1.Influence of excipients on the mechanical properties of microcapsules

AIM: To use the increasing rate of hygroscopic weight added to represent the mechanical properties of microcapsules, and to study the influence of excipients on the mechanical properties of microcapsules. METHODS: To develop a method of processing the microcapsules, which simulates the rubbing, shearing effect on the microcapsules during manufacturing, and calculate the increasing rate of hygroscopic weight added to study the influence of excipients on the mechanical properties. RESULTS: The excipients could affect the mechanical properties of microcapsules. CONCLUSION: The mechanical properties of microcapsules is the important quality standard, We can use it for selecting excipients used for microcapsulation.
2.The kinematic analysis of Dorsiflexion Ankle Foot Orthotics on pelvic control of hemiplegic patients

Objective To observe the effects of dorsiflexion ankle foot orthotics (d-AFO) on pelvic control of hemiplegic patients by analyzing the time-space gait parameters and 3D pelvic motion parameters.Methods Fifteen hemiplegic patients (14 male and 1 female) were enrolled in this study.The time-space gait parameters and pelvic 3D motion parameters were collecte with a Motion Analysis System while the patients were asked to walk under 3 conditions:wearing d-AFO (5°dorsiflexion),f-AFO (0°dorsiflexion) or barefoot.Results It was found that,while wearing d-AFO,the velocity,the paralyzed step length and the nonparalyzed step length were significantly increased,and the step width was significantly decreased when compared with those while walking barefoot (P < 0.05).Comparison between wearing d-AFO and f-AFO showed that the sagittal and coronal plane inclinations,but not the transverse rotation of pelvis were significantly decreased when wearing the d-AFO (P < 0.05).Conclusions d-AFO can significantly improve pelvis control and increase gait stability in hemiplegic patients.
3.Airway humidfi icationd urin g mechanical ventilation

Humidificait on by artificial means must be provided when the upper airway is bypassed during mechanical ventilation.Enough humidification is very important during mechanical ventilation.Heated humidification( HH) and heta and moisture exchanger( HME) are the most commonly used types of artificial humidification in this situation.In this article, their advantages and disadvantages are itn roduced.HH na d HME are compared in humidification performance,effect no ventilator mechanics and association with venti-lation associated pneumonia.Depending on the clinical scenario, humidifier is reasonably selected, and the selection may changeo ver time according the treatment effect.
5.Extraction of anti-asthma activitive protein components from fresh Earthworm (Eisenia foetida)

AIM:To identify the optimal extraction of anti-asthma active protein from fresh Earthworm(Eisenia foetida).METHODS:Extraction time,extraction solvent and pH were selected as influencing factors.90 quinea pigs were randomly divided into 9 groups(control,montelukast,low temp,room temp,acidic buffer extract,alkaline buffer extract,aqueous extract,neutral buffer extract,powder earthworm group).Anti-asthma effciency was compared among groups.extraction time,saturation of ammonia sulfate buffer volume were selected as factors of orthogonal design.RESULTS:The optimal extraction for anti-asthma protein was at 4 ?C,adding 2 fold volume of pH 6.8 buffer phosphate,homogenating,standing and extracting for 3 hours,taking the supernataut and centifuging,salting out with 80% ammonia sulfate,suspecting with 20 mmol/L,pH6.8 buffer phosphate.So Earthworm protein was gained from the clear supernatant.CONCLUSION:The extraction technique may keep its original activity.
6.Combination of IGF-1 with CEA, CYFRA21-1, NSE for the diagnosis and prediction of treatment response in lung cancer

Objective To evaluate four tumor markers of insulin-like growth factor 1((IGF-1), CEA, cytokeratin fragment antigen 21-1 (CYFRA21-1), neuron-specific enolase (NSE) for the diagnosis and prediction of treatment response in human lung cancer. Methods Serum samples were taken from three groups: 91 patients with lung cancer, 30 healthy adults and 15 patients with benign pulmonary diseases. Serum IGF-1 was assayed by radioimmunoassay and CEA, CYFRA21-1, and NSE by electrochemiluminescence immunoassay. The differences among the three groups were determined by Kruskal-Wallis one-way analysis of variance (ANOVA) and with Mann-Whitney rank-sum test. Diagnostic efficacy was evaluated by ROC curves. Results The four serum tumor marker levels were significantly higher in lung cancer group, as compared with the benign and the healthy (IGF-1:χ2=26.95,P<0.001, CEA:χ2=49.11,P<0.001; CYFRA21-1:χ2=40.63,P<0.001; NSE:χ2=14.76;P<0.001). The diagnostic sensitivities of IGF-1, CEA, CYFRA21-1 and NSE was 75.6% (34/45), 53.3% (24/45), 66.7% (30/45) and 42.2% (19/45) respectively for lung cancer. The diagnostic sensitivity of IGF-1 combined with CYFRA21-1 was 95.5 %( 43/45) and that of IGF-1 combined with CEA and CYFRA21-1was 97.8%(44/45). Only IGF-1 and CYFRA21-1 showed significant changes before and after treatment (IGF-1: χ2=5.99,P=0.014; CYFRA21-1:χ2=4.99, P=0.025) in cancer group. Conclusions Serum IGF-1, CEA, CYFRA21-1 and NSE are all valuable for lung cancer diagnosis and the combination of those parameters can enhance the diagnostic efficiency. Serum IGF-1 and CYFRA21-1 may also be useful for evaluating the treatment response in lung cancer.
7.Pharmacodynamics and pharmacokinetics of epidural ropivacaine in patients undergoing renal transplantion

Objective To investigate the pharmacodynamics and pharmacokinetics of ropivacaine for epidural anesthesia in renal transplantion. Methods Eight patients undergoing renal transplantation (group R) and eight patients with normal renal function (group C) were included in this study. Nine patients were male and seven female. Age ranged from 27-52 years and body weight from 46-89 kg. Radial artery was cannulated under local anesthesia for blood pressure monitoring and blood sampling. An epidural catheter was placed at L1-2. An epidural bolus of 0.75% ropivacaine 16 ml was given. The level of sensory block was tested every 3 min within 30min after epidural ropivacaine and then every 15 min.The degree of motor block was assessed using Bromage score every minute (0 = the patient can move his/her lower limb freely, 3 = the patient can not flex his/her hip, knee and ankle) . Arterial blood samples were taken before, during and after surgery for determination of plasma concentration of Alphal acid glycoprotein (?1-AGP) using an ultrafiltration technique. Results Demographic data including gender, age and body weight were comparable between the two groups. The onset time of sensory block in group R (20.00?4.63)min was significantly shorter than that in group C (32.14?8.09) min, while duration of sensory block in group R (60.00?27.69) min was significantly tonger than that in group C (43.13 ? 27.64) min. Objective To study the hygroscopicity of glucosamine sulfate powder.Methods Hygroscopic studies were conducted on glucosamine sulfate powder at 5℃ and 30℃ used gravimetric method,determined the equilibrium moisture content(EMC) and drew the moisture sorption isotherms.The experimental values were regressed using the BET model,calculated the monolayer moisture content.Results At 30℃,when water activity exceeded 0.43,samples agglomerated,browned and lose adsorbed water,EMC declined;At 5℃,the isotherm had typical ob-S shape,the EMC increased with water activity raising.At 30℃ and 5℃,the monolayer moisture content were 4.8633,5.5740g/100g respectively.Conclusion At 5,30℃ the appropriate aw for storage appeared to be below 0.3056,0.2784,respectively.
9.Clinical application of minimally invasive direct cardiac surgery: 108 cases report

Objective The aim of this study is to summarize primarily the application range and the clinical effect of the minimally invasive direct cardiac surgery (MIDCS).Methods From April 2010 to August 2011,108 operations had been performed in our centre using MIDCS,among of which 44 males and 64 females,mean age (44.3 ± 16.4 ) years old,mean weight (60.0 ± 12.0 ) kg.These operations included 29 MVR operations (replacement with mechanical valve 19,with tissue valve 10),17 AVR operations( replacement with mechanical valve 11,with tissue valve 6 ),5 MVP operations,6 MVR + AVR operations (replacement with mechanical valve 5,with tissue valve 1 ),12 VSD repair operations and 23 ASD repair operations,9 MIDCAB operations,1 MVR + CABG oparation,2 PECD correction,1 repair of ruptruec chordae tendineae of tricuspid septum and 1 Ebstein deformity correction operation.Concomitant procedures included 19 TVP operations and 11 radiofrequency ablation operations.Except MIDCAB operations,other operations are performed with closed type extracorporeal circulation.Conversion to median sternotomy was necessary in only two patients ( 1.9% ).Results Mean cardiopulnonary bypass time and aortic eross-clamp time were ( 104.4 ± 59.3 ) minutes and ( 66.7 ± 52.8 ) minutes respectively,74 patients ( 92.5 % ) recover to beat automatically after heart ceased operation.Median mechanical ventilation time was ( 14.5 ± 11.9 ) hours,Median intensive care unit stay was ( 18.8 ± 15.3 ) hours,median hospital stay post operative was (6.9 ± 2.9) days.Median incision length was (5.3 ± 1.2 )cm.Median draining volume was( 337.6 ± 240.9 )ml in the first day after operation,No transfusion occurred in 72 patients(66.7% ).Hospital mortality was 0.No re-exploration for bleeding and sternal wound infection.When leaving hospital,94 being of heart functional class Ⅰ,8 of class Ⅱ,6 of class Ⅲ.Conclusion MIDCS was associated with good operative effect in the near future,superior safety and broad application range.Furthermore MIDCS has minimally invasive,less postoperative bleeding,fewer blood transfusions,good cosmesis,as well as the absence of sternal wound infection.It may be safely performed on selected patients with low postoperative mortality and morbidity.
10.Ultrastructure changes of osteosarcoma cells inactivated by alcohol in different concentrations

Objective:To study the ultrastructure changes of osteosarcoma cells(OS-732) inactivated by alcohol of two different concentrations and clinical application value.Methods:Osteosarcoma cells(OS-732) were inactivated by 75% and 95% alcohol and observed by light and electron microscope.And its viability was detected by MTT method. Results:Cell internal structure changed significantly and irreversible damage formed. MTT method proved that inactivated cells had no viability.Conclusion:These two inactivation methods were effective. Cell internal enviroment was damaged very seriously and cell was led to death. These two inactivation methods could provide choices for clinical limb protective operation.
