1.Correlation between drug effects and serum HPLC fingerprint of active compositions of Radix paeoniae alba and Radix et Rhizowa Glycyrrhizae

AIM: To study the correlation between the drug effects and serum HPLC fingerprint from active compositions of Radix paeoniae alba and Radix et Rhizowa Glycyrrhizae. METHODS: Orthogonal design of L_9(3~4) of the different combinations of three active compositions of paeoniflorin,glycyrrhiza flavoues and Glycyrrhizic acid were prepared,antalgic pharmacological experiments of mice were carried out.The fingerprints of the serum were tested by HPLC method and different chromatographic peaks were obtained.Correlation analysis was applied to interelationship between the area of each peaks and the pharmacologic data. RESULTS: Correlation coefficients were different between pear areas and analgetic coffec. CONCLUSION: This study provides a significant try of investigating the compatibility of composite herbal medicines.
2.Distribution of flurbiprofen axetil in cerebral-spinal fluid after intravenous administration

Objective To examine the distribution of flurbiprofen axetil in cerebral-spinal fluid (CSF) by determining the CSF concentration of flurbiprofen after iv administration. Methods Seventy-two ASA Ⅰ or Ⅱ patients of both sexes aged 18-75 yr weighing 54-82 kg undergoing spinal or combined spinal-epidural anesthesia for lower extremity or lower abdominal surgery were studied. Flurbiprofen axetil 1 mg/kg was injected intravenously.CSF 2 ml and venous blood 3 ml were obtained simultaneously every 5 min after iv injection for 45 min (T1-9 ) for determination of flurbiprofen concentration using high performance liquid chromatography, and the CSF/blood flurbiprofen concentration ratio was caculated. Results Flurbiprofen was not detected in CSF at T1,2 after iv injection in 3 and 4 patients. The CSF flurbiprofen concentration was significantly higher at T4-9, and CSF/blood flubiprofen concentration ratio higher at T5-9 than at T3 ( P < 0.05). There was no significant difference in CSF flurbiprofen concentrations among T4-9 ( P > 0.05 ) Conclusion Flurbiprofen is detected in CSF after iv injection, the CSF flurbiprofen concentration peaks at 20 min after iv injection and it lasts until 45 min after iv injection.
3.Quantitative determination of ophiopogonin D and ophiopogonin D′ in Fibre Ophiopogon enriched extract by macroporous resin

AIM: To establish a HPLC-ELSD method for determining ophiopogonin D and ophiopogonin D′ in Fibre Ophiopogon enriched extract by macroporous resin. METHODS: The determination was performed by HPLC-ELSD on APOLLO C_(18) column(100 A,5 ?,4.6 mm?250 mm),using acetonitrile-H_2O(50∶50) as a mobile phase,flow rate was at 1.0 mL/min,column temperature was at 25℃,detector:ELSD(Neu:70℃,Eva:90℃,gas flow:1.5 SLM). RESULTS: The linear range of ophiopogonin D was 4.424-22.12 ?g,r=0.999 7,The average recovery was 97.6%,RSD=1.51%.The linear range of ophiopogonin D′ was 3.848-19.24 ?g,r=0.999 8.The average recovery was 98.4%,RSD=4.8%. CONCLUSION: The method is convenient,sensitive and accurate.
4.Ultrastructure changes of osteosarcoma cells inactivated by alcohol in different concentrations

Objective:To study the ultrastructure changes of osteosarcoma cells(OS-732) inactivated by alcohol of two different concentrations and clinical application value.Methods:Osteosarcoma cells(OS-732) were inactivated by 75% and 95% alcohol and observed by light and electron microscope.And its viability was detected by MTT method. Results:Cell internal structure changed significantly and irreversible damage formed. MTT method proved that inactivated cells had no viability.Conclusion:These two inactivation methods were effective. Cell internal enviroment was damaged very seriously and cell was led to death. These two inactivation methods could provide choices for clinical limb protective operation.
5.Subfascial Endoscopic Communicating Branch Vein Ablati on in Treating Communicating Branch Vein Insufficiency (Report of 57 Cases)

Objective To study the effectiveness of subfascial endoscopic communicating branch vein ablation in treating communi cating branch vein insufficiency.Methods Fifty-seven cases of primary lower ext remity vein valve insufficiency complicated by communicating vein insufficiency diagnosed by venography and color duplex were treated with subfascial endoscopic communicating branch vein ablation, and their information was analyzed retrosp ectively.Results Swelling was observed in leg of 7 cases after operation, and disappeared within one week spontaneously. Incision infec tion occurred in 2 cases and skin ambustion happened in 2 cases. Superficial vei n varicosis disappeared in all case, skin pigmentatin declined significantly and ulcers healed in 2-3 weeks.Conclusion Subfascial endoscopic communicating branch vein ablation is an excellent choice in treating communicating branch ve in insufficiency.
6.Control Hospital Infection to Realizet Potency Feeling among Malignant Tumor Patients

OBJECTIVE To strengthen the hospital infection control to realize the potency feeling among malignant tumor patients under the conduct of psychological nurses and other medical staff.METHODS The related hospital infection control system and the psychological countermeasure of nursing were formulated,we used two groups of comparisons methods around implements,counting and analyzing the effect of the hospital infection control among malignant tumor patients.RESULTS The psychologically healthy instruction enabled the patient positively to coordinate the treatment,the ratio of hospital infection from the original 21.33% dropped to 15.41%,and make the certain dynamic function to the self-realization of the patients' potency feeling.CONCLUSIONS The hospital infection control is an essential and important link to the nursing of the malignant tumor patients;the psychologically healthy instruction is an effective way to promote the self-realization of patients potency feelings.
7.Quantitative analysis of microcirculation in renal solid lesions with gamma curve fitting technique of contrast-enhanced ultrasound

Objective To evaluate the role of gamma curve fitting technique of contrast-enhanced ultrasound in quantitative analysis of microcirculation in renal solid lesions. Methods A total of fifty patients with renal parenchyma solid lesions were performed contrast-enhanced ultrasound. The images were analysed by computer with gamma fitting analysis of contrast-enhanced ultrasonic system. The quantitative parameters were obtained by the time-intensity curves, such as ascending slope (a3), descending slope (a2), arrival time (AT), time to peak intensity (TTP), basic intensity (BI), peak intensity, amplification (AMP), area under the curve (AUC), mean transit time (MTT) and perfusion index (PI). The parameters were compared between renal malignant and benign solid lesions. Results Fast-in and fast-out was the main perfusion mode in renal malignant tumors while slow-in and slow-out was found in renal angiomyolipoma (AML). The perfusion modes in renal malignant tumors and renal AML were fast-in and fast-out in 28 cases and 0 case, fast-in and slow-out in 4 cases and 1 case, slow-in and fast-out in 5 cases and 1 case, and slow-in and slow-out in 1 case and 10 cases, respectively. There were significant differences in the quantitative parameters such as a2, AUC and PI between renal malignant tumors and renal AML obtained by the time-intensity curves (P<0.05). Conclusion Gamma fitting analysis of contrast-enhanced ultrasound system can provide quantitative information of microcirculation of renal tumors, which helps to differentiate benign renal tumors from malignant ones.
8.Relationship between gut mucosal barrier dysfunction and immune response in patients with early severe acute pancreatitis

Objective To explore the relationship between gut mucosal barrier dysfunction and immune response in patients with early severe acute pancreatitis (SAP).Method 46 patients with early SAP were enrolled into this study from January 2010 to December 2012.Results The levels of endotoxin,the lactulose/mannitol (L/M) ratio,the D (-)-lactate concentration,and the proportion of HLA-DR-positive monocytes all decreased from a high level,while the frequency of Tregs increased during the first 14 days.The Th1/Th2 ratio decreased,with a decreased Th1 and an increased Th2 profile in the first week,but it subsequently increased,with an increased Th1 profile.Conclusions The data from this study showed that immunosuppression and immune imbalance happened in patients with early SAP,and showed a positive relationship with gut mucosal barrier dysfunction in patients with early SAP.Thus,early protection of the function of intestinal mucosal barrier is important in the treatment of SAP.
9.Mcrosurgical reconstructions of hepatic arteries in pediatric liver transplantation performed by a single surgeon:115 cases report

Objective Hepatic artery (HA) reconstruction is one challenging procedure in pediatric liver transplantation (PLT).Here we review the first 115 microsurgical reconstructions of HA in PLT performed by a single surgeon,aiming to demonstrate the learning curve and the problems encountered.Methods From July 2016 to January 2017,a series of 115 microsurgical reconstructions of HA in PLT for end-stage liver disease were finished by one single surgeon with 4-year liver surgery experience and 2-week microsurgical training.HA reconstruction was performed with an operating microscope (Carl-Zeiss S88).Reconstruction was completed with interrupted sutures with 8-0 or 9-0 Prolene using the double clip for fixation.The blood flow was examined by Doppler scan daily after PLTs in first week and then once in 2nd week and first month for patency.A total of 143 artery anastomoses were performed in 115 PLTs.The age ranged from 3 months to 9 years.Indications for PLT included biliary atresia (105/115),Alagille syndrome (5/115),PFIC (3/115),Caroli disease (1/115),methylmalonicacidemia (1/115) and glycogen storage disease (1/115).Most of the PLTs were living donor liver transplantation (107/115),along with OLT (5/115) and split LT (3/115).Results The diameter of the arteries was mostly less than 2 mm (98/115).Up to date,one HA thrombosis (HAT) occurred at D8 after LT and 4 cases suspected as temporal HA stenosis (HAS) around 2 weeks after LT,which manifested as low velocity (<20 cm/s) and resistance index (<0.50) by Doppler.The HAT case failed in emergent re-anastomosis,but had a spontaneous recanalization at 3 weeks and is now in good condition without biliary problem.All the HAS children recovered to normal flows at first month.All children with HA complications started warfarin upon detection,with a targeted INR between 1.5-2.0.There were 6 deaths in this series including 5 cases of infections and 1 case of graft failure.Learning curve suggested a two phases growth (first 44 cases practicing phase vs.next 71 cases mature phase),which can be attributed to experience accumulation in terms of precise of manipulation,choice of inflow arteries for better match and stronger pulsation,avoidance of length redundant,prevention of kink.All the HAT and HASs happened in practicing phase while outcomes were excellent in mature phase.Moreover,time for each anastomosis was significantly shortened in second phase from 45-70 min to 30-55 min.Conclusion Microsurgical technique is highly safe in pediatric HA reconstruction,especially for very tiny arteries.It is possible to achieve low risk of complications for a new surgeon with adequate experience in liver surgery and microsurgical training.However,more surveillance and timing anticoagulation therapy is required before the mature of microsurgical technique.
