4.Effects of glutamine on immune function in elderly patients with malnutrition

Objective To evaluate the effects of glutamine on immune function in elderly patients with malnutrition.Methods Twenty-four elderly patients with malnutrition were equally randomized divided into glutamine group and control group.Patients in both two groups were administered with enteral nutrition containing equal nitrogen and calorics for consecutively 7 days.In the glutamine group,patients were also supplied with oral glutamine (7.5g tid).Changes of plasma IgG,IgA,and IgM levels,percentages of T-lymphocyte subsets (CD3,CD4,CD8),and CD4/CD8 ratio were determined before and after treatment.Results Plasma IgG and IgA lev-els,percentages of CD3 and CD4,and CD4/CD8 ratio were significantly higher in glutamine group than in control group (P<0.05 or P<0.01).Conclusion Glutamine can improve the immune function in elderly patients with malnutritiop and promote their recovery.
5.Systemic Monitoring of Blood Glucose Level in Small for Gestational Age Infants

Objective To investigate the distribution and duration of abnormal blood glucose level in SGA infants. Methods Blood glucose level was systemicly monitored in 37 SGA infants for (133?115) h(range 16~524 h, median 93 h), who were non NICU hospitalized and no feeding or IV infusion when admitted at (2 6?2 4) h (range 0.25~10.0 h, median 2.0 h) of age. Results During monitoring period, there was hypoglycemia in 19 cases (51%),hyperglycemia in 2 cases (5%), and both hypo and hyperglycemia in 3 cases (8%). The median of last time abnormal glucose level appeared was 10 hr after birth, its 95% confidence interval was 5~55hr after birth. Multiple regression analysis shown delivery models bad relationship with glucose level at admition (p= 0.013). The blood glucose level was higher in vaginal delivery babies. Since 1950s,the idea of liver transplantation (LT) was proposed,the technique of LT had been gradually and widely applied in the field of surgery.The development of LT experienced three stages,including animal experiment,clinical probation and clinical application.It is well known that LT is the only effective therapeutic method for terminal stage of liver diseases.In liver transplantation,the ischemia-reperfusion injury is difficult to avoid,and an important factor causing poor liver function.Meanwhile,hemodynamic volatility and internal environment disorder are important prognostic factors.How to reduce ischemia-reperfusion injury of the transplanted liver and improve intraoperative hemodynamic changes in liver transplantation is a clinical research focus.Recent studies has indicated that orthotopic liver transplantation retrograde perfusion method can effectively reduce the transplanted liver ischemia-reperfusion injury and improve liver function early after transplantation,and thus significantly improve the success rate of liver transplantation and survival.

The widespread use of antibiotics has been effectively controlled serious life-threatening infections.The trituration,exploitation and use of antibiotics has been rapidly developed and applied to many fields.Due to the extensive use of various antibiotics,the emergences of drug-resistant bacteria and the rates of bacterial resistance have increased unceasingly which has been a common focus in the world.Resistant pathogen can reduce the effectiveness of antibiotics,and which is a serious threat to human health.De-escalation therapy of antibiotics can quickly and effectively control serious infections,and avoid bacterial resistance,and induce fungal infections.The rational use of antibiotics in pediatrics is very imperative,which includes strengthening management of antibiotics,and improving awareness and vigilance of the adverse reactions of antibiotics,and consummating specification of medication.Then,realize the correct,safe,rational medication of antibiotics.

It summarizes the practical researches of self-handicapping from 7 aspects: (1)manifestations and types of self-handicapping behaviors;(2)evaluative conditions that typically elicit self-handicapping;(3)motives underlying self-handicapping; (4)factors influencing on self-handicapping; (5)consequences of self-handicapping; (6) measurements of self-handicapping. On this basis, it raises some questions in self-handicappping researches and points out the future direction.

Multidrug resistance to chemotherapeutic drugs is an important reason for clinical chemotherapy failure. The mechanisms of the classical drug resistance such as P-gp, MRP have been clarified. Hence, it is very important to further study the non-classical mechanism of multidrug resistance. We introduced the effects on MDR, the possible mechanisms of glutathione and related enzymes, as well as the relationship between structure of GSH analogues and the transport activity in this paper.
