 12-24 hours after ICU admission,whereas the control group received EN 72 hours-5 days later.The function of intestinal mucosal barrier was evaluated by the reabsorb concentration of disaccharides lactulose/mannitol (L/M).In addition,the body mass index (BMI),body temperature,urine L/M ratio,serum albumin,pre-albumin,and ventilation days were recorded or calculated.Results On the seventh day,the L/M ratio was (0.036 ±0.004) in the EEN group,which was significantly lower than that (0.108 ±0.020) in the control group (t =2.746,P <0.01) ; the average body temperature was significantly lower in the EEN group than in the control group [(38.25 ± 1.20) ℃ vs (38.92 ± 1.40) ℃ ; t =2.683,P < 0.05)] ; the incidences of adverse reactions such as constipation and diarrhea were significantly lower in the EEN group [16.7% (4/23) vs 27.3% (6/22),P<0.05].The weaning rate within 2 weeks also favoured the EEN group [90% (18/20) vs 80% (16/20),P < 0.05].Compared with the control group,the nutritional status of serum albumin and pre-albumin also showed a favourbale trends in the EEN group.Conclusions EEN can improve intestinal mucosal barrier and increase the weaning possibility in patients with mechanical ventilation.
7.Accuracy of plasma DNA concentration for evaluation of prognosis in patients with sepsis

Objective To investigate the accuracy of the plasma DNA concentration in evaluating the prognosis in patients with sepsis.Methods One hundred and sixty patients with sepsis were enrolled as the sepsis group (group SE).Another 109 patients without sepsis hospitalized during the same period served as the control group (group C).The venous blood sample was taken on admission for determination of plasma DNA concentration by polymerase chain reaction,C reactive protein (CRP) concentration by ELISA.APACHE Ⅱ score and SOFA score were evaluated at 24 h after admission.The 160 patients with sepsis were divided into two groups according to the result of prognosis:survival group ( n =103) and death group ( n =57).Results Compared with group C,the plasma DNA concentration,CRP concentration,APACHE Ⅱ score and SOFA score were significantly increased in group SE (P<0.05).Compared with survival group,the plasma DNA concentration,APACHEⅡ score and SOFA score were significantly increased in death group ( P < 0.05).The areas under receiver operating characteristic (ROC) curves of the plasma DNA concentration was significantly larger than those of APACHE Ⅱ score and SOFA score (0.81(95% CI,0.74-0.88) versus 0.68(95% CI,0.60-0.77),or 0.72(95% CI,0.63-0.82)).Conclusion The plasma DNA concentration can accurately evaluate the prognosis in patients with sepsis.As compared with the plasma CRP concentration,APACHE Ⅱ score and SOFA score,the plasma DNA concentration is more accurate to evaluate the prognosis in patients with sepsis.
8.Logistic regression analysis for the risk factors of the reflux during enteral nutrition in patients receiving mechanical ventilation

Objective To study the risk factors for the reflux of enteral nutrition in the mechanical ventilated ( MV) patients. Methods The data of reflux in 79 MV patients fed by enteral nutrition were collected. These patients were consecutively admitted to intensive care unit (ICU) of the Navy General Hospital from Jan. 2009 to Dec. 2010. Patients' performance conditions,patients' body position,location of feeding tube, way of feeding,tracheotomy,drug intervention were recorded. Simple logistic regression was used to analyze all the risk factors, and statistically significant variables were selected and adopted in multivariate and unconditioned Logistic regression analysis. Results The subjects averagely aged (58.6 ± 15. 3)years old,including 44 males and 35 females;with an APACHE Ⅱ score of 18. 1 ±4.0. Univariate analysis showed that age,position,location of feeding tube,way of feeding, tracheotomy et al were all assoctied with reflux. Advanced age( >60 y) ( OR = 4.577,95% CI 1. 459 - 14. 363, P =0. 009) was an independent risk factor for reflux; semisupination at 30° (OR =0.201,95%CI 0.057 -0.708,P = 0.013),nasalenteral tube (OR =0.267,95% CI0.072 -0.993,P = 0.049) ,and tracheotomy (OR = 0.232,95%CI0.070 -0.763,P =0.016) were independent protecting factors for the reflux. Conclusion Advanced age ( > 60 y) is a high risk factor for the MV patients with enteral nutrition;, semisupination at 30° .nasalenteral feeding,tracheotomy are low risk factors for the MV patients with enteral nutrition.
9.Relationship between expression of Her-2 gene and the transfer number of axillary lymph node and its influence on prognosis

Objective To investigate the relationship between expression of Her-2 gene and the transfer number of axillary lymph node and its influence on prognosis.Methods A total of 351 cases with primary breast cancer from January 2008 to January 2011 were selected.The expression of Her-2 gene was detected by immunohistochemical method,and analyzed the relationship between it and the transfer number of axillary lymph node and its influence on prognosis.Results The expression of Her-2-,+,++ had no correlation with the transfer number of axillary lymph node (x2 =3.757,1.650,2.379,P > 0.05),while the expression of Her-2 +++ had correlation with the transfer number of axillary lymph node (x2 =8.681,P < 0.05).The 2 years survival rates in the expression of Her-2-,+,++,+++ were 77.01% (201/261),85.00% (34/40),29.17%(7/24),1 1.54% (3/26),and which in the expression of Her-2--+ was significantly higher than that in the expression of Her-2 ++-+++ (x2 =61.605,P < 0.01).The transfer number of axillary lymph node was 0 and 1-3,the 2 years survival rate in the expression of Her-2--+ was significantly higher than that in the expression of Her-2 ++-+++,and there was significant difference (x2 =61.605,14.747,P < 0.05).The transfer number of axillary lymph node was 4-9 and ≥ 10,there was no significant difference in the 2 years survival rate between the expression of Her-2--+ and Her-2 ++-+++ (x2 =3.691,3.482,P > 0.05).Conclusions The expression of Her-2-,+,++ has no correlation with axillary lymph node metastasis,and the 2 years survival rate in the expression of Her-2--+ is higher than that in the expression of Her-2 ++-+++,while Her-2-has no difference with Her-2 + in prognosis.While the transfer number of axillary lymph node ≤ 3,the expression of Her-2 gene may be an important prognostic factor.
10.Vestibular and limb peripheral nerve impairment in auditory neuropathy

Objective: To investigate the impairment status of vestibular and limb peripheral nerve of patients with auditory neuropathy, improve the understanding of auditory neuropathy in general. Method: Vestibular function tests and nerve conduction velocity (NCV) examinatoin were performed on 28 young patients with auditory neuropathy which confirmed by clinical auditory tests diagnosis from March 1999 to November 2000. There were 14 males and 14 females, ranging in age from 22 to 28 years old. Results: Vestibular dysfunction was encountered in 22 of 28 (78.57%) suffering from auditory neuropathy. Limb peripheral nerve impairment was found in 11 of 28 patients (39.29%) of auditory neuropathy. The caloric responses were normal symmetric responses in 6 of 28 (21.43%,6/28), and weaken bilaterally in 20 of 28 (71.43%,20/28)respectively. On the NCV examination, both motor conduction velocity (MVC) and sensory conduction velocity (SCV) were normal in 17 (60.71%,17/28), abnormal in 4 (14.29%,4/28). Four cases showed abnormal MCV and SCV. And pure MCV abnormality and pure SCV abnormality were found on 4 and 3 cases respectively. Conclusion: The pathological process affecting the auditory nerve may also affect the vestibular nerve and other peripheral nerve. This seemed possible in view of fact that auditory neuropathy may affect one nerve (mononeuropathy) or multiple nerves (polyneuropathy).
