1.Bacteriologic Study on Normal Conjunctival Flora and Change of Antibiotic Susceptability.

We evaluated the distribution of normal conjuctival flora and the change of antibiotic sensitivity of cultured bacteria. Specimens were obtained from inferior conjunctiva cul de sac with sterile cotton-tipped applicator from healthy 216 human beings. Antibiotic sensitivity test was done by disk diffusion method. The results indicated that overall positive culture rate was 47.7% and major organisms were Coagulase-negative Staphylococcus(55.7%), Staphylococcus aureus(16%), Gram negative bacteria(7.5%), Streptococcus(4.7%). Antibiotic sensitivity test of cultured Gram positive bacteria demonstrated less than 60% rate of susceptability in gentamicin, tetracyclin, erythromycin and 80.5% rate of susceptability in ciprofloxacin. Gram negative bacteria demonstrated less than 60% rate of susceptability in cephalothin, ampicillin. From the above results, we concluded that antibiotic resistance of normal conjunctival flora was increased from the past and the principle of antiviotic eye drop use should be emphasized.

2.2 Cases of Mumps Orchitis Treated with Systemic Interferon-alpha2.

Mumps orchitis represents the most common complication of mumps infections and occurs in 5 to 37% of the populations. The most important danger is the risk of testicular atrophy up to 50% of affected testis which results in sterility. Two patients with mumps orchitis received systemic treatment with Interferon-alpha2(INF-alpha2)(3x06IU per day) for 7 days. All acute symptoms and signs of mumps orchitis disappeared within 4-7 days of INF-alpha2 therapy. No incidence of testicular atrophy was observed at the 19 and 6 months follow-up, respectively. No significant adverse effects of drug were occurred during treatment. We recommend the immediate interferon therapy, as soon as mumps orchitis manifests, to relieve clinical manifestation and to prevent testicualr atrophy and resulting infertility.

3.Options in Intracoronary Radiation Therapy.

Coronary restenosis is still regarded as Achilles' Hill of interventional cardiology despite relentless efforts of many investigators. Recent experimental and clinical studies have suggested that both gamma and beta radiation can be reduce restenosis after angioplasty. Currently, intracoronary brachytherapy for the prevention of restenosis has become a new evolving treatment modality in interventional cardiology. This report discusses a physical aspect of gamma and beta radiation, initial clinical results and delivery systems used in intracoronary brachytherapy. We shall take a brief overview of methods and their advantages in intracoronary brachytherapy. Future work should provide further insight for the best way of treating restenosis.

4.Ameloblastic fibro-odontoma(AFO) in the maxilla: a case report.

Ameloblastic fibro-odontoma(AFO) is a rare mixed odontogenic tumor. It is composed of connective tissue characteristic of an ameloblastic fibroma and calcified tissue as a complex or compound odontoma. AFO usually presents itself as an asymptomatic swelling of jaw or failure of tooth eruption. The lesion usually occurs in individual less than 30 years old. The differential diagnosis of this tumor includes odontoma, ameloblastoma, and ameloblastic fibroma. This report describes an ameloblastic fibro-odontoma occurring in maxilla of sixteen-year-old female. The lesion was treated by surgical enucleation and curettage without extraction of the involved canine(#23). This patient has shown no sign of recurrence during postoperative 34 months. So we report our case with review of literatures.

5.Comparative Analysis of Minimally Invasive Plate Osteosynthesis Using Periarticular Plate and Intramedullary Nailing in Distal Tibial Metaphyseal Fractures.

PURPOSE: To compare results between minimally invasive plate osteosynthesis using a periarticular plate and intramedullary nailing in distal tibial metaphyseal fractures in two treatment groups. MATERIALS AND METHODS: Sixty-one cases of distal tibial metaphyseal fractures from December 2008 to December 2009 were evaluated. The minimal follow-up period was 12 months. Thirty patients treated by minimally invasive plate osteosynthesis using a periarticular plate were Group A; 31 patients treated by intramedullary nailing were Group B. We compared and analyzed the results of each group by radiological and clinical assessments. RESULTS: The mean bony union time was 16.4 weeks in Group A and 17.2 weeks in Group B. The mean operation time was 45 minutes in Group A and 48 minutes in Group B. The mean radiation exposure times were 4.2 minutes and 4.8 minutes, respectively. VAS scores were 0.7 points and 0.5 points in each respective group. In Group A, the VAS score was 1.7 points when we applied pressure on the skin around the plate. The mean Olerud and Molander Ankle Score was 87.4 points and 86.3 points, respectively. A superficial wound infection occurred in 1 case in each group, and angular deformities more than 5 degrees occurred in 2 Group B cases. CONCLUSION: No significant differences in results were observed between the two groups. However, a higher incidence of angular deformity was seen in the intramedullary nailing group. Therefore, we must be careful during surgery.

6.Pectoral nerve block (Pecs block) with sedation for breast conserving surgery without general anesthesia.

Most regional anesthesia in breast surgeries is performed as postoperative pain management under general anesthesia, and not as the primary anesthesia. Regional anesthesia has very few cardiovascular or pulmonary side-effects, as compared with general anesthesia. Pectoral nerve block is a relatively new technique, with fewer complications than other regional anesthesia. We performed Pecs I and Pec II block simultaneously as primary anesthesia under moderate sedation with dexmedetomidine for breast conserving surgery in a 49-year-old female patient with invasive ductal carcinoma. Block was uneventful and showed no complications. Thus, Pecs block with sedation could be an alternative to general anesthesia for breast surgeries.

7.Non-intubated thoracoscopic surgery for decortication of empyema under thoracic epidural anesthesia: a case report.

General anesthesia is the main strategy for almost all thoracic surgeries. However, a growing body of literature has reported successful cases of non-intubated thoracic surgery with regional anesthesia. This alternative strategy not only prevents complications related to general anesthesia, such as lung injury, incomplete re-expansion and intubation related problems, but also accords with trends of shorter hospital stay and lower overall costs. We experienced a successful case of non-intubated thoracoscopic decortication for a 68-year-old man who was diagnosed as empyema while the patient kept spontaneously breathing with moderate sedation under thoracic epidural anesthesia. The patient showed a fast recovery without concerns of general anesthesia related complications and effective postoperative analgesia through thoracic epidural patient-controlled analgesia device. This is the first report of non-intubated thoracoscopic surgery under thoracic epidural anesthesia in Korea, and we expect that various well designed prospective studies will warrant the improvement of outcomes in non-intubated thoracoscopic surgery.

8.Focal Sparing of the Fatty Liver Caused by a Nontumorous Arterioportal Shunt.

We present two cases of focal fatty sparing caused by a nontumorous arterioportal shunt. Two patients underwent ultrasonography (US) as a part of routine medical screening, which revealed a hypoechoic lesion in the diffuse fatty liver. Each lesion appeared to be an arterioportal shunt on triphasic mutidetector computed tomography (CT) and was seen as being slightly hyperdense on noncontrast CT. No tumors were delineated around or within the arterioportal shunt. Both lesions remained unchanged on six-month follow-up US.

9.Oronasal fistula reconstruction using tongue flap with simultaneous iliac bone graft: a case report

The ultimate goal of cleft palate repair is to achieve an intact palate with the separation of the oral and nasal cavities. However, some patients develop an oronasal fistula in the secondary palate after palatoplasty. Postoperatively, a secondary palatal oronasal fistula may develop, leading to functional problems. In this study, we describe a patient with recurrent oronasal fistula and alveolar cleft with multiple failed previous reconstructions at another clinic. The oronasal fistula and alveolar cleft were repaired using a tongue flap and an iliac bone graft, respectively. The patient demonstrated excellent clinical progress with no recurrence of the oronasal fistula at the 1-year follow-up.
10.Relationship between Bony Alignment of Foot and Scoliosis in Children and Adolescent

Purpose:
Foot alignment affects the overall balance and alignment of the body. This study examined the relationship between the foot parameters and scoliosis in children and adolescents through simple radiographs.
Materials and Methods:
Two hundred and forty-one outpatients under 15 years old from 2013 to 2022 were evaluated. Patients with an abnormal leg length discrepancy or pelvic tilt were screened. The patients included were divided into four groups. Each group was tested to determine if they had scoliosis. The foot anterior-posterior/lateral X-rays were selected for the foot parameters, and the EOS system was used for the spline and pelvis parameters.
Results:
A t-test of all groups showed no statistically significant difference as the p-value exceeded 0.05. An analysis of variance (ANOVA) comparing the Cobb’s angle and foot parameters did not show a significant correlation. On the other hand, female and older patients had a higher Cobb’s angle.
Conclusion
No direct relationship was noted between scoliosis and the foot parameters that could be measured radiographically among children and adolescents. In addition, no correction between the Cobb’s angle and foot parameters was found.
