1.COVID-19 Convalescent Plasma Therapy in Korea

A global effort was made to respond to COVID-19 using convalescent plasma therapy. The United States Food and Drug Administration Emergency Use Authorization facilitated rapid deployment, enabling the treatment of 94,287 patients by August 2020. The guidelines continuously evolved to emphasize the importance of a high titer plasma and specific immunocompromised patient groups. Korea has developed guidelines for treatment with convalescent plasma based on the successful treatment of two patients. By December 2023, convalescent plasma was collected from 67 donors at nine blood centers and transfused to 67 patients at 10 hospitals in Korea. The Association for the Advancement of Blood and Biotherapies recommendations, based on recent research, are believed to encompass the therapeutic effects of convalescent plasma therapy in COVID-19 patients. Korea initially considered convalescent plasma but experienced difficulties in development because of the switch to hyperimmune globulin and monoclonal antibodies. The insights gained from COVID-19 convalescent plasma treatment will be helpful in future pandemics caused by new infectious agents, underscoring the importance of domestic readiness for timely policy implementation.
2.New expandable metallic stents: an experimental study in vessels of dogs.

Three-types of expandable metallic stents were constructed to evaluate the differences between the stents : one, formed in a cylindrical zigzag pattern with stainless steel wire (Gianturco stent), another, made by coating the Gianturco stent with silicone rubber(Silicone stent), the third, made by coating the Gianturco stent with 24 carat gold(God stent) for tissue acceptance. A total of 69 stents(each 23 stents of Glanturco, Silicone, and Gold stents) of 8-12 mm in diameter and 20 mm long were placed into normal abdominal aorta, inferior vena cava and iliac artery of nine adult dogs for 2 weeks to 11 months. It was more difficult to introcuce Silicone stent into an introducing sheath than Gianturco or Gold stent due to the thickness of silicone rubber, Four Gianturco stents and three Silicone stents showed migration but Gold stent did not. Luminal narrowing or occlusion was noted in 3 Silicone stents, in 2 Gianturco stents, and in 1 Gold stent. Neointimal proliferation over the stent wires was more rapid and even in God stent than Silicone or Gianturco stent. Although further study is needed, Gold stent seems to be better than Gianturco or Silicone stent as an endovascular graft material.

3.Inspection and Evaluation of Blood Cold Chain

Background:
Due to the importance of the cold chain related to vaccine transportation, it is necessary to establish a blood cold chain operation strategy by checking the status of blood transportation from blood donation to transfusion.
Methods:
The blood transportation system and blood storage facilities were checked by inspecting the Korean Red Cross (KRC) Blood Centers and its affiliated supplier and Hanmaeum Blood Center. The status of the cold chain was evaluated through an interview with the quality control manager or blood supply team leader. For the hospital, the status was investigated from the perspective of the 30-min rule. A questionnaire survey was conducted for medical technicians and nurses working at the hospital.
Results:
Data on temperature during transport of blood components were computerized, and all standards were met. A nationwide network that could supply blood from the blood supply center to the hospitals within 2 hours was established. The frequency of temperature monitoring in the blood transport box and constant temperature check in the transport box during long-distance transport were evaluated.
Conclusion
This study confirmed that blood storage and transportation in Korea complied with the cold chain standards of high-income countries or higher. The evaluation of the cold chain is a constantly evolving process requiring continuous attention. When standards for storage and transportation of blood products are established, strict regulations and examinations will be required.
4.The Effects of Isokinetic Strengthening of Trunk Muscles on Balance in Hemiplegic Patients.

OBJECTIVE: To identify the effects of isokinetic strengthening of trunk muscles on balance in hemiplegic patients after stroke. METHOD: All participants were ambulatory hemiplegic patients, injured at least 6 months prior to study. The patients (n=16) were randomly divided into two groups. The control group received neurodevelopmental treatments and gait training 3 times a week. In addition to the same treatments provided for the control group, the experimental group received trunk isokinetic strengthening exercises using isokinetic dynamometer 3 times a week. Trunk muscle peak torque and balance in the experimental group were compared with those in control group at baseline and 4 weeks after treatments. RESULTS: There were no significant differences in age, lesion of hemiplegia, time after stroke onset, trunk muscle strength and Berg balance score before treatments between two groups. In the experimental group, peak torques of trunk extensor increased significantly at 60degrees/sec and 90degrees/sec at 4 weeks (p<0.05), but there were no significant improvements in peak torques of trunk flexor at all degree and extensor peak torques at 120degrees/sec. Both the extensor and flexors showed no significant changes in the control groups. In the experimental group, a mean percent change in peak torque involving the trunk extensor was significantly increased at 60degrees/sec. In the experimental group, Berg balance scores, 10 m gait velocity, sit to stand and gait 3 m and return, and 10 seconds stair up and down were improved (p<0.05). No significant improvements were noted in the control group. CONCLUSION: Isokinetic strengthening of trunk muscles can improve balance in hemiplegic patients.

5.Metaplastic Carcinoma of the Breast with Chondroid Calcification: A Case Report.

Metaplastic carcinoma is a rare form of breast carcinoma in which a variety of metaplastic changes occur. These commonly involve squamous or spindle cells, but pure chondroid metaplasia is relatively uncommon. We report a case of metaplastic carcinoma of the breast which mainly involved chondroid metaplasia and in which chondroid calcifications were seen on mammograms.

6.Blood Management System in Japan.

Information on the blood safety management system in Japan was collected by visiting the Ministry of Health, Labour and Welfare (MHLW), Japanese Red Cross Blood Service Headquarter, Kanto-Koshinetsu Block Blood Center, and Yurakucho Blood Room of Tokyo Metropolitan Blood Center, in July 2014, to improve the quality of the blood management system in Korea. In Japan, all blood products are supplied by the Japanese Red Cross Blood Service. In April 2012, the function of screening tests and blood product production of the provincial blood centers was transferred to the block blood centers. Donor suitability is assessed by medical doctors and EKG was tested for donors over 40 years old annually. To prevent bacterial contamination, the shelf life of platelets was shortened to 4 days after production, but routine bacterial screening test was not performed. Adverse reactions and infection following transfusions are reported to MHLW through the Red Cross Blood Service, and the case was reviewed by Pharmaceuticals and Medical Devices Agency (PMDA). Before transfusion, HBsAg, anti-HBs, anti-HBc, anti-HCV, and HCVcAg of the recipient is tested, and testing for HIV antigen is performed if the recipient has risk factors for HIV infection. Even when hepatitis B NAT is positive, look back is not performed if anti-HBc is negative and there is no history of blood donation within 125 days before the current donation. Good Manufacturing Practice (GMP) for blood centers was introduced in the 1990s and PMDA performs the test every 5 years. In introduction of GMP in Korea, it is necessary to decide how to absorb the expense.

7.Changes of serum CK and LD Isoenzymes at Tourniquet Application.

Serum creatine kinase (CK) and lactic dehydrogenase (LD) isoenzyme concentrations were studied in 44 patients during orthopedic operations that required the application of a pneumatic tourniquet. CK and LD isoenzymes were measured at preoperation, during tourniquet, and after tourniguet release. In the general anesthesia patients, LD isoenzyme values were not significantly changed during and after touniguet release. CK-BB was more significantly increased (p<0.05) after tourniguet (2.65+/-5. 40IU/l) than at preoperation (0.36+/-1.17IU/l). In the regional block patients, LD isoenzyme values were not significantly changed during and after tourniguet application. CK-BB was more significantly increased (p<0.05) at after tourniguet (0.90+/-1.95IU/l) than at preoperation (0.12+0.48IU/l). But regardleas of tourniguet application time, the values of LD isoenzymes were not significantly changed. CK-BB values were more significantly (p< 0.05) increased after tourniguet release than at preoperation, regardless of tourniguet application time.

8.Hydroquinone suppresses IFN-β expression by targeting AKT/IRF3 pathway.

Previous studies have demonstrated the role of hydroquinone (HQ), a hydroxylated benzene metabolite, in modulating various immune responses; however, its role in macrophage-mediated inflammatory responses is not fully understood. In this study, the role of HQ in inflammatory responses and the underlying molecular mechanism were explored in macrophages. HQ down-regulated the expression of interferon (IFN)-β mRNA in LPS-stimulated RAW264.7 cells without any cytotoxicity and suppressed interferon regulatory factor (IRF)-3-mediated luciferase activity induced by TIR-domain-containing adapter-inducing interferon-β (TRIF) and TANK-binding kinase 1 (TBK1). A mechanism study revealed that HQ inhibited IRF-3 phosphorylation induced by lipopolysaccharide (LPS), TRIF, and AKT by suppressing phosphorylation of AKT, an upstream kinase of the IRF-3 signaling pathway. IRF-3 phosphorylation is highly induced by wild-type AKT and poorly induced by an AKT mutant, AKT C310A, which is mutated at an inhibitory target site of HQ. We also showed that HQ inhibited IRF-3 phosphorylation by targeting all three AKT isoforms (AKT1, AKT2, and AKT3) in RAW264.7 cells and suppressed IRF-3-mediated luciferase activities induced by AKT in HEK293 cells. Taken together, these results strongly suggest that HQ inhibits the production of a type I IFN, IFN-β, by targeting AKTs in the IRF-3 signaling pathway during macrophage-mediated inflammation.

9.Large Pleural Effusion Causing Right Atrial Collapse and Tachycardia.

We observed one patient with large pleural effusion causing severe dyspnea, tachycardia, and severe right atrial collapse, which findings were completely resolved after thoracentesis. Our report shows that massive pleural effusion also can make severe right atrial collapse and symptom like cardiac tamponade, and thoracentesis can improve this condition.

10.The Anti-inflammatory Effect of Retinoid on Streptozotocin-induced Diabetic Nephropathy.

BACKGROUND: An inflammatory mechanism has been suggested to contribute to the progression of diabetic nephropathy. Although retinoid, a known anti-inflammatory agent, has been reported to be beneficial in some experimental renal diseases, it has not been shown whether it prevents disease progression in diabetic nephropathy. Therefore, we investigated whether all-trans retinoic acid inhibits inflammatory changes and improves renal function during the early stages of diabetic nephropathy in streptozotocin-induced diabetic rats. METHODS: We evaluated anti-inflammatory effect of retinoid on streptozotocin-induced diabetic nephropathy. Anti-inflammatory effect was determined by the expression of monocyte chemoattractant peptide-1 (MCP-1). RESULTS: Urinary protein excretion was significantly higher in diabetic rats at four weeks after the induction of diabetes mellitus compared with controls, and proteinuria in the group with retinoic acid treatment was decreased (1.25+/-0.69 vs. 0.78+/-0.72 mg/mg Cr, p=0.056). Urinary excretion of MCP-1 was rapidly increased at two days after induction of diabetes mellitus in diabetic rats, and further increased until four weeks of age compared with control rats. Retinoic acid treatment suppressed to 30% reduction of the urinary level of MCP-1 compared with vehicle treated diabetic rats (119.3+/-74.2 vs. 78.1+/-62.7 pg/mg Cr, p=0.078). Immunohistochemistry revealed a significant increase in staining for MCP-1 protein in the diabetic kidney, and retinoic acid treatment significantly suppressed intrarenal MCP-1 protein synthesis. CONCLUSION: Retinoic acid suppressed proteinuria and inflammatory changes in diabetic rats. These results suggest that retinoic acid may have an anti- inflammatory effect in diabetic nephropathy.
